1.Endovenous laser treatment of varicose veins of the legs:a report of 170 patients

Objective To assess the effects of treating varicose vein with endovenous laser treatment(EVLT). Methods 810nm wavelength diode laser machine was used to treat 170 patients with varicose veins of the legs. After treatment, leg bandage compression was used for 2 days, and then changed to elastic stockings for 6-8 weeks. Patients were examined with Doppler ultrasound at follow-up. Results Post-operative follow up(1-12 months) of EVLT in 170 patients indicated 100% rate of greater saphenous vein closure on Doppler ultrasound, and no significant complications. All of the symptoms, disappeared or were significantly improved. Conclusions The results of EVLT of varicose veins have been favorable. This minimally invasive technique appears to be safe, and results in very effective closure of great saphenous vein.

3.Expression of AIB1 in bladder cancer and its significance

Objective To investigate the expression of AIB1 protein in bladder cancer and its significance.Methods 30 cases of bladder cancer and 30 adjacent normal tissues were collected from 30 patients underwent radical or TURBt.The level of AIB1 protein expressed in these tissues were detected by immunohitochemical method.Results The positive rate of AIB1 protein in bladder carcinoma was higher than that in the normal tissues [43.3 % (13/30) vs 3.3 % (1/30),x2 =4.316,P < 0.05].The positive rate of AIB1 in T3 stage was higher than that in T1 or T2 stage [62.5 % (5/8) vs 25.0 % (2/8),42.9 % (6/14),x2 =4.623,P < 0.05].No correlation was found between the expression of AIB1 protein and the sex,age,multiplicity or pathological grade (P > 0.05).Conclusions There is a close relationship between AIB1 and bladder cancer.AIB1 might have great value on the prognosis of patients with bladder cancer.
4.Application of mind mapping on adverse effect caused by medication via PCA

Objective To evaluate the effect of mind mapping on adverse effects caused by medication via patient control alagesia pump(PCA).Methods One hundred and seventy-nine patients with PCA pump after operation were divided into control group(n=90)and observation group(n=89)according to the admission time.The observation group was nursed with mind mapping and the control group with routine method. The incidences of adverse effects caused by medication via PCA pump were compared between the two groups.Result The incidences of adverse effect in the observation group and the control group were 19.1%and 32.2%respectively,with statistical significance(P<0.05).Conclusion Nursing with mind mapping for medication via PAC pump can improve nurses’clinical ability in observation and nursing and therefore effectively prevent and deal with adverse effect caused by medication via PCA pump to relieve patients’pain and improve nursing quality.
5.Genotyping of Clinical Isolates of Malassezia from Patients with Tinea Versicolor by PCR-RFLP

Objectives To develop a rapid genotyping method of clinical isolates of Malassezia from patients with tinea versicolor by PCR-RFLP,and to evaluate reliability of the approach as compared with biochemical classification.Methods Tween assimilation test and catalase reaction were carried out to identify 74 isolates of Malassezia species from patients with tinea versicolor and 7 Malassezia reference strains.The sequence of 28S rDNA of Malassezia species was amplified by PCR,and then the product was analyzed by RFLP with Eco88I,Bsp143Ⅱ and BshNⅠ,respectively.Results M.restricta,M.obtusa and M.pachydermatis were successfully identified by three restriction endonucleases.M.restricta was found to be more diverse from the other 6 species in genetic homology.By comparison with PCR-RFLP technique,a possible mistake was discovered with biochemical method.Conclusion PCR-RFLP is a promising molecular biological technique,which could rapidly and correctly classify Malassezia species.
6.Application of clinical nursing pathway in patients with hand-foot-and-mouth disease associated with vital myocarditis

Objective To study the effect of clinical nursing pathway in patients with hand-foot-and-mouth disease associated with vital myocarditis .Methods A total of 76 cases of patients with hand-foot-and-mouth disease associated with vital myocarditis between December 2011 and December 2013 were recruited and divided into control group(n=38)and observation group(n=38) randomly.The control group was treated by normal therapy , while the observation group was treated by clinical nursing pathway .The effect and satisfaction of two groups were observed and compared .Results The hospital stay of the observation group was (12.8 ± 3.58) days, which was significantly shorter than that in the control group ( 18.7 ±5.31 ) days, and the difference was statistically significant (t =9.25,P<0.05).The hospital cost in the observation group was reduced by almost 20%compared with the control group .The observation group had one case with nosocomial infection, while there were six cases in the control group , and the difference was statistically significant (χ2 =6.08,P<0.05).The satisfaction rate of the observation group was 94.74%(36/38), while the control group was 78.95%(30/38), and a significant difference was found ( t =14.08, P <0.05).Conclusions The clinical nursing pathway applied in patients with hand-foot-and-mouth disease associated with vital myocarditis can shorten hospital stay , reduce nosocomial infection and improve the satisfaction of patients .
7.Median effective interval of ropivacaine combined with hydromorphone for labor analgesia

