1.Value of universal primer PCR for diagnosing bacterial and fungal infec-tion of central nervous system

Objective To understand pathogen spectrum of bacterial and fungal infection of central nervous system (CNS),and evaluate the etiological diagnostic value of universal primer polymerase chain reaction (PCR).Methods Data about patients with suspected or confirmed bacterial and fungal infection of CNS from January 2009 to March 2015 were collected,species of pathogens from cerebrospinal fluid (CSF)were analyzed,DNA from patients’CSF were performed PCR amplification and sequencing with universal primers of bacterial 16S rRNA and fungal 28S rRNA, PCR detection results were compared with CSF culture during the same period.Results A total of 400 patients were with confirmed or suspected bacterial or fungal infection of CNS,132 of whom were with positive CSF culture.150 pathogenic isolates were detected,including 48 isolates of gram-positive bacteria,90 gram-negative bacteria,and 12 fungi;the top three isolated bacteria were Acinetobacter baumannii (n =32 ),coagulase negative staphylococcus (n=16)and Klebsiella pneumoniae (n=13);the most common fungus was Cryptococcus neoformans (n=8).CSF from 88 infected patients and 20 non-infected patients were selected for PCR amplification,the sensitive of PCR am-plification assay was higher than the culture method (35.23% [31/88]vs 28.41 %[25/88],χ2 =4.17,P <0.05).
2.Effects of parenteral nutrition in combination with chemotherapy on the nutritional status and immunity function in patients with malignant tumors of digestive tract

Objective To study the effect of parenteral nutrition combined with chemotherapy on nutritional status and immunity function of patients with gastrointestinal cancer.Methods 100 gastrointestinal cancer patients were treated as research subjects,and they were randomly divided into the treatment group and the control group.The control group was not given special treatment during chemotherapy,while the observation group patients were given nutritional support according to the gastrointestinal function after admission.The nutrition indicators of ALB,TF,PA, immune function parameters IgA,IgG,IgM,T lymphocyte subsets and T cell immune function indicators(CD +4 ,CD +8 and CD +4 /CD +8 )were counted after the four courses.Results There was no significant difference between the preop-erative and after fourth courses of treatment(t =0.093,P >0.05)between the two groups in nutrition.Within group comparisons,data of Tf and PA levels in the observation group were significant different compared with the control group after fourth courses of treatment (t =3.302,P <0.05).After the first 4 postoperative course,IgM index was statistically significant(t =4.135,P <0.05)different between the two groups,three indicators within the observation group were higher than the preoperative level indicators,the difference was statistically significant(t =3.426,P <0.05).After the first 4 postoperative course,the CD +4 level was significantly higher in the observation group,the difference was statistically significant(t =0.087,P <0.05).Comparison between the two groups after the first four courses,the CD +4 /CD +8 level was higher than that of preoperative patients with a significant difference(t =3.359,P <0.05).Intra -group comparison of the control group,four courses of CD +8 was significantly lower than that of the pre-operative patients(t =3.265,P <0.05).Conclusion Parenteral nutrition combined with chemotherapy can improve the nutritional status of patients with malignant tumors of alimentary tract and improve the body's immune function, give parenteral nutrition support treatment and provide a theoretical basis for the digestive tract malignant tumor during chemotherapy.It is worthy of clinical popularization.
3.Effects of parenteral nutrition combined with chemotherapy on immune function in patients with digestive tract malignant tumor

Objective To study the effect of parenteral nutrition combined with chemotherapy on immune function in patients with digestive tract malignant tumor. Methods One hundred patients with digestive tract malignant tumor were selected, and they were divided into the observation group and the control group by random digits table method with 50 cases each. The patients in control group during chemotherapy did not receive special treatment, and the patients in observation group were given parenteral nutrition support according to the gastrointestinal function during chemotherapy. The nutrition index, immune function index and T lymphocyte subsets after treatment were observed. Results The transferrin, prealbumin, IgA, CD4+and CD4+/CD8+levels after treatment in observation group were significantly higher than those in control group: (2.41±0.53) g/L vs. (2.15±0.57) g/L, (0.28±0.07) g/L vs. (0.20±0.08) g/L, (3.25±1.19) g/L vs. (2.85±1.01) g/L, 0.403±0.065 vs. 0.323±0.083 and 2.15±1.02 vs. 1.35±0.73, while the IgM in observation group was significantly lower than that in control group: (1.02 ± 0.35) g/L vs. (1.48±0.78) g/L, and there were statistical differences ( P<0.05). There were no statistical differences in body weight, albumin, IgG, and CD8+between 2 groups (P>0.05). Conclusion Parenteral nutrition combined with chemotherapy can improve the immune function in patients with digestive tract malignant tumor, and provide a theoretical basis for the treatment of parenteral nutrition combination with chemotherapy.
4.Antitumor effects of the polysaccharides from Gloiopeltis furcata on H_(22) tumor bearing mice

