1.Role of Tumor Necrosis Factor-Alpha Monoclonal Antibody in Cell Apoptosis after Combined Liver and Kidney Transplantation in Rats

Objective To investigate the potential role of tumor necrosis factor-alpha (TNF-?) in apoptosis after combined liver and kidney transplantation in rats. Methods Eighty rats which had combined liver and kidney transplantation were randomly paired, were divided into study group (n=20) and control group (n=20). 40 ml of 4 ℃ sodium chloride and anti-TNF-? monoclonal antibody (30 ml was infused from portal veins to donated livers and 10 ml from renal arteries to donated kidneys) were infused to the study group (0.1 mg/kg weight),and the same quantity of 4 ℃ sodium chloride was infused the control group. Venous blood was drew at different phases after the transplantations to detect the function of kidney and liver. The level of TNF-? and the cell apoptosis were detected in the transplanted tissues of liver and kidney by ELISA and terminal deoxynucleotidy transferase mediated dTUP-biotin nick-end labeling (TUNEL). 2.Analysis of 53 Patients with Sjogren' s Syndrome and Idiopathic Pulmonary Fibrosis

Objective To explore the occurrence, clinical feature and the associatrd factors of Sjogren' s syndrome(SS) complicated with Idiopathic Pulmonary Fibrosis(IPF) to build up knowledge for early diagnosis. Methods The clinical characteristics of 160 cases damitted SS patients collected from January 2002 to August 2008 were retrospectively investigated including RF, chest - X, ESR, blood -gas analysis,pulmonary function tests,chest HRCT,ANA and so on. Results The patients with IPF accounted for 33.12% of total SS patients, including 12 cases of male. These patients presented strongly positive ANA,strongly positive RF etc,especially in the male. Conclusion The occurrence of SS - IP was related to the disease' s activity. FHRCT and pulmonary function test should taken early in order to get early diagnosis and treatment.
3.Effect of Motor Imagery on Patients Accepted Radiotherapy after Operation for Cerebral Glioma

Objective To observe the effect of motor imagery therapy on patients accepted radiotherapy after operation for cerebral glioma. Methods 34 patients were randomly divided into control group (n=17) and observation group (n=17). Both groups accepted routine physiotherapy during radiotherapy, 45~60 min/d, 5 times a week for 4 weeks, while the observation group underwent motor imagery additionally, 15 min/d for 4 weeks. They were evaluated with Fugl-Meyer Assessment (FMA), Manual Muscle Test (MMT) and modified Barthel Index (MBI) before and after treatment., and with Karnofsky Performance Scale (KPS) 1 year later. Results There was no significantly difference in the scores of FMA, MMT and MBI between 2 groups before treatment (P>0.05). There was no significantly difference in the score of FMA in both groups before and after treatment (P>0.05). The score of MMT improved in the observation group after treatment (P<0.01). The scores of MBI improved in both groups after treatment (P<0.05). And there was no significantly difference in the D-value score of FMA, MMT and MBI between 2 groups (P>0.05). After 1 year follow-up, KPS showed that the number of survival patients and their quality of life were better in the observation group than in the control group (P<0.05). Conclusion Motor imagery therapy can improve the muscle force, activities of daily living and long-term quality of life in patients during radiotherapy after neurosurgery for cerebral glioma.
4.The cost-effectiveness analysis on standardized management of hypertension in community

Objective To discuss the effectiveness of standardized management of hypertension and its significance of economics.Methods Taking prospective,randomized and controlled trial design to screen hypertension in four communities of Changping Town.The patients in the four communities were randomly divided into an intervention group and a control group.The intervention group was taken the standardized management and the control group was taken conventional management.Results The treatment rate and control rate of the intervention group was higher than the control group.Necessary inputs of per capita administration costs for every 1% increase of treatment or control rate in the intervention group were less than the control groups by 1.08 and 1.36 Yuan respectively.The per capita administrative costs of intervention group were higher than control group,while the per capita comprehensive costs of intervention group were lower than control group.Compared with the control group,every 1 Yuan of per capita administrative costs increases,the comprehensive costs would be reduced 7.58 Yuan in the intervention group.Conclusion Standardized management of hypertension in community can significantly improve the prevention and treatment effects.Moreover,it has good cost-effective value and saving social resources.

