1.Air Disinfection with Chlorine Dioxide in Saps

Objective To observe the effect of air disinfection with chlorine dioxide(ClO2)in the saps and determine the concentration change in order to ensure the safety for people to enter the saps.Methods The disinfection was done at the concentration of 1 000 mg/L and the spray dose of 10 ml/m3 and then to count the total bacteria and fungi with the cascade impactors in the saps before and after air disinfection and to monitor the concentration of ClO2 with the air ClO2 monitor(iTX).Results The 94.6% of the total bacteria and 92.1% of the fungi died in 30 minutes after disinfection.The concentration of ClO2 in the air decreased from 10 mg/m3 to 0.8 mg/m3 81 minutes later and to 0.3 mg/m3 96 minutes later,after 120 minutes,it was 0 mg/m3.Conclusion The air disinfection effect of ClO2 used at the dose of 10 ml/m3 and the concentration of 1 000 mg/L is satisfactory.96 minutes after disinfection,the concentration of ClO2 is safe for human health.
2.The Investigation of Air Microorganisms Pollution in Underground Spaces

Objective To understand the air pollution by microorganisms in the underground spaces. Methods 51 underground spaces were investigated, the impact and sedimentation sampling methods were employed and the total bacterial count, mycetes, streptococcus and anaerobes in air were determined. ELESA was used to detect aflatoxin B1 in aspergillus flavus isolated from air. Results The total bacterial count in the air in underground spaces accorded with the military health standard. In the semi-closed spaces,the mycetes counts were 5 950 cfu/m3 and 9 500 cfu/m3 in aisle and rooms respectively, significantly higher than those in control group (P
3.Effect of interventional treatment for patients with coronary heart disease on brain natriuretic peptide and hsCRP levels

Objective To investigate the effect of interventional treatment in coronary heart disease (CHD) in elderly patients with brain natriuretic peptide and high-sensitivity C-reactive protein (hsCRP) levels.Methods 130 elderly patients undergoing coronary angiography interventional treatment were selected for the observation group,30 cases with normal coronary arteries angiography were chosen as the control group.In 46 cases of acute myocardial infarction(AMI) group,34 cases of unstable angina pectoris(UAP) group and 50 patients with stable angina pectoris (SAP) group and 30 cases coronary angiography normal control group(NOR),the hs-CRP and plasma brain natriuretic peptide(BNP) differences were determined and compared.Results CHD (AMI group,UAP group and SAP group) in patients with BNP levels were (548.6 ± 200.4) mg/L,(521.0 ± 198.7) mg/L and (175.8 ± 119.4) mg/L,which were significantly higher (59.3 ± 45.7) mg/L in the control group,hsCRP levels only in patients with acute myocardial infarction and unstable angina was significantly increased,the difference was statistically significant (t =13.74,10.65,all P < 0.05).3d after PCI and three months prior CHD hsCRP increased to (45.8 ± 15.3)ng/L,and then decreased to (8.6 ± 4.4) ng/L,significantly different (t =10.76,14.54,all P < 0.05) ; while BNP levels have been falling,first down (247.4 ± 70.1)mg/L,3 months after the falls (69.8 ± 20.0)mg/L,the difference was significant(t =11.47,19.64,all P<0.05).Conclusion The levels of brain natriuretic peptide,hsCRP are reflections of coronary stenting in patients with coronary heart disease early postoperative inflammatory reaction and evaluate a sensitive indicator of disease,which may play an role in stent restenosis occurs.
4.Clinical value of TNF-αand hypersensitivity-C reactive protein in elderly patients with chronic heart failure

Objective To investigate the clinical value of tumor necrosis factor-alpha ( TNF-) and hypersen-sitivity-C reactive protein ( hs-CRP ) in elderly patients with chronic heart failure ( CHF ) .Methods 85 elderly patients with CHF were randomly selected as the CHF group ,and 85 healthy pepole were chosen as the healthy control group .All subjects were taken fasting blood in the morning ,the TNF-αand hs-CRP levels were detected and compared between the two groups .The differences of plasma TNF-αand hs-CRP levels in different grades of CHF patients were also observed .Results The TNF-αand hs-CRP levels of the CHF group were significantly higher than those of the healthy control group(t=110.86,25.63,all P<0.05).With the elevation of heart function classification ,the TNF-αand hs-CRP levels gradually increased ,and compared with the previous grade ,the differences were statistically signifi-cant(F=7.65,16.75,all P<0.05).The results of linear correlation analysis showed that the levels of TNF-αand hs-CRP were negatively correlated with the LVEF (r1 =0.78,P<0.05;r2=0.71,P<0.05).The TNF-αand hs-CRP were positively correlated (r =0.83,P <0.05).Conclusion The plasma TNF-αand hs-CRP levels of elderly patients with CHF are increased ,and their levels are closely related with the heart function .
5.Significance of mediastinum drainage tube in prevention and treatment of intrathoracic anastomotic leak after esophagectomy

