2.Effects of rotating-knee system versus hinge-knee system replacement in knee salvage of tumor

BACKGROUND: With the development of n0eo-adjuvant chemotherapy, 5-year survival rate of bone tumors has significantly increased. Limb salvage has replaced amputation to become a major means of treating knee tumors and has achieved good results. OBJECTIVE: To investigate and compare the effect of knee joint reconstruction using hinge-knee system and rotating knee system. DESIGN, TIME AND SETTING: Comparative observation. The patients were selected from Department of Spine and Orthopedic Surgery, First Affiliated Hospital of Gaungxi Medical University between June 1999 and March 2006. PARTICIPANTS: A total of 72 patients with tumor round the knee were selected from the First Affiliated Hospital of Gaungxi Medical University, and 9 cases were lost in follow-up. Of the remaining 63 cases, there were 35 males and 28 females, aged 31 years (range, 15-58 years); 40 cases underwent knee joint reconstruction using hinge-knee system and 23 using rotating knee system. METHODS: The tumor excision of superior tibia was performed according to Ennecking principle. After joint reduction of hinge-knee prosthesis, the bolt was adjusted and locked; the high polymer polyethylene pad was placed in rotating knee system, and the joint rotation axis was assembled. The tumor excision of inferior tibia was performed from the medial thigh along medial vastus muscle posterior margin till medial patellar ligament of superior tibia. After joint reduction of hinge-knee prosthesis, the bolt was adjusted and locked; the high polymer polyethylene pad was placed in rotating knee system, and the joint rotation axis was assembled. MAIN OUTCOME MEASURES: The treatment effect was evaluated according to MSTS functional scores, ambulation scores, flexion-extension-angle, knee rotation and infection, relapse and operative time. RESULTS: The 63 patients were followed up for 9 months to 8 years. In hinge-knee system group, operative time was (2.663±0.336) hours; there were 5 cases of relapse, 3 cases of metastasis/death and 2 case of infection. In rotating knee system group, operative time was (2.572±0.288) hours; there were 3 cases of relapse, 2 cases of metastasis/death, and 2 case of infection. One case in rotating knee system group developed fracture and dislocation of the polyethylene bush of rotating axis and there was no syndrome after changing fittings. No prosthesis loosening was found in two groups. With exception of patients with relapse and metastasis/death, 50 cases were evaluated, and no significant difference was found in MSTS scores between two groups (P > 0.05); the total score and single item score of ambulation scores in rotating knee system group were significantly greater than hinge-knee system group (P < 0.05). The extension angle in two groups reached or was close to 0°, and the flexion-extension angle of hinge-knee system group (119.375±17.490)° was similar to rotating knee system group (125.000±15.340)° (P=0.260). CONCLUSION: The rotating knee system displays superior function than hinge-knee system in knee salvage after surgery. Ambulation scores revealed that the knee joint function of the rotating knee system is superior over hinge-knee system, while the MSTS scores did not. Therefore, ambulation scores is more sensitive and appropriate to evaluate the function after knee salvage. There were no significant differences in operative time and flexion-extension ability and syndrome between two methods.
3.Protective effect of naomaitong injection against global cerebral ischemia reperfusion injury in rats

