1.Differences of the molecular phenotypes and the histogenesis between dermatofibroma and dermatofibrosarcoma protuberans

Objective:To explore the histogenesis and differentiation of dermatofibroma (DF) and dermatofibrosarcoma protuberans (DFSP). Methods: Clinical information and microscopic characteristics of 26 cases of DF and 26 cases of DFSP were investigated. The immunohistochemical study was performed on microarray sections by a panel of antibodies including FactorⅩⅢa, HLA-DR, CD34, CD14, S-100, MSA, and Ki67. Probe was labeled by in vitro transcription. The mRNA expression levels of TGF-? and bFGF were investigated by in situ hybridization. Results: All cases showed positive for Factor ⅩⅢa,HLA-DR and CD34 to different extent. The medians of positive rates in DF were FactorⅩⅢa 90%, HLA-DR 70%, and CD34 5%, and in DFSP were FactorⅩⅢa 10%, HLA-DR 5%, and CD34 80%. CD14 was positive in 3 cases of DF and 1 case of DFSP. S-100 was positive in 6 cases of DFSP and 2 cases of DF. MSA was positive in 5 cases of DFSP and 3 cases of DF. In all cases, positive rate of Ki67 was less than 5%. The mRNA expression levels of TGF-? was elevated in DF in comparison with DFSP. Conclusion: Both DF and DFSP can differentiate to dendritic cells (DC) in different degree. Considering the character of microscopic features and immunohistochemical phenotype, cells of DF are much similar to mature DC, while those of DFSP much similar to immature dermal reserve cell (DRC). The differences of cell differentiation between DF and DFSP result in different prognosis. DF is a benign tumor, while DFSP a low grade malignant tumor. The different expression of FactorⅩⅢa and CD34 may be helpful to differential diagnosis of DF and DFSP.
2.Application of Xpert MTB/RIF assay on rapid detection of MTB and rifampin-resistant

Objective To investigate clinical value of Xpert M TB/RIF assay on rapid detection of M TB and rifampin‐resistant . Methods 408 cases of tuberculosis were detected by acid‐fast stain ,Solid culture and Xpert MTB/RIF ,the results were compared . Results All the samples were detected by acid‐fast stain ,culture and Xpert MTB/RIF assay ,the positive rates were 25 .00% (102/408) ,42 .16% (172/408) and 58 .09% (237/408) ,respectively .The positive rates of Xpert MTB/RIF assay were higher than acid‐fast stain and culture method ,and the difference is statistically significant(P<0 .05) .There are 168 cases of specimen which were tested positively by both culture and Xpert M TB/RIF ,the RIF resistance rates from standard drug susceptibility testing and Xpert MTB/RIF assay respectively were 26 .19% (44/168) and 27 .38% (46/168) ,and had no statistical significance(P>0 .05) .Accord‐ing to the result of drug susceptibility with proportion method ,the sensitivity and specificity of Xpert MTB/RIF in detecting RIF resistance were 97 .73% (43/44) and 97 .58% (121/124) .Conclusion Xpert MTB /RIF can quickly and accurately detect MTB and RIF resistance ,it has important clinical significance .
3.A correlation study between septic acute kidney injury and immune function

