OBJECTIVE: To investigate the effect of PNS on experimental atherosclerosis in rabbits, including the serum levels of TG, TC, LDL-C, level of MDA, activity of SOD and plaque area. METHODS: White Japanese rabbits were divided into normal control group, AS model group, low dose PNS group and high dose PNS group. Administration was for 12 consecutive weeks. The serum levels of TG, TC, LDL-C, MDA and activity of SOD were determined before experiment and at the end of the 12th week, respectively. AIM To study the pharmacokinetics of arbidol capsule in Chinese healthy volunteers.METHODS A single oral dose of arbidol capsule 200 mg was given to 20 healthy volunteers respectively.Plasma samples were prepared based on a simple liquid-liquid extraction.The extracted samples were analyzed by HPLC equipped with UV detection.Pharmacokinetic parameters were calculated by 3P87 software. RESULTS The main pharmacokinetic parameters of arbidol were as follows:c(max)(418±s 241)μg·L-1,t(max)(1.3±1.2)h,t(1/2α)(1.9±2.3)h,t1/2β(14±5),hAU0-t(2 633±1 071)μg·L-1,Vc/F(0.7±0.6)L,CL(0.08±0.03)L·h-1,CONLUSION The pharmacokinetics of arbidol capsule in human body accord with two-compartmetn open model.The study will offer the pharmacokinetic parameters for the clinical application of arbidol.
3.Influence of different placing time of child peripheral blood on blood routine parameters

Objective To study the influence of different placing time of child peripheral blood on blood routine parameters in order to increase the working efficiency of clinical blood routine test.Methods Each 300 μL of peripheral blood was collected from the right hand ring finger in 50 healthy children in our hospital from January 2015 to January 2016.The blood routine was performed by using the whole blood cells analyzer at instantly after blood collection,at 0,5,10,15,30 min after blood collection under the room temperature (20 to 25 ℃).Results With the measured results at 15 min as the control,the detection results at the other times points had no statistical difference (P>0.05).Compared with the detection results at 5 min,the white blood cell count(WBC) and platelet volume distribution width (PDW),lymphocyte absolute value (LYM),neutrophil percentage absolute value (NEU),platelet count (PLT),red blood cell volume distribution width (RDW),hemotocrit (HCT) and plateletcrit(PCT) were statistically different (P<0.05).Conclusion Reasonably arranging time,eliminating pre-analysis error and reducing the influence of peripheral blood placing time on blood routine parameters have an important significance to accurately judge the clinical dat.It is recommended that the blood routine detection time should be controlled within 10-30 min in order to increase the working efficiency of clinical detections.
4.The influence of curculigoside on the cognitive function of senile dementia rats and the expression level of Hippocampus Bcl-2, Bax and Estrogen Receptor

