1.Clinical research on prevention effects of Chinese medicine tea bag on calcium oxalate calculus with urinary passage

Objective To discuss the clinical prevention effects of chinese medicine tea bag on calcium oxalate calculus with urinary passage.Methods Calcium oxalate calculus with urinary passage patients were told the differentiation of symptoms and signs for classification of syndrome,choosing humid heat model、qi-stagnancy and blood stasis model、renal deficiency model with 120 patients in each group.Every model was divided into the experiment group and the control group with 60 patients in each group.We examined the rate of calculus recidivation withsonography.the urine levels of calcium,phosphonium,uric acid,ethanedioic acid after six months and one years.Results The rate of calculus recidivation at 6 months:humid heat model:the experiment group 7.55%(4/53),the control group 16.33%(8/49);qi-stagnancy and blood stasis model:the experiment group 3.57%(2/56),the controlgroup 12.09%(7/58);renal deficiency model:the experiment group 4.26%(2/47),the control group 9.30%(4/43);the total rate of calculus recidivation of the experiment group was 5.13%,while the control group was 12.67%,it had statistical significance(x2=4.60,P<0.05).The rate of calculus recidivation at 1 year:humid heat model:the experiment group 15.69%(8/51),the control group 28.26%(13/46);qi-stagnancy and blood stasis model:the experiment group 9.26%(5/54),the control group 20 69%(12/58);renal deficiency model:the experiment group 9.10%(4/44),the control group 23.69%(9/38);the total rate of calculus recidivation of the experiment group was 11.41%,while the control group was 23.94%,it had statistical significance(x2=7.90,P<0.05).In same model,the experiment group were significantly more different than those in control group at same time point(t=3.32,3.96,2.23,2.96,5.05,2.56,3.44,3.74,3.03,3.85,3.01,3.86,P<0.01,P<0.05).Conclusion Chinese medicine had preventive effect on calcium oxalate calculus with urinary passage,which worth to further study.
2.Correlation of survivin, p53 and Ki-67 in laryngeal cancer Hep-2 cell proliferation and invasion

Objective:To investigate the mechanism of survivin, p53 and Ki-67 on Hep-2 human laryngeal cancer endothelial cell proliferation and invasion.Methods:Laryngeal squamous cell carcinoma and paracancerous normal tissues were collected, total RNA was extracted from tissues,survivin,p53and Ki-67gene mRNA expression levels in laryngeal cancer and the adjacent tissues were detected by Real-time PCR. Human laryngeal cancer Hep-2 epithelial cells were selected,survivin gene was overexpressed, and cell proliferation was detected by MTT.p53 andKi-67gene expression changes in overexpressedsurvivin gene were detected by Western blot. Changes in Hep-2 cell invasive ability were studied whensurvivin was overexpressed as detected by Transwell invasion assay.Results: In the adjacent tissues, survivin,p53andKi-67 gene relative expression levels were 1.72 ± 0.9, 13.7 ± 5.7 and 5.7 ± 1.3, respectively; while in cancer tissues, gene relative expression levels were 53.7 ± 8.3, 66.7 ± 5.2 and 61.0 ± 3.1, respectively, which was significantly increased. As detected by MTT, relative cell survival rate within 12 h ofsurvivinoverexpression were: load control group, (88.5±1.6)%; overexpressed group, (90.3±1.9)%. Transwell invasion assay results indicated that overexpressedsurvivincould significantly increase the relative survival rate of cells. Conclusions:Expressions ofp53,Ki67 and survivin are increased in cancer; and there is a positive correlation betweensurvivin, p53andKi67 expressions in laryngeal carcinoma.
3.Expressions and correlations of Survivin, Ki67 and p53 in laryngeal squmous cell carcinoma.

OBJECTIVE:
To investigate the expression of Survivin, p53 and Ki67 in laryngeal carcinoma and the relation with clinical data.
METHOD:
Immunohistochemical staining (SP) was used to detect expression of Survivin, p53 and Ki67 of 64 cases with laryngeal carcinoma, 26 cases with precancerosis, 34 cases with vocal polyps.
RESULT:
The positive expression rates of Survivin, p53 and Ki67 were 59.4%, 68.8%, 65.6% respectively in laryngeal carcinoma, which were significantly higher than those in precancerosis and vocal polyps (P < 0.01). The expression of Survivin, p53 and Ki67 in laryngeal carcinoma were significantly statistical different in TNM stage and lymph node metastasis (P < 0.05), but were not correlated with patients' ages, the pathological grades, 3 years and 5 years surviving rates (P > 0.05). The expression of Survivin, Ki-67 and p53 was positively correlated (r = 0.607, 0.541, 0.648, P < 0.01) in laryngeal carcinoma.
CONCLUSION
Survivin, p53 and Ki-67 may play an important role in the carcinogenesis and progress of laryngeal carcinoma. They may play synergetic roles in the process of carcinogenesis of laryngeal carcinoma.

