1.Effect of cumin total flavonoids on experimental gastric ulcer and its mechanism

AIM: To discuss the effects of cumin total flavonoids on experimental gastric ulcer and the mechanisms.METHODS: Three rat gastric ulcer models induced by pylorus ligation, ethanol or reserpine were selected.Based on these models, the gastric juice, pH value, ulcer index and suppressive rate were recorded to determine the inhibitory effects of cumin flavonoids on rat gastric ulcers.In addition, the concentrations of gastrin (GAS), prostaglandin E2 (PGE2) and epidermal growth factor (EGF) were detected to further uncover the mechanisms of these effects.RESULTS: Cumin flavonoids exhibited significant anti-gastric ulcer effects on the three models.Low-dose treatment group (P<0.01) demonstrated higher inhibitory effects than high-dose group (P<0.01), and both low-and high-dose groups had lower effects than cimetidine treatment.Furthermore, cumin flavonoids were able to profoundly decrease the amount of gastric juice, free acidity, total acidity and output of total acid, and enhance the expression of PGE2 and EGF, but down-regulate the level of GAS in rat serum in a dose-dependent manner.CONCLUSION: Cumin flavonoids might inhibit experimental gastric ulcers by affecting and regulating the gastric juice, PEG2, EGF and GAS.
2.Effects of Lanthanum on Cell Multiplication and Intracellular Free Ca~(2+) Concentration in Primary Cultured Astrocytes of Rats

Objective To study the effects of lanthanum on cell multiplication and intracellular Ca2+([Ca2+]i) concentration in primary cultured astrocytes of rats.Methods The primary cultured astrocytes of rats were exposed to 0.25,0.5,1.0 and 2.0 mmol/L LaCl3 for 24 h and 48 h.The cell multiplication was measured by MTT;[Ca2+]i concentration was determined by the fluorescent Ca2+ indicator Fura-2/AM.Results After astrocytes were incubated with 0.25,0.5,1.0 and 2.0 mmol/L LaCl3 for 24 h and 48 h,the survival rate of astrocytes was 86.24% and 82.99%,82.26% and 79.14%,52.32% and 49.97%,38.04% and 35.15% respectively,which was significantly decreased compared with the control group(P

Perioperative pain management play a critical role in rapid recovery and long term outcome in patient undergoing total knee arthroplasty.With the advancement of peripheral nerve block, technique such as femoral nerve block, sciatic nerve block, abductor canal block have play an important role in perioperative pain management.Local infiltration analgesia have also gained popularity.The ultimate goal of perioperative pain management is to ensure analgesia effect and maintain good motor function of lower extremity.We are still in searching of a safe, effective, analgesia without motor block.Currently multimodal analgesia seems to be the most favorable choice.
4.Clinical study of octreotide in the treatment of intestinal endotoxemia

Objective To observe the clinical effect of continuous intravenous pumping of octreotide in the treatment of intestinal endotoxemia.Methods Eighty patients with intestinal obstruction and non-surgical treatment were divided into group Ⅰ with 34 cases who received conventional-treatment and group Ⅱ with 46 cases who received conventional-treatment combined with octreotide 24 h continuous intravenous pumping.White blood cell count ( WBC ),diamine oxidase (DAO),D-lactic acid (D-LA) and endotoxin were detected before treatment and at 24 h,48 h,4 d after treatment.Results The content of WBC,DAO,D-LA and endotoxin in two groups all reached peak at 48 h after treatment.The difference of the content of WBC,DAO,D-LA and endotoxin between two groups had no statistical significance at 24 h after treatment (P > 0.05).The content of WBC,DAO,D-LA and endotoxin of group Ⅱ at 48 h and 4 d after treatment were lower than those of group Ⅰ.And the difference at 48 h after treatment had statistical significance[(18.40 ±0.10)× 109/L vs.(20.60 ± 2.36) × 109/L,(6.12 ± 1.02) kU/L vs.(8.02 ± 1.54) kU/L,(2.14 ±0.21) mg/L vs.(3.34 ± 0.04) mg/L,(1.65 ±0.16) kEU/L w.(2.23 ±0.36) kEU/L] (P < 0.01).While the difference at 4 d after treatment had no statistical significance(P> 0.05 ).Body temperature at 48 h after treatment,gastrointestinal decompression capacity,anus exhaust time of group Ⅱ were (37.60 + 3.01 )℃,(320.00 ± 76.14) ml/d,(54.00 ± 0.94) h respectively,and they all were superior to those of group Ⅰ[(38.50 ± 2.21 ) ℃,(500.00 ± 80.32) ml/d,(68.00 ± 1.02) h] (P <0.01).Conclusions Continuous intravenous pumping of octreotide can effectively protect the intestinal mucosal barrier function,improve intestinal permeability,reduce the trmslocation of intestinal flora,inhibit the incidence and development of enterogenous endotoxemia.And it provides new evidence to support the clinioal application of octreotide in patients with intestinal endotoxemia.
5.Assessment and treatment of poststroke spasticity

Spasticity is a common complication of stroke.It may cause the function and ability of patients to be restricted significantly and affect the quality of life of both patients and their caregivers.The estimated prevalence of poststroke spasticity (PSS) is 19% to 42.6%.PSS can be usually assessed by the Modified Ashworth Scale,Modified Tardieu Scale,and electromyography.Therapeutic methods of PSS include physical therapy and drug treatment.The treatment options should be individualized.
6.Development of peptidic MERS-CoV entry inhibitors.

