1.Comparative study of color Doppler flow imaging and spectral flow Doppler with pathology in breast lumps

Objective To evaluate the correlation of pathology with color Doppler flow imaging and spectral flow Doppler in breast lumps for raising the diagnostic rate of breast lumps.Methods One hundred and sixteen cases with breast lumps were examined by color Doppler ultrasonography in comparison with their pathological results.Results ① In the group of malignant lumps,peak wave of contractive period often appeared ahead; contractive waves rose and descend quickly; beginning wave of diastolic period often appeared below the middle point of descending wave of contractive period; the latest wave of diastolic period often did not appear or appeared contrast wave .② In the group of benign lumps,peak wave of contractive period often appeared normal; contractive waves rose and descend slowly; beginning wave of diastolic period often appeared above the middle point of descending wave of contractive period ;the latest wave of diastolic period often appeared.Conclusions Spectral flow Doppler is an important index in differential diagnosis of breast lumps.

3.Primary stability of mini-screw:buccal cortical thickness in maxillary posterior area of malocclusion

BACKGROUND:Mini-screw stability is primarily related to alveolar bone cortical thickness. It is necessary to learn cortical thickness to choose suitable implanting sites and predict success rate. OBJECTIVE:To evaluate the buccal cortical thickness in maxilary posterior area of Class II Division I malocclusion adolescents. METHODS: Fifty-two adolescents (including 26 male and 26 female) of Class II Division I malocclusion scanned by Cone Beam Computer Tomography were involved in this study. The buccal cortical thickness of six posterior interradicular sites (14-15, 15-16, 16-17, 24-25, 25-26, 26-27) at four bone levels (2, 4, 6, 8 mm) from the alveolar crest in both boys and girls were measured. RESULTS AND CONCLUSION: (1) There was no statisticaly significant difference in alveolar cortical thickness between left and right sides in the girls (P > 0.05); cortical thickness of 5-6 and 6-7 at 8 mm was thicker than that at 2 mm (P < 0.05) and cortical thickness of 6-7 at 2 mm was smaler than that at 8 mm (P < 0.05). The cortical thickness increased from crest to base of alveolar crest and reached the peak at 8 mm. Cortical thickness of 6-7 was thicker than 4-5 at 8 mm depth (P < 0.05). The results of males were the same as females. (2) There was no statisticaly significant difference in alveolar cortical thickness of the same site between males and females (P > 0.05). Al sites in this study were suitable for mini-screw implantation; from the first molar to posterior dental arch, buccal cortical thickness is wel-distributed; it is dangerous for implantation in the areas around the first molar more than 6 mm, where the frequent presence of the maxilary sinus is observed. There is an increase of buccal cortical thickness from crest to base of alveolar crest, obtaining good initial stability.
4.Early nutritional support for very low birth weight infants

Objective To study the role of early intravenous nutrition given aggressively combined with early minimal feeding on very low birth weight infants (VLBWI),and to evaluate the clinical value of intestinal barrier protein and MicroRNA.Methods All of 62 cases of VLBWI admitted in NICU,the Maternal and Child Health Hospital of Nantong Affiliated to Nantong University from January 2006 to June 2014 were recruited.Sixty-two VLBWI were randomly divided into group A and group B.Thirty infants in group A were exposed to conventional intravenous nutrition.Thirty-two infants in group B were treated with early intravenous nutrition aggressively combined with early minimal feeding.The time of birth weight recovery,days with intravenous nutrition,hospital stay and complications were recorded.The liver and kidney functions,electrolytes,blood gas analysis were monitored.Enzyme-linked immunosorbent method was used to detect intestinal fatty acid binding protein (Ⅰ-FABP),an intestinal barrier protein in plasma.Infection related MicroRNA155 was detected with fluorescent quantitative polymerase chain reaction (RT-PCR).Results Group B was superior to group A in weight loss after birth [(13.70 ± 3.10) % vs (5.46 ± 2.64) %,P < 0.05],shorter recovery time of body weight [(12.20 ± 3.38) d vs (6.82 ± 3.20) d,P < 0.05],fewer days with intravenous nutrition [(29.62 ± 4.16) d vs (20.80 ± 3.20) d,P < 0.05] and shorter hospital stays [(44.60 ± 6.32) d vs (28.91 ± 4.36) d,P < 0.05].Compared with group A,the infants in group B had less complications,including hyperbilirubinemia (31.2% vs 56.7%),extrauterine growth retardation (34.3% vs 73.3%),cholestasis (6.2% vs 23.3%),feeding intolerance (15.6% vs 53.3%) and necrotizing enterocolitis (0 vs 16.7%) (all P < 0.05).Although Ⅰ-FABP had a higher plasma concentration in group A than that of group B [(9.083 ± 1.059) μg/L vs (7.563 ± 0.739) μg/L],the difference was not significant (t =1.190,P =0.076 4).However,the plasma levels of Ⅰ-FABP in infants with necrotizing enterocolitis were significantly higher than those of group B [(19.500 ± 3.510) μg/L vs (7.563 ±0.739) μg/L,t =5.231,P =0.035 0].The expression of MicroRNA155 in group A was markedly higher than that of group B (2-△△ct were 0.81 ± 0.12 and 0.24 ± 0.08,respectively,P < 0.05).Conclusions Giving aggressive intravenous nutrition early combined with early minimal feeding was safety and effective to VLBWI,which was of benefit to their growth and development,reducing complications and shorting hospital stays.The detection of intestinal barrier protein Ⅰ-FABP and MicroRNA155 is useful for monitoring feeding complications of VLBWI.
5.Comparison of clinical features between radiological isolated syndrome and classical multiplesclerosis

