1.Genetic Reveal Study of NKX2.5,TBX5 and GATA4 Genes in Patients With Tetralogy of Fallot

Objective:To investigate mRNA expression of cardiac related genes of NKX2.5,TBX5 and GATA4 in patients with tetralogy of fallot(TOF). Methods:A total of 10 TOF patients(TOF group)from 4 months to 8 years with the mean age of 3.5 years were recruited in our hospital from June to December 2005.The patients were diagnosed by typical clinical manifestation and cardiac color echocardiogram, the diagnosis was confirmed by cardiac surgery.6 non-congenital heart disease children were selected as Control group, and they were from 4 months to 9 years with the mean age of 3.8 years.The myocardial total RNA was extracted,the related cDNA was obtained by RT-PCR.The product cDNA was amplified with fluorescent quantitative PCR in order to compare the differences of NKX2.5,TBX5,GATA4 and GAPDH mRNA expression between TOF group and Control group. Results:NKX2.5 mRNA expression in TOF group was statistically decreased than that in Control group,while there were no statistical changes found in TBX5 and GATA4 mRNA expression between TOF group and Control group. Conclusion:The mRNA expression of NKX2.5,TBX5 and GATA4 were found in myocardium development.TOF was possibly related to decreased NKX2.5 mRNA expression.
2.Vasovagal syncope and serotonergic system

The role of serotonergic system in vasovagal syncope (VVS) has been more concerned.Serotonin is a kind of neurotransmitter which is widely distributed in central nervous system.There are kinds of serotonergic receptors on postsynaptic membrane,among which 5-HT1A receptors participate in the mechanism of central blood pressure depression and promote the release of hormones such as prolactin and adrenocorticotrophic hormone.Acute administration of selective serotonin reuptake inhibitors (SSRIs) during head-up tilt test (HUT) blocks the reuptake of serotonin in the synapse space and thus increases stimulation of the serotonin receptors,which induces syncope episode.Long-term administration of SSRIs downregulates the postsynaptic serotonergic receptors,thus it weakens the reaction to the rapid change of serotonin concentration, which results in rapid depression of sympathic nerves and prevention of syncope episode.These demonstrate that central serotonergic system may participate in the pathogenesis of VVS and SSRIs are important for the diagnosis and treatment of VVS.
4.The interference of common methods in determination creatinine

Objective To investigate the interference of common methods in determination of creatinine. Methods Standardized with Fuller′s earth, Jaffe′s method, enzymatic method (ultraviolet), enzymatic method (colour), enzymatic method (electrode) were compared and evaluated. Results In eight interference materials, Jaffe′s method was positively affected by pyruvic acid (≥0.186 mmol/L), α-ketoglutaric acid(≥0.0615 g/L), ascorbic acid(≥0.82 mmol/L) and negatively affected by bilirubin(≥165.5 μmol/L). Enzymatic method (ultraviolet) was not interfered. Enzymatic method (color) was positively affected by creatine(≥0.21 mmol/L) and negatively interfered by dopamine(≥0.28 mmol/L). At last, enzymatic method (electrode) was positively interfered by creatine(≥0.62 mmol/L). Conclusion Enzymatic method(ultraviolet) is the best in the above methods.
5.A study on thyroid injury in rats with sepsis

Objective To explore the effects of sepsis on the ultrastructure and function of thyroid in rats.Methods This study was performed in the Animal Department of Fujian Medical University,the Key Laboratory of Fujian Provincial Cardiovascular Institute and the Electron.Microscopic Department of Fujian Medical University.Thirty-two pathogen-free male Sprague-Dawley rats weighing 180-220 g selected,and were randomly (random number) divided into control group (C group,n =8),12 hour group (n =8),24 hour group (n =8) and 36 hour group (n =8).The 12 h group,24 h group and 36 h group were established to be sepsis groups.The sepsis model was made by cecal ligation and puncture (CLP).The samples of blood and thyroid tissue of rats in 12 h group were taken 12 hours after modeling,and then the same procedure was done in the rats of 24 h group and 36 h group separately 24 hours and 36 hours after modeling.Blood serum was obtained by centrifugation to detect the thyroid function using measurement of different of kinds of hormones (T3,T4,fT3,fT4,TSH).Thyroid tissue was available to determine the expression of caspase-3,cell apoptosis and change of ultra-microstructure.Raw data were analyzed using SPSS19.0 software.Caspase-3 grey value was determined by Image J software.Thyroid hormone levels were presented as mean ± standard deviation.The differences in levels of different kinds of thyroid hormone among all the groups were determined by one-way ANOVA.TUNEL positive staining and caspase-3 expression were presented as median (P25,P75).Results Compared to the control group,the T4 decreased significantly (P<0.05) in the 12 h group,the 24 h group and the 36 h group,both the tT3 and the fT4 decreased significantly in the 24 h group and the 36 h group (P <0.05),and the T3 decreased significantly in the 36 h group (P < 0.05).The expression of caspase-3 in the 24 h group was higher than that in the C group (P < 0.05).There was significant difference in rate of celt apoptosis in thyroid tissue among those four groups,and both the 24 h group and the 36 h group had higher rate of cell apoptosis than the C group significantly (P <0.05).In those sepsis groups,a series of pathological ultrastructural changes were found in thyroid follicular epithelial cells as following:the nucleus became smaller and more distinctly stained,and quite a few vacuoles were visualized in cell.Furthermore,the cell membrane was broken,and nuclei showed pyknosis.Conionclus In sepsis rats,the significant changes of different kinds of thyroid hormone in serum are similar to those found in the thyroidal illness syndrome,the expression of caspase-3 and cell apoptosis in thyroid tissue increase,and pathological change of thyroid follicular epithelial cells can be observed under electron microscopy.
6.Association between genetic polymorphism of CYP3A5 and blood concentration/dose ratio of cyclosporine combined with calcium channel blockers in renal transplant patients

