1.The experimental study and preliminary clinical application of C_7 nerve transfer to restore the lower limb function of paraplegia

Objective Based on investigating the results of brachial plexus nerve transfer to repair the femoral nerve in rats, C7 nerve root transfer to femoral nerve autograft to reconstruct the function of quadriceps femoris of paraplegia was designed and evaluated clinically. Methods 16 SD rats were randomized into 2 groups. Group A: the radial nerve of the left side was severed, and the proximal end of the radial nerve was transferred to the femoral nerve of the lower limb on the same side through the vascularized sciatic nerve bridging in one stage; Group B: the radial nerve anastomosed with the bridging nerve at first, then 3 months later the femoral nerve was sectioned, and the distal end of which anastomosed with the bridging nerve. Electrophysiology and histology including amplitude of compound action muscle potential, muscle wet weight, cross section areas were used to evaluate the effects at sixth month after operation. Furthermore, two patients with paraplegia were treated by the C7 nerve root transfer to reconstruct the function of quadriceps femoris. The C7 was sectioned at the distal of nerve root , and proximal end was used for motor nerve, the vascularized tibial nerve sectioned at the middle of the crus and dissected to the rump along the posterior of lower limb was reversed to the ipsilateral cervical incision through the tunnel and used for bridging nerve to anastomose with the proximal end of C7. When the nerve fiber regenerated into the hip through bridging nerve, the femoral nerve was sectioned and distal end anastomosed with bridging nerve. Results There was no significant difference between the two groups in the results of nerve regeneration and amplitude of compound action muscle potential, but group B was superior to the group A in muscle net weight and section area of muscle fiber. One case suffered paraplegia had already been repaired the double side femoral nerve by the method, after 41 months follow up, the muscle power of quadriceps was 4 grade on the right side and 3 grade on the left. The other was followed up for 24 months, the muscle power of quadriceps was 2 grade on both sides. Conclusion C7 nerve transfer to repair the femoral nerve through the vascularized tibial nerve bridging in two stages could reconstruct the movement and sensation function of lower limb of paraplegia.
2.Inhibitory effect of Fluor Protector on demineralization of bovine enamel caused by beverages

Objective: To investigate the inhibitory effect of Flu or Protector on the demineralization of bovine enamel caused by some beverages. Methods: Forty bovine teeth were equally divided into experimental and control groups, each of them were subdivided into two groups. The enamel sp ecimens of the experimental group were pretreated with Fluor Protector and then exposed to carbonated drinks and fruit juice respectively,the enamel specimens o f control group were exposed to beverages directly. All the enamel specimens wer e exposed to beverages 5 times a day and for 5 minutes each time. After 7- day exposure,the concentration of calcium and phosphate in the beverages were measu red by EL 312e Micro-Plate. Results: The concentration of calci um and phosphate in the corresponding beverage of experimental group was lower t han that in control group in the same expossure time(P0.05).Conclusions: Fluor Protector can inhibit the demineralization of bovin e enamel caused by beverages.The inhibitory effect of Fluor Protector decreases as the exposure time prolonges.
3.Impact of GSTM1 and GSTP1 gene polymorphisms on erythrocyte GST activity

Aim The current study was conducted to investigate the effect of GSTP1 codon 105 polymorphism, alone and in combination with GSTM1-deletion polymorphism, on erythrocyte GST activity in 196 Han Chinese. Methods GST activity was measured in healthy Chinese by a spectrophotometric method (n=196;101 males and 95 females; age range 21~81 years; median 43.5 years). GSTM1 polymorphisms were analyzed by a PCR-Multiplex procedure, whereas GSTP1 polymorphism was analyzed by PCR-RFLP. Results The frequency of GSTM1 null genotype was 56.1% and the frequency of I/I, I/V, and V/V genotypes was 60.7%, 35.2% and 4.1%, respectively, in Han Chinese. The mean erythrocyte GST enzyme activity for I/V genotype group(3.53?0.63 U?g -1Hb) was significantly lower than that for I/I genotypes (4.25?1.07 U?g -1Hb, P=0.000), while significantly higher than that for V/V genotypes (2.44?0.67 U?g -1Hb, P=0.004). In GSTM1(-) group, the GST activity of carriers of GSTM1(-)/GSTP1- I/I is significantly higher than that of GSTM1(-)/GSTP1- I/V or-V/V, however, in GSTM1(+) group, there is no difference between different subgroups. There was no significant difference in the mean GST activity among different age groups. Erythrocyte GST activities were significantly higher in females than in males, but not significantly. Conclusion The GST activity measured by CDNB-based assay is probably strongly correlated with the GSTP1 105Val genotype, although other GST enzymes would tend to dilute the GSTP1 genotype effect.
4.Application of fast-track surgery in perioperative period of colon cancer: a randomized controlled clinical trial

Objective This study was designed to compare the safety and efficacy of fast track surgery (FTS) with conventional perioperative management in colonl cancer patients undergoing radical resection.Methods From May 2011 to May 2013,66 patients with colorectal cancer were randomly assigned to FTS group and control group.Outcomes were assessed by length of postoperative hospital stay,medical cost,nutritional status,gut function,and postoperative complication.Results FTS group was associated with a significantly shorter postoperative hospital stay,lessmedical cost,earlier first passage of flatus,less loss of body weight in the postoperative period compared with control group,and there were no differences in morbidity or mortality between two groups.Conclusion The radical resection under the guidence of fast track rehabilitation is safe and effective method.Significantly reduce patients' perioparative stess response and accelerate the recovery.
5.Application of Traditional Chinese Medicine and Psychological Theory to Scale Development of Social Adaptation Degrees of Special Service Personnel

