1.TRANSMISSION ELECTRON MICROSCOPICAL OBSERVATIONS ON THE EFFECTS OF PYQUITON ON PAGUMOGONIMUS SKRJABINI IN DOGS

Transmission. electron microscopical observations on the effects of pyquiton on po-gumogonimus skrjabini in dogs were carried out. 24 hours after oral administration of single dose of 120mg/kg pyquiton to dogs infected with Pagumogonimus skrjabini, the tegumental cytoplasmic projections swelled, the mitochondria in the ground substance; were concentrated, the secretory bodies decreased, the nucleolus and chromatin in the tegumental cells and the musculature were dissolved and the muscular microfilaments were disordered. Simultaneously, the nucleus and nucleolus of the gut epithelia were swollen, the chromatin concentrated, the secretory body disappeared and the microvilli on the surface of digestive tract adhered together and ruptured. Finally, the mechanism of pyquiton in killing Paragonimus was discussed on the basis of the findings in this study (Plates 1-8).
2.Observation of clinical effects of radiofrequency ablation in the treatment of cervical spondylotic radiculopathy

Objective To compare the clinical effects of conservative therapy and radiofrequency ablation in the treatment of cervical spondylotic radiculopathy,to analyze the reason and discuss the surgical indications.Methods Fifty-five patients with cervical spondylotic radiculopathy treated in Shijingshan Hospital from May 2013 to May 2016 were selected and retrospectively analyzed,including 30 patients received conservative treatment (conservative group),25 patients undergone radiofrequency ablation (operation group).Visual analogue score (VAS) was used to assess the degree of pain relief in the two groups before treatment and 1 week,1 month after treatment.The side effects in the two groups and the postoperative complications of the patients were observed.Results The VAS score of the operation group and the conservative group in 1 weeks after treatment were (3.2±0.5) points,(5.2±0.7)) points,1 months (2.8±0.3) points,(5.8±0.4) points,respectively,the degree of pain were lower than those collected before treatment (8.0±0.9) points,(7.6±0.8) points,separately,the differences were statistically significant (P<0.05),and the degree of pain relief in the operation group was more obvious than that in the conservative group,there was significant difference between the two groups (P<0.01).Two groups of patients were treated with medication without any obvious discomfort.The surgery of the operation group was successful,and no complications such as nerve and spinal cord injury,vascular injury,local hematoma,puncture needle breakage and intervertebral space infection were found.Conclusion Radiofrequency ablation is more effective for the treatment of cervical spondylotic myelopathy in the short term than conservative treatment,and radiofrequency ablation treatment is a recommended choice for patients with no surgical contradiction.
3.Percutaneous kyphoplasty combined with zoledronic acid for the treatment of single segment osteoporotic lumbar vertebralcompression fractures

Objective To evaluate the clinical efficacy of percutaneous kyphoplasty combined with zoledronic acid for the treatment of single segment osteoporotic vertebral compression fractures (OVCF).Methods Clinical datas of 90 cases patients with postmenopausal suffered single segment vertebral compression fracture who were treated in Beijing ShiJingshan Hospital of Capital Medical University from March 2010 to March 2013 were retrospectively analyzed.The patienrs were randomly divided into three groups according to the different treatment methods,20 patients receivedbed rest treatment as conservative group,40 patients were undergone percutaneous kyphoplasty as operation group,30 patients were given percutaneous kyphoplasty combined with zoledronic acid as combination group.Visual analogue score (VAS) were recorded before treatment and 1 week,6 months after treatment,and lumbar vertebrae bone mineral density (BMD) was measured before treatment and 2 years after treatment.The subsequent fracture of the vertebrae was analyzed after 2 years of treatment.Results There were 4 patients were not followed up.Three patients in conservative group suffered complications,but no complications were observed in operation group and combination group.The VAS score of the combination group and operation group at 1 week after treatment was significantly different compared with conservative group((3.0± 1.4) points vs.(3.2± 1.2) points vs.(8.0± 1.1) points,P<0.05).At the 6 months after treatment,there were no significant differences among the 3 groups on VAS score (P >0.05).Lumbar vertebrae BMD of combination group increased at 2 years after treatment,compared with conservative group and operation group the difference were significantly((-1.8±0.6) vs.(-3.1±0.5) vs.(-2.7±0.3),P <0.05).The incidence of the subsequent fracture of the vertebrae reduced of combination group,compared with conservative group and operation group the difference were significantly (3.3% (1/30) vs.27.8% (5/18)vs.21.1% (8/38),P< 0.05).Conclusion Percutaneous kyphoplasty combined with zoledronic acid for the treatment of OVCF-patients with postmenopausal and single segment fracture is not only safe,effective,but also can obviously relive pain and reduce the risk of subsequent fracture of the vertebrae.
4.Advances on the combined immunotherapy for advanced unresectable hepatocellular carcinoma