4.Effect of intraurethral irrigation with honeysuckle water-decoction on formation of bacterial biofilm on the surface of catheter

Objective To investigate the effect of intraurethral irrigation with honeysuckle water-decoction on formation of bacterial biofilm on the surface of the catheter. Methods Eighty male patients,who had no history of catheterization and no urinary infection with negative results of urine bacterial culture,were enrolled in the study. All patients were randomly divided into 4 groups with 20 cases in each: group A (patients underwent intraurethral irrigation with honeysuckle water-decoction); group B (intraurethral irrigation with 25% PVP); group C (intraurethral irrigation with normal saline ) and group D (no intraurethral irrigation). The urethral catheters were removed at the 7th day and the bacterial biofilm on the surface of the catheter was examined by scanning electron microscopy (SEM). Results SEM demonstrated uneven- distributed matrixes coated on the surface of the catheter, and bacteria adhered to the matrixes. The average thickness of biofilm in group A, B, C and D was (17.876 ±11.788)μn, (31.090±15. 006)μm,(33.716±11.927)μm, (67.762±23.588)μm, respectively. The average thickness of biofilm in group A, B, C was significantly thinner than that in group D (P<0.01), and the thickness in group A was thinner than that in group B and C (P<0.01), but there was no significance difference between group B and group C (P>0.01). Conclusions Intraurethral irrigation with honeysuckle water-decoction can effectively reduce the formation of bacterial biofilm on the surface of the catheter.

The authors reviewed the practice of integrated health care delivery system( IDS) at home and abroad, and based on experiences of collaborations between medical service institutions in Zhejiang province,proposed the strategic positioning,responsibilities and service innovation of urban public hospitals in a regional medical service system. It is held that the direction of China′s health care reform should move towards IDS in the future,and such hospitals should play an active role in the process via integration of its own resource and provide multi-level,diversified services for the regional health care system.
6.Management of advanced head and neck cancer with intertissue injection of 32P radioisotope

OBJECTIVE To observe the effect of 32P intertissue injection on advanced head and neck cancer. METHODS Eight patients with advanced head and neck cancer were treated with 32P injection into the tumors according to the size of the cancer as determined by CT scan. RESULTS Local tumors were controlled evidently and the pain of the patients were alleviated. The survival periods of the 8 cases ranged from 3 to 18 months (average 14.2 months). CONCLUSION 32P intertissue injection offer an useful method for patients with advanced head and neck cancer .
7.An objective evaluation of nasal airway obstruction by enlarged adenoids in children.

OBJECTIVE:
To evaluate the clinical application value of nasal airway obstruction objectively and quantificationally by acoustic rhinometry (AR) in children with adenoid hypertrophy.
METHOD:
Thirty patients with adenoid hypertrophy were examined with AR and questionnaire at preoperative and eight weeks after surgery. The values of AR were compared with those of 27 normal controls.
RESULT:
There were significant differences between preoperative and postoperative in NAR and NPV (P < 0.01), no significant difference was observed between preoperative and postoperative in MCSA (P > 0.05); There were significant differences in NAR, NPV, MCSA between the patient group and the control group (P < 0.01); There were significant differences between postoperative and the control group in NPV and MCSA (P < 0.01), no significant difference was observed between postoperative and the control group in TNAR (P > 0.05); There was negative correlation between MCSA and symptom scores (r = -0. 519, P < 0.05). No significant correlation was observed between NAR and NPV and symptom scores (P > 0.05).
CONCLUSION
Acoustic rhinometry was an objective and quantitative method for evaluating nasal airway in children with adenoid hypertrophy. In interpretation the relationships of the results of acoustic rhinometry reflection measurements and subjective symptoms, MCSA should be the main reference.

8.Effects of resilience and cognitive evaluation on quality of life of burn patients

Objective:To understand the status of life quality, resilience and disability acceptance of burn patients, and to explore the impact of mental resilience and disability acceptance on the quality of life.Methods:Connor-Davidson Resilience Scale (CD-RISC), Acceptance of Disability Scale (ADS) and Burn Specific Health Scale-Brief (BSHS-B) were used to carry out questionnaire survey for a total of 315 burned patients who were in rehabilitation phase.Results:For burned patients, the total score of resilience was 57.12±14.34, the total score of disability acceptance was 78.20 ±9.99, and the total score of quality of life was 128.29 ±37.26. The correlation analysis showed that the total score of resilience was positively correlated with quality of life for burned patients ( r value was 0.646, P<0.01); the total score of disability acceptance was positively correlated with the total score of quality of life for burned patients ( r value was 0.846, P < 0.01). Multiple stepwise regression showed that the total score of disability acceptance, severity of burn, sleep state, ability and endurance of negative emotions were the main predictors of the quality of life for burned patients. Conclusions:Great attention should be paid to the quality of life of burned patients, and psychological problems of high-risk patients should be found as soon as possible and reasonable intervention should be given to improve the resilience and disability acceptance of patients, so as to improve their quality of life.
9.The application value of adaptive statistical iterative reconstruction-Veo combined with full-field organ dose modulation technique in low-dose CT urography

