1.Sentinel lymph node micrometastasis in papillary thyroid carcinoma

Objective To investigate the optimal methods of detecting micrometastasis of sentinel lymph node(SLN) in papillary thyroid carcinoma(PTC).Methods Data of 51 consecutive PTC patients without clinical evidence of cervical lymph node metastasis were analyzed.They were conducted with SLN localization with blue dye,technetium-labeled sulfur colloid or the combination of them from Aug.2007 to Sep.2010.55 SLNs from 18 cases were selected.No metastasis was found to these 55 SLNs by routine pathological section.The 55 SLNs were serially sectioned at a 50 μm interval and stained by both HE and immunohistochemistry for detecting micrometastasis.Results SLN was successfully identified in all the 51 cases,with SLN identification rate of 100%.Among the 18 cases without metastasis to SLN by routine pathological section,5 cases were found micrometastasis by step sectioning plus immunohistochemically stains.The false negative rate was reduced from 15.4% by routine pathological section to 2.6% by step sectioning plus immunohistochemically stains.Conclusion Cytokeratin immunohistochemistry on series sections is a reliable method in detecting SLN micrometastasis in PTC.
2.Sentinel lymph node biopsy guided neck dissection in patients with papillary thyroid carcinoma

ObjectiveTo evaluate combined radioisotope and methylene blue dye method for identifying sentinel lymph node (SLN) for modified radical neck dissection of papillary thyroid carcinoma (PTC). MethodFifty-one consecutive PTC patients without clinical evidence of locoregional lymph node involvement were enrolled in the study between August 2007 and September 2010.5 h ( rangel.5 - 8 h) before the surgery,one single intratumoral injection of 74 MBq in a volume of 0.4 ml 99mTc -Dextran was administered under ultrasound guidance and 1% methylene blue dye was injected into the parenchyma surrounding the primary tumor intraoperatively.Preoperative lymphoscintigraphy,intra-operative hand-held gamma probe detecting and blue dyed lymph node were used to identify the SLN.All SLNs were sent for frozen-section and the specimens of routine selective neck dissection were stained with haematoxylin and eosin (H&E). ResultsSLNs were identified in 48 of 51 cases (94.1% ) with combination method.SLN identification rate were 66.7%by methylene blue dye method and 90.2%by radioisotope method respectively.Final pathologic examination revealed that 30 cases ( 58.8% ),including 3 cases who had negative SLNs,had lateral neck occult lymph node metastasis.The rate of occult lymph node metastasis in level Ⅱ,level Ⅲ,level Ⅳ and level Ⅴ were 17.6%,52.9%,29.4% and 0%.Thus,the sensitivity,specificity,accuracy, and positive and negative predictive values of SLN biopsy were 90%, 100%,94.1%,100% and 87.5%,respectively. ConclusionsSLNB is feasible and safe,the findings correlate with lateral lymph node status.Therefore,SLN biopsy is a good method for estimating the status of lateral lymph node in patients with clinical negative lymph node papillary thyroid carcinoma.
3.Mechanism of lncRNA-miRNA-related Signaling Pathways in Gastric Cancer and Chinese Medicine Intervention: A Review

Gastric cancer (GC) is one of the most common malignant tumors in the digestive system, with high morbidity and mortality. Early clinical symptoms of GC are not obvious, and most of them have entered the advanced stage after discovery, which greatly reduces the clinical cure rate and affects the quality of life of patients, and the prognosis is very poor. In recent years, with the continuous exploration in the field of bioinformatics, it has been found that micro-RNA (miRNA) and long non-coding RNA (lncRNA) exist as non-coding RNA (ncRNA) without translation ability, and regulate the expression levels of related signal proteins by acting on a certain target, thereby activating or inhibiting a certain signaling pathway, which plays an important role in assisting diagnosis, guiding clinical medication, and judging prognosis in the progress of GC. Chinese medicine is easily accepted by patients because of its good curative effect and less side effects. In the present basic studies, with the interaction mechanism between miRNA, lncRNA and signaling pathways as the breakthrough point, various studies on the regulation of related signaling molecules and signaling pathways by Chinese medicine have been carried out. A large number of experimental data have proved that the development of GC is closely related to the interaction of miRNA, lncRNA, and related signaling pathways, and Chinese medicine, with multi-target, multi-mechanism, and multi-pathway characteristics, affects various signaling molecules and signaling pathways and intervenes in the progress of GC cells. This paper reviewed the basic research on lncRNA, miRNA molecules, and main signaling pathways involved in the occurrence and development of GC, and summarized specific molecular mechanisms of Chinese medicine in the regulation of each signaling pathway, hoping to provide references for modern research of Chinese medicine in the intervention of GC progress at the molecular level.
4.Content Determination of 10 Kinds of Chemical Components in Ligusticum chuanxiong under Different Storage Conditions by HPLC

