1.Clinical Value of Detecting Serum GP7 3 and Related Indexes of Liver Fibrosis Levels in Patients with Hepatitis B Cirrhosis

Objective To investigate the relationship between serum Golgi protein-73 (GP73)level and related indexes of he-patic fibrosis in patients with hepatitis B cirrhosis diseases and discuss the clinical value of GP73.Methods The level of GP73 was detected by ELISA;the levels of type Ⅳ precollagen (Ⅳ-C)and hyaluronic acid (HA)were measured by radio-immunity in serum from 11 patients with acute hepatitis B (AHB),31 patients with chronic hepatitis B (CHB),71 patients with Hepatitis B Cirrhosis (LC),23 patients with primary hepatic carcinoma (PHC)and 34 healthy people with check-up. The clinical changes and relationship between the GP-73 level and the parameter of fibrous degeneration were studied.Re-sults The GP73 level in CHB group was 118.0±82.2μg/L;the GP73 level in LC group was 154.4±87.7μg/L and the GP73 level in PHC group was 196.5±89.6μg/L.Compared with the normal control group,statistically significant differ-ence were found in GP73 level of CHB group,LC group and PHC group (t values were -7.284,-7.909 and -9.609,re-spectively;P<0.01).As the progress of liver cirrhosis,the levels of GP73,Ⅳ-C and HA all increased gradually and the level of GP73 was significantly positively correlated to Ⅳ-C and HA (r valves were -0.212 and 0.487,respectively;P<0.01). Conclusion The GP73 was high expressed in chronic liver disease,especially in liver cirrhosis.The GP73 was significantly associated with the occurrence of hepatitis b liver fibrosis.There fore,GP73 could be used as a better index to monitor the fi-brosis degree.

3.Drug Resistance Mutations and Genotypes of Hepatitis B Virus P in Nantong Area and Its Clinical Significance

Objective To investigate the mutation characteristics and genotype of hepatitis B virus resistance gene in Nantong area,and provide scientific basis for clinical rational drug.Methods A total of 158 cases of chronic hepatitis B (CHB)pa-tients who were received with nucleos (T)ide analogues therapy for at least 2 years as the research object,and 30 cases of CHB patients who were not received nucleos (T)ide analogues for the treatment as the control group.PCR-sequencing method was used to detect the HBV P resistant gene and genotype,meanwhile,observe the relationship between three main mutation model and the levels of ALT and HBV DNA was also investigated.Results B genotype was detected in 42 (26.58%)out of 158 CHB patients,and 116 cases (73.42%)were C genotype.A total of 131 patients with different site mutations in P region,the mutation rate were 82.91%.There were totally 11 HBV mutation sites,including the main muta-tion site:M204I,L180M,M204V,A181V and A181T,the frequency of drug resistance were 41.14%,37.34%,22.15%, 11.39% and 10.13%,respectively.Moreover,11 mutation sites had 21 mutation patterns.In lamivudine (LAM)resistance associated mutations,the L180M and M204V sites were mainly co-occurrence,followed by M204I alone.In adefovir dipivoxil (ADV)resistance associated mutations,A181V was the main mutation site.Whereas,the drug resistance rate of entecavir (ETV)was low.Conclusion The main genotypes of HBV were type B and C in Nantong area,and C type was the dominant genotype.The resistance mutations mainly concentrated in LAM and ADV resistance associated mutations,while the resist-ance rate of ETV was low.Multi-locus drug-resistant mutation detection may help to detect viral resistance and guide clinical treatment better.
4.Comparison of the release and percutaneous penetration in vitro between psoralen ointment and gel

AIM: To compare the release and percutaneous penetration in vitro among 2 kinds of the psoralen ointments and its gel. METHODS: Dissolution tester and Franz diffusion cells were adopted as the release and percutaneous penetration in vitro, and HPLC was used to measure the amount of psoralen in the releasing solution, receiving solution and the skin. RESULTS: The release and percutaneous penetration of psoralen from the 3 kinds of preparation conformed to the zero kinetic equation. The steady-state penetration rate of psoralen in the gel was higher than that of the ointment, and there was no significant difference between 2 kinds of ointments. CONCLUSION: Gel is a better choice for percutaneous penetration of psoralen.
5.Comparative analysis of the bile culture and drug susceptibility test in the patients with community-acquired acute cholecystitis and the patients with community-acquired acute cholangitis in Chongqing during 2014 to 2016

Objective To understand whether it had differences in bile culture and drug susceptibility test between patients with community-acquired acute cholecystitis and patients with community-acquired acute cholangitis at present.Methods 169 patients with community-acquired acute cholecystitis and the 97 patients with community-acquired acute cholangitis in our department were selected prospectively,in which 16 patients suffered from both community-acquired acute cholecystitis and community-acquired acute cholangitis.Bile sample was extracted in operations and delivered to perform aerobic culture and drug sensitivity test.The bile samples coming from cholecyst were divided into group A,while the bile samples coming from bile duct were divided into group B.Results The positive rates of bile culture in group A and group B were separately 24.9% and 64.9%(P<0.05).The result of the sample coming from cholecyst and the result of the sample coming from bile duct were not the same in 4 patients of the 16 patients who suffered from both community-acquired acute cholecystitis and community-acquired acute cholangitis simultaneously.The main bacteria of acute cholecystitis included Escherichia coli(28.0%),Klebsiella pneumonia(24.0%)and Enterococcus faecium(16.0%).The main bacteria of acute cholangitis included Escherichia coli(39.7%),Klebsiella pneumonia(19.2%)and Enterobacter cloacae(12.3%).The antibiotics which were lowly resistant to gram negative bacteria in the two groups(A and B) included amikacin(0%/0%),ertapenem(0%/0%),imipenem(0%/7.0%)and piperacillin/tazobactam(0%/7.0%).The antibiotics which had lower resistant rates to gram positive bacteria in the two groups included tigecycline(0%/0%)and linezolid(0%/0%).The proportions of the multidrug-resistant strains in the two groups were separately 24.0% and 35.6%(P>0.05).Conclusion Some difference exist in the results of bile culture between patients with community-acquired acutecholecystitis and patients with community-acquired acute cholangitis,but the main bacteria of the both two types of biliary tract infection are Escherichia coli and Klebsiella pneumonia.The drug resistance is serious in the both infections,and the most sensitive antibiotics to gram negative bacteria include amikacin,ertapenem,imipenem,piperacillin/tazobactam,and the most sensitive antibiotics to gram positive bacteria include tigecycline,linezolid in the both infections.

