1.Optimization of the immunologic diagnostic criteria for rheumatoid arthritis

Objective To optimize the immunologic diagnostic criteria of rheumatoid arthritis commonly used in clinical,to find methods to diagnose rheumatoid arthritis quickly,accurately and economically.Methods The levels of RF,CRP,ESR,anti-CCP,AKA,CP I,anti-MCV in blood of the patients with rheumatoid arthritis were simultaneously measured, and their sensitivity and specificity was compared.Results The sensitivity and specificity of antiMCV was 76.8% and 90.6% ,anti-CCP 67.2% and 90.3% ,AKA 75.2% and 84.7% ,RF 72.8% and 79.1%.The sensitivity and specificity of them were higher.The specificity of GPI(92.1% ) was high,but the sensitivity of it (46.4% )was low.The sensitivity and specificity of other criteria were low.Conclusion anti-MCV,anti-CCP,AKA and RF could be preferentially used in the diagnosis,therapy and monitor of curative effect of rheumatoid arthritis.
2.Allogeneic stem cell transplantation in multiple myeloma

Allogeneic stem cell transplantation is the only treatment which may cure multiple myeloma (MM).The high transplantation related mortality (TRM) limits the wide clinical application of myeloablative allogeneic stem cell transplantation.Non-myeloablative allogeneic stem cell transplantation can reduce the TRM,but the risk of relapse and progress of the disease may get increased. Autononmyeloablative allogeneic stem cell tandem transplantation can reduce both the TRM and the relapse and progress of the disease. At present,there exist many new methods which may improve the effect of allogeneic stem cell transplantation for MM in clinical.
3.Correlation between hypertensive disorder complicating pregnancy with homocysteine,DˉD and hsˉCRP

Objective To investigate the relationship between hypertensive disorder complicating pregnancy(HDCP)with homo-cysteine,D-D and high sensitive C-reactive protein(hs-CRP).Methods 80 HDCP women included 38 cases of gestational hyperten-sion,26 cases of mild preeclampsia and 16 cases of severe preeclampsias,and at the same time 36 normal late-term pregnant women and 30 non-pregnant normal women were selected as the control group.The plasma Hcy was determined by the enzymatic cycling assay,D-D was determined by the latex immune turbidimetry and hs-CRP was determined by the nephelometry immuno assay,re-spectively.Results The plasma Hcy,D-D and hs-CRP levels in the HDCP group were significantly increased compared with the normal non-pregnancy women group and the normal late-term pregnancy women group controls and showed the increasing trend with the aggravation of disease condition,the differences had the statistical significance(P <0.01).Conclusion Timely monitoring the plasma Hcy,D-D and hs-CRP levels can effective conduct the adverse pregnancy prediction,thus timely takes the medication in-tervention for correcting the occurrence and development of the disease condition,and provides the significant reference indexes for evaluating the change and prognosis of the disease condition in clinic.

5.Changes of IL-4, IL-5, TGF-β and IL-10 in pediatric steroid-sensitive nephrotic syndrome

Objective:To clarify the dynamics of Th2 related cytokines IL-4、IL-5 and the Treg T cell related cytokines TGF-β、IL-10 in children with primary steroid-sensitive nephrotic syndrome ( SSNS) from the nephrotic phase before steroid treatment to the re-mission phase.Methods:All 36 SSNS patients and 18 healthy controls matched for age , gender, body weight and height were enrolled , and their clinical characteristics were evaluated .Serum cytokine level was measured by ELISA assay .The serum levels of interleukin (IL)-4, IL-5, IL-10 and transforming growth factor (TGF)-βwere compared and correlated with serum albumin and cholesterol .Re-sults:Serum IL-4 and IL-5 levels in SSNS of nephrotic phase were higher than serum IL-4 and IL-5 levels of SSNS of remission phase and the healthy control , respectively .The serum TGF-βlevels of the nephrotic phase were significantly lower than those of remission phase or control group , whereas the serum IL-10 levels showed no significant difference between nephrotic phase and remission phase of SSNS or control group.The serum IL-4 levels had a negative correlation with serum albumin (R2 =-0.694,P=0.000), and a posi-tive correlation with serum albumin levels (R2 =0.658,P=0.000), whereas the serum TGF-βlevels had a positive correlation with serum albumin (R2 =0.838,P=0.000), and a negative correlation with serum albumin levels , had (R2 =-0.722,P=0.000). Conclusion:This study indicates that IL-4, IL-5 and TGF-βlevel is related to the pathogenesis of pediatric SSNS .
6.The value of thromboelastography in evaluating the risk of bleeding in patients with acute leukemia