OBJECTIVE: To establish the method for the content determination of chlorogenic acid, ligustrazine, ferulic acid, senkyunolide I, senkyunolide H, coniferly ferulate, senkyunolide A, n-butylphtalide, Z-ligustilide and n-butylidenephthalide in Ligusticum chuanxiong under different storage conditions. METHODS: HPLC method was adopted. The determination was performed on Boston C18 column with mobile phase consisted of acetonitrile-0.5% acetic acid solution (gradient elution) at the flow rate of 1.0 mL/min. The detection wavelength was set at 285 nm, and the column temperature was 30 ℃. The sample size was 5 μL. RESULTS: The linear range of chlorogenic acid, ligustrazine, ferulic acid, senkyunolide I, senkyunolide H, coniferly ferulate, senkyunolide A, n-butylphtalide, Z-ligustilide n-butylidenephthalide were 0.030 53-0.519 01 μg (r=0.999 5), 0.001 02-0.017 34 μg (r=0.999 9), 0.012 83-0.218 11 μg (r=0.999 5), 0.007 63- 0.129 71 μg (r=0.999 7), 0.001 76-0.029 92 μg (r=0.999 5), 0.054 74-0.930 58 μg (r=0.999 9), 0.215 80-3.668 60 μg (r=0.999 9), 0.018 02-0.306 34 μg (r=0.999 7), 0.232 50- 3.952 50 μg (r=0.999 9), 0.002 40-0.040 80 μg (r=0.999 5).The limits of quantitation were 2.073 7, 0.556 6, 0.753 8, 0.231 5, 0.306 9, 0.925 2, 2.295 3, 4.624 0, 3.215 3, 0.910 5 ng, respectively. The detection limits were 0.622 1, 0.167 0, 0.226 1, 0.069 4, 0.092 1, 0.277 6, 0.688 6, 1.387 2, 0.964 6, 0.273 1 ng, respectively. RSD of precision, stability and repeatability tests were all less than 5% (n=6). The recovery rates were 95.90%-103.28% (RSD=2.99%, n=6), 88.24%-107.84% (RSD=4.89%, n=6), 95.06%-102.08% (RSD=3.97%, n=6), 93.67%-101.05% (RSD=1.02%, n=6), 94.81%-104.33% (RSD=2.34%,n=6), 94.41%-105.59% (RSD=4.32%, n=6), 92.76%-104.83% (RSD=1.95%, n=6), 87.22%-102.56% (RSD=2.89%, n=6), 94.04%-99.52% (RSD=0.92%, n=6), 88.51%-103.83% (RSD=4.89%, n=6), respectively. At 5 ℃ and 15 ℃, no obvious deterioration was observed in medicinal materials. At room temperature, some samples were moth-eaten and mildewed. The content ranges of 6 batches of samples were 0.047 7%-0.160 8%, 0.006 1%- 0.022 7%, 0.048 2%-0.172 2%, 0.023 3%-0.145 2%, 0.004 6%-0.030 7%, 0.085 2%-0.835 4%, 0.182 6%-2.112 7%, 0.009 9%- 0.098 3%, 0.614 9%-3.176 2% and 0.005 7%-0.036 9%, showing decreasing trend; the decrease rate was in descending order 5 ℃<15 ℃<room temperature; at the same storage temperature, the decrease rate of polyethylene plastic bag was lower than that of polypropylene woven bag. CONCLUSIONS: This method is accurate and feasible, and can be used for simultaneous determination of 10 kinds of components in L. chuanxiong. It is suggested that L. chuanxiong medicinal materials should be sealed and packed in dry and cool places and should not be stored for a long time.
5.Health economic evidence for colorectal cancer screening programs in China: an update from 2009-2018