Objective Curculiginis extract has good scavenging effects on oxygen free radicals, and there is no curculiginis extract for AD research at home.The aim of this study was to explore the influence of Curculigoside on the cognitive learning and memory functions of senile dementia model rats induced by D-galactose combined with β-protein amyloid β25-35 (Aa25 35) and the expression level of hippocampus B-cell lymphoma-2 (Bcl-2), Bax, estrogen receptor (ER) protein and mRNA.Methods Forty SD rats weredivided into the control group (equal saline was intraperitoneal injected for six weeks consecutively, and equal saline was injected to the bilateral hippocampus in the seventh week), the model group (galactose was intraperitoneal injected for six weeks consecutively, and 4 mmol/L Aβ25-35 was injected to the bilateral hippocampus in the seventh week), the low dosage group and the high dosage group randomly (the stomach irrigation of low dosage 24mg/kg or the high dosage 72mg/kg curculigoside was applied every day, for 8 weeks), 10 rats in each group.After the treatment , the Morris water maze was conducted to test the space learning and memory functions of rats;the flow cytometry method was conducted to analyze the neuronal apoptosis conditions;the western blot was conducted to detect Bcl-2, Bax and ER protein level;the Realtime PCR was conducted to detect Bcl-2, Bax and ER mRNA level.Results Morris water maze test results: Compared with the model group, the escape latency (EL) of the rats of the model group, the low dosage group and the high dosage group on the 2nd, the 3rd, the 4th and the 5th day significantly extends (P<0.01), and the high dosage group significantly shortens (P<0.05) than the low dosage group.Space exploration test results: Compared with the proportion of the distance swum within the quadrant by the rats in the model group in the total distance [(30.14±5.10)%], that of the low dosage group, the high dosage group and the control group [(41.98±4.78)%, (46.17± 9.31)% and (50.13±6.81)%] significantly increases (P<0.01);in addition, the high dosage group is higher than the low dosage group (P<0.05).Compared with the apoptosis rate of rates in the model group[(7.11±1.78)%], that of the high dosage group, the low dosage group and the control group[(5.53±1.45)%], (4.11±1.03)% and (3.08±1.33)% significantly decreases (P<0.05).Compared with the control group, the Bcl-2 and Bax protein expression of the model group significantly increases, and the cell Bcl-1 and Bax of cells significantly decreases (P<0.05).Compared with the model group, the Bcl-2 and ER protein expression as well as the Bcl-2/Bax of the high dosage group and the low dosage group significantly increase, and the Bax protein expression significantly decreases (P<0.05).Compared with the mRNA expression (1.00 and 1.00) of Bcl-2 and ER in the control group, the mRNA expression (1.31 and 1.98) of the Bcl-2 and Bax in the model group significantly increases (P<0.05);compared with the mRNA expression (1.13 and 1.08) of Bcl-2 and ER in the model group, the Bcl-2 (1.95 and 2.01) and mRNA expression (1.81 and 1.96) of ER of the high dosage group and the low dosage group significantly increase (P<0.01).Conclusion Curculigoside plays an improving role in space learning and memory functions of senile dementia model rats, with the mechanism may be related to the protection to the hippocampus neurons by restraining Bax expression and neuronal apoptosis through the up-regulation of hippocampus ER and Bcl-2 expressions.
5.Efficacy of Nicorandil for Treating the Patients With Cardiac Syndrome X and Its Impact on Vascular Endothelial Function

To observe the clinical efficacy of nicorandil for treating the patient with cardiac syndrome X (CSX) and its impact on vascular endothelial function. Methods: A total of 140 CSX patients were randomly divided into 2 groups: Control group, the patients received conventional anti-angina therapy and Nicorandil group, based on conventional anti-angina therapy, the patients received additional oral nicorandil treatment. n=70 in each group. All patients received resting emission computed tomography (ECT) and treadmill exercise ECG stress test (TET). Blood levels of endothelin (ET-1), high-sensitivity C-reactive protein (hs-CRP) and nitric oxide (NO) were examined before and 3 months after treatment. Results: Compared with pre-treatment condition, the attack frequency of angina pectoris and positive rate of ECT were decreased after treatment in both groups, P<0.05; in Nicorandil group, the suspicious positive rate and positive rate of TET were reduced after treatment, P<0.05. Compared with Control group, Nicorandil group had the much lower suspicious positive rate and positive rate of TET after treatment, P<0.05. Blood tests indicated that compared with pre-treatment condition, ET-1 and hs-CRP were decreased, NO was increased after treatment in both groups, all P<0.05; blood levels of ET-1, hs-CRP and NO were different between 2 groups after treatment, all P<0.05. Conclusion: Nicorandil could inhibit inflammatory factors, elevate endothelial function and therefore improve micro vascular angina symptoms, increase exercise tolerance obviously.
6.Effects of acupuncture on the number and degranulation ratio of mast cells and expression of tryptase in synovium of rats with adjuvant arthritis.

