1.AFFERENT CONNECTIONS OF SUBSTANTIA NIGRA IN THE RAT

The substantia nigra(SN)has been considered to be related to motor function.The recent physiological study has shown that the SN is not only one of theelements of extrapyramidal system,but is also related to the sensory system and takepart in the modulation of pain.In order to provide morphological evidence for itsfunctional aspect,42 rats have been employed for study by retrograde transportmethod of horseradish peroxidase(HRP),using microinjective and microiontophoreticdelivery system through vertical and oblique approaches.The results showed that the main labeled nuclei and neuro-structures were thecaudoputamen nucleus,dorsal raphe nucleus,central amygdaloid nucleus,globuspallidus,cerebral cortex,median raphe nucleus,locus coeruleus,periaqueductal grayand parabrachium nuclei.Topographical organization of the projections from these labeled nuclei andneuro-structures to SN was studied in detail.Among the structures studied the labeled cells were found most numerous incaudoputamen nucleus(CP).The projection fibers start in a order from the antero-dorso-medial to postero-ventro-lateral part of CP and end in the medial to lateralof SN in succession.Topographical projection from dorsal raphe nucleus to SN was also found.Thedorsal raphe nucleus only project to the medial two-thirds of SN.The fibers from central amygdaloid nucleus and locus coeruleus terminate in thelateral two-thirds of SN.Only the medial one half of SN receives the projection fibers from globuspallidus.In cerebral cortex,the main regions where labeled cells could be seen wereinsula,frontal and parietal regions.The projection fibers coming from median raphe nucleus terminate to the medialone-third of SN.When HRP was injected into lateral or median one-third of SN,numerousand clear cut labeled cells can be seen in the ventro-lateral region of periaqueductalgray.The projection from parabrachium nuclei terminates to the lateral one half ofSN.

3.Curative Effect of Bifid Triple Viable Capsules in Patients after Laparoscopic Cholecystectomy

Objective:To discuss the curative effect of bifid triple viable capsules in the patients after laparoscopic cholecystecto-my ( LC) . Methods:Totally 100 cases of patients with LC were selected and divided into the observation group and the control group at random. The patients in the two groups underwent LC under the general anesthesia, and were given routine medical treatment after the operation. The patients in the observation group were additionally given bifid triple viable capsules, 630mg per time, twice a day for 1 week as the treatment course. The changes in the occurrence of diarrhea, alteration of intestinal flora and plasma D-lactic acid of the patients and drug adverse reactions in the two groups were observed and compared after the medical treatment. Results:One week after the operation, the occurrence of diarrhea and alteration of intestinal flora of the patients in the observation group were lower than those in the control group (P<0. 05), and compared with those before the treatment, the plasma D-lactic acid levels of the patients in the two groups were declined (P<0. 05 or P<0. 01), and the declining rate in the observation group was much higher than that in the control group (P<0. 05). Totally 3 and 5 cases of drug adverse reactions were appeared in the observation group and the control group, respectively with no significant difference (P>0. 05). Conclusion: The application of bifid triple viable capsules in the pa-tients after LC can effectively reduce the plasma D-lactic acid levels, occurrence of diarrhea and alteration of intestinal flora with prom-ising security, which is good for the postoperative recovery of the patients.

The effect of loss-of-function of Attractin (Atrn) on the male mouse reproduction system was examined in the study. The weights and pathological changes of testes and epididymes were compared between Atrn mutant (Atrn ( mg-3J )) mice and wild-type mice (C3HeB/FeJ) at different months of age. The number and motility of sperms were measured in the mutant and control mice. Furthermore, the testicular lactate dehydrogenase (LDH) and succinate dehydrogenase (SDH) in these animals were detected. The fertility potential of the sperms was observed in vivo and in vitro. The results showed that the testes of 3-month-old Atrn ( mg-3J ) mice experienced no significantly different pathological changes from the control mice at the same month of age but the SDH activity was substantially reduced. In the 5-month-old mutant mice, as compared with the control mice, mild vacuolation was found in the testes, the density and motility of sperms were decreased in the epididymes, the sperm fertility was impaired and the testicular enzyme activity was reduced. It is concluded that the age-related Atrn gene progressively loses its function and can cause testis vacuolation and impaired sperm function, which may be responsible for the impairment of male reproductive ability.
6.Fecal calprotectin in estimation of activity of peptic ulcers