Objective To explore the correlations between septic acute kidney injury (SAKI) and immune condition and provide the clinical basis of predictable diagnosis and treatment in patients with SAKI.Methods Patients diagnosed with sepsis admitted to department of emergency intensive care unit of Peking Union Medical College Hospital between January 1st,2013 and September 30th,2014 were retrospectively studied.A total of 91 patients with sepsis were included,and they were divided into secondary immune deficient (SID) group (n =46) or control group (n =45).According to the diagnostic criteria and stage of the guidelines of Kidney Disease:Improving Global Outcomes 2012 (KDIGO-AKI 2012),patients in each group were divided into non-SAKI group (n1 =16,n2 =23)、KDIGO-1 group (n1 =15,n2 =13)、KDIGO-2 group (n1 =11,n2 =1) or KDIGO-3 group (n1 =4,n2 =8).The morbidity of each stage and the renal index along with the progression of SAKI was also compared in patients with SAKI in two groups.Results While there was a significant difference in the morbidity of KDIGO-2 (23.9% vs.2.2%,x2 =0.321,P =0.002) in patients with SAKI between immune deficient group and control group,the morbidity of KDIGO-1 and KDIGO-3 had no significant difference (KDIGO-1:32.6% vs.29.8%,x2 =0.040,P =0.701;KDIGO-3:8.7% vs.17.8%,x2 =-1.805,P =0.200).There was also no significant difference in the renal index (△Scr、△eGFR) and progression of SAKI (elapsed days from the diagnosis of sepsis to the occurance and most severe stage of SAKI) in patients with SAKI in two groups (P > 0.05).Conclusion There was no significant difference of the severity of septic acute kidney injury in patients with and without secondary immune deficiency.Inflammatory mediators-induced kidney injury in the process of immune response may not be the main mechanism in SAKI.
4.The correlation analysis between renal artery resistance index and renal function in patients with thrombotic microangiopathy

Objective To explore the correlations between renal artery resistance index (RRI) and renal function in patients with thrombotic microangiopathy (TMA) so as to provide the clinical basis for predictable diagnosis and treatment in patients with acute kidney injury (AKI).Methods Patients diagnosed with thrombotic microangiopathy admitted to department of emergency of Peking Union Medical College Hospital between August 1st,2014 and March 31th,2015 were enrolled.Intrarenal arteries resistive index of right kidney was detected in all cases on admission by color Doppler flow image.The serum creatinine (SCR) and glomerular fihration rate (GFR) were measured at the same time.According to the diagnostic criteria of the guideline of Kidney Disease:Improving Global Outcomes 2012 (KDIGO-AKI 2012),patients were divided into non-AKI group and AKI group.The intergroup difference was compared and the correlation between RRI and SCR as well as between RRI and GFR were assessed.RRI,SCR and GFR were measured again at the most severe stage of kidney injury.The above index were marked as RRI*,SCR and GFRmin.At the same time,△RRI (RRI*-RRI),△SCR (SCRmax-SCR) and △GFR (GFR-GFRmin) were calculated.According to the stage classification of KDIGO-AKI 2012,36 patients diagnosed with AKI during their hospitalization were divided into KDIGO-1 group (n =10),KDIGO-2 group (n =10) or KDIGO-3 group (n =16).The intergroup difference of RRI* was compared and the correlation between △RRI and △SCR as well as between △RRI and △GFR were assessed.Results When RRI > 0.7 was used as the diagnostic threshold for AKI,the sensitivity was 92.3% and the specificity was 80.1%.RRI was positively correlated with SCR (r1 =-0.728,P<0.01;r2=-0.709,P<0.01) and negatively correlated with GFR (r1 =-0.728,P<0.01;r2 =-0.709,P <0.01) in all patients at the time of admission and the most severe stage of kidney injury.While there was a significant difference in the RRI* among KDIGO-1,KDIGO-2 and KDIGO-3 groups (F =37.979,P =0.Q01),and there was no significant difference in △RRI (F =0.634,P =0.537).The △RRI was not correlated with △GFR or △SCR.Conclusions RRI can be used as a marker for diagnosis of AKI and the evaluation of renal function in patients with TMA,but it is not helpful to reflect the trends of renal injury especially for the critically ill patients.
5.Reassessment of the pathological diagnosis in 33 cases of malignant fibrous histiocytoma

