1.Analysis review of current situation of objective structured clinical examination in assessing domestic medical students

In this paper,we analyzed the current status of objective structured clinical exam-ination (OSCE) in the assessment of the domestic medical students, the results achieved and the problems encountered. Meanwhile,we proposed that we can optimize the station,unify SP selection and training and improve the evaluation content and subject in order to enhance the operating effect of OSCE.
2.Clinical results of transepithelial corneal collagen cross-linking for pediatric keratoconus

Background Keratoconus is a progressive corneal thinning and protrusion disease that develop in the age of puberty,resulting in a certain extent impairment of visual function.Corneal collagen cross-linking (CXL) increases the stiffness of the cornea through the photooxidation of ultraviolet A (UVA) and riboflavin,with the aim to postpone and prevent the progression of keratoectasia.Objective This study was to evaluate the safety and efficacy of transepithelial CXL in the treatment of pediatric keratoconus.Methods Ten eyes of 9 pediatric patients with keratoconus undergoing transepithelial CXL were enrolled from February 2010 to March 2013 in Affiliated Eye Hospital of Wenzhou Medical University,with the mean age was (15± 1) years (range from 13 years to 17 years).After topical anesthesia (0.1% tetracaine) for 15 minutes,0.5% riboflavin was applied until it was saturated in the anterior chamber,then UVA with the intensity of 3 mW/cm2 was performed on the cornea for 30 minutes.The uncorrected distance visual acuity (UDVA),corrected distance visual acuity (CDVA),refractive error,topography,corneal thinnest thickness were measured 7 days,1 month,3 months,6 months and 1 year after operation.Endothelium cell density (ECD) was measured 7 days afier operative.Results The mean corneal reepithelization time was (1.4±0.8) days.The UDVA and CDVA were significantly improved from preoperative 1.02±0.16 and 0.34±0.20 to postoperative 0.77±0.18 and 0.25 ±0.15,respectively (t =4.251,3.750;both at P<0.05).The refractive sphere and spherical equivalent significantly changed from preoperative (-7.15±3.00)D and (-9.26±3.23)D to postoperative (-5.28±2.05) D and (-7.05±2.08) D 1 year,respectively (t =-2.515,-2.597;both at P<0.05).Maximum Kvalue (Kmax) was significantly decreased from preoperative (64.1 ± 11.9) D to postoperative (61.8 ± 10.4) D (t =2.304,P<0.05).The refractive cylinder,corneal thinnest thickness and ECD showed no significant differences between preoperation and postoperation (t =-1.331,0.328,1.205;all at P>0.05).Stromal opacity was observed in 2 eyes 3 months and 6 months after operation,respectively.Conclusions Transepithelial CXL is safe and effective in prolonging or halting progression in adolescent keratoconus.
3.Effects of various health management on pre-diabetics

ObjectiveTo explore the effects of different health management on adults with prediabetes.MethodsA total of 1182 pre-diabetics were enrolled in this prospective observation study and assigned to one-by-one interview group,in situ education group,communicative interview group and control group.Three intervention groups received life style intervention ( including health education and risk factor control) for 2 years.Analysis ofvariance,x2 test or q test were used for data analysis.ResultsAfteryear intervention,fasting plasma glucose (FPG)and 2 h plasma glucose of the intervention groups were significantly decreased ( P < 0.05 ).FPG of the one-by-one interview group and the in situ education group was significantly lower than that of the communicative interview group at 6 months ( P < 0.05 ). At 24months,FPG of the one-by-one interview group was lower than that of the in situ education group (P <0.05).The percentage of participants who showed appropriate blood glucose control was increased in the intervention groups. In addition,blood pressure,triglyceride,cholesterol,creatinine and body weight of those in the intervened groups were significantly improved compared with the control group ( all P < 0.05 ),although electrocardiogram results and arteriosclerosisshowed nosignificant changes(P > 0.05 ).Conclusion Health management may contribute to life style change and hypertension control of prediabetics.Furthermore,in situ education may be the most effective life style interventions.
4.Development of knowledge, attitude and practice questionnaire on prevention and control of occupational diseases.