BACKGROUND: Naomaitong injection is a Chinese herbal compound preparation for treatment of ischemic cerebral vascular disease, acting on resisting calcium overload, regulating the imbalance between thromboxane (TXA) and prostaglandin (PG) and blocking lipid peroxidation mediated by free radical so as to protect cerebrum.OBJECTIVE: To observe the effects of naomaitong injection on water and Ca2+ contents, activity of superoxide dismutase (SOD), levels of lipid peroxide (LPO), 6-keto-PG lα and TXA, and compare it with danshen injection.DESIGN: Randomized control experiment was designed.SETTING: Experimental Center of Chengdu University of Traditional Chinese Medicine.MATERIALS: The experiment was performed in Experimental Center of Chengdu University of Traditional Chinese Medicine from October 1997 to February 1998, in which, 72 healthy male Wistar rats were employed, rangroup: Abdominal injection was done with physiological saline 1.67 mL/kg,group): Abdominal injection was done with compound danshen injection groups (naomaitong No.1, No.2 and No.3 groups): abdominal injection was done with naomaitong injection 3.33, 1.67, 0.84 mL/kg successively,twice/day.METHODS: Totally 48 hours after medication, under anesthetized state,the rats in every group were sacrificed to collect brain tissue. The two hemispheres were cut into two pieces from the middle. One of them was prepared into brain tissue homogenate at low temperature. Radioimmunologic analysis method was used to measure 6-keto-PG 1o and TXA B2 levels so as to evaluate the balance between PG and TXA systems. The modified pyorgallol autoxidation and thiobarbituric acid (TBA) colorimetric method were applied to determine SOD activity and LPO level respectively so as to evaluate lipid peroxidation mediated by free radical. The dry and wet weights of other piece were weighed immediately on electronic scale and the water content of brain tissue was calculated to evaluate brain edema. Atomic absorption spectrophotometry (AAS) was used to determine Ca2+content in brain tissue so as to evaluate calcium overload.tissue of rats in every group.content in brain tissue of rats in every group: That in model group was higher remarkably than normal group [(82.27±1.32)%, (77.24±1.36)%;(267.47±15.69), (37.55±13.23) μg/g, P < 0.01]. The water contents in 4 treatment groups were decreased of various degrees. The effect in No.1 group was the strongest [(78.74±1.41)%] and that in danshen group was the weakest [(81.45±1.52)%]. Ca2+ content in danshen group was decreased of various degrees, indicating dose-effect dependence, but, which was near to ty and LPO level in brain tissue of rats in every group: SOD activity in model group was lower remarkably than normal group [(86.18 ±3.17),(131.86±4.67) μkat/g, P < 0.01]. After treated with naomaitong of 3 dosages, that was all improved, indicating dose-effect dependence (P < 0.01). The effect of No.1 group was the strongest [(119.02±4.00) μkat/g],SOD activity in danshen group was near to model group (P > 0.05). LPO level in model group was higher than normal group [(52.46 ±3.25),(32.29±2.23) μmol/L, P < 0.01]. LPO level of every treatment group was lower significantly than model group and the therapeutic effects of No. 1, 2,3 groups were superior to danshen group [(35.68±2.86), (41.54±2.47),1α and TXA B2 in brain tissue of rats in every group: Content of 6-ketoPG 1α in brain tissue of model group was lower remarkably than normal group (P < 0.01). That was improved in all of 4 treatment groups, in which,the therapeutic effects of No.1, 2, 3 groups were superior to danshen group [(43.84±2.98), (35.01±4.32), (29.97±3.81), (22.89±3.64) ng/g, P < 0.01].TXA B2 content in brain tissue of model group was higher remarkably than normal group (P < 0.01). After treatment, the 4 treatment groups lowed significantly TXA B2 content in brain tissue compared with model group,indicating dose difference. That in danshen group was lower than No. 1, 2,3 groups [(40.58±1.34), (32.85±1.43), (34.31±1.39), (37.27±1.52) ng/g, P <0.01].CONCLUSION: Naomaitong injection alleviates brain edema, resists calcium ion, regulates imbalance between TXA and PG systems, improves activity of anti-oxidase and is against injury of free radical so as to protect the structure of brain tissue and achieve therapeutic effects, indicating a certain dose-effect relationship. The effect of naomaitong injection is superior to that of compound danshen injection.

5.Interventional diagnosis and treatment in emergent uterine artery bleeding after curettage

Objective To discuss the possibility of interventional diagnosis and treatment of uterine artery bleeding after curettage. Methods Select the emergent patients with uterine artery bleeding after curettage as the basis for study. Three women with uterine artery bleeding underwent femoral artery puncture and transcatheter uterine artery embolization. Bilateral selective artery angiographies were performed, and then underwent artery embolization after exhibiting the bleeding sites. Results Three patients were promptly and correctly diagnosed and arterial embolization were then attempted under DSA. No recurrence of bleeding during the angiographic and clinical follow up simultaneously with no serious complication. Conclusions Uterine arterial DSA and interventional embolization of uterine artery are effective in the diagnosing and treatting emergent uterine artery bleeding after curettage.
6.Experimental research on inhibition of carotid artery atherosclerosis induced by endostatin gene mediated by ultrasound targeted microbubble destruction in rabbits

Objective To explore the inhibition effect on angiogenesis and plaque growth of carotid atherosclerosis by transfection of endostatin gene using microbubbles combined with ultrasound exposure.Methods Twenty rabbit models of carotid atherosclerosis were randomly divided into 3 groups:group A,microbubble+ ultrasound; group B, control plasmid + microbubble + ultrsound; group C, ES plasmid +microbubble+ ultrasound. Two weeks after surgery, ultrasound/microbubble mediated gene transfer was performed,and it was performed once again three weeks after the first transfection. Ultrasonography and digital subtraction angiography(DSA) were performed at the time of 14 weeks. The carotid arteries were taken to detect the neointima and angiogenesis, and the expression of endostatin was detected using pathological means. Results The imagings of ultrasound showed that the intima in group A and B were thick significantly with larger plaques, and the lumen became stenosis with the peak systolic velocity increasing,however,in group C,the parameters mentioned above were significantly less than those of group A and B ( P<0.05). Pathological results displayed that intima-media thickness (IMT), intima thickness (IT), intima thickness/media thickness (IT/MT), intima area (IA), intima area/media area (IA/MA) and neointimal stenosis rates were greater in group A and B, however, they were less in group C ( P<0.05).The number of neovascularization and vascular endothelial growth factor(VEGF) expression in group A and B were more than those of group C. There was more endostatin positive expression in carotid arteries and anterior tibial muscles of group C, while there was nearly no expression in group A and B. Conclusions Under the conditioned ultrasonic irradiation, ultrasound/microbubble mediated endostatin gene transfection can inhibit the development of carotid atherosclerosis in rabbits, which might provide a safe and effective strategy for gene therapy of atherosclerotic disease in future.
7.Determination of 4-( 4-Amino-3-fluorophenoxy )-N-methylpyridine-2-carboxamide in Regorafenib by LC-MS/MS