Objective To study the antitumor effect and the mechanism of the polysaccharide from Gloipeltis furcata on H_(22),tumor bearing mice.Methods The polysaccharides from Gloipeltis furcata were administered by oral route in mice bearing H_(22) tumor.The treatments lasted for 7 days.The inhibition rate against H_(22) tumor and the indices of thymus, spleen and liver were measured.In addition,the levels of antibodies against H_(22) tumor and GPT in serum,and GPT,GOT,MDA and SOD in liver were measured using commercially available kits.Results The administration of the polysaccharides from Gloipeltis furcata(200 mg?kg~(-1)?d~(-1) and 400 mg?kg~(-1)?d~(-1) ) for 7 days,the inhibition rates of H_(22) tumor were 35.64%(P

[Summary] The cytomorphological features of 17 cases of confirmed MTC were analyzed. cytomorphological features were moderate to marked cellularity, isolated cells alternate with clusters in variable proportions, plasmacytoid, polygonal, round, and(or) spindle-shaped cells, mild to moderate pleomorphism, finely or salt and pepper-like chromatin, frequent binucleation/ multinuleation, rare bizarre giant cells may be seen, nuclear pseudoinclusions are occasionally noted, unremarkable nucleoli, cytoplasm is granular and variable. Amyloid is often present. MTC could be adequately diagnosed by FNA, According to the clinical characteristics and cellblock immunochemical staining can be helpful in diagnosis of MTC.
6.Expressions of Slug, BRAF V600E and STIP1 proteins and their correlation with capsular invasion and regional lymph node metastasis in papillary thyroid carcinoma

Objective To observe the expressions of Slug, BRAF V600E and STIP1 proteins in papillary thyroid carcinoma (PTC), and to explore their correlation with capsular invasion and regional lymph node metastasis. Methods Slug, BRAF V600E and STIP1 expressions in 107 cases of differentiated PTC were examined by immunohistochemical staining. The expressions of three proteins and clinicopathological data were statistically analyzed. Results Positive rates of Slug, BRAF V600E and STIP1 in PTC were 65.4 % (70/107), 61.7 % (66/107) and 66.4 % (71/107), respectively, and overexpression of Slug, BRAF V600E and STIP1 was significantly associated with capsular invasion and regional lymph node metastasis in PTC (P< 0.05). There are a significant correlation between expression of Slug and BRAF V600E in PTC (r= 0.235, P< 0.05). Conclusion Overexpression of Slug, BRAF V600E and STIP1 proteins is associated with capsular invasion and regional lymph node metastasis in PTC, which maybe useful for predicting regional lymph node metastasis and prognostic evaluation.
7.The Expression of GAP-43 in Cochlear Nucleus and Inferior Colliculus after Unilateral Cochlear Damage in Rats

Objective To study the changes of the expression of growth associated protein -43(GAP 43) in cochlear nucleus and the inferior colliculus (IC) in rats after unilateral cochlear damage at different time points after surgery ,and explore the remodeling and repair process of synapse in brainstem auditory center of rats after auditory deprivation .Methods Thirty rats were randomly divided into six groups :the 3 ,7 ,15 ,30 ,60 days group (with u-nilateral cochlea damage) and the control group (without cochlea damage) with 5 rats in each group .The immuno-histochemical SP method was used to detect the expression of GAP -43 in cochlear nucleus and inferior colliculus at different time points after cochlea damaged in the control group .Results After the unilateral cochlear ear was dam-aged at 3 ,7 ,15 days ,GAP -43 in operation side cochlear nucleus were expressed and the expression level signifi-cantly increased compared with normal control group .After 30 days postoperatively ,GAP - 43 expression level decreased ,but it was still higher than normal control group .After 60 days postoperatively ,GAP - 43 expression level was slightly higher than that of normal control group .After 3 days postoperatively ,GAP - 43 expression level was higher than normal control group .After 7 ,15 days postoperatively ,expression levels were increased .After 30 , 60 days postoperatively ,the expressions gradually decreased ,but still higher than the normal control group .Conclusion After unilateral cochlea damaged ,GAP - 43 was expressed an obvious dynamic change process in cochlear nucleus and infe-rior colliculus .It may reflect the axonal regeneration and synaptic plasticity after hearing loss in brainstem auditory neurons .
8.Cause Analysis of emporal bone Anatomy Factor Underwent Radical Mastoidectomy in 36 Cases

