1.Expression of cytoglobin gene in hypoxic-ischemic brain damage of newborn rat

Objective To investigate the expression and change of cytoglobin(Cygb)gene on hypoxicischemic brain damage(HIBD).Methods Fifty SD rats of 7days old were divided into four study groups and one control group.The brain tissues were taken at 4hours,12hours,24hours and 48hours after the onset of HIBD.Cygb mRNA was determined by the reverse transcription PCR.One-way method of GraphPad Prism was used for statistics.Results The fragment length of PCR products was identical with experimental design.The expression level of Cygb gene increased at 4h after ischemia,and peaked at 24h.48h after HIBD,the Cygb gene level began to decrease.Conclusion The expression of Cygb in brain tissue increased rapidly after HIBD of newborn rats,suggesting that Cygb may have important function in the protection process of HIBD.
2.Effect of Acupuncture of Connecting Meridians on Muscle-tension after Cerebral Infarction

Objective To observe the effect of connect meridians inducing Yin from Yang on muscle-tension after cerebral infarction. Methods 82 patients with cerebral infarction in Second Hospital of Harbin from 2010 to 2014 were divided into observation group (n=41) and control group (n=41) according to hospitalization order. The observation group accepted acupuncture of connecting meridians inducing Yin from Yang, and the control group accepted the traditional acupuncture method for 2 weeks. The situation of spasticity, the motor limb function of limbs, and activities of daily living were evaluated with modified Ashworth Scale (MAS), Clinic Spasticity Index (CSI), Fugl-Meyer Assessment (FMA) and Barthel index (BI), respectively. Results Compared with the control group, the score of MAS decreased (t=2.212, P=0.030), the score of CSI decreased (t=2.514, P=0.014), the score of FMA increased (t=4.288, P<0.001), and the score of BI in-creased (t=2.970, P=0.004) in the observation group after treatment. Conclusion The acupuncture of connecting meridians inducing Yin from Yang can reduce the muscle-tension of limbs, improve the motor function and increase the activities of daily living.
3.Advances in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is the most common hereditary cerebral small vessel disease.NOTCH3 missense mutation causes its coded cysteine occurring odd change and then affects the conformation and function of protein of NOTCH3.The abnormal NOTCH3 protein has vascular smooth muscle toxicity and finally deposits in the cerebral small blood vessels and causes the disease.Usually,CADASIL can be suspected by its typical clinical manifestations and neuroimaging findings.Its diagnosis needs genetic testing or skin biopsy to find the outer granular osmiophilic deposits of small vascular smooth muscle cells or immunohistochemical NOTCH3-ECD staining positive.For nearly two decades,the studies on genetics,pathogenesis,clinical manifestations,and diagnostic techniques of CADASIL have made great progress,however,many important questions have not been fully clarified and have new discoveries,such as the NOTCH3 gene mutation pattern and loci,and the relationship between gene phenotype and clinical phenotype,optimization of diagnosis process,depth study of pathogenic mechanism,exploration of new discoveries,new therapeutic targets and concepts.This article reviews the genetic characteristics,pathogenesis,and clinical diagnosis and treatment technology of CADASIL.

5.Distribufion and significance of IgG subclasses of serum antithyroglobulin antibody in Hashimoto thyroiditis