Objective To explore the role of cell cycle regulator in tuberculosis with lung cancer.Methods The ex- pression of p21~(WAF1)gene and p53 protein in 101 cases of tuberculosis with lung cancer have been detected by immunohis- toehemical method.Results total positive staining rate of p21~(WAF1)gene and p53 protein was 42.9% and 64.1%; The expression of both was correlated with histological classification and differentiation of carcinoma.Conclusion Detection of p21W AF1 gene and p53 protein expression may have significant value in the prognosis of tuberculosis with lung cancer

AIM:To clarify the relationship between serum copper/zinc ratio(CZR) and histopathological classification as well as prognosis of Hodgkin's disease (HD). METHODS:The serum copper level(SCL),serum zinc level(SZL)and CZR were determined by atomic absorption spectrophotometry(AAS)in 135 patients with HD and 100 healthy individuals.RESULTS:The results showed that SCL and CZR were higher and SZL was lower in patients with HD than in healthy individuals (P<0.01). SCL and CZR were significantly higher in the patients with lymphocyte depleted type(LD)than mixed cellulal type(MC), SCL and CZR were higher in MC type than nodular sclerosis type(NS)and lymphocyte predominance type (LP)(P<0.01). It was also found that with the development of the disease, the SCL and CZR elevated but SZL dropped. And with the alleviation of the disease, the SCL and CZR rose while SZL dropped. While the illness is relieved and the SCL and CZR markedly dropped and SCZ rose. SCL and CZR were strongly correlated with histopathologic type and clinical stage of tumor.CONCLUSION:SCL, SZL and CZR are parameters useful for clarifying histopathological classification, clinical stage and prognosis of HD.

Objective To study the feasibility and safety of performing nephrectomy together with the removal of complicated inferior vena cava tumor thrombus under profound hypothermia and arrested circulation. Methods After made the median thoraco-abdominal incision, the exploration of the abdominal organs was done. The right kidney, inferior vena cava and renal pedicle were well exposed then. After the whole body heparinization, cannulas were put into ascending aorta, superior vena cava, aortic root and right superior pulmonary vein. The body temperature was reduced to 20℃ with cardiopulmonary bypass unit and the extracorporeal circulation was stopped then. Cut open the inferior vena cava at vena renalis dextra ingress and the F16 urinary catheter was inserted into atrum dextra through inferior vena cava and inflated. The tumor thrombus was pulled out and the right kidney was removed. The inferior vena cava incision was sutured to close and the extracorporeal circulation was resumed and patient was re-warmed.Results The operation time was 330 min and the extracorporeal circulation time was 90 min, while the profound hypothermia with circulatory arrest time was 20 min. The estimated blood loss during operation was 400 ml and 6 unit red cells and 600 ml blood plasm were transfused. The patient was awaked 2.5 h after the operation, food intake resumed 4 days after operation and the patient was discharged on day 10 post-operatively. After 6 months'follow-up, there were no local recurrence and metastasis occurred. Conclusion The technique of profound hypothermia and circulation arrest could improve the safety and efficacy in the treatment of renal cell carcinoma with suprahepatic (level Ⅲ) caval tumor thrombus.
10.Epidemiological investigation of back pain and spondyloarthritis in Shougang resident communities

Objective To investigate the epidemiological features of back pain, spondyloarthritis (SPA) and ankylosing spondylitis (AS) in Beijing Shougang district. Methods Set up Chinese version of questionnaire about incidence of spondyloarthro-pathy. Employees and retired ones were drawn out from subfactory units by non-randomized sampling. 15 357 subjects were investigated, of which 12 125 questionnaires were taken. Suspected cases were then screened with sacroiliac joint X ray and HLA-B27 testing. 2009 assessment in ankylosing spondylitis (ASAS) criteria were used for diagnosing SpA. Results Back pain is common with total incidence of 42.7%, and the most common pattern is mechanical pain. The incidence of SpA is 0.58% and that of AS is 0.36%, while only 28.9% AS patients had been diagnosed before and received treatment. Conclusion The AS incidence in Shougang district is similar with the epidemiological data got from other districts of China. And knowledge of SpA and AS is needed in China.
