Objective To study the significance of ?-catenin expression in human gastric carcinomas, colorectal carcinomas and human hepatic carcinomas.Methods S-P immunohistochemical method was used to detect ?-catenin expression in 80 cases of human carcinomas of alimentary system and corresponding tumor-adjacent normal tissues by ?-catenin polyclonal antibody. 2.Research on evaluation of essential functional liver volume of solitary large hepatocellular carcinoma beforeγ-SABR

With the rapid development of medical technology, radiation therapy in the treatment of liver cancer occupies the important position. According to the biological characteristics of large liver cancer, the development of liver cancer radiotherapy at home and abroad and the technical features of stereotactic radiotherapy, it is concluded that the effect of radiation therapy of liver cancer can be comparable to those of surgery.γ-SABR is a kind of high accuracy of stereotactic radiotherapy. Before the treatment, usually do a rough evaluation according to the patient general condition, tumor size, and the liver function. The potential risk of radiation induced liver disease is very big. Therefore, the evaluation of essential functional liver volume of large liver cancer is particularly important before stereotactic radiotherapy. Draw lessons from the consensus of liver reserve function and the decision tree of liver function, the concept of essential functional liver volume and remaining functional liver volume is applied to theγ-SABR are being studied in clinical trials. This review is intend to outline the evaluation of essential functional liver volume of solitary large hepatocellular carcinoma beforeγ-SABR.
3.Chronic tibial defection in children treated with grafting of fibula with vessel pedicles and Lebone under external fixators

Objective To report one stage treatment of chronic tibial defection in children. Methods 19 children with chronic tibial defection were treated in our department when their infection was at the stage of latency. First their focuses of infection were cleared away. Next a piece of fibula with vessel pedicles whose length was equal to that of the tibial defect was taken as the support to restore the length of the tibia. Then the external fixators were applied and a mixture of antibiotic and Lebone powder was used to fill the defect. Results In 15 cases the incision healed at one stage, slight infection occurred in 4 cases whose incisions healed after treatment of 3 to 4 weeks. Follow-ups of 0.5 to 1.5 years showed that no infection occurred again, bone defects healed well and limb function was satisfactory. Conclusion To treat chronic tibial defection in children at one stage, combined therapy should be applied. Grafting of fibula with vessel pedicles and Lebone under external fixators is a good one, because it can shorten the treatment time at no cost of satisfactory effects.
4.The laboratory differential diagnosis of Henoch-Schonlein purpura

Objective To explore the common clinical features and mechanism of HenochSchonlein purpura (HSP) in children,and provide evidence and guidance for clinical diagnosis,therapy and prognosis of HSP.Methods Prospective study.Totally 1 232 HSP children blood samples were collected during January 2010 to December 2013 in the Children's Hospital of Zhejiang University School of Medicine,and the levels of immunoglobulin,complement,T lymphocyte subsets and CRP were detected in the acute phase of HSP.The clinical data were analyzed with these indexes to explore the function of cellular and/or humoral immunity in the pathogenesis of HSP.In addition,this study detected serum IgA levels in 200 cases of sepsis rash,200 cases of urticarial and 200 cases of thrombocytopenic purpura patients and 400 cases of healthy children over the same period to our hospital respectively,in order to find out whether IgA is capable of differentiating allergic purpura rash from other similar skin rashes.Results Compared with healthy children,the IgA level was significantly higher [IgA:2.0 (0.6-7.5) g/L vs 1.1(0.6-2.1) g/L,Z =5.928,P =0.03].When IgA > 1.44 g/L,the diagnostic sensitivity and specificity to differentiate allergic purpura patients from sepsis patients were 77.43 % (954/1 232) and 80.00% (160/200),respectively.When IgA > 1.53 g/L,the diagnostic sensitivity and specificity to distinguish from allergic purpura with urticaria patients were 72.57% (894/1 232) and 74.51% (149/200).When IgA >0.91 g/L,it had a 95.14% sensitivity (1 172/1 232) and a 79.45% specificity (159/200) for the differential diagnosis of allergic purpura and thrombocytopenic purpura patients.When IgA > 1.33 g/L,the sensitivity of distinguishing allergic purpura patients from healthy children was 81.25% (1 001/1 232),while the specificity was 95.00% (380/400).Conclusions HSP children show cellular immune dysfunction.Th1/Th2 imbalance and over-activation of Th2 cells result in the increased synthesis and release of immune globulin,causing abnormalities in humoral immune.An increase in serum IgA level of HSP patients can differentiate HSP from other similar rashes.

