1.Concentrated Disinfectant Supply of Regional Medical Appliances and Its Cost Estimation

OBJECTIVE To estimate the cost of concentrated disinfectant supply of medical appliances,so as to provide the primary basis for improving the concentrated disinfectant supply model,along with regional integration.METHODS Using contrasting methodologies,the cost level before and after the concentrated disinfectant supply with regional integration was compared,and the individual cost and total cost were analyzed.RESULTS Material and human resource cost was the core of total cost.Concentration management of regional medical appliances disinfection supply could significantly reduce costs.CONCLUSIONS Integrated management of concentrated disinfectant supply could reduce cost and economize expenses.Actually,this model is worth promoting.
2.Comparison on the positive detection rate of allergen specific IgE measured with the systems between fluorescence enzyme immunoassay and western blot

Objective:To compare the difference on the positive detection rate of the main aeroallergens and food allergens specific IgE measured with the systems between fluorescence enzyme immunoassay(FEIA, referred to as ImmunoCAP system)and western blot(referred to as Allergy Screen system), in order to provide a basis for the rational application of methods and interpretation of the test results.Methods:The clinical information and sIgE test results data were collected from a total of 458 cases of allergic diseases from October 2017 to April 2019 in the outpatient clinic of Allergy Department in Beijing Children′s Hospital.All of the 458 cases were detected for a panel of common aeroallergens and food allergens sIgE level in their serum with the Allergy Screen system.Simultaneously, the above 141 cases were detected main aeroallergen and food allergen sIgE with ImmunoCAP system, while 303 cases only for aeroallergens and 14 cases only for food allergens.All of the cases were divided into three different phenotype groups according to the main target organ and diagnosis such as airway allergic disease group( N=293), skin allergic disease group( N=14)and multi-system allergic disease group( N=151). Meanwhile, three different age groups were referred to as <3 years old group( N=97), 3 to 6 years old group(3 years and 6 years included)( N=186)and >6 years old group( N=175). The same kinds of the allergens included in the two systems were house dust mite(HDM), cat dander, dog dander, egg white, milk, crab, shrimp, therefore data of sIgE to those seven allergens were compared. Results:In all the enrolled cases, the positive detection rate of HDM, egg white and milk sIgE detected by ImmunoCAP system were significantly higher than those by Allergy Screen system(30.6%, 36.1% and 43.2% vs 21.2%, 21.3% and 21.3%). Among the disease groups, the positive detection rate of HDM sIgE detected by ImmunoCAP system was significantly higher in the airway allergic disease group and multi-system allergic disease group than those by Allergy Screen system, respectively(33.7% and 24.6% vs 23.4% and 16.2%). The positive detection rate of egg white and milk sIgE detected by ImmunoCAP system in the multi-system allergic disease group was significantly higher than those by Allergy Screen system, respectively(47.6% and 47.6% vs 26.2% and 25.2%). There was no significant difference in the positive detection rate of cat dander, dog dander, crab and shrimp sIgE among the disease groups.The positive detection rate of HDM sIgE detected by ImmunoCAP system in all of the age groups were significantly higher than those by Allergy Screen system, respectively(14.9%, 26.9% and 42.3% vs 5.7%, 17.6% and 32.6%). The positive detection rate of cat dander sIgE detected by ImmunoCAP system in <3 years old group was significantly lower than that by Allergy Screen system(6.9% vs 16.1%). The positive detection rate of egg white sIgE detected by ImmunoCAP system in the <3 years old group and the 3 to 6 years old group were higher than those by Allergy Screen system, respectively(47.1% and 33.3% vs 32.4% and 15.0%). The difference in the positive detection rate of milk sIgE detected by these two methods in the <3 years old group and the 3 to 6 years old group was similar to the egg white.Conclusion:According to the analysis of seven kinds of major aeroallergen and food allergen sIgE results, there are differences between the two methods to detect HDM, cat dander, egg white and milk sIgE.It depends on the type of allergic disease and age.The positive detection rates of dog dander, crab, and shrimp sIgE detected by the two methods are consistent.In clinical application, comprehensive analysis should be made, testing methods should be selected rationally, and the results should be interpreted scientifically.
3.Cannulated screws combined with transposition of cuboid periosteal flap pedicled with fascia and lateral tarsal artery in the treatment of talus neck fractures

Objective To investigate the surgical techniques and clinical efficacy of cannulated screws combined with transposition of cuboid periosteal flap pedicled with fascia and lateral tarsal artery in the treatment of talus neck fractures.Methods From March 2008 to June 2011,12 cases with talus neck fractures were treated with cannulated screws combined with transposition of cuboid periosteal flap pedicled with fascia and lateral tarsal artery.External fixation in functional position for 12-14 weeks,and the load time was determined by X-ray fracture healing.Functional results were assessed according to AOFAS (American Orthopaedic Foot and Ankle Society) score at last follow-up.Results Twelve cases were followed up for an average of 24 months (12 to 48 months).One case of skin flap necrosis healed by dressing.All the fractures healed,and the mean healing time was 20 weeks (16 to 24 weeks).AOFAS ankle and hindfoot mean postoperative score was 82.5 (ranging from 55 to 96) points.Among them 4 cases were excellent,good in 5 cases,fair in 3 cases.Two cases presented with mild posttraumatic subtalar arthritis and pain relief after oral anti-inflammatory analgesic.One case with avascular necrosis of talus body,and X-ray displayed sclerosis of talus body,but no collapse,and the patient was told to reduce weight and regularly followed up.Conclusion The application of cannulated screws combined with transposition of cuboid periosteal flap pedicled with fascia and lateral tarsal artery for the treatment of talus neck fractures is an effective treatment,which can improves the blood supply of the talus body and reduces the occurrence of avascular necrosis of the talus.

