1.Effects of endothelin on platelet activating factor (PAF) synthesis and release by cultured Kupffer cells from CCl_4-induced cirrhotic rats

Objective To investigate the correlation between Kupffer cells and the synthesis of platelet activating factor(PAF)in experimental hepatic cirrhosis,and to elucidate the effects of endothelin(ET)on portal hypertension.Methods Thirty SD rats were randomly assigned to two groups:control group and CCl4-induced hepatic cirrhotic group.Kupffer cells,isolated from the livers of animals in both groups,were cultured for 24h.ET-1-induced PAF synthesis,and mRNA expression of PAF,ET-1 receptor and preproendothelin-1 in Kupffer cells were determined by rapid 3H-PAF scintillation proximity assay,saturation binding technique and semi-quantitative reverse transcriptase polymerase chain reaction(RT-PCR),respectively.Results Cell-associated PAF synthesis and release increased about 1.48 folds and two-folds,respectively,by cirrhotic Kupffer cells as compared to the control(1.02 ? 0.06 vs 0.69 ? 0.07 pg /mg DNA in Kupffer cells and 1.42? 0.14 vs 0.66 ? 0.04 pg/mg DNA in medium).Endothelin-1 enhanced Kupffer cells to stimulate PAF synthesis in a concentration-dependent manner,and for cirrhotic Kupffer cells,the effect was more significant than control.Cirrhotic Kupffer cells also had increased densities of functional receptors for both PAF and ET-1(exclusively ETB),but did not change the affinity of these receptors.No mRNA transcripts for the ETA receptor or preproET-1 were detected.Conclusion Kupffer cell is the main source of PAF in the cirrhotic rats.ET-1 stimulates PAF synthesis in activated Kupffer cells via ETB receptor.Since both ET-1 and PAF individually cause portal hypertension,Kupffer cells may play a role in portal hypertension associated with liver cirrhosis.
2.Clinical assessment of the effect of intravenous patient controlled intravenous analgesia and epidural patient controlled epidural analgesia in postoperative analgesia

Objective To compare the clinical effect and side effect of intravenous patient controlled intravenous analgesia (PCIA) with patient controlled epidural analgesia (PCEA). Methods Two hundred and fourty postoperative patients,who were randomly divided into three groups:group PCIA,group PCEA and control, 80 cases for each group,and the recovery of bowel movement,urinary retention,nausea,vomiting,pruritis,respiratory depression,postpartum hemorrhage,lactation and recovery of uterus were observed after operation. Results Overall patients in two PCA groups were satisfied with the postoperative analgesia. The incidence of urinary retention in group PCIA were significantly lower than that in group PCEA( P 0 05). There were no remarkable differences between two PCA groups and control in the incidence of postartum hemorrhage, lactation and recovery of uterus. Conclusions Both PCIA and PCEA have excellent analgesic effect. PCIA has lower incidence of urinary retention,and has higher sedative incidence compared with PCEA.
3.Advances in Study on Endoscopic Diagnosis of Early Gastric Cancer

Gastric cancer is one of the common malignant tumors in digestive system. The incidence of gastric cancer in China is higher than that in developed countries. The prognosis of gastric cancer is closely related to the stage of cancer, and the prognosis of advanced gastric cancer is poor. Improving the detection rate of early gastric cancer is the key to improve the survival of patients with gastric cancer. Endoscopic technology developed rapidly in recent years,various forms of endoscopy have been applied in clinical practice,and the detection rate of early gastric cancer was increased. This article reviewed the advances in study on endoscopic diagnosis of early gastric cancer.
4.Synthesis of platelet activating factor(PAF) and expression of PAF receptor in Kupffer cells in portal hypertension in rat