objective To evaluate the distribution and significance of IgG subclasses of antithyroglobulin antibody in sera from patients with Hashimoto thyroiditis.Methods Sera from 112 patients with Hashimoto thyroiditis were collected and patients were divided into 3 groups,i.e.hypothyroidism,subclinical hypothyroidism and euthyroidism.Antigen specific ELISA was used to detect the distribution of IgG subclasses of antithyroglobulin antibody.Results The positive rates of IgG subclasses of TgAb were IgG1 90.2%,IgG2 58.0%,IgG3 19.6%and IgG4 87.5%respectively.The mean geometric titers of IgG1 in sera from patients with hypothyroidism and subclinical hypothyroidism were 1:450.8 and 1:245.5 respectively,both being significantly higher than that with euthyroidism(1:8.7,P<0.01).The mean geometric titers of IgG2 in sera from patients with hypothyroidism and subclinical hypothyroidism were 1:37.3 and 1:3.2 respectively,both being also significantly higher than that with euthyroidism(1:0.2,P<0.01 and P<0.05 respectively)and that with hypothyroidism was significantly higher than that with subclinical hypothyroidism(P<0.05).Conclusion The distribution of IgG subclasses of antithyroglobulin antibody in sera from patients with Hashimoto thyroiditis was predominantly IgG1,IgG2 and IgG4.High titers of IgG1 and IgG2 implicated the possibility of development from subclinical hypothyroidism to overt hypothyroidism.
6.Effects of changes of risk factors in PCI patients on the incidence of cardiac adverse events

Objective To investigate changes of risk factors in patients after percutaneous coronary intervention ( PCI) , and analyze the relationship between it and the risk of occurrence of adverse cardiac events. Methods Totally 868 patients after PCI in the follow-up database center of a tertiary hospital in Harbin from March to September 2012 were selected as the research object. 2 years after surgery, the changes of blood lipid, body mass index (BMI), smoking, blood pressure and other risk factors were analyzed.Results There was no significant change in BMI and systolic blood pressure 2 years after PCI compared with prior, while low-density lipoprotein cholesterol ( LDL-C) , triglyceride ( TG) and total cholesterol ( TC) significantly decreased compared with preoperative, and high-density lipoprotein cholesterol (HDL-C) increased (P<0.05). In addition, risk stratification of adverse cardiac events was in the low-risk and intermediate-risk group 2 years after PCI, and it was significantly higher compared to the previous, and percentage of high-risk group and very high-risk group decreased (P<0.05).Conclusions The change of risk factors in PCI patients leads to the reallocation of risk stratification of cardiac adverse events, which reminds that medical staff should adhere to strengthening long-term secondary prevention interventions after PCI, to improve the quality of life of patients.
7.Applications and effect evaluations of continued nursing care in functional rehabilitation after limbs intra-articular fracture operation

Objective To explore the influences of continued nursing care on the recovery of intra-articular fractures of joint function of limbs and the quality of life after operation. Methods A total of 493 intra-articular fracture of limbs postoperative patients,who were discharged from our department from January 2015 to December 2016,were randomly divided into observation group and reference group.After operation,the reference group used self-management mode to practice joint functional exercise,while the observation group uses continued nursing mode to guide physical therapy to recover joint functions.After they discharged 12 weeks,we compared these two groups of patients with satisfaction of nursing care,joint function recovery and quality of life. Results After 12 weeks of operation, in observation group, the patients' joint function recovery rate was close to 79.27%(195/246), which was significantly higher than the rates 55.47%(137/247)in reference group,the difference was statistically significant(χ2=32.947,P<0.01). Respectively, the physiological function score,vigor score, mental health score, physiology role score, body pain score, health condition score, social function score and health score of the observation group in the quality of life assessment were (72.66 ± 4.41), (89.27 ± 4.10), (93.01 ± 3.05), (88.03 ± 3.19), (91.68±3.99),(76.78±4.86),(79.11±4.68),(85.57±7.07).The scores of control groups were(65.71±3.41),(64.02±4.51),(61.43±4.29),(66.49±4.52),(64.24±4.30),(62.02±6.24),(62.94±4.50),(65.02±7.72).The comparative difference of the two groups has statistical significance (t=12.48- 60.15, P<0.05). Meanwhile,in the satisfaction rate of nursing care,the great satisfaction rate of observation group(70.73%, 174/246) was much higher than reference group (37.65%, 93/247), the difference was statistically significant (χ2=59.789, P<0.01). Conclusion Offered outside continued nursing care to the intra-articular fracture of limbs postoperative patients can effectively improve the joint function of patients,their qualities of life and can considerably increase satisfaction of nursing care. Thus,it should to be clinically promoted.
8.Application of FOCUS-PDCA Cycle Management in Reducing the Incidence of Irrational Medical Orders of Parenteral Nutrition