Objective: To observe the effects of acupuncture on synovial pathology, synovial mast cell degranulation and tryptase expression and to investigate the relationship between the functions of mast cells and effects of acupuncture on early adjuvant arthritis in rats. Methods: Forty-six male Wistar rats were randomly divided into normal control group (n=16), untreated group (n=15) and acupuncture group (n=15). Adjuvant arthritis was induced by injection of 0.1 mL Freund's complete adjuvant in right hind limb footpad. Normal control group and untreated group received no acupuncture treatment, while rats in the acupuncture group were treated with sterilized disposable stainless steel needles inserted perpendicularly as deep as 2 to 3 mm at Xuanzhong (GB39), 6 mm at Shenshu (BL23) and 7 mm at Zusanli (ST36) for eight times (15 min each time) every two days. Setting the modeling day as the 0 day of the experiment, the body weight and paw volume of the rats were measured every three days from the 0 day. In the end, synovial tissues of the right hind ankles were sampled and made into paraffin sections. Then they were firstly stained with hematoxylin-eosin for observing synovial pathology to evaluate the effects of acupuncture on adjuvant arthritis, then stained with toluidine blue for observing the number and degranulation ratio of synovial mast cells and finally detected by immunohistochemical staining method to investigate the expression of tryptase in synovium. Results: Compared with the untreated group, the body weight of rats in the acpuncture group was increased significantly (P<0.05), while the paw volume decreased obviously (P<0.01). Hematoxylin-eosin staining showed that acupuncture significantly inhibited inflammatory cell infiltration, synovial cell hyperplasia, and synovial fibroplasia in synovium of rats with adjuvant arthritis as compared with the untreated group (P<0.05). Toluidine blue staining showed that acupuncture could significantly diminish the numbers of total and degranulated mast cells in rats with adjuvant arthritis (P<0.01), which were significantly higher in the untreated group than in the normal control group (P<0.01). Showing by immunohistochemical staining, the expression of tryptase in synovium in the acupuncture group was decreased as compared with the untreated group (P<0.01). Analyzed by Spearman's bivariate correlation, the number of mast cells and degranulation ratio of mast cells were positively correlated with the pathological scores (r=0.837, P<0.01; r=0.634, P<0.01). Conclusion: Acupuncture can improve pathological condition of inflammatory synovium in rats with early adjuvant arthritis by inhibiting the function of synovial mast cells, which may play an important underlying role in the immunoregulation of acupuncture on adjuvant arthritis.
7.Chemical Constituents of Endophytic Fungus Aconitum Transsectum from Alternaria Solani

Objective: To study the chemical constituents of endophytic fungus Alternaria solani from Aconitum transsectum. Methods: The compounds were isolated and purified by column chromatography , and their structures were elucidated based on the spectroscopic analysis (1H-NMR, 13C-NMR and MS).Results:Totally 15 compounds were isolated from A.solani and characterized as 20-hydroxyl-ergosta-4,6,8(14),22-tetraen-3-one(1),ergone(2),stellasterin(3),episterol(4),ergosterol peroxide(5),cyclo(D)-Pro-(D)-Leu(6),cyclo-(L-Val-L-Pro)(7),cyclo-(S-Pro-S-Ile)(8),tentoxin(9),porrotoxin(10),7-dehydroxyl-zinniol(11),zinniol (12),8-zinniol methyl ether (13),3-methoxy-4-methyl-5-(3′,3′-dimethylallyloxy)-2-methoxymethyl benzoic acid (14) and pseudo-monas aeruginosa H2-sesquiterpene lactone (15).Conclusion: Totally 15 compounds are isolated from endophytic fungus Alternaria solani, and compounds 2-10 and 12-15 are isolated for the first time .
8.Clinical analysis of extramedullary plasmacytomas in the head and neck.

OBJECTIVE:
To summarize the clinical features and the experiences of diagnosis and treatment of extramedullary plasmacytomas (EMPs), and to enhance the cure rates of EMPs.
METHOD:
Clinical data of 8 patients with EMPs in head and neck, who treated in our hospital from Jan. 1990 to Dec. 2004, were reviewed. Of 8 cases, 3 cases with the tumors occurred in nasal cavities, 1 in maxillary sinus, 2 in nasopharynx, 1 in posterior wall of oropharynx.
RESULT:
Eight patients who were all pathologic confirmed EMP accepted surgical resections of the tumors, and 4 of 8 cases were boosted radiation therapy post-operation. Four cases were still alive disease-free for more than 1, 5, 10 and 12 years after treatments, respectively. One died of local recurrence in 2 years, 1 died of multiple myeloma in 3 year, and 1 died of heart attack in 2 years after treatments, respectively. One lost follow-up.
CONCLUSION
EMPs in head and neck are low potential malignancy tumors. The diagnosis of EMPs mainly depends on clinical manifestations and pathological results. Surgery and radiation therapy are the main treatments for EMPs in head and neck.