6.Effects of different acupuncture depths of Lianquan (CV 23) for dysphagia after stroke: a randomized controlled trial.

OBJECTIVETo explore the effects of different acupuncture depths of Lianquan (CV 23) for dysphagia after stroke.
METHODSTwo hundred and fifty-one patients with dysphagia after stroke were randomly divided into a deep acupuncture group (group A, 85 cases) a shallow acupuncture group (group B, 83 cases) and a glossopharyngeum acupuncture group (group C, 83 cases). Based on the conventional treatment, the glossopharyngeum acupuncture which was pricking without needles retained at lingual surface, posterior pharyngeal wall of the affected side, Jinjin (EX-HN 12) and Yuye (EX-HN 13) was used in the three groups. After the glossopharyngeum acupuncture, Lianquan (CV 23) was acupunctured 60-70 mm in the group A and 30-40 mm in the group B. The needles were all retained for 30 min. The treatment was applied once a day for 30 days in the three groups. Water swallowing test evaluation scale was observed before and after treatment, and the efficacy was assessed in the three groups.
RESULTSThe total effective rate was 95.3% (81/85) in the group A, which was better than 85.5% (71/83) in the group B and 83.1% (69/83) in the group C (both P < 0.05). The water swallowing test scores were decreased apparently in the three groups (all P < 0.01), and scores in the group A were the most obvious decline (P < 0.01). The decrease of scores in the group A was more remarkable than those in the group B and the group C after two weeks of treatment (both P < 0.05).
CONCLUSIONAcupuncture at Lianquan (CV 23) can effectively improve the dysphagia after stroke. The acupuncture depth can affect efficacy and the effect of deep acupuncture is better.

7.JPH2 gene expression in persistent atrial fibrillation with mitral valve disease

Objective This research is to explore the Junctophilin-2 ( JPH2 ) expression in persistent atrial fibrillation with mitral valve disease.Methods The left atrial tissue samples were taken from 34 patients with mitral valve disease who underwent cardiac surgery.16 patients were in sinus rhythm, 18 patients had persistent atrial fibrillation.Western blot tech-nique was used to test the JPH2 expression, and the RT-PCR method was used to test the JPH2 gene expression.Results The Westernblot results showed JPH2 expression down-regulated in persistent atrial fibrillation group (0.94 ±0.29 vs.1.53 ± 0.61,P<0.01).However, there was no difference with JPH2 mRNA expression between atrial fibrillation patients and sinus rhythm patients(1.76 ±1.38 vs.1.15 ±0.94, P>0.05).Expression of miRNA-24 was significantly up-regulated in patients with persistent atrial fibrillation(4.49 ±4.30 vs.1.72 ±1.08, P<0.05).There was no significant difference in mean age, gender, ejection fraction and NYHA among the two groups.But the left atrial diameter was significant larger in patients with persistent atrial fibrillation compared to those in sinus rhythm(P=0.02).Conclusion Junctophilin-2 protein down-regulation may be associated with atrial fibrillation , cause left atrial remodeling and contraction dysfunction .

