1.Expressions of Cyclin D1 and Ki67 in Esophageal Squamous Cell Carcinoma and Their Clinical Significance

Objective To estimate the expressions of cyclin D1 and Ki67 in esophageal squamous cell carcinomas and their clinical significance. Methods The expressions of cyclin D1 and Ki67 were detected by immunohistochemistry in 28 specimens esophageal squamous cell carcinoma (ESCC) and 33 samples esophagitis tissue. Results The positive expression rate of cyclin D1 in ESCC and esophagitis tissues was 60.7% (17/28) and 33.3% (11/33) respectively (?2=4.573,P=0.032),while the mean Ki67 label index (Ki67 LI) was (49.21?25.15)% and (11.62?9.87)% respectively (t=7.908,P=0.000). The positive expression rate of cyclin D1 in TNM stage I tissues was 14.3% (1/7),which was significantly lower than that in stages Ⅲ and Ⅳ tissues[85.7% (6/7) and 100% (5/5),P=0.029 and 0.015,respectively]. In cases with lymph node metastasis,the positive rate of cyclin D1 expression was 90.9% (10/11) that was significantly higher than that in those without lymph node metastasis (P=0.016). The 28 patients with ESCC were followed up for 6 to 34 months [mean,(25.0?4.2) months],during which 19 patients survived,4 patients died of deterioration of the primary diseases (3 cases) or cerebrovascular affair (1 case),and the other 5 patients was lost to follow-up. Conclusions The expression of cyclin D1 is correlated with advanced tumor and lymph node metastasis. The Ki67 is highly expressed in ESCC tissues; no relationship exists between the level of the expression and the pathological differentiation type and TNM stage of ESCC.
2.Progress of Drug Treatment for Gastrointestinal Neuroendocrine Neoplasm

Neuroendocrine neoplasm (NEN)is a group of heterogeneous tumor originated from the neuroendocrine system,and gastrointestinal neuroendocrine neoplasm (GI-NEN)is the mostly frequently seen. The incidence of GI-NEN is increasing year by year,but the efficacy of clinical treatment is unsatisfactory. In recent years,new progress has been achieved in medical therapy of GI-NEN,including somatostatin analogues,interferon,mTOR inhibitor,vascular endothelial growth factor inhibitor and chemotherapeutic drugs. Peptide receptor radionuclide therapy also provides an additional option for treatment of metastatic GI-NEN. This article reviewed the recent progress of drug treatment for GI-NEN.
3.Clinical features and surgical treatment of extrahepatic growing hepatocellular carcinoma

Objective To study the clinical characteristics, diagnosis and treatment of extrahepatic growing hepatocellular carcinoma(HCC). Methods The clinical data of 11 patients with extrahepatic growing HCC were analysed retrospectively. Results The mean diameter of the tumors was (12.4?4.3)cm.All the tumors in the 11 patients had complete capsule formation. The numbers of tumors located in the left, right and caudate lobe of the liver were 6,3 and 2 respectively. Surgical treatment included segmentectomy in 6 cases , lobectomy in 4 cases, and unresectable in 1 case. The 1-, 2-, and 3-year survival rates were 80.1%,62.3%, and 47.6% respectively. Conclusions Although the size of extrahepatic growing HCC is large,the resection rate is high and prognosis is good. The resection of hepatic segments or lobes containing the lesion should be done in radical operation of this tumors.
4.Advances of resveratrol synthase gene in the application of genetic engineering and biofunctional investigation.

Resveratrol synthase (RS) plays a key role in resveratrol (Res) biosynthesis. RS gene has been formerly reported to be transformed into many plant species and microorganisms, and to play certain roles in metabolic and regulation processes. In this paper, the transformations of RS gene in plants, and the related changes of biological properties, such as metabolites, anti-pathogen activities, anti-radical properties, and developmental characters in transgenic plants, as well as the production of resveratrol in microbes by utilizing RS gene were summarized. Moreover, the application prospects of RS gene in bioengineering were also addressed.

5.Factors Associated with Fungal Infection Following Pancreatoduodenectomy

Objective To investigate the relevant factors for fungal infection following pancreatoduodenectomy and offer the theoretical foundation for preventing the emergence of complications after operation. Methods Medical records from 562 consecutive patients who underwent pancreatoduodenectomy in this hospital from 1995 to 2005 were retrospectively reviewed by using single factor and non-condition Logistic regression analyse. Results ①Seventy-eight patients (13.9%) developed invasive fungal infection. The most frequently isolated fungal were Candida albicans accounted for 67.0%, and followed by Candida glabrata, Candida papasilosis and Candida tropicalis and gastrointestinal tract was the most common infection site, followed by respiratory tract, abdominal cavity. ②Fungal infection occurred significantly more often in patients with the length of time in parenteral nutrition, antibiotic use or abdominal cavity complications. Conclusion The most common infection site and isolated fungal associated with pancreatoduodenectomy were gastrointestinal tract and Candida albicans. Abdominal cavity complications such as pancreatic fistula, biliary fistula and abdominal infection and extended use parenteral nutrition and antibiotic are the most important factors leading to invasive fungal infection after pancreatoduodenctomy. Eliminating the various risk factors will decrease the incidence of fungal infection.
6.A Study of Instantaneous Biological Effects of Blast Waves with High-speed Photography

