1.Effects of adjusted proportional macronutrient intake on serum uric acid,blood lipids,renal function,and outcome of patients with gout and overweight

Objective To study the effects of adjusted proportional macronutrient intake on serum uric acid,blood lipids,renal function,and nutritional status of patients with gout and overweight.Methods Sixtyseven patients with gout and overweight were randomized into high protein group and low purine group for 6 months.Each patient had had at least one gouty attacks during the six months before enrollment.Dietary recommendations consisted of calorie restriction to 6276 kJ per day with 40% derived from carbohydrate,30% from protein,and 30% from fat,and the refined carbohydrates were replaced with complex ones and saturated fats with mono-and polyunsaturated ones.High protein group didn't limit purine intake.The two dietary consisted of isoenergetic (6276 kJ).Dietary of low purine group consisted of 60% derived from carbohydrate,10% from protein,and 30% from fat,and limited of parine < 150 mg/d.At onset and 6 months after intervention,fasting blood samples were taken for determination of uric acid(UA),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),fasting plasma glucose(FBG),serum creatinine(SCr),blood urea nitrogen(BUN),albumin(ALB),total protein(TP),and hemoglobin(Hb);the number of gout attacks was recorded.Results Sixty-one persons completed the study.Dietary measures resulted in weight loss[(65.75 ± 3.26)vs(69.31 ± 7.78)kg,P =0.043)and a decrease in the frequency of sixmonth gout attacks(17 vs 28,P =0.000).After the trial,compared to low-purine group,UA[(420.25 ±36.78)vs(466.81 ±41.97)μmol/L,P=0.000],TC[(5.69±0.46)vs(6.49±1.04)mmol/L,P=0.007),TG[(2.15 ±0.54)vs(3.13 ±0.62)mmol/L,P =0.000]and FBG[(5.63 ±0.82)vs(6.49 ±0.64)mmol/L,P =0.000]decreased significantly in the high protein group.There were no significant differences in LDL-C[(3.55 ±0.80)vs(3.73 ±0.72)mmol/L,P =0.445],SCr[(96.63 ± 16.97)vs(94.28 ±13.35)μmol/L,P =0.597],and BUN[(4.89 ± 1.25)vs(4.46 ± 1.21)mmol/L,P =0.208]between two groups.HDL-C[(1.24±0.22)vs(0.89±0.26)mmol/L,P=0.000],ALB[(41.56±5.37)vs(37.08±5.94)mmol/L,P=0.014],TP(72.97 ±5.41)vs(67.27±7.87)g/L,P=0.005],and Hb[(142.86±12.31)vs(114.25 ± 12.98)g/L,P =0.001]increased significantly in the high protein group.Gouty attacks were reduced and decreased by 48.48% and 22.22% in high protein group and low purine group,respectively.The differences between the two group was statistically significant(P =0.000)Conclusion Change in proportional macronutrient intake is helpful to reduce body weight and thus is beneficial for lowering UA,improving lipid metabolism and nutritional status,and decreasing the frequency of gout attacks.
2.Expression of interleukin-8 protein in villi and decidua of patients with recurrent spontaneous abortion

ObjectiveTo study the expression of interleukin-8 (IL-8) protein in villi and decidua of patients with recurrent spontaneous abortion(RSA). MethodsThe local and quantitative expressions of IL-8 in villi and decidua of 30 RSA patients were shown and measured by immunohistochemistry and Western blotting. Results IL-8 protein immunohistochemical signals were located in the chorioepithelium cytoplasm and decidual cells plasma in RSA group. While in control group the immunohistochemical staining was negative in decidul cells, but positive in glandular epithelium. The quantitative analysis of IL-8 protein by Western blotting, intensity of the immunostaining was higher in RSA group than that in the control group. Conclusion The higher expression of IL-8 in villi and decidua is releated to RSA, IL-8 might take part in the pathologic process of RSA.
4.Effects of whole grain-bean mixed staple food on intestinal microecology and metabolic parameters of obese people

