1.Protective effects of penehyclidine hydrochloride on transient forebrain ischemia reperfusion injury in gerbils

AIM:To study the protective effects of penehyclidine hydrochloride(PHC) on transient forebrain ischemia reperfusion injury in gerbils.METHODS:The model of transient forebrain ischemia reperfusion was established in gerbils by bilateral carotid artery clamping.The effects of PHC on neurological function scores and the morphous of hippocampal pyramidal neuron of gerbils were observed after receiving transient forebrain ischemia reperfusion.SOD activities and contents of MDA in the hippocampus and cortex of gerbils were measured.RESULTS:In the groups of PHC(0.08),(0.24)(mg?kg~(-1)) and atropine,the stroke index was decreased,compared with the ischemia-reperfusion group after the gerbils received transient forebrain ischemia reperfusion for six hours.PHC could reduce the degree of injury in hippocampal pyramidal neuron after ischemia reperfusion for three days.CONCLUSION: PHC has protective effects on transient forebrain ischemia reperfusion injury in gerbils.
2.Study on anticonvulsant mechanism of ketamine

Aim To observe the effects of ketamine on Na+,K+-ATPase,Ca2+-ATPase and NOSase activity in different cerebral cortex in convulsive mice.Methods The mice were randomly divided into blank group,normal saline(NS) group and ketamine 25 mg?kg-1 (KetⅠ),50 mg?kg-1(KetⅡ) group. The animals of blank group were killed directly.Convulsion was induced by intraperitoneally(ip) strychnine(1.5 mg?kg-1) in other groups,and correspond drugs were administered ip before five minutes.Action variety of mice was observed. Animals were killed on 30 minutes after strychnine injection.The activity of Na+,K+-ATPase,Ca2+-ATPase,TNOSase and iNOSase were assessed by spestrophotometric analysis in different cere-bral cortex(forehead,parietal and occipital area).Results Ketamine group could decrease mortality completely. The duration of tonic state in KetⅡ group was significantly shorter than that in KetⅠgroup.Compared with blank group,Na+,K+-ATPase and Ca2+-ATP ase activities were decreased in the group of NS and KetⅠ,and recovered normal level in the group of KetⅡ at parietal and occipital area. 3.Establishment and Application of 18-STR Database in Han Population from Zhejiang Province

[Objective] To construct a database for the genetic polymorphism of 18 STR loci (D8S1179, D21S11, D7S820, CSFIPO, D3S1358, TH01, D13S317, D16S539, D2S1338, D19S433, VWA, TPOX, D18S51, D5S818, FGA, PentaE, PentaD, SE33) in Hart population from Zhejiang province. To investigate the application of 18 STR loci in the field of paternity testing and prenatal diagnosis. [Methods] Fluorescent dye labeling multiplex STR-PCR, capillary electrophoresis and DNA sequencer GeneScan were adopted in genotyping 598 unrelated samples collected from Han population in Zhejiang province. 18-STR database was established and analyzed. Population comparison was conducted between Han population in Zhejiang province and 8 other population. 15-STR and 18-STR identification system were compared in 497 paternity testing cases. [Results] We observed the distribution of 18 STR loci in Han population meet Hardy-Weinberge equilibrium and was different from other 8 population (X~2 test, P>0.05). Statistical results showed that the heterozygosis (He) ranged from 0.630 to 0.942. The combined power of discrimination was>0.9999999999. Compared with 15-STR identification system, higher paternity index scores and higher exclusion rate were obtained with 18-STR identification system in dual-case paternity test and mutation identification. One trisomy 21 fetus was found in a prenatal paternity test case which had two characteristic genotypes in 2 STR loci of D21S11 and Penta D. [Conclusions] The 18 loci were relatively highly genetic polymorphic in Zhejiang Han population and could be used for paternity testing. Some STR loci could be used in prenatal diagnosis for aneuploidy.
4.Reduction in bile acid pool causes delayed liver regeneration accompanied by down-regulated expression of FXR and c-Jun mRNA in rats.