Objective To study the changes in PAF and its receptor in Kupffer cells in experimental cirrhosis and to evaluate the role of activated Kupffer cells in portal hypertension. Methods Kupffer cells, isolated from the livers of control and CCl_4-induced cirrhotic rats, were cultured in serum-free medium overnight. PAF synthesis and release by Kupffer cells were determined 24 h later by rapid ~3H-PAF scintillation proximity assay, and the expression of PAF receptor in Kupffer cells by saturation binding technique and semi-quantitative reverse transcriptase polymerase chain reaction (RT-PCR). By immunohistochemisty, the distribution of PAF in Kupffer cells was surveyed. 5.Detection of Plasma Endothelins and Serum Nitric Oxide in the Patients with Sudden Sensorineural Hearing Loss

Objective To investigate the roles of endothelins and nitric oxide in the pathogenesis of sudden sensorineural hearing loss(SSHL). Methods 26 patients with SSHL in Changsha area from October 2000 to April 2001, and 18 healthy subjects in the same period were enrolled in this study. Plasma endothelins level was detected by radioimmunoassay, and serum nitric oxide(NO) level was detected by cadmium-reduced spectrophotometry. Results The plasma endothelins level in the SSHL patients was significantly higher than that in the healthy subjects, and the serum NO level in the SSHL patients was significantly lower than that in the healthy subjects(P0.05). Conclusion The increase of ET level and the decrease of NO level may play an important role in the pathogenesis of SSHL, which may serve as an index to predict the hearing improving or not, but were not related to the degree of hearing loss.
6.Extraction Technology of Total Flavonoids for the Bone Healing Formulation

OBJECTIVE:To optimize the extraction technology of total flavonoids for Bone healing formulation in order to use it for preparation research. METHODS:With the content of total flavonoids as the index,L9(34)orthogonal test was employed to investigate the effects of volume fraction of the solvent ethanol,the amount of solvent,extraction times and extraction time on the extraction of total flavonoids for Bone healing formulation to determine the optimal levels of the factors,and verification tests were conducted. RESULTS:The optimal extraction technology of total flavonoids was 1 h reflux extraction for 3 times,with 70% etha-nol 10 times as much as the amount of medicinal materials. Verification tests showed the average content of total flavonoids was 62.03 mg/ml(RSD=0.84%,n=3),that is to say,6.20 g total flavonoids might be extracted from 100 g medicinal materials for the formulation. CONCLUSIONS:The optimal technology is stable and feasible and can be used for the extraction of total flavo-noids for Bone healing formulation and provide a experimental basis for the preparation of Bone healing liniment.
7.Transdermal Behavior in vitro of Total Flavonoids and Its Monomer Component in Guyu Liniment

OBJECTIVE:To investigate transdermal behavior in vitro of total flavonoids and its monomer components in Guyu liniment. METHODS:Vertical Franz diffusion cell was adopted to perform a test on excised mouse skin as transdermal barrier. UV spectrophotometry was used to determine the content of total flavonoids,and HPLC to determine the content of monomer flavo-noid hydroxysafflor yellow A(HSYA)to observe transdermal absorption in vitro within 12 hours. RESULTS:The accumulative per-meation quantity Q of the total flavonoids and HSYA in Guyu liniment increased with time(t),demonstrating a significant correla-tion with t1/2,and transdermal absorption was in conformity with Higuchi equation (r=0.995 6,0.999 5);permeate flux of total flavonoids and HSYA were 126.24,47.516μg/(cm2·h). CONCLUSIONS:The transdermal behavior of total flavonoids in Guyu lin-iment is similar to that of HSYA. Both belong to matrix diffusion-type transdermal drug delivery system,with the characteristic of long-term sustained release.
8.Analysis of prognostic influencing factors for early non-small-cell lung cancer after radiotherapy

