1.Early treatment of multiple patients with extensive burns combined with burst injury of the lung

Objective To explore effective treatment methods and experience of management and organization in the early period of multiple patients with extensive burns combined with blast injury of the lungs. Methods To summary and analyze the climical date of 16 patients with extensive burns the combined with blast injury and experience of management and organization. Results One patient died of severe blast injury of lung 5 days after,one died of multi-organ dysfunction syndrome, the others recovered well and the burn, healed well. Organization and the course of resuscitating patients were effective, logistic support was strong and all patients were transported and transferred successfully. Conclusion Individualization fluid infusion based on exact clinical observations and increaseing the ratio of colloid fluid would help to prevent shock accompany burst injury of lung. Early and suitable mechanical ventilation mode to improve ventilation/gas exchange function were important measures to manage inhalation injuries. To plan directive salvage schemes for emergency and powerful logistic support,and transporting and transferring patients in time would help to increase treatment efficiency of multiple patients.
2.Fingerprint analysis of Spica Prunellae by HPLC

Objective To establish a method of fingerprint analysis on Spica Prunellae by HPLC.Methods The HPLC method was used,chromatographic conditions were Betasil C18 column(250 mm ? 4.6 mm,5 ?m),acetonitrile-0.1% phosphric acid system as mobile phase,0-50 min acetonitrile 10%→25%,50-60 min acetonitrile 25%→32%;flow rate 0.7 mL/min;detective wavelength at 210 nm,column temperature 25 ℃.Results Establishing the fingerprint of Spica Prunellae,16 common peaks were found in the HPLC fingerprint of Spica Prunellae and good similarities with correlation coefficients over 0.95 were found in the fingerprints between the herbs from different sources and standard fingerprint,which could be utilized for the identification of Spica Prunellae.Conclusion The method is accuate,simple,and could be used for the quality control of Spica Prunellae.
3.Expression of MCP-1 in renal tissues of patients with IgA nephropathy

Objective To investigate the differential expression of monocyte ehemoattraetant protein-1 (MCP-1) in renal biopsy tissues of patients with IgA nephropathy, and to analyse the association between these 2 markers and their effect on various pathologic types of IgA nephropathy. Methods According to pathologic type, 88 renal biopsy tissues of patients with IgA nephropathy were divided into 4 groups: a minimal change group, a mesangial proliferative glomerulonephritis group, a focal sclerosing glomerulonephritis group, and a diffused sclerosing glomerulonephritis group. Immunohistochemical staining was used to detect the in situ expression of MCP-1 and CD68 on renal biopsy tissues. The expression levels were semi-quantified by image analysis and clinical data were collected from the patients. Results The differences in glomerular MCP-1 expressions were not statistically significant among all groups, while the tubulointerstitial MCP-1 expressions were statistically different among the 4 groups, with the average scores of 1.43 ± 0. 60, 5.98 ±0.92, 10. 60 ± 0.76 and 11.65 ±0.39 for minimal change group, mesangial proliferative glomerulonephritis group, focal sclerotic glomerulonephritis, and diffused sclerotic glomerulonephritis group, respectively. The tubular and interstitial CD68 scores were 0. 75 ± 0. 71, 5. 87 ± 0. 96, 10. 42 ± 0. 61, and 11.40 ±0.49 for the 4 groups, with significant differences in both MCP-1 and CD68 among the 4 groups. Correlation analysis indicated a positive correlation between tubulointerstitial MCP-1 and CD68 (r = 0. 688, P < 0. 01) . MCP-1 in tubulointerstitial was significantly correlated with 24 h urinary protein excretion (r=0.531, P<0.01). Conclusion MCP-1 plays a critical role in mac-rophage infiltration in the kidney. MCP-1 is associated with the severity of tubulointerstitial damage and clinical prognosis.
4.The mediating effects of college students' online social support on the relationship between the online self-disclosure and the internet altruistic behavior

Objective To explore the mediating effects of college students' online social support on the relationship between online self-disclosure and internet altruistic behavior.Methods Using cluster random sampling,the self-disclosure questionnaire for Chinese undergraduates with their internet friend,the online social support scale of adolescence and the internet altruistic behavior scale of undergraduates were applied to 351 college students.Results The score of college students' online self-disclosure,online social support and internet altruistic behavior were respectively(2.22±0.45),(2.93±0.59) and (2.04±0.46).The college students' online self-disclosure,online social support and internet altruistic behavior were positively correlated with each other (P<0.01).College students' online self-disclosure not only predicted internet altruistic behavior directly,but also predicted internet altruistic behavior indirectly through online social support.The direct effects and indirect effects were 0.200 and 0.265.The ratio of the mediating effect on the total effect was 57%.The fit index of mediation model was x2/df=2.87,RMSEA =0.07,CFI =0.94,TLI =0.93,GFI=0.92,IFI=0.94,NFI=0.91.Conclusion The college students' online self-disclosure not only affects the interuet altruistic behavior directly,but also can take the same effect on online social support indirectly.

