Objective To observe the changes of the function of vascular endothelial secretion, levels of OX-LDL and body-weight of obese children after exercise intervention. Methods Thirty children with obesity at the age of 8 to 14 years and 30 normal children were enrolled in this experiment. The obese children (exercise group) received a session of 1 hour's exercise each day, 5 days a week for 8 weeks. The levels of ET, 6-K-PGF1?,NO, OX-LDL and body-weight were then measured and compared with that of their pre-exercise state and that of the normal children (control group). Objective To explore the ways of repairing the large long-bone and skin defects as single stage procedures. Methods Five clinic cases were studied, firstly, the arteriae fibularis were restructuered into one or two vaso-nasa which nourished the related fibula segments and the overlying skin, then, binding all of the fibula segments and turn it into a bulky bone with a periosteal vaso-nasa wraparound. The graft was rich in blood supply so osteogenesis activity was vigorous under the periostum, the interspace in the graft soon disappear and the graft increased in diameter gradually. Results All of the harvested skin flaps survived and the wound healed on schedule, besides, the involved fractures were union 1 year later and the grafts were strong enough for weight-bearing and walking 2 year later, Neither fracture occur nor morbidity was created at the donor site, No problem was caused at the ankle. Conclusions It's appropriate way to repair large long-bone and skin defects with a vascularized fibula segments complex transfer incorporationg the overlying skin that was supplied via the same peroneal vessel pedicle.

OBJECTIVE:To evaluate the situation of the use of cardiovascular drugs and its trend of develop?ment.METHODS:The cardiovascular drugs used during2001～2003were analyzed retrospectively in respect to the sale amount,DDDs and etc.RESULTS:The sale amount of cardiovascular drugs was increasing year by year;the antihypertensives and antianginals ranked at the first two places in respect to the sequence of the amount of money and the DDDs;the sale amount of joint-venture products was the largest,and there was a high proportions of national essential drugs consump?tion.CONCLUSIONS:There was a stable and rational structure of cardiovascular drugs in our hospital.Domestic substitute drugs should be exploited for medical researchers in the future so as to reduce the cost of medicine.

Objective To clone and sequence the cDNA encoding(S)-N-methylcoclaurine-3'-hydroxylase from Coptis chinensis.Methods The cDNA,encoding(S)-N-methylcoclaurine-3'-hydroxylase,was amplified by RT-PCR with cDNA library of tender leaf as the template.Results The full-length cDNA of(S)-N-methylcoclaurine-3'-hydroxylase(named as CYP80B3) had 1 680 bp with an open reading frame encoding 488 amino acids of protein.The CYP80B3 had 95%,82%,70%,and 68% amino acid sequence homology to the sequence of(S)-N-methylcoclaurine-3'-hydroxylase from C.japonica,Thalictrum flavum,Eschscholzia californica and Papaver somniferum,respectively.The sequence was reported to the GenBank and coded as EF492879.Comparison of sequence with(S)-N-methylcoclaurine-3'-hydroxylase from C.japonica showed CYP80B3 possessed the same functional regions involved with 3'-hydroxylation of(S)-N-methylcoclaurine.Conclusion The cDNA encoding CYP80B3 from C.chinensis was cloned and reported.This work underlays the first step for exploring the pathway of benzylisoquinoline alkaloid biosynthesis and for improving the content of benzylisoquinoline alkaloid in C.chinensis.

Objective To determine the capability of emergency physicians (EPs) after goaldirected training to make accurate judgement and assessment of left ventricular systolic function (LVSF) as they own manipulated the hand-held echocardiography.Methods Eighty-one patients with acute dyspneic symptom admitted into emergency department of Xinhua Hospital Affiliated to Shanghai qaotong University School of Medicine from November 2011 to February 2012 were enrolled for a prospective,observational study.Patients with a history of trauma or acute myocardial infarction diagnosed by electrocardiogram were excluded.Four EPs after a intensive course of goal-oriented training in a good command of trans-thoracic echocardiography (TTE) in 81 emergency patients using hand-held echocardiography.EPs attempted to obtain images at the parasternal (long and short axis),apical,and subcostal positions,and visually estimated left ventricular ejection fraction (LVEF) and categorized LVSF as normal function,mild or moderate or severely depressed function.The results of echocardiographic LVEF got by EPs were compared quntitatively with those measured by an professional echocardiographer.The kappa statistical test by using SPSS version 13.0 software was used to allow for comparison in agreement between EPs and the professional echocardiographer's interpretations of TTE findings.Results Using the results of TTE measured by the professional echocardiographer as agold standard,EPs correctly distinguished the normal LVSF from decreased LVSF in 89％ patients.The rate of positive predictive value for the EPs identifying any abnormality in LV function was 83％ and the rate of negative predictive value was 93％.The kappa coefficient for the agreement between EPs and the professional echocardiographer' s interpretations for any abnormality in LV function was 0.77 (95％ CI:0.70-0.84,P ＜ 0.01).EPs correctly placed LV function into one of three categories in 68 of 81 cases (84％),The kappa coefficient for the agreement was 0.71 (95％ CI:0.64-0.78,P ＜0.01).Conclusions Emergency physicians after a intensive course of training in mastering echocardiography can accurately determine the left ventricular systolic function.