The present study attempted to examine the effects of bile acid pool size on liver regeneration after hepatectomy. The rats were fed on 0.2% cholic acid (CA) or 2% cholestyramine for 7 days to induce a change in the bile acid size, and then a partial hepatectomy (PH) was performed. Rats fed on the normal diet served as the controls. Measurements were made on the rate of liver regeneration, the labeling indices of PCNA, the plasma total bile acids (TBA), and the mRNA expression of cholesterol 7alpha-hydroxylase (CYP7A1), farnesoid X receptor (FXR), and transcription factor c-Jun or c-fos. As compared with the normal and CA groups, the rate of liver regeneration was decreased on the day 3, and 7 after PH; the peak of the labeling indices of PCNA was delayed and the labeling indices were significantly reduced on the day 1; the TBA were also decreased on the day 1; the expression of FXR decreased but that of CYP7A1 increased at any given time; at the 1st, and 3rd h, the expression of c-Jun was declined in the cholestyramine group. The reduction in the bile acid pool size was found to delay the liver regeneration, which may be caused by the down-regulation of FXR and c-Jun expression.
5.Karyotype analysis of 12 841 cases of amniotic fluid cells and risk assessment of missed diagnosis in molecular techniques

Objective To evaluate the distribution of fetal abnormal chromosome karyotype in mid-pregnancy and analyse the possible misdiagnosis risks of molecular techniques in clinical prenatal diagnosis.Methods Fetal karyotype ( fetal cell collected from amniotic fluid ) in Prenatal Diagnosis Center of Zhejiang Province between 2001 and 2010 were retrospectively analyzed on distribution according to 7 different referral indication:positive screening for trisomy 21, trisomy 18, advanced maternal age , abnormal history of pregnancies , abnormal family history , fetal structural abnormalities and others.The combination of trisomy 21, trisomy 18 and trisomy 13 ( T21/18/13 Group) and the aneuploidies of chromosome 21, 18, 13, X, Y (21/18/13/X/Y Group) were further analyzed based on the current molecular target detection range.Results There were 462 cases out of 12 481 with chromosomal abnormality (3.60%, 462/12 841), with 215 cases of high risk (detection rate 1.67%, 215/12 841) and 247 cases of low risk (detection rate 1.92%, 247/12 841).Under different indications , the detection rate on abnormal chromosome of high risk (high-risk CA) is different,“abnormal fetal ultrasound” is the highest(27.27%,24/88).Among the high-risk CA, T21/18/13 Group accounted for 72.56%(156/215), while the 21/18/13/X/Y Group accounted for 94.88%(204/215).For the 7 regular indications , the high-risk CA distribute different;Except the T21/18/13 Group and 21/18/13/X/Y Group, the rates of other abnormal chromosome karyotype in the high risk CA were 0.28%( 2/719 )-12.5%( 11/88 ) and 0.06%( 4/6 915 )-1.14%( 1/88 ) according to different indication, respectively.Conclusions The distribution of abnormal karyotype were different under different referral indication;the detection power and possible misdiagnosis risks were varied under different indication for each molecular technique.It was suggested that doctors should select suitable molecular technique according to different clinical indications and each molecular method has its own limitations .
6.Investigation of curative effect on renal cancer patients adopting thymopetidum combined with cytokine-induced killer cells in peroperative period

