Chinese Journal of Orthopaedics 2021;41(1):58-66
Acetabular fracture is an intra-articular fracture, and its treatment principle is also equivalent to that of intra-articular fracture, namely anatomical reduction of articular surface and firm fixation. Therefore, the choice of surgical approach is of great importance to the surgical exposure, reduction quality and fixation effect of acetabular fracture, which directly affects the surgical curative effect. Proper surgical approach is not only good for clear surgical exposure, easy operation, and ideal reduction quality and fixation effect, but also has great advantages in shortening the operation time, reducing intraoperative bleeding, reducing surgical trauma and avoiding surgical complications. The selection of surgical approach for acetabular fractures is closely related to the fracture injury mechanism, fracture classification, and clinical manifestations, etc.. The surgical efficacy depends on the fracture reduction quality, fixation effect, and surgical trauma degree, etc., which are closely related to the selection of surgical approach. At present, the alternative surgical approaches are generally divided into anterior approach and posterior approach. However, due to the complexity of acetabular fractures, no single surgical approach can fit all acetabular fractures, and each approach has its own indications, advantages and disadvantages.

Objective To explore the biomechanical mechanism of long-term complication of cervical spinal operations by measuring pressure changes of zygapophysial joints using pressure sensitive film (PSF) after cervical spine anterior operations. Methods C4,5 anterior discectomy and C4,5 anterior plate fixation were done on 10 fresh adult cervical spine specimens (C2~T1). PSF was put into zygapophysial joints and pressure changes were measured in positions of extension, left/right lateral bending and axial rotation on a universal testing machine. Results After cervical spine anterior operations, pressure of the adjacent facet joints increased. The differences were significant in extension and axial rotation (P

Objective To evaluate the efficacy of modified arytenoid adduction in the management of patients with unilateral vocal fold paralysis(UVFP).Methods A retrospective review was performed on 22 patients who underwent modified arytenoid adduction for UVFP between February 2001 and December 2007.Pre-,and 3 months postoperative aspiration, perceptual(GRBAS),acoustic data(fundamental frequency,F_0,fundamental frequency perturbation, jitter, amplitude perturbation, shimmer, normalized noise energy,NNE)and aerodynamic(maximal phonatory time,MPT,mean airflow rate,MFR)were analyzed statistically. Results The ratings of postoperative aspiration were significantly decreased than that of the preoperation(P<0.0001).There was a significant decrease in GRBAS scales postoperatively versus preoperatively(P<0.0001).The mean values of voice acoustics parameters (F_0,jitter,shimmer,NNE)were significantly decreased, the maximum phonation time were significantly longer, and the mean airflow rate were significantly decreased after operation than that of the preoperation(P<0.001).Conclusion Modified arytenoid adduction is an effective medialization technique that can restore satisfactory speech and prevent aspiration in patients with UVFP.

Objective To compare the efficacies of total gastrectomy (TG) and proximal gastrectomy (PG) for patients with upper gastric cancer. Methods Databases including Medline,Cochrane Library,Web of Science,China National Knowledge Infrastructure,Wanfang database were searched to retrieve literatures on surgical treatment of upper gastric cancer which were published from January 1980 to October 2011.According to different surgical procedures, all the patients were divided into PG group and TG group.Meta analysis were performed by RevMan 5.1.Categorical variables were presented by odds ratio (OR) and 95% confidence interval (95%CI).Results Thirteen literatures including 2622 patients with upper gastric cancer were retrieved. There were 1464 patients in the TG group and 1158 patients in the PG group. There was no significant difference in the 1-year survival rate between the 2 groups (OR =1.23,P > 0.05).The 3-and 5-year survival rates of patients in the TG group were significantly higher than those of the PG group (OR =1.74,1.45,P < 0.05).There were no significant difference in the 5-year survival rates of patients in TNM Ⅰ,Ⅱ,Ⅳ stages between the 2 groups (OR =0.94,1.31,2.03,P > 0.05),while the 5-year survival rate of patients in TNM Ⅲ stage of TG group was significantly higher than PG group (OR =2.29,P < 0.05) The overall recurrence rate of TG group was slightly lower than that of PG group, with no significant difference OR =0.44,P > 0.05).The local recurrence rate of TG group was significantly lower than that of PG group (OR =0.29,P < 0.05).There was no significant difference in the distal recurrence rate between the 2 groups (OR =0.60,P > 0.05).Conclusions The medium and longterm efficacies of TG are superior than that of PG.The stage of cancer should be taken into account to determine the plan of individual treatment.

Objective To investigate the effect of β-elemene on SGC7901 gastric cancer cell line and the potential proteins involved. Methods Human SGC7901 gastric cancer cells were treated with different concentrations ofβ-elemene.Cell viability was assessed.A proteomic method, isobaric tags for relative and absolute quantitation (iTRAQ),was employed to detect the proteins altered by β-elemene.Protein expression was validated by Western blot. Results β-elemene inhibited the viability of SGC7901 gastric cancer cells in a dose-dependent manner.Altogether,147 upregulated proteins and 86 downregulated proteins were identified in response to β-elemene treatment in SGC7901 gastric cancer cell line.Among them, the expressions of p21-activated protein kinase-interacting protein 1 (PAK1IP1 ),Bcl-2-associated transcription factor 1 (BTF)and topoisomerase 2-alpha (TOPIIα)were validated by Western blot and the trends were consistent with iTRAQ results.Top pathways involved inβ-elemene treatment in SGC7901 gastric cancer cell line included ribosome signaling, peroxisome proliferator-activated receptors (PPARs)signaling pathway, regulation of actin cytoskeleton, phagosome, biosynthesis and metabolism of some amino acids. Conclusion Our results suggest a promising therapeutic role of β-elemene for gastric cancer. The differentially expressed proteins give us better insights into the potential mechanisms involved in gastric cancer treatment using β-elemene.

Objective To investigate the effect of negative-pressure wound therapy (NPWT) on the migration behavior of rat adipose-derived stem cells (ADSCs).Methods Eight (20 ±-2)kg Bama miniature pigs respectively received the sham operation (group A,n=16),continuous negative pressure suction (group B,n =8) and intermittent negative pressure suction (group C,n =8).The pigs were observed for 3 months after 72 h treatment. The effect of NPWT on the migration behavior of ADSCs was observed at the cellular level. In order to avoid the influence of serum-free on cell viability,ADSCs were extracted and treated with continuous negative pressure suction or intermittent negative pressure suction for 8 h.The migration ability of ADSCs was detected by transwell. The proliferation ability of ADSCs was detected by MTT.The expression of hypoxia inducible factor-1α (HIF-1α) was detected by Western blot. Results The wound healing effect of continuous negative pressure suction group and intermittent negative pressure suction group was better than that of the control group, and compared with the control group, the number of migrating ADSCs cells had statistically significant difference (P<0.05,P<0.01),but cell proliferation had no statistically significant difference. The expression of HIF-1α was consistent with the migration results of ADSCs.Conclusion NPWT can promote the migration of rat ADSCs by up-regulating HIF-1α protein expression.
