1.Quality criteria of xiaozhining tablet

Objective: To establish the quality criteria for xiaozhining tablet. Methods: TLC method was applicated to determine and differentiate the Scutellaria , the Radix salviae miltiorrhizae, the Rheum, the Leonurus of the tablet. HPLC method was used for detecting the content of emodin in xiaozhining Tablet. Results: Four main components of the tablet was determined well. The regression equation by HPLC was A=2.43?10 7C+6.06?10 4(r=0.999 7). Curcumin had good linear relation within the range of 0.5~0.05 mg/ml. The average recovery rate was 96.70%(RSD=1.06%). The average value of emodin was 0.021 4 mg. Conclusion: The method of determination is reliable. It can be used as the basis for the quality control of xiaozhining Tablet.
2.The expression and significance of Survivin, Smad4/dpc4, APC gene in large intestinal adenoma and adenocarcinoma

Objective To observe the solo-allied expression of Survivin, Smad4/dpc4 and APC gene, and study the relationship between the three genes and biological behavior of colorectal cancer. Methods The expressions of Survivin, Smad4/dpc4 and APC gene were detected by SP immunohistochem-istry among the cases from 40 eu-intestine ,80 large intestinal adenoma and 80 CRC. Results The positive rate of survivin protein in the eu-intestine, the large intestinal adenoma and CRC was 0,35.0% ,75.0% , respectively. The positive rate of Smad4/dpc4 protein was 100% , 95. 0% , 78. 8% , and the positive rate of APC protein was 100% ,80.0% ,45.0% , respectively. In the eu-intestine, APC + Smad4/dpc4 Z- expressed in 40 cases, with the incidence of 100%. In the large intestinal adenoma, APC + Smad4 expressed in 38 patients, with the incidence of 47. 5% , and the three genes expressed in 29 patients with the incidence of 23. 8%. In the CRC, two genes expressed in 57 patients, with the incidence of 71. 3% , and three genes expressed in 28 patients, with the incidence of 35.0%. Conclusion The detection of survivin was a new indicator in early diagnosis of CRC. It was significant in the diagnosis of CRC that energetic search for the positive rate level realm of survivin. The catastrophe or absence of Smad4/dpc4 gene not only induced the genesis of CRC but also encouraged its growth. Smad4/dpc4 was ant- carcinomatous gene intimately correlated to the CRC. Detection of APC gene had very important significance, it could be helpful to conduct the research of tumorous aetiology, nosogenesis and early diagnosis. The allied detection of Survivin, Smad4/dpc4, APC gene played a very important role in early diagnosing and healing the CRC.
3.Acute adverse effects of radiotherapy on HIV-positive patients with malignancy

Objective To observe and evaluate acute toxicities in a series of human immunodeficiency virus (HIV)-positive cancer patients receiving radiation therapy.Methods The study retrospectively reviewed the acute radiation reaction of radiation therapy of 14 HIV seropositive patients diagnosed with carcinoma between Feb 2008 and Dec 2013 at the Yunnan Tumor Hospital during the radiotherapy period and 1 month following treatment.Acute adverse effects were classified according to the site of radiation therapy and analyzed using the Common Terminology Criteria for Adverse Events (CTCAE) version 3.0.Results Seven patients experienced interruptions or delays in treatment,and 2 stopped treatment entirely.The most common acute adverse effects were skin reactions and mucous membrane reactions,including dermatitis,stomatitis or diarrhea.Eight patients had grade 3 acute adverse effects,including 6 patients with grade 3 skin reactions and 2 patients with grade 3 mucosa reactions.Conclusions Radiotherapy is an effective treatment for HIV positive patients with tumors,however it frequently induced severe acute radiation responses.
4. Analysis of CT signs in 85 cases with peripheral lung cancer