OBJECTIVE:To reduce the incidence of irrational medical orders for parenteral nutrition,and promote the rational use of parenteral nutrition. METHODS:The medical orders for parenteral nutrition of the first quarter of 2016 in general surgery de-partment of our hospital were collected,and the number and types of its irrational medical orders were summed up. Then FO-CUS-PDCA(Find-organize-clarify-understand-select-plan-do-check-act)cycle management was adopted to analyze and improve the existing problems in issuing medical orders for parenteral nutrition. The improved(the third quarter of 2016)medical orders for par-enteral nutrition were collected,the number and types of its irrational medical orders were summed up,and management effect was evaluated. RESULTS:Establishing nutrition support group,strengthening the training and communication of medical staff,adding prescription evaluation module for parenteral nutrition in hospital information system and a number of measures had made the inci-dence of irrational medical orders for parenteral nutrition in general surgery department declined from 48.25%(1433/2970)before improvement to 5.67%(120/2118)after improvement. The incidences of cation excess,inappropriate selection of drugs and inap-propriate compatibility in irrational types were 0. CONCLUSIONS:FOCUS-PDCA cycle management can reduce the irrational rate of medical orders for parenteral nutrition and promote the rational use of parenteral nutrition in hospital.
9.Clinical analysis of 39 cases of retroperitoneal paraganglioma

Objective:To summarize the clinicopathologic features and clinical diagnosis and treatment experience of retroperitoneal paraganglioma.Methods:This study retrospectively analyzed the clinical, pathological and follow-up data of 39 patients admitted to the First Affiliated Hospital of Xi'an Jiaotong University from 1 Oct 2012 to 1 Oct 2022 for retroperitoneal paragangliomas undergoing resection.Results:There were 19 males and 20 females with tumor being functional in 11 cases (28%) and non-functional in 28 cases (72%). CT angiography showed that the tumors were distributed around the abdominal aorta and inferior vena cava in most cases. All 39 patients underwent tumor rescetion.Patients in laparoscopic group had shorter operation time and postoperative hospital saty compared with open sugery [(135±66)min vs. (194±67)min, t=-2.529, P=0.016; (6.6±2.2)d vs.(9.6±4.8)d, t=-2.096, P=0.043], while there was no statistically significant difference between the two groups in terms of intraoperative blood loss [(152±151)ml vs. (361±608)ml, t=-1.169, P=0.250]. There were no major postoperative complications in the laparoscopic group, and pulmonary infection in 1 case and intestinal obstruction in 1 case in the open group. Thrity-six cases were followed up, ranging from 2 to 115 months, 1 patient in the laparoscopic group died 1 year after surgery due to recurrence and metastasis. In the open group, 1 case recurred 2 years later and was discharged after the second operation, and 1 case died of recurrence 2 years after surgery. Conclusions:Surgery is indicated for retroperitoneal paraganglioma. Adequate perioperative management is the key to the success of the operation. Laparoscopic surgery is superior to open surgery in terms of operation time and postoperative recovery .

Objective:To determine the relative correction factor of pre-vitamin D and simplify the calculation method of vitamin D assay.Methods:By studying the calculation method of vitamin D content in drug standards of various countries,HPLC was used to determine the relative correction factor of pre-vitamin D,and the influencing factors of determination were investigated.Results:The relative correction factors of pre-vitamin D at 254 nm and 265nm wavelength were determined by statistical analysis of 7 laboratories in China.Conclusion:Using the pre-vi-tamin D relative correction factor method to calculate the total amount of vitamin D simplified the experimental steps can be simplified by the pre-vitamin D relative correction factor method to calculate the total amount of vitamin D and the random operating errors can be avoided.The method is rapid and accurate,and lay a solid foundation for further improving the standard of vitamin D preparations.