Objective To explore the significance of mediastinum drainage tube in prevention and treatment of intrathoracic anastomotic leak after esophagectomy. Methods The morbidity and prognosis of intrathoracic anas-tomotic leak after esophagectomy in 180 patients with esophageal or cardia carcinoma (group A) who used mediasti-num drainage tube in operation from 2006 to 2008 was studied retrospectively,and were compare with 154 patients (group B) without mediastinum drainage tube from 2004 to 2006. Results The morbidity of intrathoracic anasto-motic leak after esophagectomy was 1.67% (3/180) in group A,and 4.55% (7/154) in group B,but no difference between them(χ2=1.4807, P>0.05) were found. Compared to group B, the final diagnosis of anastomotic leak in group A was early [(6.7±0.6) vs (10.0±2.1) days after operation (t=2.62, P<0.05)] and the duration of fe-ver was short [(5.3±1.5)vs (43.0±20.9) hours (t=4.39, P<0.01)]. The prognosis of 3 cases in group A was well, who all recovered without operation and had shorten hospital days than group B [(23.7±5.9)d vs (45.3± 5.0)d,t=4.86,P<0.01)]. In 7 cases of group B,there were 5 cases underwent second operation,but only 3 cases recovered, the case fatality rate was 2.60% (4/154), higher than in group A (0, Fisher, P<0.05). Conclusions Although the mediastinum drainage tube used or not used in esophagectomy is unrelated with the occurrence of anas-tomotic leak, and the significance to use it is that it can help to discover anastomotic leak early, and control mediasti-num infection effectively without reoperation,and improve patients' prognosis.
6.Surgical treatment of invasive pulmonary fungal infections in patients with hematologic malignancies

Objective To summarise the experience of surgical treatment of invasive pulmonary fungal infection in patients with hematologic malignancies.Methods From Jan.2000 to Dec.2012,78 hematologic patients with invasive pulmonary fungal infections,including 49 males and 29 females with average age of 29.6 years,underwent surgical treatment.Preoperatively,all patients with 1 to 3 courses of chemotherapy had received average 4 weeks antifungal treatment.There were 58 cases received conventional open thoracic surgery and 20 cases received VATS including lobectomy,segmentectomy,wedgectomy.Results The procedures were successful,the average operation time w as(125.3 ± 35.7) min and intraoperative blood loss was(253.1 ± 42.8) nl.There were no severe postoperative complications.The main complications were excessive effusion (10.2%) and persistence airleak (7.7%).The hospital days was(7.8 ± 2.5) days,no reoperation and death case in 30 days after operation.The follow-up was from 6 to 24 months,38 cases(48.7%) received prophylaetic antifungal treatment,no ease with fungi spread and recurrence.Conclusion The surgical treatment is safe and useful for the hematologic patients with invasive pulmonary fungal infections,when internal medicine conservative treatment is invalid.
7.Clinical observation of recombinant human brain natriuretic peptide in the treatment of acute non-ST-segment elevation myocardial infarction patients with heart failure

Objective To investigate the the efficacy and safety of recombinant human brain natriuretic peptide (rhBNP) in the treatment of acute non-ST-segment elevation myocardial infarction patients with heart failure.Methods 80 patients with heart failure caused by acute non-ST-segment elevation myocardial infarction were chosen,they were divided into the observation group and control group based on a random number table,each group included 40 patients.The observation group were treated with intravenous injection of recombinant human brain natriuretic peptide,the control group were treated with intravenous nitroglycerin.vital signs,dyspnea,brain natriuretic peptide (BNP),associated hemodynamic changes and adverse reactions of two groups were compared before and after treatment.Results The total effective rate in observation group was 87.5% (35/40),which were significantly higher than 50.0% (20/40) of the control group,there were significant difference (x2 =13.09,P < 0.05).In the observation group,after treatment,the left ventricular ejection fraction (LVEF) and average hourly urine output were significantly higher than those of before treatment (t =26.40,3.22,all P < 0.05),and the brain natriuretic peptide (BNP) concentration levels were significantly lower than those of before treatment (t =14.11,all P < 0.05).The control group before and after treatment LVEF,BNP and changes in urine levels were not significantly.Conclusion In the treatment of heart failure caused by acute non-ST-segment elevation myocardial infarction,the recombinant human brain natriuretic peptide (rhBNP) has significant effect,feasible and safe with few side effects,which has a high clinical value.
8.Effects of human gastric cancer cell line SGC-7901 by p21~(WAF1/CIP1)gene transduction plus cisplatin

