Objective:To evaluate the expression and relationship of Ki 67 and microvessel density (MVD) in transitional cell carcinoma of bladder (TCC).Methods:With anti Ki 67 McAb and anti FⅧRAgMcAb,immunohistochemistry was used to examine the expression of Ki 67 and MVD in 65 cases of TCC,and 8 cases of normal bladder tissue as controls.Results:It was shown that the expression of Ki 67 in TCC was positively correlated to tumor MVD.These two indices increased with the escalation of the tumor grade ( P
2.Percutaneous vertebroplasty with low-dose bone cement for acute osteoporotic vertebral compression fractures

BACKGROUND:Percutaneous vertebroplasty technique has become an effective means for clinical treatment of spinal osteoporotic fractures, but there is the risk of bone cement leakage.
OBJECTIVE:To explore the clinical effect of percutaneous vertebroplasty with low-dose bone cement for acute osteoporotic vertebral compression fractures.
METHODS:From September 2008 to February 2011, 32 patients with osteoporotic compression fractures were treated by percutaneous vertebroplasty. According to the dose of bone cement, the patients were divided into low dose group (2-4 mL) and routine dose group (4-6 mL). In addition, another patients who were hospitalized over the same period for acute osteoporotic compression and could not receive vertebroplasty due to urgical contraindication served as control group.
RESULTS AND CONCLUSION:The pain relief and vertebral height restoration rate of the low dose group and routine dose group were significantly better than the control group (P<0.05). The bone cement leakage rate and last fol ow-up incidence of adjacent segment vertebral fractures of the low dose group were also significantly lower than those in the routine dose group (P<0.05). Vertebroplasty with modified low dose bone cement can achieve satisfactory clinical effects, and effectively reduce the leakage of bone cement and incidence of adjacent segment secondary fractures.
3.Clinical study of low-dose bone cement injection for treating osteoporotic compression fractures

Objective To explore the clinical effect of low-dose bone cement injection in percutaneous vertebroplasty for treating osteoporotic vertebral compression fractures .Methods 41 cases of osteoporotic vertebral compression fractures from February 2009 to February 2012 were treated with percutaneous vertebroplasty .The patients were divided into the low-dose group and the conventional dose group according to the amount of bone cement injection .The postoperative VAS score ,cement leakage rate ,verte-bral height restoration degree and incidence rate of adjacent segment fracture were observed ,evaluated and compared .Results The follow-up period ranged from 3 months to 15 months(average 11 .2 months) .The postoperative pain relief effect in the conventional dose group and the low dose group was similar .The vertebral height restoration rate of the conventional dose group was superior to that of the low-dose group .In the aspects of the bone cement leakage rate and adjacent segment vertebral secondary fracture ,the low-dose group was superior to the conventional dose group .Conclusion In the procedure of percutaneous vertebroplasty ,applying the low-dose bone cement injection can reach the satisfactory clinical effect ,at same time can effectively reduce the complication oc-currence rate of bone cement leakage and adjacent segments secondary fractures .

