2.Clinical application of incision cataract extraction after manual fragmentation and emulsification of nucleus for traumatic lenssubluxation and cataract

AIM: To observe the surgical effects of the taumatic lens subluxation and cataract after manual fragmentation and emulsification of nucleus and foldable intraocular lens implantation.
METHODS: A 3. 0mm tunnel limbus incision was operated through the predicted bulbar conjunctiva and sclera on 26 cases ( 26 eyes ) with taumatic lens subluxation ( suspensory ligament rupture range less than 120 ) and cataract (Ⅰ ~ Ⅲ) . And after the manual fragmentation and emulsification of nucleus, foldable intraocular lens was implantated. Intraocular lens loop was imbedded in the middle of the lens zonular ligament breakup to reset the pouch. The surgical complications and postoperative vision changes were observed.
RESULTS:Three month after operation, 22 eyes had a intraocular lens centric position taking up 85% of the whole. Four eyes had a slightly eccentric position ( 1 ~2mm), taking up 15% of the whole. 21 eyes had their visual acuity 0. 5~0. 8, taking up 81% of the whole. Five eyes of visual acuity was 0. 2~0. 8. Within 24h intraocular pressure of 12 eyes (46%) after operation were elevated, and returned to normal after 2~7d. There was no severe complication during operation and postoperation.
CONCLUSION: The manual fragmentation and emulsification of nucleus and foldable intraocular lens implantation of the traumatic lens subluxation and the cataract through the 3. 0mm corneal sclera limbus tunnel incision is a simple and effective surgery.
3.Correlation research on contents of podophyllotoxin and total lignans in Sinopodophyllum hexandrum and ecological factors.

To explore the correlation between the ecological factors and the contents of podophyllotoxin and total lignans in root and rhizome of Sinopodophyllum hexandrum, podophyllotoxin in 87 samples (from 5 provinces) was determined by HPLC and total lignans by UV. A correlation and regression analysis was made by software SPSS 16.0 in combination with ecological factors (terrain, soil and climate). The content determination results showed a great difference between podophyllotoxin and total lignans, attaining 1.001%-6.230% and 5.350%-16.34%, respective. The correlation and regression analysis by SPSS showed a positive linear correlation between their contents, strong positive correlation between their contents, latitude and annual average rainfall within the sampling area, weak negative correlation with pH value and organic material in soil, weaker and stronger positive correlations with soil potassium, weak negative correlation with slope and annual average temperature and weaker positive correlation between the podophyllotoxin content and soil potassium.

6.Separation, purification and primary reverse cholesterol transport study of Cordyceps militaris polysaccharide.

The authors designed to separate, purify and determine the monosaccharide composition of the polysaccharide from Cordyceps militaris, and study its effect on reverse cholesterol transport in vivo by isotope tracing assay. Polysaccharides were separate and purify by ion exchange column Q-sepharose Fast Flow and size exclusion column Sephacryl S200HR; the molecular weight and monosaccharide composition of the polysaccharides were determined by high performance gel permeation chromatography and high performance liquid chromatography coming with pre-column derivation, respectively. Finally, three purified polysaccharides CMBW1, CMBW2 and CMYW1 were obtained, their total carbohydrate contents were 87%, 89%, 95%, respectively; their protein contents were 6.5%, 1.3%, 2.8%, respectively; their molecular weights were 772.1, 20.9, 13.2 kDa, respectively; CMBW1 was composed of mannose, glucosamine, rhamnose, glucuronic acid, glucose, galactose and arabinose with a molar ratio of 7.25: 0.17: 1.29: 0.23: 6.30: 11.08: 0.79; CMBW2 was composed of mannose, glucosamine, galactose and arabinose with a molar ratio of 2.40: 0.16: 2.92: 0.24; CMYW1 was composed of mannose, glucosamine, glucuronic acid and glucose with a molar ratio of 0.59: 0.57: 0.45: 25.61. Polysaccharide at 50 mg x kg(-1) could significantly improve the transport of 3H- cholesterol to blood and excretion from feces. All of the three purified polysaccharides CMBW1, CMBW2 and CMYW1 were heteropolysaccharide; and they could improve reverse cholesterol transport in vivo, the underlying mechanisms are being studied.