Objective To evaluate the prognosis influencing factors of early non-small cell luny cancer (NSCLC) after radiotherapy.Methods 81 early NSCLC patients received definitive radiotherapy and were eligible.Among these patients,60 were diagnosed as squamous cell carcinoma,16 were adenocarcinoma and 5 were diagnosed through imaging instead of pathology.45 patients received conventional radiotherapy,36 patients received three dimensional conformal radiotherapy (3D-CRT),All of them received a total dose of 50-96 Gy with a median dose of 67.8 Gy. Kaplan-Meier survival curves and Cox regression model analysis were applied to evaluate the survival and prognostic factors. Results The median survival time was 34 months.The 1-,3- and 5-year survival rates (OS) were 88.7 %,41.9 %,21.8 %,respectively.Karnofsky performance status≥80,Clinical stage, diameter≤4 cm and the therapeutic effect were associated with improving overall survival.Cox hazards model showed that Karnofsky performance status≥ 80 and diameter≤4 cm were likely to be independent positive prognostic factors. Conclusion Karnofsky performance status and tumor diamater can be used to evaluate the prognosis of early NSCLC after radiotherapy.
9.Single-incision laparoscopic cholecystectomy without using titanium-clips

Objective To explore the safety and feasibility of single-incision laparoscopic cholecystectomy without using titanium-clips. Methods Data of 1016 patients(group A) undergoing single-incision laparoscopic cholecystectomy without using titanium-clips were compared to that of 874 patients(group B)undergoing two-port laparoscopic cholecystectomy without using titanium-clips by t test and chi square test for operating time,operative hemorrhage,the length of postoperative hospital stay and postoperative pain.Results In group A,1001 cases were successfully operated on with single-incision laparoscopic cholecystectomy,with conversion to classic four-port laparoscopic cholecystectomy in 15 cases,while 874 cases in group B were operated on with two-port laparoscopic cholecystectomy.There were no bile leakage,biliary tract injury or death in both groups.There were no difference in operating time[(34.5 ±5.2) min vs (32.0±7.4)min,t=0.063,P=0.526],the length of postoperative hospital stay[(3.1±0.8)d vs(3.2±0.7)d,t=1.073,P=0.326]and operative hemorrhage[(56.5±17.8)ml vs (55.2±15.9)ml,t=0.812,P=0.425](P>0.05) between the two groups,but the postoperative pain was less severe in Group A than that in group B (P=0.000<0.05). Conclusions The single-incision laparoscopic cholecystectomy is feasible,safe,less traumatic and more cosmetic.
10.Biological properties of human periodontal ligament stem cells under inflammatory microenvironment

BACKGROUND:The periodontal ligament stem cels can promote periodontal tissue regeneration, providing a new way for the treatment of periodontitis. OBJECTIVE:To observe the inflammatory microenvironment effects on the biological properties of periodontal ligament stem cels. METHODS: Periodontal ligament stem cels from healthy controls and patients with periodontitis were primarily cultured by tissue digestion method, purified using limited dilution method, and identified through detection of CD146 and STRO-1. Then, passage 3 cels were taken and denoted as normal control and inflammation groups folowed by osteogenic induction. RESULTS AND CONCLUSION:Purified cels from two sources both expressed STRO-1 and CD146. Periodontal ligament stem cels in the inflammation group showed higher multiplication capacity, but the osteogenesis ability was lower compared with the normal control group. The expressions of Runx2 mRNA and Osterix mRNA were dropped significantly after the stimulus of tumor necrosis factor-α (P < 0.05), but the interleukin-1β and interleukin-6 did not have a significant impact. Tumor necrosis factor-α at 0.1 and 1 μg/L had no significant effects on the expression of Runx2 mRNA, but the expression of Runx2 mRNA was decreased significantly after treatment with 10 μg/L tumor necrosis factor-α (P< 0.05). It is confirmed that the molecular signaling mechanism inside the periodontal ligament stem cels is changed under inflammatory microenvironment, so that the differentiation capacity of cels from the inflammatory sources is lowered. Moreover, tumor necrosis factor-α is one of the key factors and its optimalconcentration is 10 μg/L.