Objective To explore the clinical value of fecal calprotectin (FCP) in peptic ulcer (PU) as an non-invasive indicator of disease activity compared with gastroscope. Methods The study was conducted in 62 patients with PU confirmed by endoscopy ( PU group) and 30 subjects with normal findings under endoscopy ( control group). Fecal sample ( weight 5-10 g) was collected within 3 days after endoscopy and FCP was measured by emzyme-linked immunosorbent assay (ELISA). The case history and clinical data were collected as well. Results The level of FCP in PU group was significantly higher than that in control group ( 154. 72 μg/g vs. 25. 18 μg/g, P < 0.001 ). In patients with PU at active stage ( n = 32), the level of FCP was significantly higher than that at scar stage (n =30,318.34 μg/g vs. 54. 10 μg/g, P <0. 01 ), and that in control group (25.18 μg/g, P <0.01), while there was no significant difference in FCP between the latter two groups ( P >0. 05 ). The level of FCP had no significant correlation with the location, size or number of the ulcer. Among patients in PU group, the level of FCP in patients presented with haematemesis or melena ( n = 20) was significantly higher than that in patients presented with other symptoms ( n = 42, 1257. 41 μg/g vs. 92. 77 μg/g, P < 0. 01 ). Conclusion The level of FCP is closely correlated with the activity of PU, which is significantly higher at active stage than that at scar stage, as well as in PU patients with bleeding than those without. Measurement of FCP is a convenient and noninvasive method with well compliance of patients, which might be used as an indicator of disease activity in PU.
7.Fifteen Years' Blood Pressure Change in 1079 Workers

Aim To study the change of blood pressure in 1079 workers for fifteen years and the effect of anti-hypertension drugs therapy. Methods We investigated the blood pressure of 1079 workers who were enrolled in six organizations in the year 1983 and 1998 respectively. Results During the 15 years: (1)Mean value of blood pressure: systolic blood pressure increased 22 mmHg in man and 16.9 mmHg in women; diastolic blood pressure increased 9 mmHg in man and 12.7 mmHg in women (P<0.05);(2)The incidence of hypertension increased by 25.03% in man and 28.28% in women;(3)The prevalence rate of hypertension is 27.9 percent in people with initially normal blood pressure (1.86%/y) and 72.6 percent in initially broder line hypertension (4.84%/y);(4)The control rate of hypertension is 2.9 percent;(5)The incidence of stroke is highly related to hypertension (P<0.001);(6)46.6 percent patients had a regular drug therapy, mainly reserpini complex(25.2%);(7)Drug therapy has no obviously effect of the control of hypertension and the incidence of stroke. Conclusion Both the mean value of BP and the incidence of hypertension were increased with age. The control rate of hypertension was low and the drug therapy shows little advantage. We should do mach more works to popularize the knowledge of the prevention of hypertension. Improve people's self-prevention. Regular cheek should be given to the hypertension patients.

9.High-risk primary disease and medical factors of ventilator-associated pneumonia in neonates: A Meta-analysis

Objective To provide the evidences for the management strategies of ventilator-associated pneumonia (VAP) in neonates,we systematically reviewed all related studies and analyzed the high-risk primary disease and medical factors of VAP in neonates.Methods We retrieved all related studies in CNKI,Wanfang,VIP,CBM,Pubmed and Embase and evaluated their quality by Newcastle-Ottawa Scale and analyzed all data by qualitative and Meta-analysis.Results There were 12 case-control studies with higher methodological quality and involving 1 994 neonates and with 708 VAP patients.Six studies involving 872 neonates were included,the odds ratio of respiratory distress syndrome(OR=2.81) and malnutrition(OR=5.18) had significant differences between VAP and non-VAP group.Seven studies involving 1 110 neonates were included and the odds ratio of patients with corticosteroids (OR=3.12),central inhibitors (OR=2.31),antacids (OR=4.35) and Gamma globulin with large doses (OR=2.35) had significant differences between VAP group and non VAP.Four studies involving 554 neonates were included and the odds ratio of patients with closed chest drainage (OR=1.81)and umbilical vein catheterization (OR=9.19) had significant differences between VAP group and non VAP.Six studies involving 1 139 neonates were included and the odds ratio of patients with parenteral nutrition (OR=1.82)and blood transfusions (OR=2.49) had significant differences between VAP group and non VAP.Conclusions Our study confirms that the respiratory distress syndrome and malnutrition corticosteroids,central inhibitors,antacids,Gamma globulin with large doses,closed chest drainage,umbilical vein catheterization,parenteral nutrition and blood transfusions are important risk and early-warning factors.
10.Medication Compliance Investigation and Pharmaceutical Care for 50 Cases of Hypertension

Objective: To understand the medication compliance of inpatients with essential hypertension, analyze the influencing factors in medication compliance of the patients and perform the targeted medication education and pharmaceutical care.Methods: Medication education and guide were conducted in 50 cases of patients with essential hypertension based on the main contents in guidelines for prevention of hypertension.Every patient completed regular follow-ups in two months after being recruited.The relevant information about the treatment and medication were collected and analyzed.Results: Through the medication education and pharmaceutical care, the patients had a better understanding on the relevant knowledge of hypertension, rational drug use and treatment.Among the 50 patients, the percentage of completely following the doctor's advice on the hypertension medication was 86%, that of quitting smoking and limiting alcohol was 82%, and that of optimized life way was 78%, and totally 82% of the patients effectively controlled blood pressure or improved markedly when compared with the situation on admission.Conclusion: Medication education and pharmaceutical care conducted by clinical pharmacists is very important for the patients with high blood pressure.It can help patients build a good lifestyle or improve the quality of life by improving the medication compliance of patients, alleviating complications and reducing adverse drug reactions.