ObjectiveHeart failure is closely associated with a defected calcium-induced calcium release (CICR) between the transverse tubular (TT) invagination of plasma membrane and terminal cistemae of sarcoplasmic reticulum (SR) in cardiac myocytes.The underlying cause of this defect is not well understood.Any factors impacting the TT and SR connecting may reduce the excitation-contraction coupling efficiency.Junctophilin 2 (Jph 2) is a cardiac protein anchoring SR to TT.This research is to explore the JP-2 expression in the cardiomyopathy heart failure myocytes.Objective Myocardium specimens of the lateral segments of left ventricule were collected from cardiomyopathy heart failure patients and the transplantation donors.MethodsGroup A:heart failure cases diagnosised as cardiomyopathy and samples were collected from the left ventricle lateral wall.Group B:control samples from the transplantation donors which not used for the recepients reason.Electron microscopy technique was used to test the mean junctional distance between TT and SR.Westernblot technique was used to test the Junctophilin 2 expression and the RT-PCR method was used to test the JP-2 gene expression.The data were analysises with the SPSS 12.0 software,P < 0.05 was accepted as different significantly.ResultsSamples were collected from 14 patients with severe heart failure and 6 control cases.Electron microscopy ultrastructure results showed in an average 100 μm2 of the myocardial cells area the coupling numbers of the control group and heart failure group were 60 and 112 (P <0.001 ).The electrical micrography mean junctional distance between TT and SR was significantly increased from the control group ( 16.2 ± 3.2) nm to the ( 19.3 ±4.3 ) am in the heart failure group( P <0.001 ) ). The Westernblot results showed Junctophilin 2 versus GAPDH expression down regulated in the heart failure group comparing to the control group (7.2% vs 15.3 %,P < 0.05 ).The RT-PCR implied the JP-2 gene versus GAPDH expression also down regulated in the heart failure group comparing to the control group (37.5% vs 98.8%,P < 0.01 ).ConclusionConclusion JP-2 gene down-regulation may be one of the earliest change in the heart failure molecular mechanisms.
9.Application and economic effects of digital three-dimensional reconstruction in hip hemiarthroplasty for intertrochanteric femoral fractures in the elderly

BACKGROUND:Digital three-dimensional reconstruction technology is gradually applied to orthopedic diseases with the advantages of visualization,accuracy and non-invasiveness,but there is less evidence-based support for its use in artificial hip hemiarthroplasty for intertrochanteric fractures of the femur in the elderly. OBJECTIVE:To investigate the application value and economic effects of digital three-dimensional reconstruction techniques in artificial hip hemiarthroplasty of intertrochanteric fractures of the femur in the elderly. METHODS:One hundred and thirty elderly patients with intertrochanteric femur fractures admitted to Zunyi First People's Hospital from January 2019 to December 2022 were selected and randomly divided into a control group(n=65)and an observation group(n=65).Artificial hip hemiarthroplasty was performed in both groups.The control group adopted the film template measurement method for manual preoperative planning while the observation group adopted a digital three-dimensional reconstruction technique.Preoperative planning and intraoperative actual application of prosthesis compliance rate,fibrinogen,D-dimer,bilateral femoral eccentric distance difference,bilateral lower limb length difference,Harris hip function score,visual analog scale score,excellent and good rate of hip function,complications,and hospitalization cost were observed in both groups. RESULTS AND CONCLUSION:(1)The proportion of acetabular side and femoral side prosthesis in grade 0(fully compliant)was higher in the observation group than that in the control group(P<0.05).(2)Fibrinogen and D-dimer levels in the observation group were lower than those in the control group 3 days after surgery(P<0.05).(3)The difference in bilateral femoral eccentric distance and the difference in bilateral lower limb length in the observation group were smaller than those in the control group immediately after surgery(P<0.05).The differences in Harris and visual analog scale scores were not significantly different between the two groups preoperatively,6 and 12 months postoperatively(P>0.05).There was no significant difference in excellent and good rate of hip function between the two groups 12 months postoperatively(P>0.05).(4)There was no significant difference in the complication rate between the two groups(P>0.05).The hospitalization cost of the observation group was higher than that of the control group(P<0.05).(5)It is indicated that digital three-dimensional reconstruction technology applied in artificial hip hemiarthroplasty of intertrochanteric femoral fracture in the elderly can not only accurately determine the prosthesis type before surgery,but also accurately reconstruct the bilateral lower limbs offline,but its hospitalization cost is high.