Purpose:To study the effects and overexpression of p21 WAF1/CIP1 gene (p21 gene) on the SGC 7901 cell line plus cisplatin. Methods:An adenovirus expression vector with full length cDNA of p21 gene insert was constructed (ad p21) and transferred into SGC 7901 cell line with p53 mutation, the effects of ad p21 on the growth of SGC 7901 cell line was examined in vitro and in vivo . Results:The expression of p21 gene in SGC 7901 cell was confirmed by FCM, The growth of the Ad p21 gene transfected SGC 7901 cells was significantly inhibited as compared to mock (Ad LacZ) transfected SGC 7901 in vitro and induced the transfected cells appearing apoptotic, Combined with chemotherapeutic agent cisplatin (DDP), it could more strongly inhibit SGC 7901 cell growths.Conclusions:p21 gene can inhibit SGC 7901 cells growth and shows enhanced therapeutic efficiency for SGC 7901 cells when combined with DDP, especially in vivo experiment. Its use for gene therapy of cancer, especially when combined with other apoptosis inducing agents is promising.
9.Effect of stress-induced hyperglycemia on myocardial perfusion and prognosis in elderly patients ;with acute myocardial infarction undergoing primary percutaneous coronary intervention

Objective To investigate the effect of stress-induced hyperglycemia (SHG) on myocardial perfusion and clinical prognosis in elderly patients with acute myocardial infarction (AMI) who underwent primary percutaneous coronary intervention (PCI). Methods In this study, 459 elderly patients with first-time occurrence of acute ST-elevation myocardial infarction (STEMI) who underwent primary PCI within 12 h from January 2012 to January 2015 were enrolled and followed up. All patients were divided into three groups according to serum glucose (SG) on admission: normal group (SG<7.0 mmol/L, 148 patients), SG elevation group (SG 7.0-11.1 mmol/L, 169 patients) and SHG group (SG>11.1 mmol/L, 142 patients). Myocardial perfusion indexes, including ST segment resolution (STR), TIMI myocardial perfusion grade (TMPG), peak value of creatine kinase (CK)-MB, left ventricular ejection (LVEF), and major adverse cardiac events (MACE) of patients in three groups were measured and compared after emergency PCI. Results The blood glucose levels were increased, ST-elevation 2 h after PCI were well declined, the percentages of patients with TMPG 2-3 were decreased and peak values of CK-MB were increased in the three groups (P<0.05). After 12 months of follow-up, Kaplan-Meier survival analysis showed that cumulative non-events survival rates in three groups had significantly different: 89.2% (132/148) vs. 85.8% (145/169) and 76.1% (108/142), P<0.05. Multivariate Cox regression analysis showed that SHG was the independent predictor for the occurrence of MACE in patients undergoing PCI after adjusting for age and gender (P<0.05). Conclusions SHG in elderly patients with STEMI can decrease myocardial perfusion level after primary PCI, which will lead to high incidence of MACE.
10.Trastuzumab inhabits the process of radiation induced Her-2 nuclear transport in breast cancer cell line SKBR3

Objective To observe the influence of trastuzumab on DNA break repair and Her-2 nuclear import after radiation in breast cancer cell line SKBR3,and discuss the radiosensitivity mechanism of trastuzumab.Methods Clone formation assay was used to analyze the difference of survival fractions between radiation group and radiation plus trastuzumab group.Confocal microscopy was applied to observe the influence of trastuzumab in the nuclear import process of Her-2 and the expression of γH2AX after radiation,which is considered as the marker of DNA double strand break.Western blotting was used to detect the expression of Her-2 and DNA-PKcs in nuclei after radiation.Results The result of clone formation assayshowed that the SF2 in radiation group was 0.547±0.046 and 0.321±0.022 in the radiation plus trastuzumab group were significantly decreased,the results of confocal microscopy showed that trastuzumab postponed the nuclear import process of Her-2 (52.80±19.74 in radiation group,21.41±10.55 in the radiation group),and increased expression of γH2AX after radiation (85.40±25.63 in radiation group,18.53±44.32 in the radiation group),and western blotting revealed trastuzumab reduced the expression of Her-2,DNA-PKcs in nuclei.Conclusion Trastuzumab can inhibit the radiation induced nuclear import of Her-2,and decrease Her-2,DNA-PKcs in nuclei to increase the DSB on early stage after radiation.