Objective To explore the main anatomical factors for relapse in patients underwent radical mas-toidectomy and discuss the preventive measures .Methods A retrospective study was carried out 36 patients who had underwent radical mastoidectomy and 100 patients (100 ears) who had been cured underwent radical mastoidectomy during July 2011 toSeptember 2013 .The patients underwent axial and coronal temporal Computed Tomography (CT) .The incidence of low -lying middle cranial fossa ,high jugular bulb ,anterior locati(36 ears)on of sigmoid si-nus ,dehiscent tympani tegmen ,dehiscent of the facial nerve canal ,large sinus tympani ,low -lying horizontal facial nerve were recorded .Results The incidence of low -lying middle cranial fossa(36 .1% ) ,anterior location of sig-moid sinus(41 .7% ) ,dehiscent tegmen tympani (30 .6% ) ,dehiscent of the facial nerve canal (41 .7% ) ,large sinus tympani(25 .0% ) ,low -lying horizontal facial nerve(22 .2% ) were higher than those of in the normal group .There was no significant difference in high jugular bulb .Conclusion The main anatomical factors for relapse in patients un-derwent radical mastoidectomy were low -lying middle cranial fossa ,anterior location of sigmoid sinus ,dehiscent tegmen tympani ,dehiscent of the facial nerve canal ,large sinus tympani ,low -lying horizontal facial nerve .
9.Correlation of plasma cytokine profiles with age, smoking and alcohol drinking behaviors

Objective To study the differences in levels of plasma cytokine profiles including interleukin-6 (IL-6),interleukin-10 (IL-10),tumor necrosis factor-alpha (TNF-alpha) and C-reactive protein (CRP) between healthy controls and people with smoking and alcohol drinking behaviors at different ages,and to investigate the correlation of plasma cytokine profiles with age,smoking and alcohol drinking behaviors.Methods The concentrations of IL-6,IL-10,TNF-alpha and CRP in 350 healthy volunteers,60 cigarette smokers and 65 people with smoking and alcohol drinking behaviors were detected by ELISA.All subjects were grouped by age,gender,smoking and alcohol drinking behavior.The differences in the levels of plasma cytokine profiles among groups were analyzed by statistics software SPSS18.0.Results The concentrations of IL-6 and CRP in 25-39years,40-49 years,50-59 years and over 60 years groups were (1.1±0.7) ng/L,(1.5±0.1) ng/L,(1.7±0.2) ng/L,(2.1±0.1) ng/L and (3.1±1.0) mg/L,(4.4±±0.6) mg/L,(6.2±1.7) mg/L,(8.0±0.8) mg/L,respectively.The concentrations of IL-6 and CRP were significantly increased with age (F=1.60,7.50,P=0.012,0.043),while the concentrations of IL-10 and TNF-alpha were not correlated with age (F=4.20,4.80,P=0.178,0.421).There were no significant differences in the concentrations of IL-6,IL-10,TNF alpha and CRP between males and females (P>0.05).The plasma CRP level was higher in smoking group than in control group [(6.8±0.6) mg/L vs.(3.1± 0.4) mg/L] and was highest in smoking-alcohol group [(9.5±0.5) mg/L vs.(6.8±0.6) mg/L,(3.1±0.4) mg/L,F=8.40,P=0.046].Compared with the control group,IL-6 level was increased and IL-10 level was decreased in smoking group and smoking-alcohol group.There were no significant differences in levels of IL-6,IL-10 and TNF-a among the three groups (F=2.90,4.30,4.80,P=0.089,0.672,1.231).Conclusions The plasma levels of IL-6 and CRP are significantly increased with age,but the plasma levels of IL-10 and TNF-a have no correlations with age.The plasma levels of IL-6,IL-10,TNF-a and CRP are not affected by gender.CRP level is significantly influenced by smoking and alcohol drinking behaviors,which should be taken into consideration to establish the reference values.
10.Value of 16S rRNA gene amplification for identification of clinical rare pathogens

Objective To evaluate the value of amplification and sequencing of 16S rRNA gene in the identification of clinical rare pathogenic bacteria,and guide the diagnosis and treatment for related clinical infection.Methods 12 bacterial isolates that were difficult,or unable to be identified with conventional laboratory methods,or with special phenotypes were collected. The 16S rRNA gene was amplified by polymerase chain reaction (PCR),then sequenced for identifying bacterial species through BLAST comparison,clinical characteristics of related infection were ana-lyzed.Results 12 clinical isolates were all positive for PCR amplification (about 1500 bp),species were all identi-fied (similarity≥99% ),the identified strains were Listeriamonocytogenes(n= 2),Brucellamelitensis(n= 2),Fu-sobacteriummortiferum(n= 1),Rothiaaeria(n= 1),Nocardiafarcinica(n= 1),Staphylococcussaccharolyticus (n= 1 ),Rhizobiumradiobacter(n= 1 ),Prevotellabivia(n= 1 ),Ralstoniamannitolilytica(n= 1 ),and Atopobium vaginae(n= 1 ). The sensitivity of 16S rRNA gene amplification was high,and the minimum detection limit of Escherichiacoli ATCC 25922 was 1.5×101 CFU/mL. Clinical data of 12 patients revealed that these strains can cause multi-sites and multi-types of infection,after patients received targeted antimicrobial therapy,11 improved, and 1 died.Conclusion Sequencing for 16S rRNA gene can rapidly and accurately identify rare,anaerobic,and difficult cultured bacteria,provide laboratory evidence for etiological diagnosis and treatment of different types of infection.
