1.Applied anatomy of the greater omentum in transplantation

ObjectiveTo observe the shape, size, position and vessels of greater omentum. To explore the design of greater omentum and clarify its clinical significance. MethodsThree fresh and 12 formalin-fixed cadaveres were dissected. All of them had no damage nor injury. 1. Macro-micro-dissection: The shape, size and position of greater omentum were observed. The vessels of the greater omentum were studied.2.Lead oxide-gelatin injection technique and three-dimensional reconstruction.Results The length of greater omentum was(24.7±6.9)cm,the width was(28.3+2.8)cm. It could be divided into 3 types: thin type,middle type and thick type, the occurrence rate was 33.3%, 46.7% and 20.0% respectively. The blood supply of greater omentum mainly came from the gastroepiploic arch composed of the left gastroepiploic artery and the right gastroepiploic artery.There were 4 main arteries of the greater omentum: ①The right epiploic artery: The external diameter at origination was (1.0±0.4)mm. ② The middle epiploic artery: The external diameter at origination was(0.7 + 0.3)mm. ③The left epiploic artery:The external diameter at origination was (1.2±0.4)mm.④ The accessary epiploic artery:The external diameter at origination was (0.5 ± 0.1)mm.ConclusionAccording to the characteristic of the anterior arch and posterior arch,the position of the middle epiploic artery divarication and the method of spread,the greater omentum is divided into 5 types.It should be according to the characteristic, when the greater omentum is designed and clipped.
2.The method of tracing the first dorsal metatarsal artery at the layer superficial to the extensor expansion in thumb and finger reconstruction

Objective To clarify the relationship between the first metatarsal web space and associated vessels and its application in dissection of the toes for thumb and finger reconstruction. Methods The relationships of the first dorsal metatarsal artery to the first deep transverse metatarsal ligament and the extensor expansion were observed on 42 adult cadaver lower limbs. Clinically 36 cases of thumb defects were reconstructed using the method of tracing the first dorsal metatarsal artery around the space of extensor expansion to dissect toes. Results The distal segment of the first dorsal metatarsal artery of Gilbert type I and type Ⅱwas located superficially to the layer of the extensor expansion.The time of harvesting the toe was shortened from 90 minutes to 50 minutes with 100%survival of reconstruction. Conclusions The distal segment of the first dorsal meatarsal artery lies constantly to the superficial layer of the extensor expansion.Consequently the location of the first metatarsal artery of Gilbert type I and type Ⅱbecomes much easier, by adopting the method of combination of sequential dissection and reverse dissection around the space of the extensor expansion.
3.THE EFFECT OF SIGNAL TRANSDUCTION MECHANISM ON BBB DAMAGE IN BRAIN INJURY OF RATS

This study was designed to explore the mechanism of signal transduction on BBB damage in brain injury. 75 male SD rats were randomly devided into 5 groups: control, sham operation, injury only, injury with Forskolin-treatment and H7 treatment.Evans blue permeation was observed qualitatively with an epifluo resence and mesured quantitatively with a spetrophotometer.The result shcwed that Evans blue significantly permeated in injuried group,and noticeably decereased in H7 treatment group and Forskolin treatment group. It indicated that Forskolin and H7 can effectively prevent BBB opening,and suggest that Forskoling and H7 may provide new ways on BBB protection.

5.PROGRESS OF TREATMENT OF WRIST DISORDER BY LIMITED INTERCARPAL ARTHRODESIS

Objective To review the recent progress in the treatment of wrist disorder by limited intercarpal arthrodesis and the related experimental study. Methods Recent original articles related to limited intercarpal arthrodesis, including clinical practice and experimental study, were extensively retrieved and carefully analyzed. Results Limited intercarpal arthrodesis could relieve pain and stabilize the wrist joint with partial motion. Conclusion With suitable indication and well selected operation approach, the limited intercarpal arthrodesis should be the optimal surgical intervention than total carpal athrodesis in the treatment of wrist disorder.
6.Clinical application of the palmaris brevis musculocutaneous flap

Objective To study of applied anatomy of the palmaris brevis musculocutaneous flap and to repair of pulp defect of thumb by free the palmaris brevis musculocutaneous flap. Methods The distributions of vascularis and nerves of the palmaris brevis musculocutaneous flap were observed on 30 upper limbs which were injected red latex. Nine cases who sufferd from pulp defects of thumb were treated by free the palmaris brevis musculocutaneous flap. Results There were four sources of blood supply to palmaris brevis musculocutaneous flap ;ulnar arterial trunk,musculocutaneous and cutaneous branches of superficial palmar branch,the musculocutaneous branches from pro fund branch of ulnar artery and the proper volar ulnar digital artery of little finger..The venous drainages of the flap were venae comitantes of arteries mentioned above, nine cases who suffered from pulp defects of thumb were treated by free the palmaris brevis musculocutaneous flap successfully. Conclusions The palmaris brevis musculocutaneous flap and pulp of thumb have similar characteristics. The blood supply to the palmaris brevis musculocutaneous flap was rich,it could be used as a sensory flap and was one of good donor regions for repairing large pulp defects of thumb.
7.Comparative Study on the Cracking Sounds During Two Rotary Manipulations of Neck