Objective To investigate the role of thrombelastography(TEG) in assessing the risk of bleeding and diagnostic value in patients with acute leukemia(AL) .Methods The TEG and PLT data were counted in 127 patients(272 sets of data) who were diagnosed with AL .Those patients were divided into two groups :group 1 (including patients with bleeding) and group 2 (in‐cluding patients with no bleeding) .The indicators(R values ,K values ,α‐angle ,MA values)and PLT count were compared between two groups .Those data with PLT<30 × 109/L of these two groups also were divided and the 4 indicators of TEG were compared between the two groups .We used the ROC curve to evaluate the sensitivity and specificity in assessing the risk of bleeding .Results According to the data in total ,the K value ,R value of the group 1 were higher than those of the group 2(P<0 .05);theα‐angle and MA value ,PLT counts of group 1 were lower than those of the group 2(P<0 .05) .In those AL patients whose PLT<30 × 109/L ,the K value of the group 1 was higher than that of the group 2(P<0 .05);theα‐Angle and MA value of the group 1 were lower than those of the group 2(P<0 .05);R values and PLT count were not different between the two groups(P>0 .05);the are‐as under the ROC curve about the PLT counts ,MA value andα‐angle were more than 0 .5 (0 .750 ,0 .740 and 0 .653) .Conclusion T EG could predict the risk of bleeding in acute leukemia patients and it could be used in clinical application .

9.Cross-sectional study of the mild cognitive impairment among elderly in Xinjiang Uygur and Han ethnic groups

Objective To understand the prevalence and distribution features of mild cognitive impairment(MCI)among elderly in Xinjiang Uygur and Han ethnic groups so as to provide evidence for etiological study and prevention. Methods From July 2007 to October 2008, according to the criteria of DSM-IV for MCI,a randomized, stratified and cluster sampling procedure was used in the Uygur and Han elder people aged ≥60 years in south Xinjiang, east Xinjiang and Urumchi region. Results A total of 2986 people were surveyed, including 1519 Uygur people and 1467 Han people, and 1435 males and 1551 females. (1) According to the age composition of national census in 2000, the total crude prevalence rate of MCI was 10.21%, and the total standardized prevalence rate of MCI was 10.58% in Uygur and Han elder people. In general Uygur and Han population, the crude prevalence rates of MCI were 9.61% and 10.84%, and the standardized prevalence rates of MCI were 10.29%and 10.86%, respectively. The prevalence of MCI was higher in Han population than in Uygur population, but there was no statistical difference(χ~2 = 1.225, P>0.05). (2) In males and females, the crude prevalence rates of MCI were 9.34% and 11.03%, and the standardized prevalence rates of MCI were 9.26% and 11.62%, respectively. There was no difference in prevalence rate between different sex populations(χ~2 =2.314,P>0.05). (3) In elder people aged 60~69, 70~79 and≥80 years, the MCI prevalence rates were 6.83%, 13.22% and 22.22% in Uygur population and 8.64%, 12.50% and 19.30% in Han population, respectively, and the prevalence rate of MCI was increased with aging in the two thnic roups(χ_(for trend)~2=34.753, 14.081, both P<0.05). (4)There were statistical differences in prevalence rates of MCI among different education levels, and it was decreased with enhancement of education levels in Uygur and Han population(χ_(for trend) = 14.785, 21.059, both P<0.05). Conclusions In Xinjiang Uygur and Han ethnic elderly people, the prevalence rates of MCI are significantly different among elderly with different ages and education levels, and it is increased with aging, but is decreased with enhancement of education levels.
10.Investigation of stroke-preventing knowledge and influential factors in hypertensive patients from the communities of Guangzhou

Objective To investigate the current situations in stroke-prevention knowledge and influential factors in hypertensive patients from the communities of Guangzhou.Methods 94 hypertensive patients visiting the community health service centers in Guangzhou participated in the survey using stroke-preventing knowledge questionnaire.Results The total score on the stroke-preventing knowledge was(62.70±18.39).By stepwise regression analysis,those female and well-educated scored higher. The score on monitoring the blood pressure was the lowest(43.09±25.77).There were mistakes in medication,physical exercises and living habits.Conclusions The hypertensive patients in the communities in Guangzhou lack of stroke-preventing knowledge.In the stroke-preventing education,those male patients as well as those with less education deserves more attention.