OBJECTIVETo develop the knowledge, attitude and practice questionnaire on the prevention and control of occupational diseases for occupational groups, and to provide a convenient and effective tool for the survey of knowledge, attitude, and behavior on the prevention and control of occupational diseases in occupational groups and the evaluation of intervention effect.
METHODSThe initial questionnaire which was evaluated by the experts was used to carry out a pre-survey in Guangzhou, China. The survey results were statistically analyzed by t test, identification index method, correlation analysis, and Cronbach's a coefficient method. And then the questionnaire was further modified, and the content of the questionnaire was determined finally.
RESULTSAfter modification, there were 18 items on knowledge, 16 items on attitude, and 12 items on behavior in the "Knowledge, attitude and practice questionnaire on the prevention and control of occupational diseases for enterprise managers"; there were 19 items on knowledge, 10 items on attitude, and 11 items on behavior in the "Knowledge, attitude and practice questionnaire on the prevention and control of occupational diseases for workers".
CONCLUSIONThe knowledge, attitude and practice questionnaire on the prevention and control of occupational diseases for occupational groups is developed successfully, and it is a convenient and effective tool for the survey of knowledge, attitude, and behavior on the prevention and control of occupational diseases in occupational groups and the evaluation of intervention effect.

5.Efficacy of improved skin expanding method on patients with arm infusion port

Objective:To explore the efficacy and safety of improved skin expanding method on patients with arm infusion port.Methods:Clinical data of 100 patients with gynecological malignant tumor receiving adjuvant chemotherapy was retrospectively analyzed, including 50 patients treated by traditional way of expanding skin(transverse and micro intubation sheath inserted into blood vessels) and 50 patients treated by modified skin expanding method, namely making an incision with both depth and length of 0.5 cm which tilting 45° from the thread toward the port. The indexes as planting time, smooth operation, incidence of secondary enlargement, patient satisfaction, port moving time and incidence of postoperative complications were compared and analyzed between the two groups.Result:Modified skin expanding method was superior to traditional skin expanding way in planting time, smooth operation, incidence of secondary enlargement, patient satisfaction, port moving time and incidence of postoperative complications.Conclusions:Compared with the traditional way of expanding skin, modified skin expanding method can adjust catheter malposition caused by connecting with the port and send the catheter back into blood vessel at the incision maintaining the smooth of the catheter. Can make the operation well going increasing the efficiency of implantation of the upper arm infusion port. Can avoid the adverse consequences from the straddle of blood vessel by catheter and port in the tunnel after the catheter indwelling, which greatly enhances the safety of patients during infusion port application period.
6.Reliability and validity of SF-36 in advanced schistosomiasis

Objective To evaluate the reliability and validity of SF-36 in patients with advanced schistosomiasis,so as to proride scientific basis for the selection of suitable tools for health measure.Methods A Chinese version of SF-36 scale was applied to evaluate the health of patients with advanced schistosomiasis by a household survey in Hanshou County of Hunan Province and Jiangling County of Hubei Province,then the reliability and validity of the scale were tested.Results Atotal of 326 patients were investigated in the two counties.The split-half reliability(with a split-half coefficient of 0.95) and the internal consistency (Cronbach'α coefficients of the eight dimensions ranged from 0.86 to 0.88)were satisfying;the convergent and discriminative validity were high with the test successful rates of 97.14%and 87.86%,respectively;the criterion validity was acceptable with a correlation coefficient between the total score of SF-36 and EQ-5D+C VAS score of 0.70.However,the construct validity seemed to be not so reasonable as only 2 dimensions out of 8 were completely in accordance with the theoretical model on factor loading.The percentages of floor effect and ceiling effect in most dimensions were not significant except RP and RE(with the percentages of floor effect of 50.31%and 48.16%,respectively).Conclusions SF-36 is appropriate to be used in patients with advanced schistosomiasis.but some items need to be improved according to the local settings of endemic areas.
7.Therapeutic effect of modified femoral neck osteotomy on the surgical treatment of ankylosing spondylitis with severe flexion deformity