Objective To compare the clinical features between radiological isolated syndrome (RIS) and classical multiple sclerosis (CMS), in order to improve the understanding of the RIS. Methods All 35 patients with RIS and 32 patients with CMS were selected. The epidemiological and clinical findings, cerebrospinal fluid, neural electrophysiological examination and magnetic resonance imaging (MRI) data were analyzed. Results There were no statistical differences in sex ratio and onset age between RIS patients and CMS patients (P>0.05). The main symptoms of in patients with RIS were headache (45.7%, 16/35), dizziness (40.0%, 14/35), hypomnesis (20.0%, 7/35) and psychiatric disorders (11.4%, 4/35). But the main symptoms of in patients with CMS were limb weakness (75.0%, 24/32), sensory abnormalities (68.8%, 22/32) and ocular symptoms (34.4%,11/32). The incidences of limb weakness, sensory abnormalities and ocular symptoms in patients with CMS were significantly higher than those in patients with RIS:75.0%(24/32) vs. 0, 68.8%(22/32) vs. 0 and 34.4%(11/32) vs. 0, and there were statistical differences (P<0.01). The 18 patients with RIS and 21 patients with CMS underwent the examination of cerebrospinal fluid, and there was no significant difference in leukocyte between patients with RIS and patients with CMS (P>0.05). The cerebrospinal fluid protein and the incidences of IgG index>0.7 in patients with RIS were significantly lower than those in patients with CMS:0.175 (0.03-0.69) g/L vs. 0.440 (0.04-1.09) g/L and 3/18 vs. 47.6%(10/21), and there were statistical differences (P<0.05). The 15 patients with RIS and 22 patients with CMS underwent the examination of neural electrophysiological, and the abnormality rates of visual evoked potential (VEP) and brain stem auditory evoked potential (BAEP) in patients with RIS were significantly lower than those in patients with CMS:4/15 vs. 63.6%(14/22) and 3/15 vs. 54.5%(12/22), and there were statistical differences (P<0.05). But there was no statistical difference in the abnormality rate of somatosensory evoked potential (SEP) between patients with RIS and patients with CMS (P>0.05). On MRI, the demyelinating lesions of RIS and CMS were both mainly distributed in the periventricular, semi-oval center, infratentorial white matter, partly involving corpus callosum or cortical. The rates of demyelinating lesions in brainstem and cerebellum in patients with RIS were significantly lower than those in patients with CMS:5.7%(2/35) vs. 34.4% (11/32) and 2.9% (1/35) vs. 25.0% (8/32), and there were statistical differences (P<0.01 or <0.05). Comparison with CMS lesions, RIS lesions mainly showed patching and stippled, and there were statistical differences (P<0.01 or <0.05). The rates of lesions enhancement and spinal cord injury in patients with RIS were significantly lower than those in patients with CMS: 2/17 vs. 45.0% (9/20) and 1/14 vs. 43.5% (10/23), and there were statistical differences (P<0.05). Conclusions There are differences in clinical findings, cerebrospinal fluid, neural electrophysiological examination and MRI appearances between RIS and CMS.
6.The efficacy of 125I radio active particle implantation in the treatment of maxillofacial malignancy of 43 cases

Objective:To investigate the effects of 125I radioactive particle implantation in the treatment of maxillofacial malignancy. Methods:43 patients with maxillofacial malignancy were treated with 125I radioactive particle implantation.The procedure was carried out according to the treatment planning system(TPS),with the particles spaced uniformly at 1 cm intervals and with the activity of 0.7 m Ciby 41 particles per case on a verage.All patients were followed up for 6-60 months.Results:The whole treatment procedurewas successful,and no particle displaced.The follow-up rate was 93.02% and treatment effective rate(CR+PR) was 90.70%.Norecur-rence was foundinall target areas.The mortality due to tumor was 9.30%and total survival rate was 74.43%.The cumulative survival rate of the patients in 0.5,1,2,3 and 5 years was 93.0%,85.7%,79.3,69.8% and 56.9% respectively.Survival periodon aver-age was 36.06-50.04 months,with the median of 43.05 months.The longest tumor-free survival period was 60 months.Radiation in-jury rate was 20.93% and only level 1 radiation injury was observedinall the cases.Facial nerve dys function was found in 2 cases and recovered after treatment.Conclusion:Treatment of maxillofacial malignancy by implantation of 125I particles is convenient and mini-mally invasive.The treatment canincrease survival rate of the patients and guaran tee the oropharynxes' function.
7.Application of comprehensive nursing intervention in children with severe burns

