Objective To investigate the clinical efficacy of optimizing perioperative nursing process and applying enhanced recovery after surgery (ERAS) model in the treatment of post-traumatic bone infection of lower limb using the membrane induction technique.Methods A retrospective case control study was conducted to analyze the clinical data of 225 patients with post-traumatic bone infection of lower limb admitted to the First Affiliated Hospital of Army Medical University from January 2015 to December 2016.There were 184 males and 41 females,aged 19-63 years,with an average age of 41.8 years.There were 123 patients with tibia infection,80 with femur infection and 22 with fibula infection.A total of 125 patients received process optimization plus ERAS (test group),and 100 patients received traditional treatment (control group).Visual analogue scale (VAS) before operation and 1,3 and 7 days after operation,incidence of deep venous thrombosis (DVT),bedridden time,sleep time,ambulation distance,length of stay in hospital and patient satisfaction were compared between the two groups.Results The VAS scores in the test group were (4.5 ± 0.7) points,(3.7 ± 0.6) points,and (2.7 ± 0.5) points at 1,3 and 7days after operation,which were lower than those in the control group (5.7 ± 0.8) points,(5.5 ± 0.8) points,(3.6 ± 0.5) points (P ＜ 0.05 or 0.01).There was no significant difference in VAS 10 days after operation between the two groups (P ＞ 0.05).The incidence of DVT in test group was 16.0％,which was significantly lower than that in the control group 39.0％ (P ＜ 0.01).The bedridden time of the test group was (3.5 ± 0.1) days,and that of the control group was (7.0 ± 0.4) days (P ＜0.05).The sleep time at 1,2,and 3 days after operation as well as the ambulation distance at 3,4,and 5 days after operation in the test group were significantly longer than those in the control group (P ＜0.05).The hospitalization time was (13.4 ± 1.2) days in the test group and (17.6 ± 1.9) days in the control group (P ＜ 0.05).The satisfaction rate was 99.2％ in the test group and 87.0％ in the control group (P ＜ 0.05).Conclusion During treatment of post-traumatic bone infection of lower limb using the membrane induction technique,nursing process optimization plus ERAS intervention program can effectively alleviate postoperative pain,improve daily activity abilities,shorten hospitalization time,improve patient satisfaction and better promote patient rehabilitation.

Objective Through the analysis of the data such as the publication of the paper and the scientific research project of urumqi disease prevention and control center from 2008 to 2017,the research ability level of the center is comprehensively evaluated.Methods In this paper,the entropy weight method is used to process and analyze the data.According to the results of information entropy and weight,the comprehensive scores of each index project are obtained and sorted.Results The information entropy of the paper is the smallest,the weight is the largest,the information entropy of the scientific progress award and the result identification is the largest,and the weight is the smallest.The top four years in the comprehensive score are:2016,2017,2014 and 2015;The ranking year is 2012,2010,2013 and 2011 respectively.The last two years are 2008 and 2009.Conclusions Urumqi center for disease control and prevention,2008-2017,scientific research and comprehensive ability level,although each year a little ups and downs,but overall present a curve upward trend,have a qualitative leap in the area of scientific research,achieved the zero breakthrough,and formed a "scientific promoting business,business support scientific research "as the new situation.

Hyaluronic acid is a linear acid mucopolysaccharide composed of repeating disaccharide glucuronic acid and N-acetyl-glucosamine units,which is widely used in medicine,cosmetics,food and other fields.Traditional studies have made significant achievements in improving the production of hyaluronic acid by optimizing the fermentation parameters,but have reached the upper limit,and the natural strains have the increasing disadvantages of high cost of fermentation medium and pathogenicity.With the rapid development of molecular biology technology and the continuous research on the genes related to hyaluronic acid synthesis,the research focus has gradually shifted to the use of genetic engineering technology to construct high yield,safe and specific molecular weight hyaluronan genetically engineered strains.Here the strategies and research progress of genetic engineering for the production of hyaluronic acid were reviewed.

Objective To compare different incidence between lateral position and horizontal position in the Burr holes irrigation drainage(BHID).The BHID is wildly known as the most frequently used neurosurgical treatment of chronic subdural hematoma(CSDH).Methods A total of 80 cases were selected which were undergone BHID during January 2010 and December 2015,and the incidence of intracranial pneumatocele was compared after operation.All of cases came from the Second People′s Hospital of Tai Zhou City.Results There were 5 cases(out of 50,10%)having moderate intracranial pneumocephalus but no numerous intracranial pneumocephalus in lateral position,while 26 cases(out of 30 cases,87%)in horizontal position emerging moderate amount or more intracranial pneumatocele.And there were 8 cases'numerous pneumatocele in the horizontal postion group.There was no tension pneumothorax emerging in both groups.Both groups had statistical difference (χ2 =46.43,P <0.01.Conclusion BHID in lateral postion is a simple and effective method for CSDH,and it is conducive to reduce postoperative intracranial pneumatosis.That is worthy of application and promotion.

Moreover, the comparative study was done between shelter CT and common CT in hospitals. In result, in order to meet maneuverability application in the field, vehicle shelter CT was restrictive by the field conditions, traffic impacts and running requirement. The application of vehicle shelter CT was affected by the factors, such as mechanical stabilization, moving precision, power fluctuations and variations of temperature and humidity, etc. The results were helpful to improve the clinical quality of vehicle shelter CT and made a base for the quality control study in the future.

