Objective To establish systematic and integrated quality standard for Citrus medica. Methods To make clear the ancient applicated history and the present situation of the research of Citrus medica. The breed resources citrus medica and its medicinal present situation were investigated, and some chemical constituents from citrus medica were extracted and separated. Qualitative and quantitative methods were employed on citrus medica by HPLC methods. Results Changes in the distribution of the two variety resources of citrus medica were discovered, and six components were separated and identificated. The identificated characteristics of the two variety was found in the HPLC fingerprint for the first time. Some methods such as character identification, thin-layer identification, check of water and ash, determination of useful ingredients and the content of extracturm were established to evaluate quality standard for citrus medica. Conclusion The research on quality control standards of the citrus media is systematic and integrated, and these methods are scientific and accurate, which are suggested to the country to adopt.

Objective To investigate the evaluation of the employing hospital about nursing undergraduate students,to understand the actual working capacity of graduates and explore the quality requirement and individual needs of nuring undergraduates according to the findings,to improve the quality of nursing undergraduates and personnel training specifications. Methods The self-evaluation questionnaires on nursing undergraduates was used to investigate eight hospitals which accepted three or more graduates of our college by letter investigation. Results The employing hospital had the highest satisfaction degree on the moral character and working attitude, respectively 98.1%and 96.2%;the satisfaction degree of management and research capability was the lowest, respectively 81.0%and 80.0%. Conclusions Overall assessment of the quality of graduates is better, the graduates' clinical competence, interpersonal relations and psychological quality is satisfactory, nursing skills and adaptability should be improved;foreign language, scientific research, administration and management capacity is rehtively weak.In the future, the college should adapt to the reeds of employers in the teaching process, strengthening the training of practical ability and adaptability, using appropriate methods to enhance research and foreign language skills, appropriately adjust training goal of undergraduates.

Objective To explore the application of mini-clinical evaluation exercise (Mini-CEX) in training the diagnostic thinking ability for clinical interns. Methods 83 interns who came to the First People's Hospital of Lianyungang for clinical practice in June 2015 were divided into the experimental group (45 interns) and the control group (38 interns) according to their schools. The two groups received the same clinical teaching according to internship program of five-year clinical medicine, while the experimental group had to take a weekly Mini-CEX training in chnical sections. The diagnostic thinking ability of the two groups was measured by the Diagnostic Thinking Inventory in Chinese version at the beginning and the end of the internship respectively.SPSS 21.0 was applied to processing the data.Measurement data were expressed as mean plus or minus standard deviation, and t test was used in the comparison between groups. Results At the end of the internship, the total score of diagnostic thinking, the score of knowledge structure in memory and the score of flexibility in thinking of the experimental group were higher than scores at the beginning statistically, and the experimental group had a statistically higher score than the control groups in knowledge structure in memory [(69.36 ± 4.28) vs.(66.95 ± 4.78),P=0.018].Conclusion Mini-CEX training can enhance the clinical diagnostic thinking ability of interns by improving their knowledge structure in memory.

BACKGROUND:The etiological factor for rheumatoid arthritis remains unclear, but the effects of nuclear factor-κB on the onset of rheumatoid arthritis have been gradual y paid great attention by rheumatologists.
OBJECTIVE:By using the RNA interference (RNAi) technique to block the signal pathway of nuclear factor-κB
mRNA of human rheumatoid arthritis synovial cells, this study explored its application prospect in the treatment of rheumatoid arthritis.
METHODS:The synovial cells were isolated, digested, and cultured for further use. In accordance with the
design principle of smal interfering RNA (siRNA), target sequences of siRNA of nuclear factor-κB were identified, and siRNA expression vector of nuclear factor-κB was synthesized and constructed. The four pGenesil-1/nuclear factor-κB siRNA expression vectors were transfected into the first passage of synovial cells that wel grew. Blank and negative control groups were set. cells at 12, 24, 48, 72 hours, 5 and 7 days after transfection were col ected, and RNA was extracted. Intracellular nuclear factor-κB mRNA expression levels were measured, and siRNA plasmid vector that could effectively inhibit nuclear factor-κB mRNA expression was screened out.
RESULTS AND CONCLUSION:Nuclear factor-κB highly expressed in synovial cells after human rheumatoid arthritis. 3#pGenesil-1/nuclear factor-κB apparently suppressed nuclear factor-κB mRNA expression in synovial cells with human rheumatoid arthritis. RNAi technique blocked nuclear factor-κB mRNA expression. Therefore, the block of nuclear factor-κB signal pathway might be a good target for rheumatoid arthritis gene therapy.

A 74-year-old man presented with yellowish red nodules and plaques on the trunk for more than one year.Physical examination showed more than 20 scattered, irregularly shaped,well-demarcated,yellowish red nodules and plaques measuring 0.5-15 cm in diameter on the trunk. The plaques on the right lower abdomen showed central ulceration and atrophy with scar formation.Plasma protein electrophoresis revealed that the levels of immunoglobulin G (IgG) and kappa light chain were 18.3 g/L and 20.70 g/L respectively.Histopathologically, large necrobiotic areas and granuloma were observed alternately in the dermis and subcutis. There were numerous Touton giant cells, foreign body giant cells and foamy histiocytes in the granuloma, and many cholesterol clefts were found in the center of necrobiotic areas with the formation of lymphoid follicles in some regions. Both eyes of the patient were involved to different degrees. He was diagnosed as necrobiotic xanthogranuloma with IgG kappa paraproteinemia and eve involvement.

Objective:To clarify the effect of voltage-gated proton channel 1 (Hv1) on the migration and invasion of breast cancer cells. Methods:The protein expression of Hv1 was detected in human breast cancer cell lines with different metastatic abilities. SiRNA technique was used to down-regulate the expression of Hvl in breast cancer MDA-MB-231 cells. Scratch and matrigel invasion methods were used to observe the effect of Hvl on the migration and invasion of breast cancer cells, and the relevant molecular mechanism was explored. Results:Hv1 was highly expressed in the highly metastatic breast cancer cell line MDA-MB-231. Hvl was more highly expressed in MDA-MB-231 cells with higher metastatic ability. The SiRNA sequence target at Hvl inhibited Hvl expression. Scratch and matrigel invasion experiments showed that the migration and invasion of MDA-MB-231 cells were significantly attenuated when Hv1 was knocked down by siRNA targeting Hv1. Zymography experiment on matrix metalloproteinase indicated that the enzyme activities of MMP-2 markedly decreased. Conclusion:Hv1 promoted the migration and invasion ability of breast cancer cells.