Objective:Since malignant fibrous histiocytoma (MFH) may be taken as an undifferentia-ted pleomorphic sarcoma (UPS), this study was conducted to reassess 33 previously diagnosed MFH cases in the past 10 years based on the latest WHO concept. And then to search for the clinicopathological features, probably tumorigenesis, and the line of differentiation of the remaining MFH/UPS cases.Methods: Thirty-three cases in tissue microarray were studied by immunohistochemistry with panels of neurogenic, myogenic, and lipogenic antibodies. Three expertise pathologists reevaluated the slides separately. Results: Among the 33 cases, 17 cases (51.5%) of MFH had their diagnoses changed, including 5 leiomyosarcomas, 3 malignant peripheral nerve sheath tumors, 1 fibrosarcoma, 1 inflammatory myofibrosarcoma, 1 giant cell tumor and 1 angiomatoid fibrous histiocytoma. The remaining 16 cases (48.5%) were finally diagnosed as MFH/UPS, among which patients were mainly old adults (median age: 63 years; range: 38 to 76 years). The median tumor size was 6.0 cm (range: 3.0 to 14.0 cm), 8 cases (50%) located in lower limb and 5 cases (31.3%) located in thigh. These tumors had marked cytological and nuclear pleomorphism. Immunohistochemistry showed that Vimentin was strongly positive in all 16 MFH/UPS (100%), Muscle-specific actin was variously positive in 8 cases (50%) and 1 case focally expressed Desmin. Eleven cases (68.8%) variously expressed CD68 (KP1) and 7 cases (43.8%) expressed CD68 (PG-M1), which were much higher than leiomyosarcoma, malignant peripheral nerve sheath tumor and liposarcoma with significant difference. Moreover, Ki67 expression rates were from 10% to 100%, including 14 cases more than 50% and 11 cases more than 70%. However, only 2 cases (12.5%) showed P53 positive. Conclusion: MFH/UPS often show marked histological pleomorphism, and the diagnosis must be made by exclusion of other definitive sarcomas, especially myogenic and neurogenic sarcoma. Only Vimentin was always expressed in MFH/UPS, while some of the tumors were positive for myogenic antigen and CD68. It was suggested that MFH/UPS might arise from primary mesenchymal cells, and some cases exhibited fibroblastic and/or myofibroblastic features. In addition, histiocytic phenotypic marker did have more expression in MFH/UPS than in other sarcomas. MFH/UPS still had certain clinicopathological characteristics.
6.An experimental study of cmpound Thallus laminaria granules volatile oil-?-cyclodextrin clathrate

Objective To study the best preparation technique of compound Thallus laminaria granules volatile oil-?-cyclodextrin clathrate and examine the stability of the elathrate.Methods The orthogonal design method was used to optimize the formulation.The yeild of the volatile oil and the content of the clathrate were used as the test target.Results The best preparation process was as follows:?-cyclodextrin:compound Thallus laminaria=12:1(g?mL~(-1)),at 50~60℃for 2 hours.Making distillation twice while adding water to the first decoction, steaming 1h after boiling;A_2 B_1 was the best extraction scheme.Conclusion The preparation process can be a scheme to obtain the clathrate with high content of effective component,which can be used in the preparation of the clathrate.
7.The mechanism of synapse changes in the region around a cerebral infarct after mobility training

Objective To investigate how mobility training affects the synapses and their functioning in the region around a cerebral infarct.Methods One hundred and fifty rats were randomly divided into 5 groups:a mo- tor training group,a normal saline group,an Ara-c inhibition group,a mevastatin group and a control group.Cere- bral infarcts were surgically induced in all 150 rats,and the level of either glial fibrillary acidic protein(GFAP)or synaptophysin,and the cholesterol content around the infarct were observed at 7,21 and 42 days using immunohisto- chemical staining and high performance liquid chromatography.Results At 7,21 and 42 days after the infarct model was induced,significant differences in the optical density of either GFAP or synaptophysin and in cholesterol levels were noted between the motor training group and the control group.Ara-c inhibition was also significantly high- er in the controls.The optical density of synaptophysin and the cholesterol levels were significantly lower in the me- vastatin group than in the motor training group.Conclusion Motor skill training can improve synapse redefinition in rats with a model of acute cerebral infarct.Astrocytes may play a crucial role by means of the secretion of choles- terol in the region around the infarct.
8.Effect of pleiotrophin on decidualization of uterine stromal cells in mice