Objective:To evaluate the efficacy of modified femoral neck osteotomy(mFNO)in the surgical treatment of patients with ankylosing spondylitis(AS)and severe spinal kyphosis combined with hip flexion contracture.Methods:A retrospective analysis was conducted on 61 AS patients(103 hips)with spinal kyphosis and hip flexion contracture who underwent pedicle subtraction osteotomy(PSO)and total hip arthroplasty(THA)from January 1,2019 to November 15,2023.Data on mFNO operation time,blood loss,preoperative and postoperative values of the angle of the trunk and lower limb(ATL),hip passive range of motion(ROM),visual analogue scale(VAS),and incidence of in-hospital compli-cations were recorded.Statistical analysis was performed using paired-samples t test.P<0.05 was con-sidered statistically significant.Results:The study ultimately included 10 cases,9 males and 1 female,with an average age of(41.30±9.03)years.These patients underwent surgery for a total of 52 times,including 19 hips both receiving mFNO and THA,and 14 times PSO.The average operation time for nine bilateral mFNO was(133.11±34.81)min,with blood loss of(433.33±187.10)mL.A unilateral mFNO took 60 min with 200 mL of blood loss.The preoperative ATL of 19 hips was 40.37°±13.66°,and the postoperative ATL value was 88.47°±12.46°(P<0.05).The preoperative VAS score was 0,while the postoperative VAS score was 5.95±1.51(P<0.05).The preoperative hip extension ROM was 37.37°±18.13°,while the postoperative hip extension ROM was-4.95°±21.24°(P<0.05).Hip flexion ROM improved from 37.37°±18.13° to 50.79°±20.36° after FNO(P<0.05).There were three cases of in-hospital complications(3/52,5.67%):One case of postoperative atelectasis fol-lowing PSO(1/52,1.92%),one greater trochanter fracture identified during THA(1/52,1.92%),and one early dislocation post-THA(1/52,1.92%).Conclusion:mFNO significantly improves the ATL in AS patients with severe spinal kyphosis combined with hip flexion contracture,facilitating PSO and THA surgeries.

9.Research progress of pH-responsive drug delivery systems in cancer immunotherapy

Abstract: Cancer immunotherapy, which is an attractive strategy harnessing the host's own immune system to remove tumor cells, has been widely used in clinical practice, yet with low response rate and immune-related adverse events. Unlike traditional chemotherapy, the targets of immunotherapy exhibit high spatial heterogeneity and are distributed in different cell types or secondary organelles, resulting in off-target and on-target toxicity, which greatly reduces the efficacy and safety of treatment. Due to the altered metabolic level, tumor tissues often display a lower pH than normal tissues. In addition, the endocytosis pathway is accompanied by continuous pumping of protons. Therefore, the variation of environmental pH values could serve as an ideal stimulus for precise drug delivery and release. In recent years, pH-responsive materials (e.g., polymers, biomacromolecules, lipid nanoparticles, biofilm, inorganic nanoparticles, and metal-organic frameworks) have been widely investigated in the field of cancer immunotherapy. This paper summarizes recent strategies of pH-responsive drug delivery systems based on different types of carriers, aiming to provide some reference for the design of next generation of tumor-targeting formulations in cancer immunotherapy.
10.Clinical diagnosis and treatment of chyle leak after laparoscopic surgery for colorectal cancer

Chyle leak or chyle ascites is a specific form of lymphatic leakage after radical laparoscopic colorectal cancer surgery,the incidence of which has been reported in the literature to be 1%-6.5%,leading to the development of hydropower imbalance,malnutrition,impaired immune function,prolonged hospital stay and even an impact on oncological prognosis.Different surgical sites,number of lymph nodes dissected,duration of surgery,prognostic nutritional index,metastasis of the main lymph nodes,operator and age are risk factors for postoperative chyle leak in colorectal cancer.It usually occurs 4-8 days after surgery and is characterized by a sudden increase of drainage after eating which is milky,odourless and tasteless,and may be combined with abdominal pain,bloating and increased body temperature.After prolonged chyle leak,patients may develop water-electrolyte disturbance,malnutrition and hypoproteinemia.The chyle test is useful in the qualitative diagnosis of chyle leak and can be performed to detect triglyceride concentrations.Conservative treatment is the preferred treatment strategy for chyle leak,and if conservative treatment fails,surgery may be considered.Knowledge of the distribution pattern of colorectal lymphatic drainage and precise control of lymph node dissection are key measures to prevent chyle leak after laparoscopic colorectal cancer surgery.
