Objective To explore the effect of terazosin hydrochloride combined with finasteride on serum TNF-αand PSA in old patients with benign prostatic hyperplasia.Methods 64 elderly patients of benign prostatic hyperplasia were randomly divided into 2 groups, 32 cases in each group.2 groups were given routine treatment and symptomatic treatment, the control group was given finasteride; the experimental group were treated with terazosin hydrochloride combined with finasteride.A cycle of 2 groups were 28 d, a total of 1 cycle of treatment.Clinical efficacy, maximum urinary flow rate, prostate volume, residual urine volume, international prostate symptom score(IPSS), serum tumor necrosis factor alpha, prostate specific antigen level and adverse reactions were compared after the end of treatment.Results Compared with before treatment, maximum urinary flow rate (Qmax) level of 2 groups increased, prostate volume, residual urine volume, international prostate symptom score(IPSS)decreased (P<0.05), compared with the control group, maximum urinary flow rate of the experimental group was higher (P<0.05), prostate volume, residual urine volume, IPSS were lower (P<0.05), 2 groups of TNF-αand PSA levels decreased after treatment (P<0.05), compared with the control group, TNF-αand PSA levels were lower(P<0.05).There was no significant difference in adverse reactions in 2 groups.Conclusion Terazosin hydrochloride combined with finasteride had a clinical efficacy of elderly patients with benign prostatic hyperplasia, can reduce serum TNF-αand PSA levels.

OBJECTIVE: To investigate the characteristics and influence factors of sensitization hy allergens in patients with allergic rhinitis in the junction of Guangdong and Guangxi provinces. METHOD: One thousand one hundred and twenty-four patients who were diagnosed with allergic rhinitis hy clinical history and symptoms underwent a skin prick test. The percent of allergens with positive skin response and Multiple Allergens Positive Scores were recorded, and their influence factors, included age, gender and test period were evaluated. RESULT: The percent of top three inhalant allergens with positive skin tests were as follow: dermatophagoides pteronyssinus (77.0%) ,dermatophagoides farina (77.1%), blattella germanica (42.3%); the percent of top three ingested allergens with loositive skintests were as follow: crab ( 39.5%), shrimp (31.1%), ribbonfish (10.8%). The average Multiple Allergens Positive Score was (3.6 +/- 2.7). The percent of allergens with positive skin response and Multiple Allergens Positive Score of different allergens changed as age, gender and text period changed, and the changing trend of them was accordant. The percent inhalant allergens with positive skin tests in dermatophagoides pteronyssinus, dermatophagoides farinae and cat fur in different gender showed a significant difference. CONCLUSION Acarine allergens were the most common allergens in patients with allergic rhinitis in the junction of Guangdong and Guangxi provinces. The percent of allergens with positive skin response and the severity of multiple allergy were influenced by age, gender and text period. The severity of multiple allergy can be measured by Multiple Allergens Positive Score.

OBJECTIVETo evaluate the clinical effects of minimal invasion and closed reduction with external fixation in treating elderly femoral intertrochanteric fracture.

METHODSFrom August 2007 and September 2013, 43 patients with elderly femoral intertrochanteric fracture were treated by minimal invasion and closed reduction with external fixation. There were 26 males and 17 females with an average age of 78 years old ranging from 68 to 92 years. Durations from injury to operation ranged from 1 to 8 days with an average of 3 days. According to AO classification, there were 22 cases with type A1, 15 with type A2, 6 with type A3. The time of bone healing were observed after operation and the clinical effects were evaluated by Harris scoring standard.

RESULTSAll patients were followed up from 10 to 18 months with an average of 13 months. The incisions obtained one-stage healing after removal of external fixation. All the fractures were healed and the mean healing time was 16 weeks (ranged, 12 to 18 weeks). Two patiens complicated with the thread needle loosening in the femoral trochanteric, 2 cases with the hip varus deformity and 1 case with the delayed union. No bone nonunion, external fixer breakage, blood vessel or nerve damage were found. According to the Harris scoring standard,the average Harris score of hip joint function was 85.89±7.36, and 18 cases got excellent results, 19 good.

CONCLUSIONMinimal invasion and closed reduction with external fixation can obtain satisfactory results in treating elderly femoral intertrochanteric fractures. It is a simple and effective method for elderly patients who cannot tolerate anesthesia and surgery trauma.

Objective: To study the effect of B 2O 3 and P 2O 5 on fluorosilicic mica glass-ceramic sintering process.Methods: Six sets of K 2O-MgO-SiO 2-MgF glass were prepared by using B 2O 3 and P 2O 5as sintering aid respectively. Green bodies of the glass powder were formed by gelcasting and sintered at 800,850,900,950 and 1 000 ℃ for 6 h respectively. The sintering and crystallization behavior were studied by thermal shrinkage , X-ray diffraction (XRD) and SEM.Results:The shrinkage rate of the glass with 2% B 2O 3 and P 2O 5 was the highest ,while that of the glass with 5% P 2O 5 the lowest. The size of the precipitated crystal from the glass ceramics containing 2% B 2O 3 and 2% P 2O 5 was bigger.Conclusion:The density and shrinkage measurements of the sintered bodies show that the glass powder of pure K 2O-MgO-SiO 2-MgF system has poor sinterability, while minor addition (