The instantaneous changes of the body cavities under the impact of blast waves were studied in three dogs. The changes were recorded with the high-speed photography of three different speeds (563, 1526, and 6526 frames/second). It was demonstrated that there was an 11.7% reduction on average of the body cavities due to compression after 0.777 kg/cm2 of overpressure was received.In correlation with the authors' previous research work and with the relevant literature, it is believed that the 11.7% reduction is likely related to the severe pulmonary injury which is resulted from the abrupt disturbance of hemodynamics after blast wave impact.
7.The prognosis of liver cirrhotic patients with acute esophageal variceal bleeding: a Cox regression analysis

Objective To evaluate the risk factors for prognosis in liver cirrhotic patients with acute esophageal variceal bleeding (EVB). Methods Retrospective analysis was carried out in patients with liver cirrhosis,who were admitted to the hospital for EVB between January 2000 and December 2006 . All patients were follow-up to August 31st, 2009. The death of the patients was identified as the end of the study. The vital signs and results of laboratory tests were recorded when patients were admitted to the hospital. Child-Pugh score/grade, model for end stage liver disease (MELD) score and Rockall score were calculated. Results Ninty-five cases (66 males and 29 females) were enrolled in the study. Among them, 60 cases died and 35 cases survived for (42. 61±33.21) months (ranged from 2 days to 114 months). Univariate Cox regression analysis showed that age, male, Child-Pugh score/grade, MELD score, Rockall score, white blood cell counts and active hepatitis B were risk factors for death in liver cirrhotic patients with EVB (P<0.05), while high level of hemoglobin and high hematocrit, endoscopic variceal sclerosis therapy or surgical treatment were protective factors (P<0.05). Multivariate Cox regression analysis revealed that Child-Pugh grade (RR= 4. 997,95%CI:2. 787~8. 960, P<0.01),Rockall score (RR= 1. 284,95%CI:1. 062~1. 553,P=0. 010), high counts of white blood cells (RR= 1. 072,95%CI: 1. 001 ~ 1. 148, P= 0. 046) were risk factors for prognosis of liver cirrhotic patients with EVB. Conclusion It is demonstrated that Child-Pugh grade,Rockall score and high counts of white blood cells are risk factors for prognosis of liver cirrhotic patients with EVB.
8.Effects of Total Saponins of Chaenomeles speciosa on Degranulation Ratio of RBL-2H3 Mast Cells

Objective To study the effects of total saponins of Chaenomeles speciosa on release of β-hexosaminidase from rat basophilic leukemia-2H3 ( RBL-2H3 ) mast cells. Methods After rat RBL-2H3 mast cells were prepared, total saponins of Chaenomeles speciosa and the RBL-2H3 mast cells were co-cultured.The toxic effects of total saponins of Chaenomeles speciosa on mast cells were detected by MTT method, β-hexosaminidase release rate was measured by fluorescence quantitative spectrophotometric method, and cell supernatants of tumor necrosis factor-α( TNF-α) release were detected by ELISA. Results After total saponins of Chaenomeles speciosa were co-cultured with RBL-2H3 mast cells with different antigen stimulation, β-hexosaminidase release rates and the levels of TNF-α of mast cells significantly decreased compared with the control group. Conclusion Total saponins of Chaenomeles speciosa inhibit the degranulation of RBL-2H3 mast cells in a dose dependent manner, which provids basis for studying mechanism of inhibiting allergic reactions.
9.Expression and significance of CD105 in endothelial cells of primary hepatocellular carcinoma

0.05). Conclusion CD105 overexpression may play a crucial role in early recurrence of HCC in patients after operation. The expression of CD105 mRNA in the tumor peripheral tissue was much higher than in the tumor center and normal liver tissue, which indicated CD105 was related to early recurrence.
10.A pilot study on photoreceptor IS/OS and subfoveal choroidal thickness in IMH with spectral domain OCT

Background The classical theory from Gass holds that idiopathic macular hole (IMH) is due to the dragging of vitreous cortex in the tangent direction on the macular region,while spectral domain OCT (SD-OCT)studies found that the diameters of inner segment/outer segment (IS/OS) absence (DIOA) region was closely associated with vision ability of IMH patients.Enhanced depth imaging (EDI) is thought to be a new method to study the relation between DIOA and subfoveal choroidal thickness (SFCT) ,but there are few relevant reports.Objective This study aimed to observe the correlation between photoreceptor IS/OS junction and SFCT before and after vitrectomy in IMH by EDI SD-OCT.Methods Forty unilateral IMH patients were enrolled in Affiliated Foshan Hospital of Southern Medical University from June 2011 to June 2013.Vitrectomy (23G) was performed on the patients by the same operator.EDI mode of SD-OCT was used to measure DIOA and SFCT in horizontal direction before and 6 months after surgery.The relationship between DIOA and SFCT was evaluated using Pearson linear correlation analysis.Written informed consent was obtained from each patient before medical assessment.Results IMH was anatomically closed after vitrectomy in 34 eyes with the closure rate 85%.The mean DIOA was (1 280±753) μm in preoperation, which was significantly higher than (656 ± 322) μm in postoperation (t =4.989, P =0.000).The mean SFCT was (130±43)μm in preoperation,showing a slight reduce in comparison with (140±38)μrn in postoperation, but there was not statistical significance between them (t =-1.407, P =0.175).Negative correlations were found between DIOA and SFCT in both preoperation and postoperation (preoperation:r=-0.748 ,P=0.000;postoperation: r=-0.686,P =0.001).Conclusions The alteration of DIOA shows a negative correlation with SFCT in IMH eyes,suggesting a contributing role of the choroid in the change of photoreceptors of IMH.