Objective:To improve the accuracy of the rotary manipulations of the neck in the treatment of neck pain, according to subjective and objective clinical findings. Methods: Two groups of 60 outpatients diagnosed with neck pain were suited to the manipulation and each group was 30 cases. The diagnosis of neck pain was made with conventional clinical evaluation, including history, palpation examinations, CT scan and X-ray. Group A received a manipulation of obliquely pulling the neck. While group B received manipulation of pulling the rotated and located neck. Subjects received a maximum of 10 treatments over a 3-week treatment period. Both treatment groups were assessed with subjective (cervicodynia, neck discomfort, upper limb numbness, vertigo and tinnitus) and objective (cracking sounds). The cracking sounds during two of the manipu1ations were recorded by a microphone positioned at both sides of the neck. The sounds were compared in patients of two groups. Statistical analysis was conducted with a one-factor within-subjects analysis of variance tests and descriptive statistic. Results: Analysis of the subjective indicated that the cases with upper limb numbness were reduced from 12 to 6 in group 2, while the cases with vertigo and tinnitus were reduced from 13 to 4 in group 1. Analysis of the objective indicated two kinds of neck rotary manipulations act not only on the rotary-side facet joints, but on the opposite to the rotary side on1y in a few cases. The results showed that manipulation of pulling the rotated and located neck has a small action range and centralized action points, while manipulation of obliquely pulling the neck has many action segments and discentra1ized action points. Conclusion: It was concluded that the rotary manipu1ations act mainly on the rotary side, not on the opposite, and that manipulation of obliquely pulling the neck has a wider action range and a better therapeutic effect on cervical upper-segment lesions, while manipulation of pulling the rotated and located neck has a limited action range and is suitable for cervical lower-segment lesions. There were some relations between the joint sounds during the manipulation and clinical curative effect.

Objective To express human single-chain antibody against amyloid?peptide 40 in pichia pastoris for passive immuniza- tion to Alzheimer's disease patients.Methods Human single-chain antibody gene from a phage display library was mutated to obliterate BamHI site and cloned into pAO815 plasmid for pT-scFvA?40 construction which was identified by PCR amplification and endonuclease digestion.Vector pT-scFvA?40 was cut by EcoRI and NotI endonucleases and the antibody gene was cloned into pPICgK plasmid to con- struct pPIC9K-scFvA?40 expression vector which was confirmed by gene sequencing.Linearized pPIC9K-scFvA?40 was used to trans- form pichia pastoris GS115 and the recombinant was induced by 0.5% methanol to express human single - chain antibody against amyloid?peptide 40.Results Gene sequencing confirmed pPIC9K-scFvA?40 orientation.Recombinants were obtained by linearized pPICgK - scFvA?40 transformation.After inducing with 0.5% methanol,the recombinant secreted proteins with 33 kD size.Conclusions Expression vector pPIC9K-scFvA?40 was constructed successfully.Human single-chain antibody against amyloid?peptide 40 was recombinantly expressed in pichia pastoris.
9.The Conjugation of Fluorescein Isothiocyanate to Annexin V and its Application

Objective To prepare a novel annexin V and conjugate it with fluorescein isothiocyanate and test their binding ability to phosphatidylserine(PS)exposed erythrocytes.Methods The annexin V fused with an metal chelating binding site was obtained from pichia pastoris culturing and methanol induction expression.The annexin V was purified from the culture supernatant crude product by ion-exchange chromatography and ammonium sulfate precipitation.The annexin V was conjugated with fluorescein isothiocyanate in boric buffer and the conjugate annexin V-FITC was purified by ion-exchange chromatography.Sheep red blood cells were treated with Glutaraldehyde to expose membrane PS.The annexin V-FITC binding to PS exposed erythrocytes was investigated by fluorescent microscopy.Results The novel annexin V was purified and concentrated from pichia pastoris culture by ion-exchange chromatography and ammonium sulfate precipitation.After conjugating with FITC,the annexin V was found to bind PS exposed erythrocytes by fluorescent microscopy.Conclusion The annexin V expressed recombinantly in pichia pastoris retains the binding ability to PS exposed erythrocytes and is applicable to apoptosis detection in vitro.
10.Inhibit effect of Curcumin on human papilloma virus replication by inducing cell autophagy

Objective To explore the effect of autophagy induced by Curcumin on human papilloma virus (HPV) replication . Methods Human cervical cancer cell SiHa were treated with Curcumin to induce autophagy .Fluorescence microscope was used to observe the autophagosome .Western blotting was performed to analyze the expression level of autophagy marker protein LC3‐ Ⅱ /LC3‐ Ⅰ .Real time‐polymerase chain reaction(PCR) and Western blotting were used to detect HPV E6 mRNA and protein expres‐sion level .After adding the autophagy inhibitor 3‐methyl adenine (3‐MA) ,real time‐PCR and Western blotting was employed to detect the expression level of HPV E6 .Results The fluorescence intensity of SiHa cells treated by Curcumin was significant in‐creased ,while the ratio of LC3‐ Ⅱ and LC3‐ Ⅰ was also significantly increased (P< 0 .05) .The expression of HPV E6 mRNA and protein were decreased significantly after induction of autophagy ,while increased significantly after adding autophagy inhibitor 3‐MA .Conclusion Curcumin might inhibit HPV replication by inducing cell autophagy .