Objective To investigate the effects of whole grain-bean mixed staple food on intestinal microecology,anthropometric,and metabolic parameters of obese people.Methods Totally 87 obese people were divided with random number table into whole grain-bean mixed staple food group (test group) and refined grains staple food group (control group).Body weight (BW),body mass index (BMI),waist circumference (WC),hip circumference (HC),waist/hip ratio (WHR),fasting plasma glucose (FPG),triglyceride (TG),total cholesterol (TC),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C),and colony count of intestinal flora (bacillus bifidus,lactobacillus,clostridium perfringens,enterobacteria,enterococcus,bacteroides) were measured at baseline and 3 months after intervention.Results Ten people were excluded from this research,6 in the test group and 4 in the control group.Mter 3 months of dietary intervention,BW,BMI,WC,HC,and WHR were significantly lower in the test group than in the control group [(69.45 ± 7.07) kgvs.(72.42 ±5.79) kg,P=0.000; (26.08 ±1.48) kg/m2 vs.(27.27 ±0.81) kg/m2,P=0.000; (82.48±9.30) cm vs.(86.96±4.93) cm,P =0.000; (90.08 ±6.57) cm vs.(92.42 ±6.67) cm,P =0.000; 0.92 ±0.11vs.0.95 ±0.10,P =0.003].The levels of FPG,TC,and LDL-C in the test group were significantly lower than those in the control group [(4.92 ± 0.75) mmoL/L vs.(5.41 ± 1.21) mmoL/L,P=0.037; (3.85±1.13) mmoL/L vs.(4.38 ±1.26) mmoL/L,P=0.046; (3.55 ±1.04) mmoL/L vs.(4.16 ± 1.40) rnmoL/L,P =0.033] ; the level of HDL-C in the test group was significantly higher than that in the control group [(1.13 ± 0.37) mmoL/L vs.(0.92 ± 0.26) mmoL/L,P =0.004].The colony counts of intestinal bacillus bifidus,lactobacillus,and bacteroides in the test group were significantly higher than in the control group [(7.94 ± 0.58) lg CFU/g vs.(6.95 ± 0.38) lg CFU/g,P =0.000 ; (7.67 ± 0.46) lg CFU/g vs.(6.96 ± 0.57) lg CFU/g,P =0.000 ; (5.53 ± 0.44) lg CFU/g vs.(4.80 ±0.23) lg CFU/g,P =0.000],while the colony count of clostridium perfringens was significantly lower than in the control group [(4.40 ± 0.49) lg CFU/g vs.(5.11 ± 0.63) lg CFU/g,P =0.000].Conclusions In obese people,whole grain-bean mixed staple food can improve anthropometric parameters,some lipid metabolic parameters,and intestinal flora.The underlying mechanism may involve promoting the survival and proliferation of probiotics,thereby improving glucose and lipid metabolism.
5.Prognostic significance of serum cystatin C in multiple myeloma

Objective To investigate the prognostic significance of serum cystatin C in multiple myeloma (MM).Methods 160 cases of newly diagnosed MM patients with average age of 61.38 years old.Determine the levels of serum cystatin C,serum creatinine,β 2-microglobulin,lactate dehydrogenase and hemoglobin before the treatment; median follow-up of 38 months,observe the relationship between serum cystatin C with overall survival (OS) and event-free survival (EFS).Results Median serum cystatin C level in 160 MM patients was higher than that in the 80 healthy controls [(0.96 ± 0.32) mg/L vs (0.71 ± 0.16) mg/L,P < 0.000].All the patients in ISS stage were divided into 3 stages.Median serum cystatin C levels significantly increased in higher ISS stages [stage Ⅰ (0.70±0.13) mg/L,stage Ⅱ (0.87±0.16) mg/L,stage Ⅲ(1.23±0.33) mg/L (P < 0.05)],serum cystatin C level was positively correlated with levels of serum creatinine (r =0.669,P < 0.000),β2-microglobulin (r =0.672,P < 0.000).lactate dehydrogenase (r =0.521,P < 0.000),and negatively correlated with hemoglobin levels (r =-0.436,P < 0.000).Using ROC analysis,patients with serum cystatin C levels > 0.95 mg/L had significantly shorter EFS and OS patients with serum cystatin C levels ≤0.95 mg/L (median EFS:30 vs 57 months,P < 0.05; median OS:43 vs 68 months,P < 0.05).Conclusion Serum cystain C is not only a sensitive marker of kidney damage,but also reflects tumor burden and delivers prognostic information in MM.
6.Analysis on the Results of Blood Lead of Children in Panyu,Guangzhou

Objective To understand the children health care and the prevention of lead pollution in Panyu in the future by re-search on the detection result of blood lead from Panyu children during 2011 and 2012.Methods The detection of whole blood lead was by atomic absorption spectrometer from body examination in 6 308 children in Panyu Center Hospital be-tween 2011 and 2012:①The blood lead result from assigned children were divided into five age groups:infancy group (1~12 months,1 511 cases),toddler age group (1~2 years,1 558 cases),preschool age group (3~6 years,1 704 cases),school age group (7~11 years,1 076 cases)and adolescence group (12~18 years,459 cases).②Based on different age group,each group was separated by male and female.③According to the detection year,each group was also put into 2011 and 2012 re-spectively.Results The overall level of children blood lead in Panyu,Guangzhou in 2011 and 2012 was 37.01±29.18μg/L, and the total poisoning rate was 1.2 9%.Along with the growth of age,the overall level of children blood lead was in up-wards trend but the poisoning rate kept in a stable level beginning from infant stage.The blood lead level of boys in Panyu was 38.87±30.18μg/L,and the poisoning rate was 1.91%;the blood lead level of girls was 34.13±27.32μg/L,and the poisoning rate was 1.29%.The blood lead levels and the poisoning rates of boys were higher than girl’s in most of age groups.Conclusion Compared with other big cities like Guangzhou,the blood lead level of children in Panyu was declined but it’s not the standard performance,and there’s a certain extent of lead pollution.So in addition to reinforce pollution con-trol from government,parents and children should enhance the knowledge,education and precaution of lead poisoning.
7.Clinical analysis of foreign body ingestion of button cell in children.

