Objective: To investigate the influence of transfection of TCRVJ37.1 gene in cytotoxicity of PBMC of healthy donor to breast cancer cell line MCF-7/S. Methods :pcDN*31vp'1 packaged by lipofectamine were transfected to healthy donor's PBMC,Flow Cytometer Analysis and MTT Colorimetric Assay were used respectively to test the expression of pcDNA3.1VB7.1 gene and the cytotoxicity of PBMC before and after gene transfection of healthy donor to breast cancer cell line MCF-7/S. Results: The expression of TCRVJ37.1 gene after transfection was obviously higher.There was distinguish differences of cytotoxicity before and after gene transfection. Conclusion:The modified healthy donor's PBMC by TCR gene could have stronger cytotoxicity to breast cancer cell line.
2.An evaluation of obscure gastrointestinal bleeding diagnosed by capsule and/or push endoscopies

Objective To compare the detection rates of capsule endoseopies with push endoseo-pies. Methods From May 2002 through January 2003 , thirty - nine patients with suspected small bowel diseases, particularly the gastrointestinal bleeding of unknown origin were examined by capsule endoscopies. Of the 39 patients. 32 complained of obscure recurrent gastrointestinal bleeding. From January 1993 to October 1996, 36 patients suffered from unexplained GI bleeding underwent push endoscopies. All patients had prior normal results on gastroseopy, colonoscopy, small bowel barium radiography, seintigraphy and/ or angiogra-phy. Results M2A capsule endoscopies disclosed abnormal small bowel findings in 26 out of 32 patients (81% ). Twenty one of 26 patients had significant pathological findings in explaining their clinical disorders with diagnostic yield of 66% (21 of 32 patients). Definite bleeding sites diagnosed by capsule endoscopies in 21 patients including angiodysplasia 8, inflammatory small bowel diseases 5, small bowel polyps 4, GI stro-mal tumor 2,earcinoid tumor and lipoma 1 , and bemorrhagie gastritis 1. Push endoscopies detected the definite sources of bleeding in 9 of 36 patients (25%). Definite bleeding sources included angiodysplasia 2, leiomyosareoma 2, leiomyoma 1 , lymphoma 1 , Grohns disease 1 , small bowel polyps 1 ,and adenocareinoma of ampulla 1. Suspected bleeding sources were seen with push endoscopies in two additional patients, and other five patients with capsule endoscopies. Conclusion Gapsule endoseopy is superior to push endoscopy in detecting obscure GI bleeding ( P
3.Capsule endoscopy in diagnosing small bowel Crohn's disease

Objective To evaluate the effectiveness of wireless capsule endoscopies in patients with suspected CD of the small bowel.Methods From May 2002 through April 2003, we prospectively examined 20 suspected CD patients by capsule endoscopies with normal results in other conventional examinations. It includes the presence of the following symptoms and signs: abdominal pain, weight loss, positive fecal occult blood test, iron deficiency anemia, diarrhea and fever. Mean duration of symptoms before diagnosis was 6 5 years.Results Of the 20 participants, 13 (65%) were diagnosed as having CD of the small bowel according to the findings of the M2A Given Capsule. Among the findings detected by the capsule were mucosal erosions (2 patients), aphthae (5 patients), granulomatous nodularity (1 patient), large ulcers (2 patients), and ulceration with incomplete obstruction (3 patients). All of the 13 patients who received medications showed good clinical improvement.Conclusion Wireless capsule endoscopies were the effective device for diagnosing patients with suspected CD, undetected by conventional diagnostic measures.It may be more likely to detect early lesions in the small bowel of patients with CD.
4.The Synthetic Treatment in Vertebral Hemangioma

Objective To assess the efficacy of radiotherapy and synthetic therapy for vertebral hemangiomas.Methods Clinical symptoms,imaging findings,therapeutic methods and following up after treatment in 22 cases with vertebal hemangiomas were retrospectively analyzed.Results Complete relief of symptoms(CR)were occurred in 19 of 22 patients(86.4%) and partial relief(PR)were achieved in 3 of 22 patients(13.6%).There was no obvious complication of radiotherapy and radiation-induced neoplasms.There was no relapse.Conclusion For vertebral hemangioma,the synthetic therapy which includes intenventional therapy before operation as well as radiotherapy after operation is of obvious superiority in relieving compression rapidly,reducing hemorrhage during operation and obtaining good efficacy.
5.Multidetector computed tomography characteristics of intra-pancreatic accessory spleen