This study was designed to compare the efficacy and toxicity of artesunate combined with NP (a chemotherapy regimen of vinorelbine and cisplatin) and NP alone in the treatment of advanced non-small cell lung cancer (NSCLC).
5.Survey of endemic situations in schistosomiasis transmission controlled or interrupted regions in Anhui Province

Objective To understand the variation rules of schistosomiasis endemic situation before and after schistosomia-sis transmission controlled or interrupted,so as to provide the evidence for the consolidation of control achievements. Meth-ods In Anhui Province,3 counties reaching the criteria of schistosomiasis transmission controlled or interrupted were selected and their historical endemic data were collected and analyzed statistically from 10 years before the schistosomiasis transmission controlled to 2008. Results In Tianchang City,the Oncomelania hupensis snail area was 3.54 hm2 in a part of the lake marsh-land in the year reaching the criteria of transmission controlled(1998),and no snails were found in the year of transmission in-terrupted(2008). No stool-test-positive resident was detected except there were 58 acute schistosomiasis cases in 1993,and the sero-test-positive rate of local residents remained at low levels(0-1.55%). In Taihu County,the snail area was 0.84 hm2 account-ing for 0.06%of its historical accumulative snail areas in the year of transmission controlled(1971),and no snails were found three years before the transmission interrupted (1983). However,the schistosomiasis endemic rebounded 12 years later (1995),there was an outbreak of acute schistosome infections(73 cases),and the snail area increased again to 133.7 hm2 ac-counting for 2.91%of historical accumulative snail areas. After that,the snail area kept on rising and the infection rates of resi-dents and bovine remained higher than 1%. In Guangde County,the snail area was 32.4 hm2 accounting for 1.90%of its histori-cal accumulative snail areas in the year of transmission controlled(1995). After that,the snail area increased progressively, and the schistosomiasis endemic rebounded and there were acute schistosome infections 5 years later. The Spearman tests showed that the sero-test-positive rate(Tianchang City)and the stool-test-positive rates of residents and bovine(Taihu County)had positive correlations with the snail areas(r=0.582,0.401,0.596,all P<0.05). Conclusion The snail status is a key for the consolidation of schistosomiasis transmission controlled and interrupted. Therefore,a valid surveillance system of snail situa-tion should be established as quickly as possible.

Objective To explore the risk factors of cerebral schistosomiasis japonica. Methods A total of 37 confirmed cases,diagnosed during 1999-2004 in Anhui Province,were involved in the study as case group. Three controls for each case were selected from the general population either without schistosomiasis,with chronic schistosomiasis or with acute schistosomiasis. The relevant risk factors on the disease were analyzed. 8.Safety and efficacy of early oral oxycodone/acetaminophen and tramadol in Chinese gynecology patients undergoing laparoscopy operation

AIM: To evaluate the safety and efficacy of oral oxycodone /acetaminophen or tramadol in early postoperative patients undergoing laparoscopic gynecological operations. METHODS: 120 gynecologic patients receiving laparoscopy operation were enrolled in a randomized,double blind, placebo controlled, multi center clinical trial with early oral analgesics if the vasual analgesia scores (VAS) was scored higher than 3.0. All patients were randomly received a single dose of oral analgesic: oxycodone/acetaminophen, tramadol or placebo, respectively. For rescue medication, PCA pump was provided in all three groups with a dose of 1 mg morphine and lockout of 5 minutes. The VAS scores, pain relief, PCA morphine consumption and side effects were evaluated at the following occasions of 0.25 , 0.5 , 0.75 , 1, 2, 4, 6, 8, 12 and 24 h throughout the study. RESULTS: The VAS scores and pain relief were significantly different in three groups at 0.75 , 1, 2, 4, 6, 8 and 12 h. The VAS scores and PCA morphine consumption was significantly lower in oxycodone/acetaminophen and tramadol groups than those in placebo group. Pain relief in oxycodone/acetaminophen and tramadol groups was better than those in placebo group. The incidence of side effects such as nausea and vomiting significantly increased in tramadol group at 24 h compared with those in the other two groups. CONCLUSION: Early oral administration of oxycodone /acetaminophen or tramadol can provide surgical patients with good and safe postoperative analgesia after laparoscopy gynecologic operation. The incidence of side effects in oxycodone /acetaminophen group is lower than that in tramadol group in this clinical trial.
9.Value of unenhanced CT-based 5-point predictive score combined with blood glucose detection for predicting short-term prognosis in patients with spontaneous intracerebral hemorrhage

