1.Investigation and anaIysis of bacteriaI spectrum and drug sensitivity test in conjunctivaI sac of cataract patients of Kazak

· ATM:To analyze bacterial spectrum and drug sensitivity in conjunctival sac of cataract patients of Kazak.
· METHODS:A total of 538 cases of conjunctival sac secretion in cataract patients of Kazak were collected.The samples were cultured and their sensibilities to antibiotics were tested.
· RESULTS: The bacterial culture was positive in 214 cases.The positive rate was 39.8%. The variety of pathogenic bacteria were mainly made up of gram positive cocci ( 88.3%), and most of them were Staphylococcus epidermidis ( 66.4%), followed by Micrococcus(9.8%).Sex had no effect on conjunctival bacteria rate in the cataract patients of Kazak, while age, place of residence had an effect on camier rate. The camier rate of conjunctival bacteria was significantly higher in people over 60 years old than that in people with age between 40 to 59 years old.And the people from city had a significant lower bacteria positive rate than those from countryside and pastoral. Most of grams were sensitive to Vancomycin, Teicoplanin, Rifampicin, Duly cloth mildew mutual and Amikacin, the tolerance was less than 20%, and they usually had higher tolerance to Penicillin, Erythromycin, Tetracycline and Chloramphenicol (>70%) .
·CONCLUSlON:Gram positivecocci is the most common bacteria in conjunctival sac in cataract patients of Kazak. Staphylococcus epidermidis was most common, followed by Micrococcus.The germ-carrying rate of conjunctival SAC in Kazakh population is associated with the patient’s age and area of residence.The clinical use of antibacterial drugs should be strictly grasp the indications, to reduce the incidence of bacterial resistance.
2.Bridging fixation with locking plate for the treatment of subtrochanteric femoral fractures of Seinsheimer type V.

OBJECTIVETo investigate the clinical effect of bridging fixation with locking plate for the Seinsheimer type V subtrochanteric femoral fracture.
METHODSFrom March 2009 to September 2014,18 cases of Seinsheimer type V subtrochanteric femoral fracture were treated by open reduction and bridging fixation with locking plate through proximal and distal approach including 16 males and 2 females with an average age of 41 years old ranging from 22 to 67 years old. Among them, 12 cases caused by traffic accident, 5 cases by falling, 1 case by heavy aboving. All cases were fresh and closed fractures. Time between injury and operation was from 4 to 9 days with an average of 6.2 days. Of them, 11 cases were fixed with reverse LISS and the other 7 cases were fixed with anatomical locking plates of proximal femur.
RESULTSThe mean time of operation was 110 min (ranged from 90 to 155 min). The mean blood loss during operation was 425 ml (ranged from 350 to 650 ml) and 16 cases got blood transfusion which was meanly 300 ml. The mean hospital time was 14 days (ranged from 12 to 18 days). The mean duration of followed up was 11.8 months (ranged from 8 to 22 months). The mean time of bone union was 6.6 months (ranged from 5 to 8 months). There was not any complication such as infection, implant failure, hip varus, external rotation deformity of low limb or fat embolism. The Sanders hip scores were 53.22 ± 6.48, the result was excellent in 12 cases and good in 6 cases at the last follow-up.
CONCLUSIONUnder the principle of biological osteosynthesis, treatment of Seinsheimer type V subtrochanteric femoral fracture with bridging locking plate fixation has such advantages as high mechanism, less interference of blood supply, stable fixation and little complication. It is a safe and idea way for the treatment of the Seinsheimer type V subtrochanteric femoral fracture.

4.Inhibitory effect of anterior cingulate cortex on spontaneous activity of thalamic ventrobasal nucleus neurons..

The purpose of this study was to investigate the influence of electrical stimulation of anterior cingulate cortex (ACC) on spontaneous activity of neurons in thalamic ventrobasal nucleus (VB). Experiments were performed on 12 male Sprague-Dawley rats weighing 250-310 g (4-5 months old). According to Paxinos and Watson's coordinate atlas of the rat, the frontal and parietal cortical areas were exposed by craniotomy, the recording electrodes were then inserted into the VB (P 2.4-4.1 mm, R 2.0-3.5 mm, H 5.2-6.8 mm) and the stimulating electrodes into the ACC (A 1.1-3.0 mm, R 0.0-1.0 mm, H 1.5-2.4 mm). Single-unit activities were recorded extracellularly in the VB by glass micropipettes (impedance 3-8 MOmega) filled with 0.5 mol/L sodium acetate solution containing saturated Fast Green. To study the effects of ACC activation on the spontaneous activities of VB cells, single electrical pulse (0.2 ms duration) was delivered to the ACC by a concentric bipolar stainless steel electrode (0.32 mm outer diameter). An effective ACC stimulation was determined for each VB neuron by gradually increasing the current intensity from 0.1 mA until either a significant change in the spontaneous activity of the VB neuron was observed, or the current intensity reached 0.4 mA. The results showed that ACC stimulation significantly suppressed the spontaneous activities in 12 out of 53 VB neurons (22.6%). (1) After the stimulation was delivered to ACC, the spontaneous activities of different VB neurons were totally suppressed for different time span. (2) There was obvious dose-effect relevance between ACC stimulation intensity and their inhibitory effect. The duration of complete inhibition was prolonged with the increases in the intensity and number of stimulation impulses in ACC. (3) The stimulation in the ACC depressed the spontaneous activities of VB neurons in different forms and this inhibition exhibited an accumulative effect. All these results indicate that the stimulation of ACC exerts an inhibitory influence on the spontaneous activities of VB neurons.