[Objective]Supracondylar fracture of the humerus is the most common fracture around elbow in children.In recent years,with the progress of minimally invasive technique,closed reduction and percutaneous Kirschner wire fixation has been widely applied in clinics to reduce the complications and surgical trauma.The relative literature concerning supracondylar fracture of the humerus in children was reviewed to find out the latest progress in classification criteria,the manipulation and reference standard of closed reduction,the configuration and effects of different percutaneous Kirschner wire fixation,and the complications and prevention measures for closed reduction and percutaneous pinning fixation.
7.The changes of serum and urine guanidinoacetic acid level after cadmium chloride-induced renal injury in rabbits

Subcutaneous injection of 1.0 mg/kg cadmium chloride in 0.8% solution was given to rabbits weighing 1.7~2.5kg 3 times a week for 10 weeks.An equal volume of normal saline was injected to the rabbits of the control group.Blood specimen from the auricle vein and 24-hour-urine of every rabbit were collected once a week.Ten weeks later the renal cortex of the rabbits WES resected,processed routinely and examined under optical microscopy,The level of guanidinoacetic acid (GAA) of the serum and urine was determined with HPLC.It was found that the serum level of GAA increased after 3 weeks of cadmium administration while urinary excretion of GAA increased slightly in the first week and decreased significantly in the second week of cadmium injection and thereafter.
8.Study on ultrasonic circulation technique to extraction of triterpenoids from Ganoderma lucidum

ObjectTo study the application of ultrasonic circulation technique for the extraction of triterpenoids from Ganoderma lucidum (Leyss.ex Fr.) Karst. Methods On the basis of conventional method, the processing steps of ultrasonic circulation treatment were added. Results The comparative experiments showed that less amounts of various solvents and shorter extraction time were needed for ultrasonic circulation extraction, with the product ratio of about 40 percent higher than that of conventional method. Furthermore, a good consistency of the target product analyzed by HPLC was found between two different extraction methods. Conclusion The ultrasonic circulation technique has a potential application to the extraction of triterpenoids from G. lucidum.
9.The effect of silent homo sapiens eukaryotic translation elongation factor 1 alpha 2 gene on the growth of pancreatic cancer xenograft in nude mice

Objective To explore the effect and the possible mechanisms of silent homo sapiens eukaryotic translation elongation factor 1 alpha 2(EEF1A2)gene on the growth of pancreatic cancer cell in vivo.Methods The pancreatic cancer xenograft models in mice were established.The mice were equally divided into control group,negative control group and EEF1A2 group,which were injected with PBS,negative control siRNA and EEF1A2 siRNA into xenograft tumors respectively.The size and weight of tumors in each group were measured.The expression of EEF1A2 and PCNA in tumor tissue of each group was detected by immunohistochemistry.The cell apoptosis rate in tumor tissue of each group was determined by TUNEL.Results In xenograft nude mice models,since the 17th day of injection,the growth of tumor size in EEF1A2 group was obviously slower than that of negative control group and control group(all P<0.05).By the end of the treatment,the tumors were cut off and weighted.The weight of tumors in EEF1A2 group(0.27g± 0.06g)were significantly lower than those of control group and negative control group(0.39g± 0.08g and 0.43g± 0.07g,P<0.05).EEF1A2 mostly expressed in cytoplasm of pancreatic cancer cell.In negative control group and control group,the positive cells distributed densely and the positive rate was(72.58 ± 25.47)% and (76.75±23.19)% respectively.The distribution of positive cells in EEF1A2 group was scattered and the positive rate was(34.78±21.36)%,the difference was statisically significant(P<0.01).The expression of PCNA at protein level in EEF1A2 group was significantly lower those that of control group and negative control group(P< 0.01).The result of TUNEL test indicated that the cell apoptosis rate in EEF1A2 group was higher than those of control group and negative control group (P<0.01).Conclusions The EEF1A2 gene can be effectively silented in vivo,which significantly inhibits the growth of pancreatic cancer cell.It may be related with inhibition of cell proliferation and promotion cell apoptosis.
10.Clinical efficacy of herbs combined with semiconductor laser irradiation in the treatment of chronic eczema

Objective To observe the clinical efficacy of herbs by internal and external administration combined with semiconductor laser irradiation in the treatment of chronic eczema.Methods Eighty-six patients with chronic eczema were divided into the herb group and the laser group by the randomized digital table,43 patients in each group.The patients in the herb group received oral administration with Chinese herbs together with application of herbs on the affected foci.The patients in the laser group received the same treatment as in the herb group and meanwhile treated with additional semiconductor laser therapy.The clinical effects were evaluated according to their itch degree and skin lesions.Result The cure rate and total effective rate were both significantly higher in the laser group than those in the herb group(P<0.05).Conclusion Compared with herbal internal and external administration only,the clinical efficacy of herbal internal and external administration combined with semiconductor laser irradiation is more effective in the treatment of chronic eczema.