Objective:
To analyze the CT signs of peripheral lung cancer.
Methods:
From July 2012 to June 2017, the clinical data of 85 patients with peripheral lung cancer examined by CT in the People's Hospital of Longyou County were retrospectively analyzed.CT findings were analyzed and compared with the results of postoperative pathological diagnosis.
Results:
CT examination showed that the lesions were located in the posterior segment of the right upper lung, the dorsal segment of the double lower lobe, the posterior segment of the left superior pulmonary apex, the anterior segment of the bilateral upper lobe and the posterior basal segment of the right lower lobe in 56 cases, accounted for 65.88%.The shape of the mass was spherical or nearly spherical in 95.29%.The diameter of the mass ≤50mm in 58 cases, accounted for 68.23%.The density of the mass was even in 72 cases, and 13 cases were inhomogeneous.The proportions of lobulation sign, burr sign, spinous process sign, pleural depression sign, bronchovascular concentration sign and vacuole sign were 94.12%, 88.24%, 89.41%, 92.94%, 52.94%, 88.24%, respectively.
Conclusion
CT findings and case-based analysis of patients with peripheral lung cancer are of great importance in the diagnosis of patients with peripheral lung cancer, and play a significant role in the clinical efficacy.

OBJECTIVE: To verify the therapeutic effect and adverse reactions of ?-receptor blocker, domestic urapidil, on severe hypertension. METHODS: Observation was carried out in a multi-center, random sampling and controlled pattern. Drug was iv injected first and then infused. At the same time, the patients' systolic pressure, diastolic pressure, heart rate, EKG, blood & urine routine, serum GPT and urea nitrogen were measured and examined.RESULTS: Of 41 cases in this series. 16 satis- factory(39.0%), 23 improved (56.1%), 2 unsatisfactory(4.9%) .The total effective rate was 95.1%. OBJECTIVE:
To identify the genetic characteristics in patients with nonsydromic hearing loss (NSHL) in Hunan province, to determine the prevalence and spectrum of mutations in GJB2 gene, and to explore the pathogenic mechanism.
METHODS:
A total of 140 sporadic patients with NSHL were enrolled after clinical examination. Molecular studies were performed by amplifing the coding region of GJB2 gene, purifying the PCR products, and sequencing directly. Sequences were analysed by DNAStar software to determine GJB2 mutations in the patients. Special method was designed to confirm the unreported mutation.
RESULTS:
We detected GJB2 mutation in 56 out of the 140 patients (40%, 56/140). Both of the 2 alleles were mutated in 29 patients and 1 allele in the other 27 patients, and the rate of allele mutation was 30.4%(85/280). Ten variations were detected, including 7 mutations and 3 polymorphisms. The deaf-causing mutations were nonsense mutation c.139G>T; frameshift mutation c.235delC and c.176-191del16; and missense mutation c.109G>A, c.344T>G, c.550C>T and c.571T>C. The unreported missense mutation was c.344T>G. The c.235delC mutation was the most prevalent mutation found in the 27 patients (19.3%, 27/140). The frequency of c.109G>A mutation was next to c.235delC found in 25 patients (17.9%, 25/140).
CONCLUSION
GJB2 mutation is a major cause for NSHL. The most common-spot in Chinese patients with NSHL is c.235delC. The unreported missense mutation is c.344T>G.

8.Summary of recommendations of guidelines for the offloading of diabetic foot ulcer

Objective To retrieve and analyze the guidelines on diabetic foot offloading, and summarize the evidence of high quality guidelines, so as to provide reference for standardizing the offloading treatment of diabetic foot in China. MethodsThe clinical practice guidelines of diabetic foot in the major guideline networks, databases and professional association websites were retrieved by computer. Three researches evaluated the quality of the guidelines, and two researches extracted evidence from documents that met the quality criteria. ResultsA total of 7 guidelines were included, and 5 aspects, 30 recommendations were extracted, including comprehensive treatment, the principle of offloading, offloading of the plantar ulcer, offloading of the non-plantar ulcer, and the gap between the evidence and practice. ConclusionsThis study summarize the best evidence of the diabetic foot offloading therapy, and provides a reference for medical institution to develop appropriate procedures and standardize the practice of diabetic foot offloading therapy. Most of the interventions recommended in the guidelines come from relatively developed countries, but their application may be limited in less developed countries or regions. Therefore, it is urgent to explore offloading methods which have positive offloading effect as well as conform to our national conditions, and apply them to clinical practice gradually in order to improve the therapeutic effect of diabetic foot and improve prognosis.
9.Stromal interaction molecule 1 promotes microglia/macrophages M1 activation through endoplasmic reticulum stress after cerebral ischemia-reperfusion

