2.Strategic thinking on hospital groups
Chinese Journal of Hospital Administration 1996;0(05):-
As a successful mode of hospital reform, the hospital group offers the key to the solution of some major problems hindering the development of hospitals. The objective of setting up a hospital group is to better realize the strategic goals of the hospitals. While hospital groups have different types and formation patterns, the key to success lies in the operating mechanism and management standards. Organizational and cultural building is of great significance to the long-term development of hospital groups.

Objective To investigate the alteration in intercellular adhesion molecule (ICAM)-l mRNA expression on the surface of human vascular endothelial cells (HUVECs) induced by LPS stimulation and the effects of hypertonic-hyperoncotic solution (HHS) . Methods HUVECs were enzymatically isolated and cultured in RPMI 1640 culture medium at 37℃ for about 7 days when a monolayer of endothelial cells has grown. The primarily cultured HUVECs were mixed with LPS 100ng/ml and incubated at 37℃ for 30min, 1,4,8, 12and 24 h. 10%NaCl and 10% hydroxyethyl starch (Fresenius) were added to RPMI 1640culture medium. The HHS concentrations were 0.25% and 0.5% respectively. HUVECs were first incubated with either HHS (0.25% , 0.5% ) or isotonic solution for 10 min, 1h and 4h and then stimulated by LPS for 4h. The ICAM-1 mRNA was measured by reverse transcriptase-PCR. Results There was slight expression of ICAM-1 mRNA on the surface of HUVECs in normal condition. ICAM-1 mRNA expression began to increase at 1h exposure to LPS and reached peak value at 4h. Both 0.25% and 0.5% HHS could prevent the LPS-induced ICAM-1 mRNA up-regulation (P

Objective To evaluate the role of postoperative electron beam radiotherapy in the management of ke-loids. Methods Between January 2004 and December 2006,116 cases of keloids were treated with 6 MeV electron beam radiotherapy within 24 h after surgical resection of the keloids. All patients had received a total of 21 Gy in seven daily 3-Gy fractions. Treatment was started at the 24th h after surgery. Patients were followed up and the information regarding treatment results, early and late radiation toxicity, and the satisfaction level by self-assessments was recorded. Results Recurrence occurred in 17 cases of keloids(14. 7%),and was correlated with site of the lesions(χ~2 =29. 91,P<0. 01). Most recurrences were observed at site of sternum (10/43) and shoulder (5/13 ). Keloid associated symptoms, e. g. itching and pain, were improved in 88. %%. For early toxicity outcomes, 100% had grade 1~2 skin erythema,7. 8% had wound delayed union, and 4. 3% had infection. For late toxicity outcomes, 30. 2% reported grade 1~2 hyperpigmentation, 11. 2% grade 1~2 hypopigmentation, and 5. 2% grade 1 telangiectasia. No severe complications or secondary malignancies were observed. 72. 4% patients described the results of treatment as excellent or good, and 15. 5% patients were not satisfied with the treatment results. Conclusion Postoperative electron radiotherapy is well tolerated and effective in preventing keloid recurrence.

Chinese Journal of Tissue Engineering Research 2014;(11):1730-1736
BACKGROUND:In recent years, many drugs emerge to control tooth movement, and scholars in China begin to investigate Chinese herbs with moderate nature and smal adverse reaction.
OBJECTIVE:To observe the relapse after orthodontic tooth movement, osteoprotegerin and osteoclast differentiation factor expression in periodontal tissue after rats were treated with local tanshinone type IIA at different doses.
METHODS:A total of 48 male Wistar rats were randomly divided into four groups:control, low dose (tanshinone type IIA 0.36 mg/d), medium dose (tanshinone type IIA 0.72 mg/d), and high dose (tanshinone type IIA 1.44 mg/d) groups. Taking anterior teeth as the anchorage, the maxil ary first molar of rats was tracted to mesial movement. In experimental groups, gingival mucosa of the first molar was local injected with tanshinone type IIA 1 day
before the force device was removed, while control group was injected with physiological saline, once a day, for 4 weeks. Immediately, 1 week, and 4 weeks after the force device was removed, the distance between the maxil ary first molar and second molar was measured and body mass was weighted. The animals were kil ed after 4 weeks, osteoprotegerin and osteoclast differentiation factor expression in maxil ary first molar and periodontal tissue were determined using immunohistochemical staining.
RESULTS AND CONCLUSION:There was no obvious change in the body weight of rats in each group (P>0.05). In low, medium and high dose groups, recurrent distance of the teeth was shorter than that in control group (P<0.05), and recurrence percentage was significantly lower than control group (P<0.05). The greater the dose was, the
smal er the degree of recurrence was. Osteoprotegerin expression in the periodontal tissue was significantly higher in the experimental groups than the control group (P<0.05), while osteoclast differentiation factor expression was significantly lower than the control group (P<0.05). The ratio of osteoprotegerin/osteoclast differentiation factor in the periodontal tissue was greater than 1 in both control group and experimental groups, and reached the peak in the high dose group. Local delivery of tanshinone type IIA has no impact on body weight of normal rats, and can effectively control the recurrence rate after orthodontic tooth movement. Within a certain range, high dose achieves the most obvious effect. Regulating osteoclast through adjusting the ratio of osteoprotegerin/osteoclast differentiation factor could be the molecular mechanism of tanshinone type IIA accelerating the periodontal tissue rebuilding.

Journal of Regional Anatomy and Operative Surgery 2013;(6):625-626,628
Objective To observe the analgesic effect and its complications of flurbiprofen axetil for spastic pain after transurethral re-section of prostate( TUR-P) . Methods 40 patients who were clinically diagnosed as benign prostatic hyperplasia and underwent TUR-P were randomly divided into the experimental group (n=20)and the control group(n=20). When the postsurgical spastic bladder pain happened, the patients in the experimental group received intravenous drip of flurbiprofen axetil 100 mg with saline 500 mL,and the patients in the con-trol group received intramuscular injection of pethidine hydrochloride 100 mg. If its effect is not good,100 mg bucinnazine hydrochloride were injected. Analgesia efficacy was assessed by visual analog scales( VAS) at 30 min,1 h,2 h,4 h,6 h and 8 h after medication. At the same time, the side-effects and complications were observed and recorded. Results There is no statistical difference between the experimental group and the control group at 30 min,1 h, and 2 h after medication(P>0. 05),but at 4 h,6 h and 8 h after medication,VAS score of the experimental group is obviously lower than that of the control group(P<0. 05). And the incidence of adverse reaction of the experimental group, such as thirst, dizziness, nausea and emesia, was lower than that of the control group(P<0. 05). But there is no significant difference in incidence of the secondary bleeding between the two groups(P>0. 05). Conclusion Intravenous drip of flurbiprofen axetil can be an ef-fective and safe way of curing postsurgical spastic bladder pain after TUR-P.
