Objective To examine the relationship between inflammatory response induced by cardiopulmonary bypass and postoperative cognitive dysfunction.Methods Twenty-five patients undergoing elective cardiac valve replacement under cardiopulmonary bypass were randomly divided into two groups:ulinastatin group(group U,n=13) and control group(group C,n=12).In group U,ulinastatin 12 000 U/kg Was given intravenously immediately after induction of anesthesia,6 000 U/kg ulinastatin Was added to the priming solution,and 6000 U/kg ulinastatin was given at 5 min before the aortic decamping.In group C,normal saline was given instead of ulinastatin.Venous blood samples were taken after induction of anesthesia,at the end of CPB,and 24 h after operation for determination of plasma IL-6 concentration and neutrophils NF-kB expression.The cognitive function of the patients was evaluated by Mini-Mental State Examination(MMSE) before and 3 d,7 d after operation.Results The concentraion of IL-6 and neutrophils NF-kB expression were lower in group U than in group C(P<0.05 or 0.01).There wag no significant difference in the incidence of postoperative cognitive dysfunction between group C and U.Conclusion Inflammatory response induced by cardiopulmonary bypass is not related to postoperative cognitive dysfunction.
4.Effects of Iotensin premedication on sympathetic responses and plasma catecholamine concentration in cervical plexus block

Objective: To evaluate the effects of lotensin on sympathetic responses following cervical plexus block and to explore the mechanism of cardiovascular responses and sympathetic nervous system activity according to plasma catecholamine concentration. Method: Sixty adult patients were randomly divided into two groups (n = 30):Group A and group B. In group B lotensin 0.15mg/kg was taken orally at night before surgery and 2h before anesthesia separately. Hemodynamics was determined before anesthesia(T_1), 5min(T_2), 10min(T_3), 15-20min(T_4)and 30min (T_5) after cervical plexus block. For measurement of plasma catecholamine concentration, blood was collected at T_1,T_3,T_4 and T_5. The study was finished before beginning of surgery and infusion. Objective:To study the validity of Binomial Forced-Choice Digit Memory Test(BFDMT) in detecting dissimulation of intellectual deficit.Methods:64 subjects with compensable head injury were assessed by BFDMT,experiential judgment and Raven's Standard Progressive Matrices (RSPM),and were finally diagnosed on intellectual deficit degrees.Results:(1)The rate of malingering was 78.1% judged by BFDMT,and 43.8% by experiential judgment.(2)All of the 16 uncertain cases by experiential judgment were assessed as malingering by BFDMT.(3)Only one case in 64 was considered as a faulted diagnosis by follow-up.Conclusion:BFDMT is useful for detecting dissimulation of intellectual deficit particularly for the difficul cases in clinical experiential judgment.
7.The Effects of Tangnaikang on IL-6 and TNF-α in Serum and Adipocytes of Diabetic KKAy Mice

Objective To observe the effects of Tangnaikang on interleukin 6 (IL-6) and minor necrosis factor-α(TNF-α) levels of Diabetic KKAy mice. Methods After diabetic KKAy mice were treated with Tangnaikang for 8 weeks,FBG was determined by glucose test-paper, IL-6 and TNF-αconcentratious in serum and adipocytes were determined by ELISA. Results IL-6 and TNF-α levels were reduced in serum and adipocytes of KKAy mice. Conclusion Tangnaikang can significantly reduce the levels of IL-6 and TNF-α in Diabetic KKAy mice, and the reduction of them can help to decrease FBG level.
8.Conversion from cyclosporine to tacrolimus in patients with chronic allograft nephropathy

Objective To investigate and compare the effect of cyclosporine A (CsA) vs. tacrolimus (TAC)-based immunosuppressive regimen on chronic allograft nephropathy. Methods Ninety-six patients who received a cadeveric kidney transplantation in our unit during Jan. 1995 to Jan. 2004 more than 12 months prior to study enrollment and who were being treated with CsA-based immunosuppressive treatment were included. All patients received allograft biopsy and were diagnosed as CAN. Patients were differentiated according to following regimen. Patients were either converted to tacrolimus (TAC group, n=58) or remained on their initial CsA-based immunosuppression (CsA group, n=39). The clinical data at study entry and after 3, 6 and 12 months including serum total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), high density lipoprotein (HDL), blood urea nitrogen (BUN), creatinine (SCr), albumin were recorded during a follow-up of over 12 months. 9.The changes of serum and urine guanidinoacetic acid level after cadmium chloride-induced renal injury in rabbits

Subcutaneous injection of 1.0 mg/kg cadmium chloride in 0.8% solution was given to rabbits weighing 1.7~2.5kg 3 times a week for 10 weeks.An equal volume of normal saline was injected to the rabbits of the control group.Blood specimen from the auricle vein and 24-hour-urine of every rabbit were collected once a week.Ten weeks later the renal cortex of the rabbits WES resected,processed routinely and examined under optical microscopy,The level of guanidinoacetic acid (GAA) of the serum and urine was determined with HPLC.It was found that the serum level of GAA increased after 3 weeks of cadmium administration while urinary excretion of GAA increased slightly in the first week and decreased significantly in the second week of cadmium injection and thereafter.
10.Changes of serum enzyme activities of healthy males under simulated high altitudes

The changes of the activities of serum CK,LDH,HBKH and AST of 11 soldiers having stayed in simulated 3500m and 4500m were observed.It was found that the activities were significantly increased in the order of CK,LDH and HBDH because of hypoxia but the changes of AST was of no statistical significance.CK/LDH ratio and pulse rate showed similar charges to serum enzymes but the changes of arterial oxygen saturation and working efficiency were the opposite to those of serum enzymes.These findings suggest that serum CK and LDH are the indicators of choice to evaluate human reactions to high altitude.