AIM:To study the relationship between the genetic polymorphism of CYP3A5 gene and the blood concentration/dose(C/D) ratio of cyclosporine combined with calcium channel blockers(CCBs) in renal transplant patients.METHODS:Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) analysis was applied to determine the genotype of CYP3A5 in 69 renal transplant patients.The blood concentration/dose(C/D) ratio of cyclosporine was compared in patients coadministrated of CCBs with different genetic polymorphism of CYP3A5.RESULTS:The cases of genotype of CYP3A51/1,CYP3A51/3 and CYP3A53/3 were 8,23 and 38,respectively.The mutation rate of CYP3A53 in renal transplant patients was 71.74%.The CsA C/D ratio of patients of CYP3A51/1 and CYP3A51/3 were 1.73 and 2.43 times higher than that in patients of CYP3A53/3.The CsA C/D ratio of patients(CYP3A51/1,CYP3A51/3) combined with CCBs had been obviously elevated(19.90?7.66 vs 36.01?8.18,34.33?7.87 vs 46.16?9.63 ng?mL-1 per mg?kg-1;P
7.Effect of icariin on proliferation and phenotype of osteoblasts interfered with titanium particles

Objective To study the effect of icariin on proliferation and phenotype of osteoblasts interfered with titanium particles. Methods Calvarial osteoblasts of newborn rats were cultured in vitro and cell identification was performed by alkaline phosphatase staining. Effect of titanium particles with different concentrations on osteoblasts proliferation was detected by CCK-8 method. Meanwhile , medium lethal concentration of titanium particles was screened out. Under this concentration , icariin with concentrations of 10-5, 10-6, 10-7, 10-8, 10-9 and 10-10 mol/L was added respectively for interference. The effect of icariin on proliferation of osteoblasts interfered with titanium particles was observed so as to obtain optimum drug concentration. Icariin with optimum concentraion was added into osteoblasts interfered with titanium particles. ALP activity of each group was detected by ELISA and calcified nodules were stained by alizarin S red to observe the effect of icariin on phenotype of osteoblasts. Results Titanium particles with different concentrations can inhibit proliferation of osteoblasts (P < 0.05), 0.5 mg/mL is its medium lethal concentration. Icariin can promote proliferation of osteoblasts interfered with particles (P < 0.05), the optimum drug concentration is 10-9 mol/L. With this concentration, icariin can significantly improve ALP activities of osteoblasts (P<0.05) and calcified nodules formation. Conclusion Icariin can promote proliferation and phenotypic expression of osteoblasts interfered with titanium particles. It may probably become an effective medicine for preventing aseptic joint prosthesis loosening.
8.Efficacy of atorvastatin combined with neuroendocrine antagonist in chronic heart failure

Objective To observe the clinical efficacy of atorvastatin combined with neuroendocrine antagonist in the treatment of chronic heart failure.Methods 70 patients of Wenzhou Medical College Theorem Clinical College of Cardiology Department from March 2014 to March 2015 were randomly divided into observation group and control group of 35 cases in each group.The control group were given the neuroendocrine antagonist, observation group were given atorvastatin combined with neuroendocrine antagonist treatment. The changes of related indicators were recorded. Results The aminoterminal pro-brain natriuretic peptide (NT-ProBNP) and ejection fraction in observation group were [(1264.7 ±174.6)pg/mL, (0.58 ±0.07)], which were higher than those in control group [(2106.5 ±164.8)pg/mL,(0.49 ±0.08)](P<0.05).The total effective rate in observation group was 91.43%, which was higher than 71.43%in control group(P<0.05).Conclusion Atorvastatin joint neuroendocrine antagonist in treatment of chronic heart failure has clinical exact effect with no significant side effects.
9.Hypertension-induced target organ damage and early identification in children

Hypertension is a global problem,affecting both developed and developing nations. The mor-bidity of hypertension in children and adolescents has increased year by year in recent years. Hypertension in children can cause target organ damages(mainly renal,cardiovascular,central neurological,or optic fundal). In clinical practice,we need to find simple and easy methods for early recognition of target organ damages,thus se-rious consequences in adult hypertension may be prevented.
10.Interspinous process device in basic study and clinical application

Interspinous process device is a kind of non-fusion implant for posterior spinal surgery.It is classified by static system and dynamic system according to its characteristics.With rapid development of internal fixator manufacture,materials for interspinous process device become increasing,such as bone allograft,titanium,poly(ether-ether-ketone) and lactoprene composite.A large number of experimental and clinical researches reveal the advantages of this technology in degenerative spinal stenosis,intervertebral original lower back pain,facet syndrome,protrusion of intervertebral disk,and lumbar instability.However,there are still many problems such as the influence on the lumbar posterior column,the mechanism to lumbar disc and the effect on the stability of spine etc.In addition,the clinical indications need to validate by long-term follow up.