In 2012, a new SARS-like coronavirus emerged in the Middle East, namely the Middle East respiratory syndrome coronavirus (MERS-CoV). It has caused outbreaks with high mortality. During infection of target cell, MERS-CoV S protein S1 subunit binds to the cellular receptor (DPP4), and its S2 subunit HR1 and HR2 regions intact with each other to form a stable six-helix bundle to mediate the fusion between virus and target cell membranes. Hence, blocking the process of six-helix bundle formation can effectively inhibit MERS-CoV entry into the target cells. This review focuses on the recent advance in the development of peptidic entry inhibitors targeting the MERS-CoV S2 subunit.
7.New fixation set-up designed for micro-tensile test

Objective:To introduce a new fixation set-up for micro-tensile test.Methods:Dentin-composite were bonded with AdperTM Single Bond 2 (SB2)and sectioned into stick-shaped specimens.Specimens from each tooth(n =6)were equally divided into Ciucchi's jig and the designed set-up(Control and experimental)groups for micro-tensile bond test according to the utilized fixa-tion set-up.The bonding interface failure mode was examined with field-emission scanning electron microscope (FESEM).Three-dimensional models of the two set-ups and the specimen were developed,stress distribution was analyzed by finite element analysis (FEA).Results:The bond strength(MPa)of experimental and control group was 32.76 ±7.43 and 43.58 ±4.72(P <0.05),the ratio of mixed failure was 28 /36 and 20 /36(P <0.05)respectively.FEA showed that the designed set-up for fixing the sticks pro-vided a uniform stress distribution along the long axis of the specimen.FEA and failure mode analysis confirmed such uniform distri-bution was also concentrated at the bonding interface.Conclusion:The new set-up is feasible for micro-tensile test.
8.Biomechanical study of improved memory alloy embracing fixators in treatment of periprosthetic femoral fracture due to hip arthroplasty

Objective To study the mechanics of improved memory alloy fixators for salvage of periprosthetic femoral fracture (PFF) after hip arthroplasty in the elderly.Methods Thirty countrymen fresh cadaveric femurs with no pathological defect,fracture,deformity or tumor were randomly divided into experimental group and control group with 15 femurs in each according to the random number table.A model of Vancouver type B1 periprosthetic femoral fracture following hip arthroplasty was induced.The fracture was treated with modified memory alloy embracing fixators in experimental group;instead general memory alloy embracing fixators in control group.All specimens were tested biomechanically.Results Under the same mechanical loading,the two groups showed respective 30% and 48% maximum differences in stress value and displacement.Results in three-point bending test did not differ significantly between the two groups (P > 0.05),but there were significant differences in axial compression and torsion test (P < 0.05 or 0.01).Conclusion The improved memory alloy embracing fixators present better resistance to compression and torsion compared to the general fixators.
9.Microsurgical treatment of aneurysms of posterior circulation

Objective To summarize surgical strategies and skills against aneurysms of posterior circulation .Methods Twenty-one cases of posterior circulation aneurysms who had undergone microsurgery between March 2008 and March 2013 were analyzed retrospectively .Two of these cases had their basilar artery aneurysms clipped via the pterion approach and five through the subtemporal approach .Five posterior cerebral artery aneurysms were clipped via the subtemporal approach , two anterior inferior cerebellar artery aneurysms were clipped via the retrosigmoid approach , three posterior inferior cerebel-lar artery aneurysms were clipped through the far lateral approach , and two vertebral artery aneurysms were clipped similar-ly approach.Results All the aneurysms were clipped successfully .According to Glasgow Outcome Scale (GOS), the cura-tive effect was poor in 3 cases(GOS 2-3 points), good in 16 cases(GOS 4-5 points) and 2 patients died(GOS 1 point). Conclusion Aneurysms of posterior circulation can be treated through microsurgery .Proper surgical strategies , operative ap-proach and delicate surgical skills are critical to the success of surgeries and curative effects .
10.The impact of neoadjuvant chemotherapy on lymph node micrometastasis and dendritic cells in patients with stage Ⅲ gastric carcinoma

Objective To evaluate the effect of neoadjuvant chemotherapy on lymph node micrometastasis and DC cell number in stage Ⅲ gastric cancer patients.Methods Lymph node micrometastases in pathologically negative lymph nodes from 76 stage Ⅲ gastric cancer patients [56 cases with neoadjavant chemotherapy (NCT) and 20 cases without],were assessed by cytokeratin-20 antibody.Dendritic cells (DC) in HE positive lymph nodes,HE negatives near tumor foci and HE negatives far foci were assessed by CD83 antibody respectively.Patients were divided into four groups:group 0 (20 cases without NCT),group 2 (two courses of NCT in 29 patients),group 3 (three courses of NCT in 16 patients),group 4 (four courses of NCT in 11 patients).Results In NCT group there were two patients with micrometastases (2/56).All the patients were with micrometastases in control group (20/20).Difference was statistically significant (P =0.000).In HE-positive lymph node group:the number of CD83-positive DC in 2 times group,3 times group,and 4 times group when compared with no chemotherapy group were statistically significant (P20 =0.001,P30 =0.000,P40 =0.000).In HE-negative lymph node near foci group:the number of CD83-positive DC in 3 times group,and 4 times group compared with 0 group were statistically significant increased (P30 =0.001,P40 =0.001).In HE-negative lymph node far foci group:the number of 4 times group was significantly higher than 0,2 and 3 group (P40 =0.000,P42 =0.000,P43 =0.001).Conclusions Neoadjuvant chemotherapy is effective in the prevention of lymph node micrometastases in stage Ⅲ gastric carcinoma and it increases the number of mature dendritic cells in lymph nodes.