Objective:This study was to systematically update the economic evaluation evidence of colorectal cancer screening in mainland China.Methods:Based on a systematic review published in 2015, we expanded the scope of retrieval database (PubMed, EMbase, The Cochrane Library, Web of Science, CNKI, Wanfang Data, VIP, CBM) and extended it to December 2018. Focusing on the evidence for nearly 10 years (2009-2018), basic characteristics and main results were extracted. Costs were discounted to 2017 using the consumer price index of medical and health care being provided to the residents, and the ratio of incremental cost-effectiveness ratio (ICER) to per capita GDP in corresponding years were calculated.Results:A total of 12 articles (8 new ones) were included, of which 9 were population-based (all cross-sectional studies) and 3 were model-based. Most of the initial screening age was 40 years (7 articles), and most of the frequency was once in a lifetime (11 articles). Technologies used for primary screening included: questionnaire assessment, immunological fecal occult blood test (iFOBT) and endoscopy. The most commonly used indicator was the cost per colorectal cancer detected, and the median (range) of the 20 screening schemes was 52 307 Chinese Yuan (12 967-3 769 801, n=20). The cost per adenoma detected was 9 220 Yuan (1 859-40 535, n=10). In 3 articles, the cost per life year saved (compared with noscreening) was mentioned and the ratio of ICER to GDP was 0.673 (-0.013-2.459, n=11), which was considered by WHO as "very cost-effective" ; The range of ratios overlapped greatly among different technologies and screening frequencies, but the initial age for screening seemed more cost-effective at the age of 50 years (0.002, -0.013-0.015, n=3), than at the 40 year-olds (0.781, 0.321-2.459, n=8). Conclusions:Results from the population-based studies showed that the cost per adenoma detected was only 1/6 of the cost per colorectal cancer detected, and limited ICER evidence suggested that screening for colorectal cancer was generally cost-effective in Chinese population. Despite the inconclusiveness of the optimal screening technology, the findings suggested that the initial screening might be more cost-effective at older age. No high-level evidence such as randomized controlled trial evaluation was found.

Benign paroxysmal positional vertigo(BPPV)is a common peripheral vestibular disorder,and at present,otolith shedding and displacement is highly recognized as the main pathological mechanism of BPPV. An increasing amount of evidence has shown that otolith particle shedding is closely associated with vitamin D,and 25-(OH)D is expected to become a potential biomarker for BPPV and an important target for the treatment of BPPV and residual symptoms after successful repositioning. This article reviews the pathophysiological mechanism of vitamin D in BPPV and residual dizziness and summarizes the association of vitamin D with BPPV and residual symptoms based on the treatment methods for vitamin D regulation.

Objective:To investigate the value of systemic inflammatory response syndrome (SIRS), pediatric sequential organ failure assessment (pSOFA) and pediatric critical illness score (PCIS) in predicting mortality of pediatric sepsis in pediatric intensive care units (PICU) from Southwest China.Methods:This was a prospective multicenter observational study. A total of 447 children with sepsis admitted to 12 PICU in Southwest China from April 2022 to March 2023 were enrolled. Based on the prognosis, the patients were divided into survival group and non-survival group. The physiological parameters of SIRS, pSOFA and PCIS were recorded and scored within 24 h after PICU admission. The general clinical data and some laboratory results were recorded. The area under the curve (AUC) of the receiver operating characteristic curve was used to compare the predictive value of SIRS, pSOFA and PCIS in mortality of pediatric sepsis.Results:Amongst 447 children with sepsis, 260 patients were male and 187 patients were female, aged 2.5 (0.8, 7.0) years, 405 patients were in the survival group and 42 patients were in the non-survival group. 418 patients (93.5%) met the criteria of SIRS, and 440 patients (98.4%) met the criteria of pSOFA≥2. There was no significant difference in the number of items meeting the SIRS criteria between the survival group and the non-survival group (3(2, 4) vs. 3(3, 4) points, Z=1.30, P=0.192). The pSOFA score of the non-survival group was significantly higher than that of the survival group (9(6, 12) vs. 4(3, 7) points, Z=6.56, P<0.001), and the PCIS score was significantly lower than that of the survival group (72(68, 81) vs. 82(76, 88) points, Z=5.90, P<0.001). The predictive value of pSOFA (AUC=0.82) and PCIS (AUC=0.78) for sepsis mortality was significantly higher than that of SIRS (AUC=0.56) ( Z=6.59, 4.23, both P<0.001). There was no significant difference between pSOFA and PCIS ( Z=1.35, P=0.176). Platelet count, procalcitonin, lactic acid, albumin, creatinine, total bilirubin, activated partial thromboplastin time, prothrombin time and international normalized ratio were all able to predict mortality of sepsis to a certain degree (AUC=0.64, 0.68, 0.80, 0.64, 0.68, 0.60, 0.77, 0.75, 0.76, all P<0.05). Conclusion:Compared with SIRS, both pSOFA and PCIS had better predictive value in the mortality of pediatric sepsis in PICU.

To acknowledge the availability and rates of annual transition of outcomes during the progression and regression stages of colorectal cancer (CRC) and related diseases, by pooling global follow-up studies on the natural history of CRC.

Till March, 2017, data was collected through systematic literature review over multiple databases, including PubMed, Embase, Cochrane and Chinese Biology Medicine (CBM) disc.