Objective To analyze high alumina deformation Micro-CT findings of bone in patients with skeletal fluorosis in Shuicheng Guizhou Province.Methods Bone deformation children,youth and middle-aged patients with fluorosis in Goumi and Zhichang Townships Shuicheng County,coal-burning pollution endemic fluorosis areas,were selected as case group,and non-bone deformation children,youth,and children from non-fluorosis endemic areas as controls.Tibia and the anterior superior iliac spine tissue were obtained through orthopedic surgery and etiology examination,and resin embedded without decalcification.Resin-embedded bone tissue was scanned using micro-CT; relevant parameters were analyzed with ABA special bone analysis software INVEON Research Workplace and three dimensional reconstruction processing software Micview.Results ①The anterior superior iliac spine cancellous bone:compared between bone deformation children and bone non-deformation children in the diseased areas,there was an increasing tendency of the following items:relative volume of trabecular bone(0.337％ vs 0.229％),absolute thickness (μm:139 vs 133),quantities within a unit length (number/mm:2.44 vs 1.72),density woven degree of trabecular bone(number/mm:2.22 vs 1.54) and bone mineral density(mg/cm3:1 033 vs 918),while relative bone area of trabecular bone(mm2/mm3:14.5 vs 15.1) and space pitch (μm:0.274 vs 0.567) declined.Compared between bone deformation youth and bone non-deformation youth in the diseased areas,relative volume of trabecular bone was lower(0.217％ vs 0.437％),relative area increased (mm2/mm3:16.9 vs 11.6),absolute thickness reduced(μm:118 vs 172),trabecular number reduced (number/mm:1.83 vs 2.54),and space pitch increased (μm:0.427 vs 0.222),but density woven degree of trabecular bone increased (number/mm:4.61 vs 1.54),bone mineral density decreased(mg/cm3:977 vs 1 108),osteopenia,osteoporosis,bone mineral decreased,and an increase in the number of trabeculae crossing number.② Tibia bone tissue:compared between bone deformation children and bone non-deformation children in the diseased areas,relative volume of tibia trabecular bone increased(0.435％ vs 0.206％),relative volume of trabecular bone (mm2/mm3:12.3vs 12.4),and thickness (μm:188 vs 161) not changed obviously,trabecular number increased (number/mm:2.43 vs 1.28),space pitch reduced(μm:0.238 vs 0.621),density woven degree of trabecular bone decreased(number/mm:2.40 vs 3.48),bone mineral density increased(mg/cm3:1 047 vs 952),in general presented trabecular thickening,increased number and increased bone mineral.Compared between middle-aged patients with fluorosis in the diseased areas and children in non-fluorosis endemic areas,relative volume of trabecular bone (0.346％ vs 0.206％) and area (mm2/mm3:13.8 vs 12.4) increased,thickness of the trabecular bone reduced (μm:144 vs 161),trabecular number increased (number/mm:1.98 vs 1.28),space pitch decreased (μm:0.318 vs 0.621),and density woven degree of trabecular bone decreased (number/mm:2.60 vs 3.48).Conclusions The results of trabecular bone microstructure and bone mineral density have showed that the combined effects of aluminum and fluorine on human bone tissue at different developmental stages are different.High aluminum and fluorine load before the sexual development of children for trabecular bone thick dense,shows an increasing in bone mass and bone mineral deposition of bone sclerosis image.Bone deformation youth shows osteopenia osteoporosis and bone mineral deposition is reduced.Bone volume is slightly increased,the number of trabecular bone is increasing,trabecular structure is fine in middle-aged patients with skeletal fluorosis.

Objective To investigate the methods to cover the donor site of wrap around flap from toe. Methods In 34 cases, 8 thumbs reconstructed with wrap around flap from great toe, 8 thumbs and 14 fingers reconstructed with half nail flap from fibular side of second toe, and 4 fingers reconstructed with dorsal nail flap from second toe. The donor site were covered with pedicle flap (first metatarsal dorsal flap, second toe digital flap, neighbor flap from dorsal of second toe, and lateral tarsus flap) or free flap (medical plantar flap, fibular artery flap, and lateral tarsus flap). Results Twelve cases were followed up after 2 months, normal gait were found, 6 cases were followed up after 4 months, (4 cases for first metatarsal dorsal flap, one for fibular artery flap and one for internal volar flap). 2 cases recovered defend sensibility and showed a bit dark, the cases which reconstructed with medical plantar flap and second toe digital flap had nearly normal sensibility. The outward appearance of the donor site were chubbiness, flexible, and no complaint were found. Conclusion Covered the donor site of wrap around flap from toe with these flaps can avoid complications like pain and ulcerate, and have a satisfactory appearance.

Objective To improve the outward appearanee in toe-to-hand transfer,a reshaped procedure of second toe was studied. Methods In reconstructing a finger with second toe transfer,a piece of flap from the fibular side of the second toe,or/and a H-shaped flap from the tibial of great toe were designed and transferred to the volar side in order to obliterate the constriction below the toe pulp. Results In 17 patients,22 fingers reconstructed with these procedure,ouly one rotation flap necrosis partly.The appearance of the newly reconstructed thumbs and fingers were improved,the pulp of reconstructed finger was similar to a natural finger, the narrow neck of second toe was well-stacked,and the interpbalangeal joint was straighten. Conclusion Reconstructed finger with this reshaped second toe is similar to a normal finger.

Objective To apply wireless net technology in our military's medical equipment more effectually. Methods The author analyzed the existing wireless net technology and its domain of application to find out the application actuality of wireless net in our military's medical equipment. Results The wireless net technology is widely used in the medical equipment and has good application prospects. Conclusion The author will continue to deepen the application research in wireless network technology in hope to improve the level of informatized military medical equipment.