BACKGROUND: The dynamic changes of pneumocyte apoptosis and aspartate-specific cysteine proteases-3 (caspase-3) expression in lung tissue of rats during the process of lung ischemia/reperfusion (I/R) injury and the possible action mechanisms remain unclear.OBJECTIVE: This study was to observe the dynamic changes of pneumocyte apoptosis and caspase-3 expression in the rat lung tissue during the process of lung I/R injury, and to analyze the role of pneumocyte apoptosis and the possible action mechanism.DESIGN: A randomized controlled animal experiment.SETTING: Emergency Center, First Hospital, Nanjing Medical University.MATERIALS: This study was carried out in the Animal Laboratory of the First Hospital of Nanjing Medcial University and Nanjing Center for Radioimmunity between April 2006 and September 2006. Twenty-eight male healthy SD rats of clean grade, with body weight of 250 to 350 g, aged 49 to 76 days, were provided by the Experimental Animal Center of Nanjing Medical University. The involved rats were randomized into experimental group and control group, with 14 rats in each.METHODS: ①Experimental intervention: Rats in the experimental group were created into models of lung I/R injury according to the method of Eppinger et al. They were occluded for 45 minutes at the porta of lung (no systolic and diastolic reactions in lung tissue being considered as successful occlusion), and then they were reperfused (recovery of systolic and diastolic function being considered as successful reperfusion); After that, lung tissues were harvested at 3 and 6 hours after lung I/R injury, 7 rats at each time point. Each rat in the control group was subjected to a thoracotony only, but lung tissues were isolated at the same time point by the same method. ②Experimental evaluation: Apoptotic cells in the lung tissue were detected with a flow cytometer by Annexin-V-PI staining, and apoptosis rate was calculated. Caspase-3 expression in the lung tissue was observed by immunohistochemical method and image analysis. Wet to dry weight ratio(W/D) of lung tissue of rats in the two groups was calculated; the number of injured pulmonary alveoli at I/R 3 hours/that at I/R 6 hours was calculated for quantitative evaluation of injured lung tissue; Patho-morphological changes of lung tissue were observed by haematoxylin & eosin staining under an optical microscope.MAIN OUTCOME MEASURES: ①Pneumocyte apoptosis rate and caspase-3 expression in the lung tissue. ②W/D of lung tissue and quantitative evaluation of injured lung tissue. ③Patho-morphological changes of lung tissue.RESULTS: Twenty-eight rats were involved in the final analysis, without deletion. ①Pneumocyte apoptosis rates in the experimental group at I/R 3 and 6 hours were significantly increased as compared with control group (P<0.01). In the experimental group, pneumocyte apoptosis rate was decreased a little at I/R 6 hours than at I/R 3 hours (P<0.05). ②Caspase-3 expression in the lung tissue of rats of experimental group reached its top at I/R 3 hours, and was decreased a little at I/R 6 hours. At each time point, caspase-3 expression in the experimental group was increased as compared with control group (P<0.01). ③In the experimental group, the number of injured pulmonary alveoli at I/R 3 hours/that at I/R 6 hours and W/D ratios of lung tissues were significantly increased as compared with control group (P<0.01). In the experimental group, two ratios at I/R 6 hours were higher than those at I/R 3 hours (P<0.05).④In the experimental group, the structure of pulmonary alveoli was destructed, collapsed and disappeared; lots of inflammatory cell infiltration was found; Patho-morphological changes of injured lung tissue at I/R 6 hours were severer than those at I/R 3 hours. No obvious changes were found in the control group.CONCLUSION: At the early stage of lung I/R injury, the alteration of caspase-3 maybe activate pneumocyte apoptosis and induce the apoptosis of lung tissue, and thereby leads to lung injury.
6.Effect of combined application of recombinant human endostatin and paclitaxel on the proliferation and invasion of gastric cancer cell

Objective Antiangiogenesis therapy has been shown to prolong survival for patients with malignant tumor .However the present study has not been observed the clinical benefit of antiangiogenesis therapy combination with chemotherapy treated with gastric canc-er.Human recombinant vascular endothelial inhibition (endostar) as a multi-targeted anti-angiogenesis drug, the mechanism is different from other Antiangiogenesis drugs.It can block different pathways of signal transduction to inhibit angiogenesis .This study aimed to observe the effect of combined application of endostar and paclitaxel on biological behavior of gastric cancer cell lines . Methods MMT assay and Tr-answell invasion assay were respectively used to examine the inhibition rate of cell growth and invasion ability when cells were treated with va-rious concentrations of endostar and paclitaxel alone or in combination.The protein expressions of VEGF,MMP-2 and MMP-9 were examined by Western blot. Results Endostar or paclitaxel effectively inhibited the growth of MGC803 cells and the in vitro invasion of MGC803 cells in a concentration-dependent manner.The proliferation and invasion ability of combined treatment with endostar and paclitaxel was significantly lower than that of endostar or paclitaxel alone (P<0.05).Compared with con-trol group, the VEGF,MMP-2 and MMP-9 protein expressions were de-creased in experimental groups ( P <0.05).Compared with paclitaxel group, the VEGF, MMP-2 and MMP-9 protein expressions were relatively reduced in combination groups (P<0.05). Conclusion Endostar combined with paclitaxel can suppress the growth and invasion of MGC803 cells, and the decreasing VEGF , MMP-2 and MMP-9 expressions may be involved in the mechanism .
7.Basic principle and practical application of Elektra linac control software item Part and value ( IPV)