Objective To summarize the imaging features of intra-pancreatic accessory spleen (IPAS)with multidetector computed tomography (MDCT) and improve the awareness and correct diagnosis of IPAS.Methods MDCT images of seven consecutive patients with surgically and pathologically confirmed IPAS were reviewed retrospectively.The investigated features included the location,size,shape,margin,density,and enhancement of the lesions.Results Four patients were male and three were female with a mean age of 49 years old.All the lesions were located at the dorsal side of parenchyma under the capsule of pancreatic tail.Three lesions were in round-like shape,and 4 in oval shape and all were well-defined.All the lesions were mass-like without necrosis and calcification.The maximum diameter of lesion ranged from 0.9 ~ 1.8 cm with a mean value of 1.4 cm.Compared with pancreatic parenchyma,the density of lesions were homogeneous on unenhanced CT,in arterial phase,slightly increased heterogeneous density was observed in 3 patients,slightly increased homogeneous density was observed in 4 patients.All the lesions appeared as slightly increased homogeneous density in portal phase.The CT value in unenhanced phase ranged from 50 ~ 61 Hu with a mean number of 55 Hu; and it ranged from 80 ~ 110 Hu with a mean number of 97 Hu in arterial phase; and the corresponding value was from 99 ~ 120 Hu with a mean number of 102 Hu in portal phase.Among the three patients underwent MDCT angiography,neither artery nor vein was compressed or invaded,and there was no vessel connected with lesions.Conclusions IPAS has some MDCT characteristics.For small solid mass in pancreatic tail,if the density and enhancement pattern is similar to that of spleen,the diagnosis of IPAS should be considered.
6.Clinical manifestations of infant cytomegalovirus infection in different age

ObjectiveTo explore the clinical manifestations of infant cytomegalovirus infection in different age. MethodThe clinical data of 237 infants who suffered from cytomegalovirus infection was analyzed retrospectively and divided into three groups: 0-3 months old (77 cases,group A) ,4-6 months old ( 65 cases , group B ) , 7-12 months old ( 95 cases , group C ). ResultsThe incidence of respiratory infection was the highest among all infectious organs in three groups, the numbers of patients who had wheeze in group A was less than that in the other two groups[24.7%(19/77) vs. 61.5% (40/65), 61.1%( 58/95 )](P < 0.01 ).The incidence of jaundice decreased gradually as the babies grew up[23.4%( 18/77 )→7.7%(5/65 )→1.1%( 1/95 )](P < 0.05). There was no significant difference in diarrhoea and bleeding among three groups (P>0.05 ). The proportion of alanine aminotransferase], aspartate amino transferase increasing was similar among three groups, but gamma-glutamyl transferase (GGT) was different, the proportion of GGT increasing was 77.9% (60/77) in group A which was higher than that in the other two groups[10.8% ( 7/65 ), 2.1% ( 2/95 )].Granulocytopenia in group B was obviously decreased compared with the other two groups (P < 0.05 ), anemia was easily occurred in group C (P < 0.05 ). ConclusionThe injury of cytomegalovirus infection may be related to month old.
7.Clinical application of wireless capsule endoscopy in the diagnosis of obscure gastrointestinal bleeding in the elderly

Objective To evaluate the safety and effectiveness of capsule endoscopy(CE) in a large cohort of the elderly patients with obscure gastrointestinal bleeding(OGIB). Methods The demographic, clinical and diagnostic data of all geriatric patients with obscure gastrointestinal bleeding who underwent CE between May 2002 and February 2007 were retrospectively analyzed. For comparison, non-geriatric patients with obscure gastrointestinal bleeding who underwent CE during the same period were selected as the control group. Results Ninety-seven geriatric patients [40men/57 women, mean age (70.84-6.8) yrs]and ninety-nine non-geriatric patients [61 men/38women, mean age (44. 4±10. 3) yrs)were retrospectively reviewed. Eighty-nine patients presented with overt OGIB while eight patients had occult OGIB in the geriatric group. The section in the nongeriatric group was ninety-one and eight respectively. No significance was seen in the gastric transit time, completion rate and delay rate between two groups. The small bowel transit time was significantly longer in the geriatric group than that in the non-geriatric group (P<0. 05). Sixty-two patients in the geriatric group obtained positive diagnosis and angiodysplasia was the most frequent lesion;while sixty-seven patients in the non-geriatric group obtained positive diagnosis and Crohn's disease was the most frequent lesion. No significance was seen in the positive diagnostic rate between two groups (P>0. 05). However, the constituent ratio of the positive diagnosis was significantly different between two groups (P<0. 01). Conclusions CE is a safe and effective procedure for geriatric patients with obscure gastrointestinal bleeding. Angioectasia accounts for the majority of positive findings in geriatric group.
8.An analysis of failure and safety profiles of capsule endoscopy