7.Pericardial Effusion in Patients with Lung Cancer Treated with Safe-Dwel Plus Drain and Intrapericardial Chemotherapy

Objective: The aim of this study was to approach an efficient way in the treatment of pericardial effusion of lung cancer. Methods: Sixteen patients with pericardial effusion of lung cancer are drained with Safe-Dwel Tlus, then cisplatin and etoposide were injected into the pericardial cavity, and the draining continually effusion to less 50 ml per day. Results: The total effective rate was 93.8% with 13 cases of complete response rate and 2 cases of partial response rate. The median survival time was 6.5 months. 2 cases died from the relapse of pericardial effusion or no effect. Eleven cases had gastrointestinal tract reaction, and 3 cases with repression of bone marrow. However all side effects were controlled by treatment. Conclusion: Safe-Dwel Tlus drain with injecting cisplatin and etoposide into pericardial cavity may be a better way to control malignant pericardial effusion of lung cancer.
8.Efficacy of phacoemulsification combined with goniosynechialysis in treating primary angle-closure glaucoma with age-related cataract

·AIM: To study clinical efficacy of phacoemulsification combined with goniosynechialysis in treating primary angle-closure glaucoma with age-related cataract. ·METHODS:Totally 66 cases (70 eyes) of patients with primary angle - closure glaucoma with age - related cataract treated in our hospital from February 2014 to February 2017 were selected as the objects of study. Retrospective analysis was implemented to their case data. There were 36 cases (38 eyes) of patients who were treated with phacoemulsification and intraocular lens implantation combined with goniosynechialysis were set as observation group, while the other 30 cases (32 eyes ) of patients who were treated with phacoemulsification and intraocular lens implantation alone were set as control group. Two groups were compared for total success rate of surgery, the situation of eyesight, intraocular pressure, chamber depth and anterior chamber angle before and after surgery, moreover, the occurrences of complications were recorded. ·RESULTS: The absolute success rate of surgery in observation group was 63%, while that of control group was 47%, and the observation group was dramatically higher than control group, with statistical significance(P<0.05). The total success rate of observation group was 87%,while that of control group was 84%, and the difference had no statistical significance (P>0.05). The difference in terms of the number of people whose eyesight were <0.1, 0.1-<0.3, 0.3-0.5 or >0.5 before surgery compared with after surgeries had statistical significance (Zobservation group=7.545, Zcontrol group=7.213; P<0.05),while the difference between groups after surgery had no statistical significance(Z=-1.456,P>0.05). After surgery, the intraocular pressure of both groups decreased significantly(tobservation group=3.323, Pobservation group=0.001; tcontrol group= 10. 394, Pcontrol group<0. 01), and the observation group was significantly lower than control group after surgery, with a difference having statistical significance (t=14.802,P<0.01). Chamber depth of both groups decreased dramatically after surgery (tobservation group= 0. 411, Pobservation group< 0. 01; tcontrol grou=15.621, Pcontrol grou<0. 01), and the difference between groups after surgery had statistical significance (t =15.246,P<0.01). The occurrence rate of complications in observation group was 11%, while that of control group was 25%,and observation group was significantly lower than control group, having statistical significance (x2=4.409,P=0.036). · CONCLUSION: Goniosynechialysis combined with phacoemulsification have obvious efficacy in treating primary angle-closure glaucoma complicated with age-related cataract. It performs better in controlling intraocular pressure after surgery with lower occurrence rate of complication.
9.Effect of topographical factors on podophyllotoxin content in Sinopodophyllum hexandrum and study on ecological suitability.

In order to find the optimal topographical factor for regionslization, the content of cimetidine in 116 Sinopodophyllum hexandrum sample collected from Sichuan, Qinghai, Gansu, Tibet, Yunnan and Shaanxi provinces, was determined. Using mathematical statistics and geographical spatial analysis of GIS analysis, the relationship between content of podophyllotoxin and influencing factors including altitude gradient and gradient position was analyzed. It is found that the optimal altitude was 2 800 m to 3 600 m, the aspect of slope north or northeast and northwest and the slope 12 degrees to 65 degrees with a high suitability degree. Considering the artificial planting, the suitable planting area for S. hexandrum is comfirmed. The topographical factor is important for S. hexandrum regionalization, but has hardly effect on podophyllotoxin content. The results of the study provide an important scientific basis for S. hexandrum production development. But there are many factors which affect suitability index and podophyllotoxin content of S. hexandrum, it is necessary to consider other factors like climate and soil while exploitation and protection of S. hexandrum.