Objective To develop a scale of social adaptation degrees of special service personnel (referred to as scale) applying Traditional Chinese Medicine and psychological theory and to assess its reliability and validity. Methods By literature review, job analysis, inventory survey, topics interview, this scale was determined in dimensions, items and items pool based on theoretical model. Results After specialist important analysis and combining the different results of statistical analysis, including Cronbach's α coefficient, resolution factor, correlation analysis and confirmatory factor analysis, 6 dimensions, 14 factors and 40 items were finally kept in the scale. The dimensions were psychological characteristics, physical condition, intelligence level, interpersonal relationships, environmental factors and moral character. The factors were self-control, self-esteem character bias, self-confidence, physiological function, learning intelligence, creating intelligence, leadership intelligence, gregariousness, cooperative, confidence feeling, natural environment, social environment and moral character. Conclusion This scale was made on the basis of general international principle, it had good reliability and validity meeting measurement standard of traditional Chinese medicine and psychological theory, and it is an effective and practical evaluation tool of social adaptation degrees of special service personnel.
7.Study on the absorption of monosialotetrahexosylganglioside sodium in rats with in situ cycle intestinal perfusion methods

Objective To investigate the duodenum absorptive character of monosialotetrahexosylganglioside sodium (GM1) in rats.Methods The contents of phenolsulfonphthalein (as indicators) and GM1 were determined with ultraviolet-visible (UV) method and high performance liquid chromatography (HPLC) method in rats with in situ cycle intestinal perfusion model.Results The ratio of duodenum absorption of GM1 was 10% in 2 h after cycle and 22% in 6 h after cycle,respectively.The Ka was (0.030± 0.012)h,and absorption t1/2 was (25.50 ± 8.56)h in 8 h after cycle.Conclusions GM1 is absorption in rat duodenum,and the accumulate absorption of GM1 is almost linearly related to the cycle time.The absorption dynamics of GM1 may be first-order kinetic process.
8.Clinical findings of 16 cases of neonatal influenza

Objective To investigate the clinical features, laboratory diagnostics, treatments, and prognosis of neonates infected with influenza. Methods The clinical data of 16 neonates diagnosed as influenza admitted to the neonatal ward from January 2015 to May 2017 were retrospectively analyzed. Results Of the 16 neonates, 11 were male and 5 were female. Mean age was 18.5 days. 75% (12/16) of them were reported to be exposed to family members with common cold- like symptoms before hospitalization. Clinical manifestations included nasal obstruction (11/16), fever (10/16), cough (10/16), and rhinorrhea (8/16). Influenza antigen rapid detection (colloidal gold method) was positive in all cases. Influenza immunofluorescence assays were performed in 15 cases, only 6.67% (1/15) was positive. Sputum culture was performed in 13 cases, 8 of which were positive. Of them, 75% (12/16) neonates were diagnosed with pneumonia. Only 12.5% (2/16) neonates were treated with neuraminidase inhibitor. All cases recovered well and were discharged after antibiotic treatment. Conclusions Neonates contacted with family members displaying common cold-like symptoms should be examined for influenza in time. The common clinical manifestations include catarrhal symptoms, fever and cough. The sensitivity of the influenza immunofluorescence assay is lower as compared with the colloidal gold method. Pneumonia may often be developed in neonatal influenza. The prognosis of neonatal influenza is satisfactory if treated.
9.Clinical experience of laparoscopic splenectomy in 62 cases of traumatic spleen rupture

Objective To analyze the safety of laparoscopic splenectomy for traumatic splenic rupture.Methods The clinical data of 62 patients with traumatic splenic rupture treated by laparoscopic splenectomy in our hospital from Jan 2013 to Jan 2018 were retrospectively analyzed.Results All 62 patients successfully underwent laparoscopic surgery except for 1 case who was converted to open surgery.One case suffered from postoperative bleeding.There was no infection,serious pancreatic leakage and other major complications.The total amount of intraoperative bleeding was 1 600 ml on average,the average operation time was 135 min,the average splenectomy time was 55 min,and the average hospital stay was 12.5 d.Conclusion Laparoscopic splenectomy is a safe and reliable minimally invasive operation for traumatic splenic rupture.
10.Clinical analysis of 8 cases of neonatal hand-foot-mouth disease

Objective To study the clinical features,treatment and prognosis of neonatal hand-foot-mouth disease.Methods Neonatal hand-foot-mouth disease patients admitted from July 2014 to May 2016 were retrospectively studied.EV71,CA16 and universal enterovirus were detected from newborn swabs by RT-PCR.Results A total of 8 neonatal patients with hand-foot-mouth diseases,6 boys and 2 girls were diagnosed at an age ranged from 7-26 days.Of them,7 patients had exposure history.Clinical manifestations including rash (8 cases),fever (6 cases),irritability (4 cases) and vomiting (1 case),nobody suffered convulsions.The results of RT-PCR showed pathogen enterovirus not-EV71 and not-CA16.There were 5 cases of pulmonary infection treated with antibiotic therapy.All of the 8 cases had a good prognosis.Conclusions Most of the handfoot-mouth diseases had exposure history without specific clinical manifestations.They had not-EV71 and not-CA16 intestinal virus infection,some patients had pulmonary infection.Therefore,neonatal hand-foot-mouth diseases need more clinical attention.