Objective:To construct the pleiotrophin (PTN )overexpression vector,and to explore the effect of PTN on the decidualization of uterine stromal cells in the mice.Methods:The specific primers containing restriction enzyme cutting sites were designed according to the PTN gene sequences published in GenBank for PCR amplification.The amplified fragment of PTN was recovered from the agarose gel and cloned into the pGEM-T vector.The pGEMT-PTN was cut by double enzyme digestion and ligated into pcDNA3.1 (+)to construct the PTN overexpression plasmid.After transfection with PTN overexpression plasmid,the expression levels of PTN mRNA in the uterine stromal cells and the expression levels of decidualization markers Prl8a2 and Prl3c1 were detected by qRT-PCR method.The uterine stromal cells transfected with pcDNA3.1 (+)empty vector were used as control group. Results:The results of identification by double enzyme digestion indicated that the bands of PTN overexpression plasmid were consistent with those of the target gene,and the clone sequencing results suggested that it had 100% homology with mouse PTN gene sequence published in GenBank.Compared with control group, the expression levels of PTN,Prl8a2 and Prl3c1 mRNA in mouse uterine stromal cells in PTN overexpression group were significantly increased (P < 0.05).Conclusion:PTN overexpression could increase the expression levels of decidualization markers in mouse uterine stromal cells,indicating that PTN might play an enhancement effect during uterine decidualization in the mice.
9.Multisystem Langerhans cell histiocytosis with diabetes insipidus as the first symptom: a case report and literature review

Objective To study the clinical features and differential diagnosis of Langerhans cell histiocytosis (LCH).Methods A case of LCH was reported and the literatures were reviewed.Results The of multisystem LCH patient,presented with a diabetes insipidus (DI) and panhypopituitarism,was 44 years old,and developed costal,tibial and femoral multiple lesions.The final diagnosis as LCH was made based on biopsy of tibia and lymph nodes.The biopsy specimen showed that the cells were infiltrated exhibiting the characteristic morphologic features of Langerhans cell (LC) with a convoluted shape,elongated nuclei exhibiting longitudinal grooves,and immunohistochemistry results revealed positive LC for the S-100,CD1a and Langerin immunostaining.Conclusions LCH may range from a solitary lytic bone lesion (for example eosinophilic granuloma) with a favorable course to a fatal disseminated leukaemia-like form.LCH typically involves the bone,lesions almost can be found in all organs.DI and CNS involvement often present as a puzzling syndrome,which renders the diagnosis problematicly,and often delays the diagnosis of LCH.The damage to the pituitary/hypothalamus axis results in life-long hormonal replacement therapy.
10.Hypoxia decreases K_V1.5 expression by 15-LO/15-HETE in rat

Aim To test the contribution of 15-HETE on expression of KV1.5 channel under hypoxia condition,using CDC or NDGA to block 15-LO/15-HETE,and to observe the effect of hypoxia on KV1.5 channel protein,mRNA expressions in cultured rat pulmonary arterial smooth muscle cells(PASMCs)and pulmonary arterials(PAs).Methods Western blot,RT-PCR and 15-LO blockers,cinnamyl 3,4-dihydroxy-[alpha]-cyanocinnamate(CDC)or nordihydroguiairetic acid(NDGA)were used to identify the role of endogenous 15-HETE on expression of KV1.5 channel in cultured rat pulmonary arterial smooth muscle cells(PASMCs)and PAs.Results(1)The expressions of KV1.5 channel protein and mRNA in PASMCs and PAs preteated with CDC or NDGA greatly increased than those of PASMCs under hypoxia group.(2)Exogenous 15-HETE added to PASMCs pretreated with CDC or NDGA greatly decreased the expression of KV1.5 than that of adding PASMCs pretreated with CDC or NDGA under hypoxia condition.Conclusion The down-regulation of KV1.5 channel expression caused by hypoxia is through endogenous 15-HETE.