Objective To explore the change of cellular immune function in renal cancer patients adopting thymopetidum (TP-5) combined with cytokine-induced killer cells (CIK) in peroperative period,and evaluate its curative effect.Methods Forty-one cases of Ⅰ-Ⅲ stage renal cancer patients adopting CIK treatment postoperatively were enrolled in this study for retrospective analysis.According to applicating condition of TP-5,the patients were divided into two groups:16 cases of combination group (TP-5+CIK) and 25 cases of CIK group.The patients in combination group were given TP-5 intramscular injection everyday from preoperative 1 d to postoperative 3 weeks,once a day.Peripheral blood mononuclear cell was collected in the patients of two groups,which was cultivated for 2 weeks and then transfused.Fasting peripheral blood 1 week (before collected peripheral blood mononuclear cell),3 weeks (before CIK retransfusion),4 weeks(after CIK retransfusion for 1 week) and 5 weeks(after CIK retransfusion for 2 weeks) after operation were examined,the peripheral blood T cell subgroups (CD3+,CD4+,CD8+) and nature killer cell (NK cell) levels were detected using flow cytometry.Results At 1 week and 3 weeks (before CIK) after operation,the levels of CD3+,CD4+/CD8+ and NK cell in combination group were significantly higher than those in CIK group:1 week after operation:0.542 ± 0.063 vs.0.491 ± 0.054,0.94 ± 0.09 vs.0.90 ± 0.12,0.247 ± 0.025 vs.0.223 ± 0.033;3weeks after operation:0.641 ±0.058 vs.0.587 ±0.062,1.71 ±0.13 vs.1.02 ±0.07,0.319 ±0.038 vs.0.264 ± 0.047).There were significantly differences (P< 0.05).At 4 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in combination group were significantly higher than those in CIK group:0.698 ± 0.041vs.0.649 ± 0.050,2.01 ± 0.11 vs.1.64 ± 0.09,0.331 ± 0.029 vs.0.289 ± 0.034.There were significantly differences (P < 0.05).At 5 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in two grotups were no significant difference (P > 0.05).At 4 and 5 weeks after operation,the levels of CD3+,CD4+/CD8+ and NK cell in two groups were significantly higher than those of 3 weeks after operation (P < 0.05).None of patients occurred acratia,hyperpyrexia,shivering and so on.Conclusions The application of TP-5 treatment in peroperative period can promote the recovery of cellular immune function,which has synergistic effect with CIK.The value of TP-5 is worthy of promotion.

8.An Experimental Study of Hydroxyapatite as Bone Graft Substitute

In this study Na2HPO4-Ca(NO3)2 system was used to synthesize hydroxyapatite (HA). Its synthetical method was simple and low in cost. With the physical-chemical characteristic observation and the animal experimentit was proved that HA was pure, with pores ranging from 20 to 200 ?m in diameter.The chemical structure of HA was similar to that of inorganic part of bones.An investigation of the histological changes and osteogenetic effectiveness of HA implanted was conducted and the mechanism of HA osteogenetic effectiveness is discussed. The study will provide scientific basis for the clinical use of HA as bone graft substitute.
9.Subtalar arthroscopy combined with locking plate fixation through anterolateral longitudinal incision of the Achilles tendon for treatment of displaced intra-articular calcaneal fractures