Recently, the clinical trials of combined immunotherapy for advanced hepatocellular were completed successfully (Imbrave150, Keynote524), which have significantly increased the objective response rate and prolonged median overall survival time. The success of combined immunotherapy provides great potential and reality for subsequent surgical intervention of advanced hepatocellular carcinoma. Nowadays, the hot topics of combined immunotherapy include the choices of combined immunotherapy, the perioperative period determination, tumor immune microenvironment, and effective biomarkers of immunotherapy selection, and so on. This paper intends to summarize the above-related issues.
5.Diagnosis and treatment of congenital giant megaureter(report of 6 cases)

Objective To present and evaluate the diagnosis and treatment of congenital giant megaureter. Methods From 1992 to 1999, 6 cases of congenital giant megaureter were diagnosis and treated, the pathogenesis, diagnosis and treatment of congenital giant megaureter were discussed. Results Resection of the kidney and ureter were carried out in all the 6 with satisfaction. Conclusions The diagnosis may be confirmed on B ultrasonography, CT, or intravenous urography. Nephroureterectomy is the principal method of treatment for congenital giant megaureter.
6.Effects of urodynamic studies on diagnosis and treatment of female stress urinary incontinence(report of 20 cases)

Objective To evaluate the effects of urodynamic studies on diagnosis and treatment of female stress urinary incontinence (SUI). Methods Urodynamic studies including the determination of stress leakage spot pressure were performed in 20 cases of SUI.Of them 6 cases (30%) had detrusor instability.Based on the types of SUI pathogenesis,7 cases were of Type Ⅲ,10 cases of mixed type (Type Ⅲ/Type Ⅱ) and 3 cases of Type Ⅱ.Of the 20 cases,16 underwent operation,including modified Gittes suspension in 12,sling with polyester pieces in 3 and MMK plus posterior vaginal wall repair in 1. Results The 16 operatied patients were followed up for 8~40 months(mean,31 months).The patients undergoing MMK or Sling operation were cured.But 3 patients (1 of Type Ⅲ and 2 of Type Ⅲ/Ⅱ) undergoing modified Gittes suspension had relapse (recurrence rate,25%). Conclusions Urodynamic studies are important to the diagnosis and differential diagnosis of SUI,and are indicative for choice of operation approaches.
7.Research progress on associating liver partition and portal vein ligation for staged hepatectomy

Associating liver partition and portal vein ligation for staged hepatectomy (ALPPS) is a new surgical management which brings hopes of cure to patients with primary or metastatic liver tumor who do not have sufficient future liver remnant (FLR).This review summarizes the current research on the development,indication,surgical procedures and safety of ALPPS.It also discusses the major concerns and unanswered doubts of ALPPS,such as the comparison with selective portal vein embolization (PVE) and the long-term oncological progression.
8.Investigation of Cardiac Failure Therapy by Transplanting Skeletal Satellite Cells

Objective Satellite cells(SCs) transplantation was tried to improve cardiac function in this experiment.Methods A myocaridium infarction was created in 23 male Suzhong pigs by ligating the diagonal ramus,and the alive pigs were divided into the experimental group and the control group.SCs were extracted,purified,cultured by modified Dorfman method,and were transplanted into myocardium by intramyocardial injection.By pathomorphological research,we observed the therapeutic outcomes of heart failure.Results ⑴10 pigs were alive(5 in the control group,and the other 5 in the experimental group).The successful rate of making mould was 43 5%.⑵After cultured in vitro for 3 days,SCs were spindle-shaped,and sticked to walls.SCs growth were stasis and each other fusing into form myotube when we delayed to divede Petri dishs or reduced the concentration of Fetus Bovine Serum(FBS) in the culture medium.⑶In the experimental group,neonate skeletal muscle cells were observed in the infarction regions,which were multi-nuclei near edges.These cells aligned in an accordance direction.Intercalated disks were not found between them.The capillary density in experimental group was much higher than the control group.Conclusions Autograft of SCS can be alive in acceptor,and improve the heart function by altering cardiac compliance and configuration,or by stimulating the blood capillary proliferation.SCs don't form intercalated disks with remnant myocardial cells,so synchro-contraction will not occur between them,but malign arrhythmia are not found.

10.Research of children's portal hypertension

Children's portal hypertension is a hemodynamic abnormalities syndrome,it refers to children with portal vein pressure exceeds 5 mmHg,or portal vein,hepatic venous pressure gradient exceeds 10 mmHg.Etiology of portal hypertension of children:portal vein obstruction,congenital hepatic fibrosis,biliary atresia.Current treatment methods are:drug treatment,varicose vein ligation or sclerosis,surgical treatment.Although the incidence of children's portal hypertension is relatively low,it can cause various serious complications,such as ascites,gastric variceal bleeding,hepatic encephalopathy and so on.To increase awareness,improve the cure rate and reduce complications,the artical will review the children's portal hypertension etiology,pathogenesis,diagnosis,and treatment progress.