Objective To investigate the effect of comprehensive nursing intervention and evaluate its role in the entire clinical course of in the treatment of children with severe burns. Methods A total of 60 children with 1-5 years old with clinical diagnosis of severe burns were randomly divided into experimental group and control group by random digits table method, 30 cases in each group. The control group received routine burn care. The experimental group received comprehensive nursing interventions on the basis of routine burn care and the intervention time covered the entire course of treatment. During hospitalization, the recovery and family social system were evaluated by anxiety scales and social support scales combined with the cure rate and hospitalization of children. Results Before intervention, the score of Anxiety Rating Scale was high in both groups of families, while Social Support Scale was lower, the difference was not statistically significant between the two groups (P>0.05). After intervention, the score of Anxiety Rating Scale and was Social Support Scale were (52.31±7.81), (31.52± 5.48)points in the experimental group, and (62.51 ± 6.52), (23.62 ± 5.62) points in the control group, and there were significant differences (t=5.491 3, 5.512 5, P<0.01). The length of stay was (18.34±3.58) d in the experimental group, and (21.82 ± 4.23) d in the control group, and there was significant difference(t=3.439 6, P<0.05). The cure rate was 93.33%(28/30) in the experimental group, and 56.67%(17/30) in the control group, and there was significant difference(χ2=10.755 6, P<0.05). Conclusions The comprehensive nursing intervention can provide comprehensive refinement of the overall care in children and their families, promote physical and psychological rehabilitation of children with severe burns, and repair their family and social support systems. The comprehensive nursing intervention has far-reaching implications for the rehabilitation of children with severe burns.
8.Research progresses of 18F-FET PET in brain gliomas

In this article, the application of 18F-FET PET in tumor diagnosis and differentiation with other diseases, treatment guidance, treatment response assessment and prognosis prediction of gliomas are reviewed.
9.Effects of breast massage at different time periods on lactation yield and breastfeeding success rate

Objective:To investigate the effects of breast massage at different time periods on lactation yield and breast feeding success rate.Methods:240 pregnant women who gave birth at scheduled time in The First People's Hospital of Wenling from January 2018 to May 2019 were included in this study. They were randomly divided into control group, observation group 1 and observation group 2 ( n = 80/group). The control group was only given routine nursing after delivery without breast massage. The observation group 1 received breast massage at 2 hours postpartum based on routine nursing. The observation group 2 received breast massage at 24 hours postpartum based on routine nursing. Breast comfort rate, the time to lactation, lactation yield at 1 and 3 days after intervention were compared between groups. The number of daily vomiting, crying and fecal empties at 4 days postpartum were compared between groups. The success rate of breastfeeding and the increases in neonatal body mass and height at 42 days postpartum were determined in each group. Results:Breast comfort rate in the control group, observation group 1 and observation group 2 was 71.25% (57/80), 93.75% (75/80) and 83.75% (67/80), respectively. There was significant difference in breast comfort rate among the three groups ( χ2 = 26.466, P < 0.05). The incidence of postpartum breast problem in the control group, observation group 1 and observation group 2 was 27.50% (22/80), 5.00% (4/80) and 15.00% (12/80), respectively. There was significant difference in the incidence of postpartum breast problem among the three groups ( χ2 = 10.478, P < 0.05). The time to lactation in the control group, observation group 1 and observation group 2 was (33.54 ± 7.28) hours, (5.56 ± 2.01) hours and (22.32 ± 4.23) hours, respectively ( F = 27.897, P < 0.05). There was significant difference in the time to lactation between groups ( F = 27.897, P < 0.05). The lactation yield in the observation group 1 and observation group 2 was significantly greater than that in the control group ( F = 6.208, 10.458, both P < 0.05). There were significant differences in the number of daily vomiting, crying and fecal empties at 4 days postpartum among the three groups ( F = 71.54, 9.26 and 10.45, all P < 0.05). The success rate of breastfeeding in the observation group 1 was 93.75% (75/80), which was significantly higher than 85.00% (68/80) in the observation group 2 and 75.00% (60/80) in the control group ( χ2 = 7.841, P < 0.05). There were significant differences in the increases in body weight and height of newborns among the three groups at 42 days postpartum ( F = 10.645, 5.789, both P < 0.05). Conclusion:Breast massage at 2 hours postpartum can greatly increase the comfort rate of maternal breast, effectively reduce the incidence of postpartum breast problem, shorten the time to lactation, increase the amount of postpartum lactation yield, and the success rate of breastfeeding.
10.Advances in Study on Role of Cathepsin S in Irritable Bowel Syndrome

Irritable bowel syndrome(IBS)is a common functional intestinal disease,and its etiology and pathogenesis are not completely clear. The pathogenesis of IBS involves disturbed gastrointestinal motility,gut hypersensitivity,intestinal inflammation,immune dysfunction and brain-gut axis abnormality. Cathepsin S(CTSS)is a proteolytic enzyme widely distributed in various cell lysosomes,and participates in a variety of pathophysiological processes. Recent studies have shown that CTSS may be involved in the pathogenesis of IBS. This article reviewed the advances in study on role of CTSS in IBS.