Objective: To establish an LC-MS/MS method for the determination of 4-( 4-amino-3-fluorophenoxy )-N-methylpyri-dine-2-carboxamide ( AFP-PMA) as a genotoxic impurity in regorafenib. Methods: The content of AFP-PMA was determined by an LC-MS/MS method. A Waters XBridge Shield RP18 column was adopted to separate the samples and the column temperature was 50℃. The mobile phase consisted of 5 mmol·L-1ammonium acetate aqueous (A)-acetonitrile (B) with gradient elution (0~9 min, 5%B→90%B) at a flow rate of 1. 0 ml·min-1. An electrospray ionization source (ESI) was used in a positive-ion and multiple reactions monitoring mode. The ion channel was m/z 262. 2→244. 1. Results:The standard curve was linear within the range of 2. 41-980. 90 ng·ml-1(r=0. 9998) and the limit of quantification was 8. 02 ng·ml-1. The limit of detection was 2. 41 ng·ml-1, which was e-quivalent to 0.000241% for the concentration of regorafenib. The average recovery was 100.95% and RSD was 2.37% (n=9). Conclusion:The method has good specificity, promising accuracy and high sensitivity, which can be used for determining the trace genotoxic impurity AFP-PMA in regorafenib.
8.Clinical Application of Transcatheter Polyvinyl Alcohol Embolization for Interventional Therapy of Central Bronchogenic Cancer

Objective To study the clinical values of polyvinyl alcohol chemoembolization and bronchial arterial infusion(BAI) in interventional therapy of central bronchogenic carcinoma.Methods 45 pathologically proved cases of central bronchogenic carcinoma in mid or late period were randomly divided into two groups(control group in 24 cases and experimental group in 21 cases).After detecting the nourishing arteries of bronchogenic carcinoma with angiography,the treating processes were performed.Control group : anticancer drugs were poured into bronchial artery.Experimental group : the feeding arteries of tumors were co-axially superselective catheterized with microcatheters,the anti-neoplasm agents were poured into everyone of feeding arteries according to the blood supply,then the feeding arteries were embolized by polyvinyl alcohol.Results Control group : the obvious effective rate(CR+PR) was 50%.Experimental group:the obvious effective rate was 90.4%.Two cases in control group obtained the chances of radical operation and chemotherapy were performed after operation,the other 22 cases were undergone synthetic treatment.Seven cases in experimental group obtained the chances of radical operation and chemotherapy were performed after operation,14 cases were undergone synthetic treatment.Three cases in control group died and none of in experimental group died in the follow-up of 2~24 months.Conclusion The short-term effect of polyvinyl alcohol embolization is better than those of BAI for interventional therapy of central bronchogenic cancer,but the long-term effect needs a large number of cases to access the real clinical value.
9.Silencing of Multidrug Resistance-Associated Protein(MRP1) Expression by siRNAs

Three pSIREN-siRNA plasmids were constructed using a pSIREN-RetroQ vector to silence the expression of muhidrug resistance-associated protein (MRP1) gene, and subsequently characterized by restriction endonuclease digestion and DNA sequencing. A truncated MRP1 and a full-length MRP1 were cloned into pEGFP-N2 and PeDNA3.1 respectively as pEGFP-MRP1T and pcDNA-MRP1. The plasmid pEGFP-MRP1T was co-transferred with each of the three pSIREN-siRNAs into HEK293 cells for MRP1T-GFP targeted silencing, and pSIREN-siRNA1 was used as the negative control, pSIREN-siRNA2 and pSIREN-siRNA3 appeared to be more effective to silence MRP1T-GFP compared to pSIREN-siRNA1 as shown by fluorescence microscopy. For the silencing of full-length MRP1 expression, HEK293 ceils were co-transferred with pcDNA-MRP1 and either of the three pSIREN-siRNAs, then subjected for Western blot analysis and MTT assays, pSIREN-siRNA2 and pSIREN-siRNA3 were able to inhibit the expression of 190 kD MRP1, but not pSIREN-siRNA1. The MDR of MRP1-transfected HEK293 ceils was abolished with pSIREN-siRNA2 or pSIREN-siRNA3 transfections. RNA secondary structure predictions demonstrated that the mRNA local free energy (△G) of the siRNA1 targeted sequence was lower, as the GC content and Tm value of siRNA1 were higher than those of siRNA2 and siRNA3. These data suggest that the local structure siRNAs and target mRNA may influence the silencing efficiency of MRP1 expression.
10.Risk factors of occurrence of ventricular fibrillation during emergency percutaneous coronary intervention for patients with acute inferior myocardial infarction