objective To analyse the incidence of technical breakdown and clinical problem leading to the failure of capsule endoscopy examination and their influence on the diagnosis and to evaluate its feasibility and safety in special patient population.Methods A retrospective study of 300 consecutive patients referred to Renji Hospital for evaluation of suspected small bowel diseases between May 2002 and May 2006 was done.This included 300 consecutive patients.The median age of the patients was 51y (range,3~91Y).The young children group was defined as less than 10 years and the elderly group as more than 80 years.Technical problems were those related to the functioning of the equipment and clinical problems were those related to the patient.The incidence and the type of above-mentioned events and their influence on the diagnosis were analyzed.The safety and feasibility of the capsule endoscopy procedure were evaluated in the young children group,elderly group and patients with pacemakers,gastrectomy and Billroth Ⅱ gastrojejunostomy,intestinal diverticula,Crohn's disease and polyp of small intestine.Results A total of 300 patients were involved.The incidence of technical problems was 1.3%.including one case of failing in activating the capsule,one case of failing in loading the data and two cases of short life of battery.Failure of diagnosis was encountered in two cases.The incidence of clinical problems was 33.0%(99 cases)and they caused 35.4%(35 cases)failure of diagnosis in the 99 cases.Three patients in the young children group were unable to swallow the capsule and endoscope-guided overtube technique was used with success in all.In the elderly group.the incidence of capsule retaining in the oesophagus and stomach was as high as 23.0%.In two patients with pacemaker no interference between pacemaker and capsule was detected.In two patients with Billroth Ⅱ gastrojejunostomy uo capsule retention occurred.In 16 patients with diverticulum,capsule retention occurred in 1 case(6.0%).In 42 patients with Crohn's disease,capsule retention occurred in 5 cases.No acute gastrointestinal obstruction was found in the 42 patients with Crohn's disease and in 5 patients with polyp of small intestine.Conclusions With capsule endoscopy technical mistakes causing failure were very rare.The majority of the clinical problems were related to the inability capsule to reach the colon during the recording time.Capsule endoscopy provides a well-tolerated,safe and effective tool to investigate the gastrointestinal diseases.especially some small bowel diseases.
9.Effect of expression of P-glycoprotein in tumor tissue on analgesic efficacy of morphine and buprenorphine in advanced cancer patients with pain

Objective To evaluate the effect of the expression of P-glycoprotein (P-gp) in tumor tissue on analgesic efficacy of morphine and buprenorphine in advanced cancer patients with pain. Methods One hundred and fifty advanced cancer patients with pain aged 51-64 yr weighing 54-65 kg were included in this study. The expression of P-gp was negative in the tumor tissue in 50 patients (group M1 and B1, n = 25 each) and positive in 100 patients (group M2 ,M3 ,and B2 ,B3, n =25 each). The PCA regimen for the 6 groups were listed in the table .Pain was assessed with VAS scores (0 = no pain, 10 = worst pain) and venous blood samples were taken for determination of blood morphine/buprenorphine concentrations at 4, 12, 24 and 48 h of PCIA. Results The six groups were comparable with respect to age, body weight, M/F sex ratio, types of cancer, baseline pain level and education. The analgesic efficacy of morphine and buprenorphine was better ( VAS scores were significantly lower)in P-gp expression negative patients (group M1 and B1 ) than in P-gp expression positive patients (group M2 and B2 ). Higher doses of morphine and buprenorphine provided better analgesic efficacy in P-gp expression positive patients in group M3 and B3 than in group M2 and B2. Plasma morphine and buprenorphine concentrations were comparable between group M1 , B1 and M2, B2 and were significantly higher in group M3 and B3 at each time point. Conclusion Positive P-gp expression in the tumor tissue can decrease the analgesic efficacy of morphine and buprenorphine in advanced cancer patients with pain.
10.Baseline value of fraction anisotropy and apparent diffusion coefficient in deep white matter of preterm

Objective To study the baseline level of fraction anisotropy (FA) and the normal value of apparent diffusion coefficient (ADC) in deep white matter of preterm and its application.Methods From Oct.2010 to Dec.2013,in Department of Neonatology,Jiangsu Province Hospital,magnetic resonance imaging (MRI) (T1,T2) and diffusion tensor imaging (DTI) were done on 13 preterm infants of less than 37 weeks of corrected gestational age (CA),42 preterm infants of term-matched age,and 15 term infants.ADC and FA were measured in genu and splenium of corpus callosum (CC),anterior limb and posterior limb of internal capsule (IC).Results 1.The ADC values in genu,splenium,anterior limb of right IC,posterior limb of right IC,anterior limb of left IC,posterior limb of left IC in CA < 37 weeks infants were higher than those in term-matched infants and in term infants.The ADC values in the 6 regions in term-matched infants and in term infants were significantly different with those in CA < 37 weeks infant(F =5.559,5.775,21.948,19.462,30.586,15.452,all P < 0.01).The differences of ADC values between CA < 37 weeks infants and term-matched infants,between CA <37 weeks infants and term infants were significant(all P <0.05),except that in CC between CA < 37 weeks infants and term-matched infants.2 The FA values in genu,splenium,anterior limb of right IC,posterior limb of right IC,anterior limb of left IC,posterior limb of left IC in CA < 37 weeks infants were lower than those in term-matched infants and in term infants.The FA values in the 6 regions in term-matched infants and in term infants were significantly different from those in CA < 37 weeks infants (F =9.835,7.500,4.811,11.430,8.674,12.666,all P < 0.01).The differences of FA values between CA < 37 weeks infants and term-matched infants (P < 0.05),between CA < 37 weeks infants and term infants were significant (all P < 0.05).Conclusions The baseline values of FA and ADC in different deep white matters were obtained.As corrected gestational age of preterm babies' increased,FA values in brain white matter increased,while ADC values decreased.The myelination in most white matter of preterm infants at matched term can catch up with that of term infants.The diagnostic value of ADC and FA needs to be studied further.