Objective:To investigate the value of a 5-point predictive score based on unenhanced CT combined with blood glucose detection for predicting short-term prognosis in patients with spontaneous cerebral hemorrhage.Methods:A total of 102 patients with spontaneous intracerebral hemorrhage who received treatment in Zhejiang Provincial People's Hospital from March 2020 to March 2022 were included in this study and analyzed retrospectively. Blood glucose level was measured and BAT score was used to evaluate hematoma enlargement. After 30 days, Glasgow Outcome Scale was used to evaluate the prognosis of patients. The relationships between blood glucose and BAT score, and between blood glucose and BAT score and prognosis were analyzed. The value of blood glucose and BAT score for predicting short-term prognosis was analyzed.Results:The Glasgow Outcome Scale results showed that among the 102 patients, 24 patients (23.53%) had poor prognosis. The BAT score and blood glucose level in patients with poor prognosis were (3.13 ± 0.68) points and (11.58 ± 2.30) mmol/L, respectively, which were significantly higher than (2.40 ± 0.59) points and (8.88 ± 1.71) mmol/L in patients with good prognosis ( t = 5.10, 5.30, both P < 0.05). Pearson correlation analysis showed that in patients with spontaneous intracerebral hemorrhage, blood glucose level was positively correlated with BAT score ( r = 0.43, P < 0.05). Spearman correlation analysis showed that in patients with spontaneous intracerebral hemorrhage, blood glucose level and BAT level were positively correlated with prognosis ( r = 0.42, 0.47, both P < 0.05). The receiver operating characteristic curve showed that the area under the curve plotted for BAT score combined with blood glucose level for predicting short-term prognosis was 0.874, which was significantly greater than the area under the curve plotted for BAT score alone for predicting short-term prognosis ( Z = 2.54, P < 0.05). Conclusion:A large proportion of patients with spontaneous intracerebral hemorrhage have a poor prognosis. The patients with a poor prognosis have higher blood glucose levels and BAT scores than those with good prognosis. Blood glucose and BAT score have a high value for predicting the prognosis of patients with spontaneous intracerebral hemorrhage.
10.Risk evaluation of schistosomiasis japonica input to potential endemic areas in Anhui province.

OBJECTIVETo analyze the impact of water transfer project from the Yangtze River to the Huaihe River on schistosomiasis transmission, and to evaluate the risk of the disease input to the potential endemic area in Anhui Province, namely the Chaohu Lake region.
METHODSFrom 2008 to 2012, 1 fixed and 3 mobile surveillance sites in the Chaohu Lake area were selected, and the schistosomiasis infection situation of 615 local residents in the fix surveillance site was investigated in autumn of 2008 and 2012, while the schistosomiasis infection situation of 1603 mobile population in the 3 mobile surveillance sites were investigated in autumn of 2008 to 2012. All people were screened by indirect hemagglutination assay (IHA), and the positive ones were then examined by sedimentation method. 303 local livestock and livestock from schistosomiasis endemic areas were examined by stool hatching method in autumn of 2008 to 2012. From 2008 to 2012, the distribution of Oncomelania snails was investigated in risk areas and suspicious areas, and the snail spreading pattern was conducted through salvaging floaters in rivers connected with the Yangtze River. In addition, the Oncomelania snails were raised in the cages on the beaches of the Chaohu Lake, a control area, from 2007 to 2010, and their survival and reproduction capacity was observed.
RESULTSIn 2008 and 2012, 301 and 314 local residents were detected by IHA, but there were no positive found. From 2008 to 2012, a total of 1603 mobile population were examined by IHA, and the positive rate of antibody was 3.1% (49/1603); 75 individuals were examined by sedimentation method, and the positive rate was 36.00% (27/75). A total of 303 livestock were examined by stool hatching method, but no one showed positive. A total of 1630 km(2) in risk areas and 3551 km(2) in suspicious areas were surveyed, but there were no Oncomelania snails found. A total of 457.6 kg floating debris were investigated, and 11 Oncomelania snails were found. From 2007 to 2010, the survival rate of Oncomelania snails in two trail areas in the Chaohu Lake and in the control area was 88% (86/98), 51% (45/89), 30% (25/71), 24% (20/84) and 92% (85/92), 54% (50/92), 23% (12/52), 17% (13/79) and 96% (85/89), 52% (44/85), 26% (18/69), 18% (14/76), respectively, there were no statistical significance between the trial areas and the control area (χ1(2) = 3.78, P > 0.01; χ2(2) = 0.27, P > 0.01; χ3(2) = 2.51, P > 0.01; χ4(2) = 1.50, P > 0.01), and filial generation snails were found in each observation area from 2008 to 2010, the number was 156-312.
CONCLUSIONThe imported infectious sources of schistosomiasis have been found in the Chaohu Lake region, the possibility of imported exogenous Oncomelania snails spreading into the Lake and surviving and reproducing there is high. The risk of schistosomiasis input to the potential endemic area in Anhui Province is predicted to be high.