Objective To investigate the clinical application value of posterior adaptive statistical iterative reconstruction-Veo(ASIR-V)algorithm combined with full-field organ dose modulation(ODM)technique in low-dose computed tomography urography(CTU).Methods Ninety patients who underwent urological contrast-enhanced and CTU examination were prospectively selected and divided into conventional dose group(group A,NI=11,posterior ASIR-V of 60%),low-dose group(group B,NI=15,posterior ASIR-V of 60%-100%with 10%interval,denoted as B1-B5 groups);full-field ODM low-dose group(group C,NI=15,posterior ASIR-V of 60%-100%with 10%interval,denoted as C1-C5 groups)according to the noise index(NI).The volume rendering(VR)and maximum intensity projection(MIP)images were reconstructed.The CT values and standard deviation(SD)values of the renal cortex and para-spinal muscles were measured,and the signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were cal-culated.The CT values of the renal pelvis,ureter,and bladder were measured,and the SNR and CNR were calculated with the para-spinal muscles as background.The subjective score was performed on the two-dimensional and three-dimensional images to evaluate the ability of CTU to display lesions.The volume CT dose index(CTDIvol),dose length product(DLP),and effective dose(ED)of the three groups were compared.Results The bladder SNR and CNR of groups B1,C1,and C2 were lower than those of group A(P<0.05).There was no statistically significant difference in the SNR and CNR of each tissue between groups B2,C3 and group A(P>0.05).The SNR and CNR of each tissue in groups B4,B5,and C5 were higher than those in group A(P<0.05).The display rate of lesions in urinary system in groups B2 and C3 could reach 100%.There was no statistically significant difference in the axial image scores between groups B2,C3 and group A(P>0.05),while the VR and MIP image scores in each group were comparable to those in group A(P>0.05).The CTDIvol of groups A,B and C were(6.08±2.11)mGy,(3.15±1.15)mGy,and(2.9±0.92)mGy,respectively.Conclusion CTU using the full-field ODM technique combined with posterior 80%ASIR-V can reduce the CTDIvol by 52.30%,and further reduce by 7.93%compared with group B,which is effective to reduce radiation to the gonads.
10.Application of auto-prescription technique combined with noise index and iterative reconstruction algorithm to the computed tomographic angiography of deep inferior epigastric artery

Objective:To explore the effects of the auto-prescription technique combined with noise index (NI) and adaptive statistical iterative reconstruction algorithm-veo (ASIR-V) on the radiation dose and image quality in the computed tomographic angiography (CTA) of the deep inferior epigastric artery (DIEA).Methods:The data of 150 cases who underwent DIEP CTA in the First Affiliated Hospital of Dalian Medical University were prospectively collected, and were randomized into groups A, B, and C, with 50 cases in each group. For group A, the conventional tube voltage was 120 kVp, and images were reconstructed using 40% ASIR-V. For group B, the tube voltage based on the auto-prescription technique was adopted, and images were reconstructed using 40%, 60%, and 80% ASIR-V (corresponding to the three subgroups B1-B3, respectively). For group C, the tube voltage based on the auto-prescription technique was employed, NI = 13, and images were reconstructed by 40%, 60%, and 80% ASIR-V (corresponding to subgroups C1, C2, and C3, respectively). Subsequently, the CT and SD values of the originating femoral artery of DIEA and the rectus abdominis at the same level as DIEA were measured on axial images of each group, the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were calculated, and the radiation doses of the three groups were recorded. Finally, the image quality and noise of DIEA in each group were independently and subjectively scored by two radiologists in a blinded manner.Results:Compared to group A, groups B and C exhibited higher CT values of femoral artery ( t= -5.18, -5.17, -5.06, -6.08, -6.08, -6.07, P < 0.05) and higher image SNR and CNR ( P < 0.05), which increased gradually with the upgrading of ASIR-V ( F = 55.45, 49.70, 53.47, 68.89, P < 0.05). The two radiologists offered consistent subjective evaluations (Kappa = 0.76-0.92, P < 0.05). The images of groups B and C displayed more distinct perforating branches and emerging points of DIEA and received higher subjective scores of intramuscular course compared to those of group A. The subjective scores of image noise in subgroups B2, B3, and C2 were not significantly different from those in group A. The radiation doses in groups B and C decreased by 15.10% and 52.85%, respectively compared to those in group A ( H = 75.21, P < 0.05). Conclusion:The combination of the auto-prescription technique with NI = 13 and 60% ASIR-V can reduce the radiation dose by 52.85% while ensuring a clear display of DIEA.