7.Semi-joint prosthesis replacement for osteosarcoma in 15 children

To investigate the effect of semi-joint prosthesis replacement in treating osteosarcoma around the knee joint,15 children with osteosarcoma around the knee joint underwent semi-joint prostheses replacement and removal of the section with tumor from October 1998 to July 2006.Imageology showed that tumor of 9 children was located at the distal femur,and 6 cases at the proximal tibia.The pathologic changes involved the upper segment of the tibia 9-11 cm in length.All the children had been diagnosed with osteosarcoma by pathological examination,which was graded as stage Ⅱ a according to Enneking staging system.All the patients underwent neoadjuvant chemotherapy preoperatively for 1-2 courses.The size of prosthesis was determined based on preoperative X-ray,CT and MRI.The length of the prosthesis should be 1.0-2.0 cm longer than that of the excised bone.Patellar and collateral ligaments were reconstructed in the artificial semi-knee joint prosthesis.All the 15 patients had a successful semi-joint prosthesis replacement.During the follow-up of 2 to 6 years,body height of the children increases by 4-6 cm,and the affected limbs shortened 1-3 cm.All children could live and study normally.There were 7 excellent,5 good,2 fine and 1 poor according to Enneking evaluation system.The semi-joint prosthesis replacement in the limb salvage surgery for child osteosarcoma around the knee joint can reconstruct knee joint function,remain normal osteoepiphysis,minimize affected limb crispation after operation,and create the condition for total knee replacement in adults.
8.BOLD-fMRI study on the basic activity of the brain in major disorder depression and their first-degree relatives

Objective To study the characteristics and differences of the amplitude of low-frequency fluctuation (ALFF) and the fractional ALFF (fALFF) of the major disorder depression (MDD) and their first-degree relatives in the resting state by functional magnetic resonance imaging (fMRI),to understand the early brain function in patients with MDD.Methods 3.0T fMRI in the resting state was performed in 10 patients with MDD (the depression group),10 first-degree relatives of MDD (the first-degree relatives group) and 10 healthy volunteers (the control group).A statistics analysis of ALFF and fALFF were performed subsequently.Results The ALFF and fALFF values measured in left cerebellum, left precuneus and left medial prefrontal cortex of MDD group were significantly lower than those of the first-degree relatives group (P<0.05).The ALFF values measured in right posterior cingulate cortex and right superior parietal lobule of MDD group were significantly lower than those of the first-degree relatives group (P<0.05).The ALFF and fALFF values measured in left cerebellum, left precuneus and left superior parietal lobule of MDD group were significantly lower than those of the control group (P<0.05).The ALFF values measured in right cingulate cortex and medial prefrontal cortex of MDD group were significantly lower than those of the control group (P<0.05).There was no significance difference found in ALFF values between the first-degree relatives group and the control group,the fALFF values measured in left superior parietallobule of the first-degree relatives group were significantly lower than those of the control group (P<0.05).Conclusion As the widespread ALFF, fALFF abnormalities of brain in MDD and first-degree relatives of MDD,a hypothesis can be get that these abnormal brain region may be associated with cognitive network disorders and emotional distress in MDD.
9.Effect of Bobath Approach Applied in Early Stage on Hemiplegia after Cerebral Infarction

Objective To observe the effect of Bobath approach applied in early stage on hemiplegia after cerebral infarction. Methods 174 cases of cerebral infarction were treated with Bobath approach and assessed using Fugl-Meyer Assessment (FMA) and Modified Barthel Index (MBI), and the patients who accepted rehabilitation within or over a month after stroke were compared. Results After treatment the scores of FMA and MBI were improved more in the patients within a month than those over a month(both P<0.01). Conclusion The Bobath approach is effective on the locomotor function and the activity of daily living in patients with cerebral infarction. The earlier it starts, the better the effect should be.
10.Influence of magnetic Fe3O4 nanoparticles in expressions of Caveolin-1 and Clathrin Heavy Chain proteins in organ tissues of rats

Objective:To investigate the influence of magnetic Fe3O4 nanoparticles in the expressions of Caveolin-1 and Clathrin Heavy Chain proteins in organ tissues of the rats, and to clarify its mechanism.Methods:Twenty-four Wistar rats were randomly divided into control group, and low,medium and high doses of magnetic Fe3O4 nanoparticle groups by weights.24 h after tail vein injection of different doses of magnetic Fe3O4 nanoparticles, the organ tissues were obtained.Western blotting method was used to detect the expressions of Caveolin-1 and Clathrin Heavy Chain proteins in the main organ tissues of the rats.Real-time fluorescent quantitative PCR was used to measure the expressions of Caveolin-1 and Clathrin Heavy Chain mRNA.Results:Compared with control group,the expressions levels Clathrin Heavy Chain protein and mRNA in liver and spleen tissues of the rats in medium and high doses groups were significantly increased (P<0.05).The expression level of Clathrin Heavy Chain mRNA in kidney tissue in high dose group was higher than those in other groups(P<0.05).The Caveolin-1 expression levels in the organ tissues of rats had no statistical differences between various groups (P>0.05).Compared with control group,the Caveolin-1 mRNA expression levels in liver, spleen, and lung tissues of the rats in low,medium and high doses groups were significantly increased (P<0.05);there were no statistical differences in the expression levels in kidney tissue of the rats between various groups(P>0.05).Conclusion:Magnetic Fe3O4 nanoparticles could enhance the expressions of Clathrin Heavy Chain in the liver, spleen, and lung tissues of the rats.Endocytosis of Clathrin Heavy Chain protein is one way for magnetic Fe3O4 nanoparticles into the liver, lung, spleen cells of the rats.