Objective To analyze the present situation of the dental instrument cleaning and disinfection in Hainan province and to explore the management mode of dental instrument cleaning and disinfection which is suitable for the provincial situation .Meth-ods By adopting the stratified random sampling method according to the hospital grades ,24 hospitals in Hainan province were per-formed the questionnaire survey on the cleaning mode of oral instruments ,layout of cleaning and disinfection room ,cleaning method and facilities ,protection measures and training of cleaning staff .Results 14 hospitals (58 .33% ) had the sterilization and supply center for conducting the centralized processing on the dental instruments .The tertiary hospitals and the second-grade hospitals had the independent cleaning and disinfection rooms with the rational layout and professional cleaning staff ;the safeguard facilities had the application in place ,the training of the related cleaning and disinfection work and the cleaning process conformed the require-ment of the standards .Among 10 first-grade and below hospitals ,only 1 hospital(10 .00% ) had the rational layout of cleaning and disinfection rooms ;3 hospitals(30 .00% ) had the professional cleaning staff ;the related training of the cleaning staff was not basi-cally carried out and the safeguard was not in place ,most of the cleaning and disinfection instruments and the cleaning process were not in accordance with the requirements .The qualification rates of instruments cleaning and disinfection in different grades of hospi-tals by the ATP bioluminescence assay were 100 .00% ,90 .00% and 80 .00% .Conclusion The existing problems are general and prominent in the hospitals of the first-grade and below .It is suggested that the regionalized disinfection and supply management mode is implemented for maximally realizing the optimized resource configuration in the disinfection and supply center .

Objective To investigate serological blood typing of the ABO locus which contradict to general law of inheritance in parentage,and the underlying reasons for severe hemolytic disease of newborn(HDN).Methods To research the family whose newborn is AB phenotypes,mother is O phenotypes and father is AB phenotypes.The familiy were genotyped by parentage tests, serological tests,PCR-SSP and direct DNA sequencing at exons 6 and 7 of ABO gene.At the salne time,HDN was detected by micro column gel Coombs (MGCT), and the primary fingerposts of the routine blood tests. Biochemical tests were dynamically observed.Results The results of parentage tests showed that three-generation pedigree have parent-child relationship. The red blood cell(RBC)of this AB phenotypes of this family members strongly agglutinated(4+)with diverse monoelonal anti-A and anti-B antibodies,and their serum did not contain anti-A and anti-B antibodies in blood anti-typing.PCR-SSP can not detect their A and B gene,but DNA sequencing at exons6 and 7 of ABO gene revealed that it had the B(A)803C→G mutation.Conclusions The genetm basis of this parentage are B(A)803G blood gene which harbored both A and B difunctionality of glyeosyhransferases.This was the first report that severe HDN resulting from a large number of A and B antigens in RBC of B(A)phenotype of a newborn,which has clinical significance on ABO locus.

Objective To investigate the effects of on-pump and off-pump coronary artery bypass grafting on plasma adrenomedullin (ADM) and endothelin-1 (ET-1) levels before and after operation in elderly patients. Methods Elderly patients aged 60 years and over with coronary artery bypass grafting were enrolled in the study and divided into off-pump group and on-pump group, each group had 20 cases. Blood samples were collected before surgery, immediately after operation and 6,24 hours, 3 days after operation. The plasma levels of ADM and ET-1 were dectecded by radio immunoassay. Results The levels of ADM and ET-1 were significantly increased immediately after operation in 2 groups of on-and off-pump. The levels of ET-1 in on-pump group and off-pump group were (67.59±10.18) ng/L and (58.73±9.64) ng/L respectively. The levels of ADM in on-and off-groups were (284.90±36.24) ng/L and (243.25±25.70) ng/L. This showed that in on-pump group, the plasma levels of ADM and ET-1 increased more significantly(P＜0.05). The plasma levels of ADM and ET-1 began to decline at 6 hours after operation, and continued to 24 hours after operation. On 3 days after operation, the plasma levels of ADM and ET-1 in off-pump group returned to the preoperative level, but they were still maintained at a high level in on-pump group.Conclusions For elderly patients, both on-pump and off-pump coronary artery bypass grafting can cause endothelial dysfunction, increasing plasma levels of ET-1 and ADM. But compared with the on-pump coronary artery bypass grafting, the off-pump coronary artery bypass grafting has less influence.

OBJECTIVE:To optimize the preparation technology for enteric-coated changbiqing granules.METHODS:The concentration of acrylic resin and the four factors like the quantity of castor oil,pulvis talci and methylsilicone oil on the yield of changbiqing enteric-coated granules were determined by orthogonal test design;The effects of the enteric-coated granule plasticizer on the quantity of castor oil and the weight increment of the coating were determined by the release rate test.RESULTS:The optimized formula of the coating solution was described as follows:the concentration of acrylic resin was5%,the relative amount of castor oil to acrylic resin was35%,the dosages of pulvis talci and methylsilicone oil were3%and1%,respectively,the weight increment of the coating was30%.CONCLUSION:Changbiqing enteric-coated granules pre?pared on the optimized technology are in line with the quality requirements and which have ideal enteric soluble drug releasing characteristics.