Objective To illustrate the basic operational principle of the control software item Part and value ( IPV) of Elekta digital medical linac and specifically explain that the control software monitors and controls the subsystems in a real-time manner by using three types of items and Parts. Methods Firstly, the composition of the control system of Elekta digital medical linac was introduced. Then, the definition and category of the items and Parts of the control software and the basic principle of different types of items were illustrated. Finally, the application of control software items and Parts in real-time monitoring and controlling the operation of subsystems was evaluated by analyzing the motion control of high-power ( HP) phase shifter. Results By analyzing the principle and citing a classical example to specifically illustrate the operational principle and application of control software IPVs, the users, especially the maintenance personnel, could master relevant theoretical knowledge, rapidly and accurately find the operational parameters during daily use and maintenance, besides analyze and resolve the problems based on the control principle of relevant subsystem. Conclusion Users, especially the maintenance personnel who master the working principle of these subprojects and their project sub-items can quickly and accurately check, calibrate the operating parameters via the control software, resolve equipment failure and avoid accidents caused by inappropriate operation in the daily use or maintenance.
8.A study of the pepsin in the sputum for diagnosis and treatment evaluation of laryngopharyngeal reflux.

OBJECTIVE:
To study the value of the pepsin in the sputum for diagnosing and evaluating the effectiveness of treatment of laryngopharyngeal reflux.
METHOD:
Thirty-six patients with the symptoms of dry pharynx, globus pharyngeus, excessive throat clearing, chronic cough and so on were divided into laryngopharyngeal reflux group and chronic laryngitis group by the results of therapeutic trial taking proton pump inhibitors for 3 months. The estimation of the reflux symptom index (RSI), the reflux finding score (RFS) and the detection of pepsin in the sputum were done before and after the treatment. The difference between two groups and the value of the pepsin were analyzed.
RESULT:
There were significant decreasing in RSI, RFS and pepsin level (P < 0.01) after the treatment in all patients. There were statistical differences between the laryngopharyngeal reflux group and the chronic laryngitis group in the changes of RSI and pepsin level (P < 0.01).
CONCLUSION
Pepsin level in the sputum might be used as a objective, effective method for diagnosing and evaluating the effectiveness in laryngopharyngeal reflux.

Non-small cell lung cancer (NSCLC) is the most common pathological type of lung cancer. The systemic antitumor therapy of advanced NSCLC has undergone renovations of chemotherapy, targeted therapy and immunotherapy, which results in greatly improved survival for patients with advanced NSCLC. Immune checkpoint inhibitors (ICIs), especially targeting programmed cell death protein 1 (PD-1)/programmed death-ligand 1 (PD-L1), has changed the treatment paradigm of NSCLC. ICIs have become the standard treatment for advanced NSCLC without epidermal growth factor receptor(EGFR) mutation or anaplastic lymphomakinase(ALK) translocation in the first- or second-line setting, and for locally advanced NSCLC following concurrent radiotherapy and chemotherapy. ICIs are also promising in adjuvant/neoadjuvant therapy. More and more ICIs have been approved domestically for the treatment of NSCLC. Led by the NSCLC expert committee of Chinese Society of Clinical Oncology (CSCO), this consensus was developed and updated based on thoroughly reviewing domestic and foreign literatures, clinical trial data, systematic reviews, experts' discussion and the consensus(2019 version). This consensus will aid domestic clinicians in the treatment of NSCLC with ICIs.
