Aim To evaluate the effect of Cdk7 silencing on the c ell cycle control, the phosphorylation level changes of Cdk2 and pRb in human he patoblastoma HepG2 cell culture in vitro, and to validate Cdk7 as a novel ta rget for anticancer therapeutics.Method Levels of Cdk7 and the phosphorylation levels of Cdk2 and pRb were measured by Western-blotting. DNA c ontents, cell cycle and apoptosis induced by Cdk7 silencing were analyzed by flo w cytometry and ultrastructural changes of cells were observed with transmission electron microscopy.Result The phosphorylation levels of pRb a nd Cdk2 and the levels of Cdk7 decreased in a concentration-dependent manner wh en the concentration was above 100 nmol?L -1. The majority of patients have experienced pain or discomfort associated with surgery or mechanical ventilation in intensive care unit (ICU), and the use of analgesic drugs can relieve patients' anxiety, improve sleep, promote the disease recovery, reduce the use of sedative drug dosage and the occurrence of delirium. Because of less adverse reactions of the non opioid analgesic drugs, in recent years gradually the doctors in ICU have paid attention to them. In this study, the related literatures have been reviewed to realize the present situation of applying non opioid analgesic drugs in ICU. It was found that nowadays the application of commonly used non opioid drugs in ICU (including ketamine, non-steroidal anti-inflammatory drugs, tramadol, lidocaine, tramadol and gabapentin) can all decrease the use of sedative drugs and opioids, reduce the incidence of adverse reactions, and gradually they will obtain more and more attention.
3.Argon plasma coagulation in the management of polyp remnants in colorectum

AIM: Remnant polyp tissues are often found after polypectomy. In this study, we assessed the outcome and safety of argon plasma coagulation (APC) in the management of colorectal sessile or semipedunculated polyp remnants after polypectomy. METHODS: Sixty-one colorectal polyps of 33 patients underwent polypectomy in Department of Digestion Medicine, Tangdu Hosptial, the Fourth Military Medical University of Chinese PLA from 2004 to 2007. Colorectal polyps type was identified by pathological examination. All polyps were removed under colonoscope with the diathermic snare, and the polyp remnants were destroyed with APC (Erbe, Germany). The power output was initially set at 40-50 W. In case of visibly insufficient coagulation, the output power was increased gradually by 5 W up until the remnants were completely ablated. Follow-up endoscopies were performed 1 and 3 months after the treatment. Pathological examination and APC treatment were performed when the recurrence of polyps was found. RESULTS: ①All 61 colorectal polyps were removed with the diathermic snare and the polyp remnants were destroyed with APC successfully only once. ②During the follow-up averaged 2.4 months, effective destruction of remnant polyp tissue was obtained in 59 polyps (96.7%) in 31 patients (93.9%). Objective To explore the clinical efficacy of transcutaneous autologous bone marrow transplantation in the treatment of delayed union after internal fixation of limb fracture. Methods 41 patients with delayed union after internal fixation of limb fracture with informed consent in our hospital were divided into two groups by number table,20 patients in control group were treated with conventional therapy,21 patients in observation group were treated with transcutaneous autologous bone marrow transplantation,the effects and the occurrence of complications of two groups were compared and observed. Results In observation group,excellent healing in 13 cases(61.9%),good in 7 cases(33.3%),failure in 1 case(4.8%),and in the control group,6 patients(30.0%),11 cases(55.0%),3 cases (15.0%),the difference between the two groups was statistically significant(u=4.1934,P<0.05);the complication rate of the observation group was 9.5%,compared with 15.0% of the control group,there was no significant difference(x2=0.034,P>0.05). Conclusion Transcutaneous autologous bone marrow transplantation in the treatment of delayed union after internal fixation of limb fracture had better efficacy and less complication.
8.Experience of eight patients treated by laparoscopic esophageal hiatal hernia repair

ObjectiveTo explore the safety and therapeutic effect of 8 patients with esophageal Hiatal Hernia treated by laparoscopic hernia repair.MethodsA retrospective analysis was performed on the clinical data of 8 patients with esophageal Hiatal Hernia form Jun.2009 to Jun. 2010.Among the participants,3 conducted 360-degree fundoplication,5 conducted partial(270-degree) fundoplication.Silk sutures were used for the repair of esophageal perforation in 4 patients,and patch repair was used for the other 4 cases.ResultsEight patients were treated by laparoscopic hernia repair,and all of them were cured without postoperative complications.The mean duration of surgery was ( 120 ± 30) min,with average blood loss ( 50 ± 12 ) ml.Patients had a mean postoperative hospital stay of(4.5 ± 2.5 )days.All the patients were followed up for 1 to 2 years,and no case was found to be relapsed.ConclusionTotal laparoscopic hernia repair is minimally invasive,with short recovery course,less pain after surgery,little complication and short hospitalized time.Laparoscopic Hernia repair should be the preferred effective operation method for patients with esophageal Hiatal Hernia.
9.The application of combined laparoscopic-colonoscopy resection in early colorectal tumor

Objective To explore the safety and clinical effect of combined laparoscopic-colonoscopy resection of colorectal tumor.Methods A total of 26 patients with early colorectal tumor was treated by combined laparoscopic-colonoscopy resection.To observe the postoperative complications,the mean operative time,mean intraoperative blood loss,mean time of gastrointestinal function recovery,and mean postoperative hospital stay were analyzed.Results All the 26 cases were operated successfully.The mean operative time was 60 ~ 162 (93.7 ± 22.5)min.The mean intraoperative blood loss was 15 ~ 120 (35.9 ± 24.2) ml.The mean time of gastrointestinal function recovery was 48 ~ 120(73.2 ± 14.5)h.The mean postoperative hospital stay was 5 ~ 13 (7.4 ± 1.8) d.No postoperative complications occurred,such as stomach leak,enterobrosis and intestinal obstruction.Follow up 6 ~ 12 months,there were no tumor residue and recurrence.Conclusions The combined laparoscopic-colonoscopy resection was located exactly,reasonable excision scope,minimally invasive,quick recovery and other advantages.It was worth of clinical application.
10.RNase MC2 manifests antitumor effects towards human hepatocellular carcinoma

Purpose To determine the effect of RNase MC2 purified from momordica charantia on cell growth of hepatocellular carcino-ma ( HCC) and its underlying mechanism. Methods MTT, colony formation and nude mice model were used to examine the activity of RNase MC2 in cell proliferation. Cell cycle analysis was done by flow cytometry. Autophagy induced by RNase MC2 treatment was observed via transmission electron microscope. Western blot was performed to detect the RNase MC2-mediated changes of proteins. Re-sults In vitro and in vivo data showed that RNase MC2 markedly inhibited HCC cell proliferation, arrested cells at G2/M phase by in-creasing expression of p53 and p21, induced autophagy via upregulating Beclin-1 and LC3-Ⅱ. Furthermore, combination of RNase MC2 and Sorafenib exerted enhanced lethal effect on HCC cells. Conclusion RNase MC2 manifests significant antitumor activities and enhances the killing effect of Sorafenib in HCC via inducing cell cycle arrest and autophagy.