Objective:To investigate the influence and mechanism of stromal interaction molecule 1 (STIM1) in microglia/macrophages M1 activation after cerebral ischemia-reperfusion injury.Methods:(1) Animal experiment: 20 male C57BL/6J mice were randomly divided into sham-operated (Sham) group, middle cerebral artery occlusion and reperfusion (MCAO/R) group, MCAO/R+si-Ctrl group, and MCAO/R+si-STIM1 group ( n=5); MCAO/R models were established in mice of the latter 3 groups; empty vector control virus and STIM1 gene knockout lentivirus were transfected into mice in the MCAO/R+si-Ctrl group and MCAO/R+si-STIM1 group. The transfection efficiency of STIM1 and the expression of microglia/macrophages M1 activation marker cluster of differentiation 86 (CD86) in each group were observed. (2) Cell experiment: primary microglia were divided into Ctrl group, oxygen-glucose deprivation/re-oxygenation (OGD/R) group, OGD/R+si-Ctrl group, OGD/R+si-STIM1 group, OGD/R+solvent group, and OGD/R+4-phenylbutyric acid (4-PBA) group; OGD/R models were established in the later 5 groups; empty vector control virus and STIM1 gene knockout lentivirus were transfected into mice in the OGD/R+si-Ctrl group and OGD/R+si-STIM1 group; cells in the OGD/R+4-PBA group were pre-treated with 1 mmol/L 4-PBA for 1 h at 24 h before OGD/R modelling to inhibit endoplasmic reticulum stress (ERS), and cells in the OGD/R+solvent group were pre-treated with 0.5% dimethyl sulfoxide (DMSO) for 1 h at the same time. Reverse transcription quantitative polymerase chain reaction (RT-qPCR), ELISA, Western blotting and other methods were used to detect the levels of CD86, tumour necrosis factor-α ( TNF-α) mRNA, interleukin (IL)-1β, and ERS-related proteins (transcription factor C/EBP homologous protein [CHOP], activated transcription factor 4 [ATF4]) in these cells. Results:(1) Animal experiment: the STIM1 expression in MCAO/R+si-STIM1 group was significantly lower than that in Sham group, MACO/R group and MCAO/R+si-Ctrl group ( P<0.05); as compared with that in the MACO/R group and MCAO/R+si-Ctrl group, the number of microglia/macrophages co-expressing CD86 and Iba-1 around the ischemic foci of mice in the MCAO/R+si-STIM1 group was significantly decreased ( P<0.05). (2) Cell experiment: as compared with those in the OGD/R group and OGD/R+si-Ctrl group, the expression levels of STIM1, CD86, and TNF-α mRNA, and supernatant IL-1β content in the OGD/R+si-STIM1 group were significantly decreased ( P<0.05); as compared with those in the OGD/R group and OGD/R+si-CTRL group, the ATF4 and CHOP expression levels in OGD/R+si-STIM1 group were significantly decreased ( P<0.05); as compared with those in the OGD/R group and OGD/R+solvent group, the CD86 level, TNF-α mRNA expression level and IL-1β content in the OGD/R+4-PBA group were significantly decreased ( P<0.05). Conclusion:STIM1 affects microglia/macrophages M1 activation after ischemia-reperfusion injury by regulating ERS level.
10.Barriers in clinical use of evidence-based urethral catheter cleaning: a qualitative study based on theoretical domains framework

Objective To explore the barriers in the clinical use of evidence-based urethral catheter cleaning. MethodsThe interview outline was developed based on the Theoretical Domains Framework (TDF) from September to October 2018. Totally 19 nurses working in the Department of Urinary Surgery at Peking University First Hospital was selected for semi-structural interview by convenient sampling, and content analysis was conducted based on the results. ResultsA total of 4 barriers related to 4 domains under the TDF were found in this study, namely, social / professional roles and identities, confidence in results, environmental factors and resources, and social impact. The subjects were extracted on this basis, which were lack of knowledge conversion process and norms in the department, poor self-management ability in the patients, and insufficient catheter cleaning resources. ConclusionsThe clinical use of evidence-based urethral catheter cleaning is affected by multiple factors, including systems, patients and resource availability. Departments need to explore and develop the norms for knowledge transformation, improve the self-management ability of postoperative patients, provide sufficient resources for catheter cleaning, and promote the clinical use of evidence-based urethral catheter cleaning.