5.The effects of sodium salicylate on the expression of GABAalpha, NR1 and hearing response properties of inferior colliculus neurons in mice.

AIMTo study the effects of sodium salicylate on the expression of GABAalpha NR1 and hearing response properties of inferior colliculus neurons in mice.
METHODSThirty-six kunming mice were divided into three groups (A, B, C,). The expression of GABAalpha NR1 were measured by using RT-PCR. The intensity-rates functions, intensity-latency functions and frequency-turning curves were recorded by extracellular electrophysiological recording techniques.
RESULTS(1) The expression of GABAalpha mRNA of B group was decreased remarkably than the control group (A group, P < 0.05), there weren't noticeable differences between A group and C group (P > 0.05). The expression of NR1 mRNA of B group was increased remarkably than the control group (A group, P < 0.01), there were noticeable differences between A group and C group P < 0.05). (2) The intensity-rates functions, intensity-latency functions were monotonic while the frequency-turning curves were more broad when sodium salicylate was given. (3) The intensity-rates functions, intensity-latency functions were non-monotonic while the frequency-turning curves were sharpened after lidocaine was given.
CONCLUSIONS(1) The results suggested that administration of sodium salicylate decreased the expression of GABAalpha while increased the expression of NR1mRNA. (2) The intensity-rates functions, intensity-latency functions were monotonic, the frequency-turning curves were more broad when salicylate was given and the changes above could be reversed by given lidocaine.

6.Randomized controlled study on electroacupuncture compound anesthesia in radiofrequency ablation for hypertrophic inferior turbinate.

OBJECTIVETo explore the feasibility of electroacupuncture compound anesthesia in radiofrequency ablation for hypertrophic inferior turbinate.
METHODSThe patients confirmed to the enrolled criteria were randomly divided into an observation group (n = 31) and a control group (n = 30). The observation group was treated with electroacupuncture at Sibai (ST 2), Xiaguan (ST 7), Hegu (LI 4) and Zhigou (TE 6) on the left side and routine local anesthesia on the right side. The control group was treated with routine local anesthesia on the both side. The feelings of pain, circulatory index and operation effect were observed and compared.
RESULTSDuring radiofrequency ablation, the pain grade of two measurements on the left side and the 2nd measurement on the right in the observation group were all lower than those in the control group (all P<0.05). In the observation group, the pain grades on the left side were lower than that on the right side (P<0.05), and the systolic blood pressure and the heart rate were lower than that in the control group when undergoing the 2nd radiofrequency ablation on the right side and on the left side, respectively (both P<0.05). There was no significant difference in operation effect between the two groups.
CONCLUSIONElectroacupuncture compound anesthesia can meet the analgesia requirement of radiofrequency ablation for treatment of hypertrophic inferior turbinate, and would be helpful to prevent cyclic fluctuation during the operation at the same time.

8.Effect of Compound Zhajin Granule on Toll-like Receptor 4 Signaling Pathway in Nonalcoholic Steatohepatitis Mice.

OBJECTIVETo observe the effect of Compound Zhajin Granule (CZG) on Toll-like re-ceptor 4 (TLR4) signaling pathway in high-fructose corn syrup induced NASH mice.
METHODSThirty 6-week-old male C3H mice were divided into the high fat and high fructose (HFHFr) group (n = 20) and the control group (n = 10) according to body weight. Mice in the HFHFr group ate high fat diet and drank 20% fructose water, while those in the control group ate common diet and drank common water. After 8 weeks mice in the HFHFr group were divided into two group according to body weight, the HFHFr group and the CZG group, 10 in each group. Mice in the CZG group were fed with high fat forage and 20% fructose water, and administered with 50 mL/kg 12. 8% CZG (prepared by hawthorn, Radix Curcumae, Alisma Orientale, Fritillaria Thunbergii, Silybum Marianum, peach seed in the ratio of 3:1.5:1.5:2:1.5:2:1) by gastrogavage. Mice in the HFHFr group were fed in the same way and daily administered with equal volume of distilled water by gastrogavage. Sixteen weeks later all mice were sacrificed. Body weight, liver wet weight, liver function, and lipid metabolism were detected. Pathological changes of liver tissues were assessed by HE staining, oil red O staining, and Masson staining. Expressions of TLR4, myeloid differentiation factor 88 (MyD88), tumor necrosis factor-alpha (TNF-α) were detected using immunohistochemical staining and real-time fluorescent quantitative PCR.
RESULTSBody weight, alanine aminotransferase (ALT), aspartate aminotransferase (AST) were obviously lower in the CZG group than in the HFHFr group (P < 0.05); oil red O stained area and density were decreased more in the CZG group than in the control group. HE staining showed ballooning inflammation was reduced more in the CZG group than in the HFHFr group. Masson staining was negative. Positive rates of TLR4 and MyD88 and mRNA expressions were significantly lower in the CZG group than in the HFHFr group (all P < 0.05).
CONCLUSIONCZG could significantly inhibit TLR4 signaling pathway of liver in NASH mice.