Objective To investigate the effective interval of programmed intermittent epidural bolus(PIEB)in labor analgesia with ropivacaine and hydromorphone.Methods A total of 101 puerpera with singleton pregnancy,head position,aged 20-40 years,BMI 20-35 kg/m2,ASA physical status Ⅰ orⅡ,gestational age 37-42 weeks,were randomly allocated into five different PIEB intervals groups:PIEB 40 minutes group(group P40,n=20),PIEB 50 minutes group(group P50,n=19),PIEB 60 minutes group(group P60,n=20),PIEB 70 minutes group(group P70,n=20),and PIEB 80 minutes group(group P80,n=22).The PIEB dose was fixed at 10 ml with the first bolus given 1 hour after the initial manual loading dose with subsequent boluses continued at intervals according to different group allocation.Analgesia was provided using PIEB with 0.1%ropivacaine combined with hydromorphone 15 μg/ml for a total of 12 ml.Based on effective analgesia rate(proportion of women who do not require additional analgesic drugs by the end of the first stage of labor).Probit regression was used to estimate median effective interval(EI50)and 90%effective interval(EI90).VAS pain scores before analgesia,20 minutes,1 hour,2,3,and 4 hours,at full opening of the cervix were recorded.The highest level of sensory blockade and motor blockade(modified Bromage score)were recorded,maternal adverse effects were recorded.Neonatal weight,1 minute and 5 minutes postnatal Apgar scores were recorded.Results Effective analgesia rates were 95.0%,84.2%,70.0%,55.0%,and 50.0%in groups P40,P50,P60,P70,and P80,respec-tively.The EI50 and EI90 were 76.5 minutes(95%CI 69.0-93.9 minutes)and 43.4 minutes(95%CI 17.6-52.7 minutes)analysing by Probit regression.There were no significant differences in VAS pain score at different time,maximum sensory block plane,modified Bromage score,incidence of maternal adverse re-actions and neonatal conditions between the two groups.Conclusion The EI50 and EI90 of PIEB 0.1%ropiv-acaine combined with hydromorphone 15 μg/ml for labor analgesia were 76.5 minutes(95%CI 69.0-93.9 minutes)and 43.4 minutes(95%CI 17.6-52.7 minutes),respectively.

Objective:
To investigate the feasibility and consistency of four-dimensional flow (4D flow) quantification of pulmonary regurgitation in patients with repaired Tetralogy of Fallot (ToF) by comparing with conventional two-dimensional flow (2D flow) and echocardiography.
Methods:
Both the 4D flow and 2D flow imaging were acquired with repaired ToF (a total of 21 patients) consecutively on 3.0 T MR scanner from May 2018 to August 2018. Pulmonary flow and regurgitant fraction were measured by a commercial post processing software Circle CVI42. All patients underwent echocardiography within one week after or before MR examination. The inter/intra-observer variability by 2D/4D f1ow and agreement between the two methods were investigated by interclass correlation coefficients (ICC) and Bland-Altman analyses. The agreement between MR and echocardiography were analyzed by weighted Kappa coefficient. The correlation between pulmonary regurgitation and cardiac function was also investigated by Pearson analysis.
Results:

Pathological insulin resistance (IR) is closely related to gestational diabetes mellitus (GDM) and adverse pregnancy outcomes in women with GDM. Increasing studies have investigated the efficacy of IR indices, such as quantitative insulin sensitivity index, homeostasis model assessment of insulin resistance, triglyceride-glucose index and sex hormone-binding globulin, in predicting GDM and related complications in recent years. This article reviews the research progress in the above topics.
10.Exploring Health Economic Evaluation Methods for Disease Screening

Objective:To explore the health economic evaluation methods of disease screening,providing insights for scholars conducting related research.Methods:By integrating relevant content from modem epidemiology,health statistics,and health economics,it investigates the differences in conducting health economic evaluations of disease screening using authenticity indicators(sensitivity,specificity)and predictive value indicators(positive predictive value,negative predictive value),and combines empirical studies with cases from the literature.Results:Authenticity indicators(sensitivity,specificity)are intrinsic measures of diagnostic tests,unaffected by the disease prevalence in the sample population,making them more suitable for conducting health economic evaluations of disease screening.Moreover,authenticity indicators are easy to obtain,and the model calculations are straightforward and convenient.Case study results demonstrate that compared to risk score screening,the Incremental Cost-Effectiveness Ratio(ICER)of fasting plasma glucose(FPG)testing for type 2 diabetes is 626,indicating that for every additional diabetic patient identified through risk score screening,an extra 626 yuan is spent.Conclusion:It is recommended to use authenticity indicators for conducting health economic evaluations of disease screening,standardize the path methods for model selection,and continuously improve the evidence quality of health economic evaluations of disease screening in China.