Objective To assess the effect of acute hypervolemic hemodilution (AHH) with different hemodiluents on activation of platelets. Method Twenty ASA I-II patients undergoing total hip replacement under general anesthesia were enrolled in this study. They averaged 54 ~ 76 years and 50 ~ 74 kg. The patients were premedicated with intramuscular phenobarbital 2 mg.kg-1 and atropine 0.01 mg. kg-1 . Anesthesia was induced with fentanyl 2ug. kg-1 , dropendol 50 ug. kg-1 , midazolam 40 ug. kg-1 and propofol 1 mg.kg-1. Tracheal intubation was facilitated with vecuronium 0. 1 mg.kg-1 . Anesthesia was maintained with infusion of propofol (2-4mg.kg-1. h-1) and vecuronium (0.06-0.2 mg. kg.h-1) and isoflurane inhalation. Radial artery and right suhelavian vein were eannulated for BP and CVP monitoring. The patients were randomly divided into 2 groups with 10 patients in each group : (A) HES group received hydroxyethyl starch (HES) 25 ml.kg-1 and (B) LR group received Ringer's lactate solution 25 ml.kg-1. In both groups HES or RL solution was infused at a rate of 30 ml-kg-1.h-1 after induetion oi anesthesia. Venous blood samples were taken before induction of anesthesia (T1 ), before AHH (T1 ), 15 min ( T3 ) , 30 min ( T4 ) and 45 min ( T5 ) after the beginning of AHH for determination of the percentage of CD62P and CD63 positive platelet using flow eytometry. Objective To analyze the levels and speciation of arsenic metabolites in urine of rats treated with sodium arsenite and sodium arsenate in order to investigate the different aspects of metabolism between sodium arsenite and sodium arsenate,thus to understand further the basic data about relationship between it's metabolism and mechanism of toxicity. Methods Seventy Wistar rats,weighting 80-120 g,were divided into 7 groups of 10 each,such as normal control group,high,middle and low sodium arsenite group and high,middle and low sodium arsenate group. After the animals were fed for one month,the urine was collected by metabolic cage in 12 hours. Applying the high efficiency liquid chromatography and hydride genesis atomic fluorescence spectroscopy (HPLC-HGAFS),the levels and speciation of arsenic metabolites were determined in urine of rats. Meanwhile,the recovery rate of dimethyl arsinic acid(DMA) would be determined to estimate the degree of accuracy of results. Results The levels of iAs~(3+),iAs~(5+) and DMA in middle sodium arsenite group[(121.66±1.26),(10.26±2.68),(200.91±0.56) μg/L]were higher than the high sodium arsenite group[(113.20±0.75),(5.16±1.32),(147.70±μ0.77)μg/L,all P < 0.05]and low sodium arsenite group[(79.35±2.12),(5.13±2.25),(56.35±1.23)μg/L,all P < 0.05]. The levels of iAs~(3+) and DMA in middle sodium arsenate group[(315.81±1.69),(245.12±1.18)μg/L]were higher than the high sodium arsenate group[(85.03±0.56),(110.34±1.04)μg/L,all P< 0.05]and low sodium arsenate group[(22.97±2.67),(15.75±2.15)μg/L,all P < 0.05]. Compared with sodium arsenate group,the levels of iAs~(3+) and DMA in high and low sodium arsenite group were higher(all P < 0.05) ; and the levels of iAs~(3+) and DMA in middle sodium arsenite group were lower(all P < 0.05). Meanwhile,the average urinary recovery rate of DMA of rats in different sodium arsenite group were 94.80%-102.70%,and the average urinary recovery rate of DMA of rats in different sodium arsenate group were 95.33%-108.40%. Conclusion The speciation and levels of arsenic are influenced by the external exposure dose,and some distinction appeared in the metabolism and metabolic path between sodium arsenite and sodium arsenate in urine in vivo.

Objective To investigate the effect and safety of duct-to-mucosa anastomosis technique for pancreaticoduodenectomy(PD).Methods The clinical data,including pancreatic fistula and other complications,of 189 patients underwent PD with end-to-side pancreaticojejunostomy at our institution from Jan 2001 to Jan 2009 were analyzed retrospectively.The definition of pancreatic fistula was threefold increase over the serum amylase level 7 days after operation,and draining volume was more than 50 ml per day.Results Totally 177 Whipple procedures were performed,while 12 pylorus-preserving pancreateduodenectomy procedures were performed.Five patients developed pancreatic fistula with a incidence of 2.65%(5/189).In which 3 were mild Cases,who fully recovered after conservative management,and the other 2 cages were cured by surgical intervention.Other complications included 9 cases of wound infection(4.76%,9/189),11 cases of empty dysfunction(5.82%,11/189),5 Cases of delayed hemorrhage(2.65%,5/189),and 4 cases of intra-abdominal infection(2.12%,4/189),and 2 patients died due to severe intra-abdominal infection and acute pulmonary infarction.Conclusions Duet-to-mucesa anastomosis technique resembles physiological state with low incidence of pancreatic fistula and delayed anastomosis hemorrhage.It may be used for different kinds of anastomosis for pancreatic stump.