9.Correlations between maximum standardized uptake value in 18F-FDG PET/CT and clinical pathological characteristics of moderately to poorly differentiated hepatocellular carcinoma

Objective:To investigate the correlations between the maximum standardized uptake value (SUVmax) in fluorodeoxyglucose F18 (18F-FDG) positron emission tomography-computed tomography (PET/CT) with the tumor differentiation,tumor size,and Ki-67 expression in patients with moderately and poorly differentiated hepatocellular carcinoma (HCC).Methods:The 18F-FDG PET/CT imaging data of 187 patients with single HCC lesion confirmed by pathology were evaluated retrospectively.According to the differentiation degree of tumor pathology,the patients were divided into moderate differentiation group,moderate-poor differentiation group,and poor differentiation group.According to the maximum diameter of the tumor,the patients were divided into ≤3 cm,3-5 cm,5-10 cm,and >10 cm group.According to the positive rate of Ki-67 expression,the patients were divided into high expression group (++,+++,and ++++) and low expression group (-and +).The SUVmax of primary tumor and the tumor to normal background ratio (TNR) were compared among groups.Furthermore,the correlations between SUVmax and pathological characteristics of HCC such as the maximum diameter of the tumor,tumor differentiation,and Ki 67 expression were analyzed.Results:The SUVmax and TNR were significantly different among moderate differentiation group (n =98),moderate-poor differentiation group (n =52) and poor differentiation group (n=37,both P<0.000 1),and the SUVmax was significantly correlated with the differentiation degree of tumor cells (r=-0.384 57,P<0.000 1).The maximum tumor diameter ≤3 cm group (n=70),3 5 cm group (n=51),5-10 cm group (n=43),and >10 cm group (n=23) had significant differences in SUVmax and TNR (both P<0.000 1),and the SUVmax was significantly correlated with the maximum tumor diameter (r=0.537 27,P<0.000 1).The SUVmax and TNR were both significantly different between Ki-67 low expression group (n=102) and Ki-67 high expression group (n=85,both P< 0.000 1),and the SUVmax was significantly correlated with the expression of Ki-67 (r=0.370 96,P<0.000 1).Conclusions:In patients with moderately to poorly differentiated HCC,the SUVmax and TNR in 18FFDG PET/CT are significantly associated with poor differentiation,large tumor size,and high expression of Ki* 67.
10.Anti-melanoma action of small molecular peptides derived from Brucea javanica (L.) Merr.globulin in vitro

Objective:The morbidity of malignant melanoma keeps increasing annually.It has high risks of metastasis,drug resistance,and poor prognosis in clinics.Moreover,the available medicines used commonly,such as dacarbazine,temozolomide,the v-Raf murine sarcoma viral oncogene homolog B1(BRAF) inhibitor vemurafenib,and the programmed cell death protein 1 inhibitor pembrolizumab,have some limitations at some extent.Therefore,a more effective therapeutic strategy is still urgently necessary.Methods:In this study,Brucea javanica(L.)Merr.globulins were hydrolyzed with pepsin,then ultra-filtrated to collect small molecular peptides (≤3 kDa).The peptides were then analyzed by anti-proliferative assay,cell-cycle distribution,apoptosis assay,and in vitro wound-scratch assay.Finally,western blotting was conducted to elucidate the underlying anti-melanoma mechanism.Results:The small molecular peptide from B.javanica significantly inhibited malignant melanoma cell proliferation with the IC50 of 2.72 μg/mL for 72 h.Further analysis indicated that B.javanica peptides arrested cell cycle at the S and G2/M phases and induced apoptosis by upregulating p21,p53,Bax,caspase-3,and cleaved PARP while downregulating Bcl-2 expression.The inhibitory migration effects were also confirmed by wound-healing assay.Conclusion:The small molecular biopeptides from B.javanica may be a promising bioactive agent candidate for melanoma treatment.