Methods:A retrospective case-series study was performed to analyzed the data of 38 patients (38 feet) with displaced intra-articular calcaneal fractures admitted to Xuzhou Central Hospital from January 2016 to May 2018. There were 21 males (21 feet) and 17 females (17 feet), with the age of (33.7±6.2)years (range, 21-53 years). According to the Sanders' classification, 7 patients were classified as type II, 27 as type III and 4 as type IV. All patients underwent the procedure of subtalar arthroscopy combined with locking plate fixation through anterolateral longitudinal incision of the Achilles tendon procedure. The operation time, intro-operative management, incision healing, and post-operative complications (nerve, vessels and tendon injuries)were recorded. B?hler angle, Gissane angle, length, width and height of the calcaneus, and bone healing time were recorded at postoperative 2 days and at final follow-up. The visual analogue score (VAS), American Orthopaedic Foot & Ankle Society (AOFAS) ankle and hindfoot scale, and Foot and Ankle Outcome Score (FAOS) were recorded at final follow-up, and were used to evaluate the clinical and functional outcomes.Results:All patients were followed up for 12-42 months [(18.7±5.3)months]. The operation time ranged from 45 to 100 minutes [(72.4±22.6)minutes]. Bone grafting was not observed, and microfracture of the posterior articular surface of the calcaneus were involved in 9 patients. The first-stage incision healing was achieved in all patients, without early complications of nerve, vessel and tendon injuries. The post-operative angles of B?hler and Gissane, length, width and height of the calcaneus were significantly improved compared to preoperative values ( P<0.01), while there were no significant difference at postoperative 2 days and at final follow-up ( P>0.05). At final follow-up, the VAS decreased from 5(3, 9)points to 0(0, 3)points, AOFAS improved from (68.3±10.5)points to (90.6±5.0)points, and FAOS improved from (66.9±9.1)points to (89.8±4.3)points, respectively ( P<0.01). According to the AOFAS ankle and hindfoot scale, the result was excellent in 27 patients, good in 8, and fair in 3, with the excellent and good rate of 92%. Conclusions:For displaced intra-articular calcaneal fractures, subtalar arthroscopy combined with locking plate fixation through anterolateral longitudinal incision of the Achilles tendon has advantages of precise fracture reduction, low postoperative complications and reliable functional outcomes.
10.All-inside subtalar arthroscopy through three portals combined with rafting screws technique for the treatment of the calcaneal fractures of Sanders Ⅱ and Ⅲ

Objective:To investigate the surgical technology and clinical efficacy of using the all-inside subtalar arthroscopy through three portals combined with rafting screws technique for the treatment of the calcaneal fractures of Sanders Ⅱ and Ⅲ.Methods:The clinical data of 33 patients (33 feet) with intra-articular displaced calcaneal fractures treated by modified all-inside arthroscopic from March 2017 to March 2019 at Department of Orthopedics,Xuzhou Central Hospital were retrospectively analyzed.There were 19 male patients,and 14 female patients,aged (32.6±11.3) years(range:20 to 55 years). According to the Sanders classification,there were 12 cases of type Ⅱ and 21 cases of type Ⅲ. The duration from injury to surgery was (101.2±32.1) hours(range:6 to 140 hours).The preoperative visual analogue scale (VAS) was 6.4±2.9 (range: 4 to 8); The preoperative American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot scale and the SF-36 score was 63.3±11.8 (range:50 to 75) and 94.8±9.1 (range:70 to 105) respectively.All the patients were managed with the all-side subtalar arthroscopy through three portals combined with rafting screws technique.The incision healing,reduction of fracture,and fracture healing were recorded.The VAS,AOFAS ankle-hindfoot scale,and SF-36 score were used to assess the pain and functional outcomes.Repeated-measurement was used to compare the scores before and after surgery.Results:The time of operation was (69.7±10.4) minutes (range:40 to 110 minutes). All patients did not undergo bone grafting.The incision healed in one stage without complications of skin sensory and peroneal tendon injury.All patients were followed up for 24 months.At the last follow-up,all the patients were satisfied with the hindfoot alignment,function,and hindfoot appearance.Postoperative calcaneal imaging indicators (calcaneal length,calcaneal width,calcaneal height,Bohler angle and Gissane angle) were significantly improved compared with those before surgery ( P<0.01).There was no statistically significant difference in the imaging indexes of calcaneus at 1 day,1 year and 2 years after surgery,indicating no loss of fracture reduction (all P>0.05).The VAS,AOFAS ankle-hindfoot scale,and SF-36 score were 1.5±0.4 (range: 0 to 2),94.0±6.5 (range:90 to 100),and 119.1±7.2 (range:105 to 130),respectively,which were better than those before surgery (all P>0.05).According to the AOFAS scoring system,27 cases were excellent,4 cases were good, 2 cases were fair,and the excellent and good rate was 94.0%. Conclusion:All-inside subtalar arthroscopy through three portals combined with rafting screws technique for the treatment of the calcaneal fractures of Sanders Ⅱ and Ⅲ provides precise reduction,reliable fixation,satisfactory function and limited complications,which is an alternative method.