9.Association between preoperative serum β 2-microglobulin concentrations and postoperative delirium in elderly patients

Objective:To evaluate the association between preoperative serum β 2-microglobulin (β 2MG) concentrations and postoperative delirium (POD) in elderly patients. Methods:The study selected patients who underwent knee or hip arthroplasty under spinal-epidural anesthesia on an elective basis at Qingdao Municipal Hospital from May 2021 to November 2022. The patients were divided into a POD group and a non-POD group based on the occurrence of POD. The study was conducted as part of the Perioperative Neurocognitive Impairment and Biomarkers Lifestyle Cohort, which was a nested case-control study. The study collected baseline data from two groups of patients and analyzed the differences between them. Logistic regression was used to identify the risk factors for POD. The stability of the regression model was tested using sensitivity analysis. The mediation model was used to examine whether cerebrospinal fluid (CSF) biomarkers mediated the relationship between β 2MG and POD. The receiver operating characteristic curve was drawn and the area under the curve was calculated to evaluate the accuracy of preoperative β 2MG concentrations and CSF biomarker concentration in predicting POD. Results:There were 57 cases in POD group and 449 cases in non-POD group. The results of logistic regression analysis showed that the increased β 2MG and CSF total tau protein (t-tau) concentrations were risk factors for POD, and the increased CSF β-amyloid 42 concentration was a protective factor for POD after adjustment for multiple confounders such as age, gender, education, Mini-Mental State Examination, history of hypertension and infusion volume ( P<0.05). The results of mediation analysis showed that the serum β 2MG′s effect on POD was partly mediated by t-tau (18.1%). The results of the receiver operating characteristic curve showed that the area under the curve of the β 2MG concentration combined with the CSF biomarker concentration was 0.742. Conclusions:Elevated preoperative serum β 2MG concentration is a risk factor for POD in elderly patients, and the relationship may be partly mediated by CSF t-tau.
10.Factors affecting target volume in adaptive radiotherapy for locally advanced nasopharyngeal carcinoma

Objective To investigate the relationships of pre-radiotherapy body weight,gender,age,EBVDNA,hemoglobin,plasma albumin,and induction chemotherapy regimen with the changes of target area and lymph node volume in adaptive radiotherapy,so as to provide a reference for the timing and population selection of adaptive radiotherapy.Methods A retrospective analysis was conducted on 34 patients who received the first course of radiotherapy at Sun Yat-sen University Cancer Center from January 2022 to November 2022.All patients underwent CT scans again after 20 sessions of radiotherapy for developing the secondary radiotherapy plans.The body weight,gender,age,tumor stage,hemoglobin,plasma albumin,induction chemotherapy regimen,and EBVDNA were collected.Results The tumor volume reduction in the primary focus was more evident in patients with pre-treatment plasma albumin≥40 g/L than in those with pre-treatment plasma albumin<40 g/L(t=3.971,P=0.001),and in patients with pretreatment EBVDNA≤4000 copies/mL than in those with pretreatment EBVDNA>4000 copies/mL(t=4.080,P=0.001).Pearson analysis showed that GTVnx volume difference was positively correlated with pre-radiotherapy GTVnx volume(r=0.444,P=0.009),right parotid gland volume difference(r=0.737,P<0.001),left parotid gland volume difference(r=0.435,P=0.010),and hemoglobin(r=0.722,P<0.001).Conclusion The reduction in tumor volume during radiotherapy is more pronounced in nasopharyngeal cancer patients with normal plasma albumin level and those with pretreatment EBVDNA≤4000 copies/mL.The pre-radiotherapy treatment volume of primary focus,parotid gland volume change before and after radiotherapy,and pre-radiotherapy EBVDNA,hemoglobin and plasma albumin levels can be used to predict the degree of tumor volume shrinkage during radiotherapy,providing a reference for the selection of the timing of adaptive radiotherapy for nasopharyngeal carcinoma.