OBJECTIVE:
To improve diagnosis and treatment level of foreign body ingestion of button cell in children.
METHOD:
Among the 14 cases with foreign bodies ingestion of button cell, 6 cases in esophageal, 7 cases in stomach and lower gastrointestinal tract, 1 case with multiple foreign bodies both in esophagus and stomach. Seven cases in stomach and lower gastrointestinal tract were closely observed without special treatment, 6 cases in esophageal, underwent esophagoscopy and removal of foreign body under general anesthesia, 1 case with multiple foreign bodies in esophagus and stomach underwent esophagoscopy and electronic gastroscopy and then removal of foreign body. All patients had symptomatic treatments after removal of foreign body. The average age is 2 years and 1 month.
RESULT:
After the removal of button cell, Foreign bodies in stomach and lower gastrointestinal tract in 7 cases were all discharged spontaneously. For the 6 cases in esophagus, 4 cases were cured, 1 case had esophageal stricture, 1 cased died. 1 case with multiple foreign bodies in both esophagus and stomach was cured.
CONCLUSION
Esophageal foreign body has strong corrosiveness and would cause serious damages even to threaten children's life. It also may lead to esophageal stricture. The key point to cure this disease is to diagnose timely and to operate as soon as possible. Because neutralize of digestive juices and low probability of incarceration, with close observation, the foreign bodies of stomach and lower gastrointestinal tract can discharge spontaneously.

8.Cell-penetrating peptide PEP-1 mediated transmembrane delivery of enhanced green fluorescent protein in vivo of mouse

Objective To investigate the in vivo transduction capability of fusion protein PEP-1-EGFP with mice.Methods Two prokaryotic expression plasmids pET15b-EGFP and pET15b-PEP-1-EGFP were constructed and transformed into E.coli BL21(DE3) to express EGFP and fusion protein PEP-1-EGFP,respectively.The expressed EGFP and PEP-1-EGFP were purified with Ni2+-resin affinity chromatography.Five hundred micrograms of EGFP and PEP-1-EGFP fusion protein were injected into mouse through caudal vein,respectively,the mice were euthanized and perfused with PBS 2 hours after administration.Then,the heart,brain,liver,spleen and kidney were removed and sectioned with a cryostat at 7 ?m for visualization with a inverted fluorescent microscope.ResultsThe brain,heart,liver,spleen and kidney injected with PEP-1-EGFP showed bright and homogenous green fluorescence whereas that with EGFP showed no green fluorescence at all.Conclusion The successful expression and purification of PEP-1-EGFP fusion protein and its efficient transduction into mice in vivo provide a basis for the research on transmembrane delivery of macromolecule drugs mediated by the cell-penetrating peptide,PEP-1.
9.Feasibility of induction with sevoflurane-midazolam-remifentanil for tracheal intubation without muscle relaxants in neck brake patients

Objective To evaluate the feasibility of induction with sevoflurane-midazolam-remifentail for tracheal intubation without muscle relaxants in neck brake patients.Methods Forty ASA Ⅰ or Ⅱ patients with cervical spine fracture with dislocation,aged 13-68 yr,scheduled for surgery under general anesthesia,were enrolled in this study.Anesthesia was induced with iv injection of midazolam 0.03 mg/kg and inhalation of 5%sevoflurane through a mask.Sevoflurane was inhaled at the initial concentration of 5%,followed by decrement of 1% every 30 s until 3%.When the eyelash reflex disappeared,remifentanil 2 μg/kg was injected slowly over 45s and 30 s later sevoflurane inhalation was stopped.The patients were mechanically ventilated after tracheal intubation.The time of disappearance of eyelash reflex was recorded.The intubation condition was evaluated using VibyMogensen score.Results All patients were successfully intubated at the first attempt.The time period from sevoflurane inhalation to disappearance of eyslash reflex was(69 ± 4)s.Coughing occurred in 3 cases during intubation.The satisfactory intubation conditions were found in 100% of cases.SpO2 > 95% in all patients.BlS was maintained at 45-55 during the period(before intubation until 3 min after intubation).Conclusion Induction with sevoflurane-midazolam-remifentail is rapid and smooth,provides good conditions for intubation and can be applied to tracheal intubation without muscle relaxants in neck brake patients.
10.Effects of different surgical methods for removal of DU fragments on the uranium concentration in rat tissues

Objective To find out the best method for elimination of uranium contamination in rat tissues as low as possible by removal of shrapnel fragments. Methods Experimental rats were divided into six groups: route group, decontamination before surgery group, decontamination in incision group, changing surgical appliances group, removing tissues around group, and comprehensive method group. Uranium concentrations in tissues and fluids in all groups were measured at 7, 14, and 21 d after operation. The efficiency of decontamination by different methods was compared. Results The highest uranium concentration in tissues was found in the route group, but the lowest in the comprehensive method group, and the second lowest in removing tissues around group. Conclusion The comprehensive method is the best one in all of the surgical removal methods. The soft tissues around DU shrapnels should be removed if they are not critical organs.