Objective To explore the possible causative factors of appearance of ventricular fibrillation (VF) during emergency percutaneous coronary intervention (PCI) in patients with acute inferior myocardial infarction. Methods Five hundred and seventy two patients with acute inferior myocardial infarction who underwent emergency PCI 24 hours after onset from May 2016 to May 2018 in Cangzhou People's Hospital were enrolled, they were divided into a VF group (52 cases) and a non-VF group (NVF, 520 cases) according to whether VF occurred or not during PCI. The differences in clinical data, characteristics of coronary artery disease and coronary artery score (Gensini score) between the two groups were compared; multivariate Logistic regression analysis was used to analyze the risk factors related to the occurrence of VF during emergency PCI; the receiver operating characteristic curve (ROC) was drawn to evaluate the efficacy of each risk factor. Results There were no statistical significant differences in sex, age, past histories of drinking alcohol, smoking, hypertension, diabetes, previous use of β blockers, aspirin, the peak values at admission of systolic blood pressure, heart rate, creatine kinase isoenzyme (CK-MB) and cardiac troponin I (cTnI), etc between the two groups (all P > 0.05). The proportions of patients with grade 0 blood flow in myocardial infarction thrombolysis (TIMI) before recanalization, with high thrombus load, criminal vessels being right coronary artery (RCA) and proximal segment of RCA, Gensini score in VF group were significantly higher than those in NVF group [TIMI 0: 80.8% (42/52) vs. 58.1% (302/520), high thrombus load: 71.2% (37/52) vs. 58.1% (302/520), criminals being RCA: 84.6% (44/52) vs. 73.7% (383/520), the occlusion site of infarction-related artery (IRA) being the proximal segment of RCA: 61.5% (32/52) vs. 41.2% (214/520), Gensini scores: 93.84±16.48 vs. 61.37±20.01, all P < 0.05]. The multiple logistic regression analysis showed that the risk factors for VF occurrence during emergency PCI for patients with acute inferior myocardial infarction included the criminals being RCA [odds ratio (OR) = 1.967, 95% confidence interval (95% CI) = 1.696-3.015, P =0.032], TIMI blood flow grade 0 before re-canalization (OR = 3.032, 95%CI = 1.248-3.675, P = 0.043), the occlusion site of infarction-related artery (IRA) being the proximal segment of RCA (OR = 2.288, 95%CI = 1.458-3.895, P =0.024), Gensini score (OR = 6.558, 95%CI = 2.168-13.359, P = 0.001] and high thrombus load (OR = 1.781, 95%CI =1.016-3.017, P = 0.033); they all were risk factors of occurrence of ventricular fibrillation during emergency PCI in patients with acute inferior myocardial infarction (all P < 0.05). ROC curve analysis showed that TIMI blood flow grade 0 before re-canalization, Gensini score and higher thrombus load had certain predictive value for VF occurrence during emergency PCI for acute inferior wall myocardial infarction; the area under ROC curve (AUC) was 0.613, 0.869 and 0.605, and 95% CI was 0.540-0.687, 0.787-0.969 and 0.521-0.675, the P value was 0.007, 0.000 and 0.012, respectively, suggesting that Gensini score had moderate predictive value for intra-operative VF, while the predictive values of TIMI blood flow grade 0 before re-canalization and higher thrombus load were relatively low. When the Gensini score had an optimal cutoff value of 96.50, the sensitivity was 85.50% and the specificity was 81.20%. Conclusion The risk factors of VF occurrence in emergency PCI for patients with acute inferior myocardial infarction are criminal vessel RCA, TIMI blood flow grade 0 before re-canalization, IRA occlusion site being proximal segment of RCA, Gensini score and high thrombus load; pre-recanalization TIMI blood flow grade 0, Gensini score and higher thrombus load all have certain predictive value for the occurrence of VF in emergency PCI for acute inferior myocardial infarction.
