Objective To explore and design quality assessment system of optimal medical service based on the BYOD and the SERVQUAL theory.Methods All items of medical service were quantified based on the theory of ERVQUAL to build a service bridge between patients and the medical team and at the same time change the traditional manual approach into a network assessment mode supported by information device.Results With the aid of BYOD technology and theory of SERVQUAL, a set of accurate service quality assessments system were built, and medical personnel could communicate with the patients timely and effectively through the system;to make comprehen-sive analysis of the hospital and to improve the quality of medical services continuously.Conclusion BYOD infor-mation technology combined with the theory of SERVQUAL can help enhance the quality of medical service and patients'satisfaction, improve staff quality and update concept and mode of service, optimize the hospital management and ultimately enhance the level of information management of medical service.
2.Adverse drug reaction induced by Ginkgo Leaf Extract and Dipyridamole Injection in our hospital:retrospective analysis on 24 cases

Obiective To study the characteristics and the correlation factors of ADR induced by Ginkgo Leaf Extract and Dipyridamole Injection on the purpose of providing reference for clinical rational administration.Methods All ADR cases that induced by Ginkgo Leaf Extract and Dipyridamole Injection reported between January 2006 and August 2009 were retrieved and statistically analyzed.Results Of all 24 ADR patients induced by Ginkgo Leaf Extract and Dipyridamole Injection,the female patients were more than the male,and the senile patients had highest incidence 41.67%.Most ADR events happened within 30min(70.83%).ADR mainly manifested as adverse reactions of skin and appendants(33.33%),followed by central nervous system adverse reaction.Conclusion Great importance should be attached to ADR induced by Ginkgo Leaf Extract and Dipyridamole Injection.Medication monitoring should be enhanced to avoid occurrence of ADR clinically.
3.Association between maternal MTHFR C677T polymorphism and neural tube defects in offsprings:a Meta-analysis

Objective To explore the association between maternal methylene tetrahydrofolate reductase (MTHFR) C677T polymorphism and neural tube defects (NTDs). Methods CBM, VIP, CNKI, Wanfang, PubMed and Web of Science databases from set up to March, 2014 were electronically searched to identify case-control studies on the relationship between maternal MTHFR C677T polymorphism and NTDs. The data were quantitatively analyzed by RevMan 5.0 software. Results A total of 25 studies were selected including 2 282 cases and 3 420 controls. Overall, the pooled OR (with 95%CI) under co-dominant model and allele contrast were 2.28(1.60-3.24), 1.25(1.02-1.53) and 1.42(1.21-1.67). Subgroup analysis showed significant association between maternal MTHFR C677T polymorphism and NTDs susceptibility in Asian populations. Conclusion The present meta-analysis suggests that MTHFR C677T polymorphism is significantly associated with maternal risk for NTDs, especially in Asian populations.
4.Aerosol inhalation of Xiyanpingversus ribavirin for treatment of respiratory tract infections in children:a meta-analysis of randomized controlled studies

Objective To evaluate the efficacy of aerosol inhalation ofXiyanpingversus ribavirin for the treatment of respiratory tract infections in children.MethodsPubMed, Embase, Cochrane database of Systematic Reviews, CNKI, cqvip and wanfangdata were searched. The randomized controlled trials(RCTs) of aerosol inhalation ofXiyanpingversus ribavirin injections for the treatment of respiratory tract infections in children were incuded. The quality of trials was evaluated and data analyses were performed with the Cochrane Collaboration’s software RevMan 5.0.Results A total of 7 trials were included, involving 857 patients(437 treated withXiyanping, 420 treated with ribavirin). Both of the clinical cure rate(50.11%vs. 40.71%) and clinical effective rate(91.08%vs. 75.95%) in theXiyanping group were significantly higher than those in the ribavirin group, withOR(95%CI) 1.49(1.13 to 1.97) and 3.54(1.87 to 6.71), respectively.Conclusions Aerosol inhalation ofXiyanping superior to aerosol inhalation of ribavirin for the treatment of respiratory tract infections in children. Owing to the poor quality and the small sample size of included trials, more larger good quality CRTs are required to confirm this conclusion.

6.Application of re-sequencing microarray in the pathogenic detection of the firstly confirmed human case of avian influenza A in Beijing

To screen and identify the possible pathogen of the firstly confirmed human case of avian influenza A in Beijing, the throat swabs and tracheal aspirates of this case were collected and the H5N1 viral nucleotide was tested with real time RT-PCR. The certification of result, screening of other pathogens in respiratory tract and sub-typing of influenza viruses were made by using re-sequencing microarray. It was found that the H5N1 viral nucleic acid was positive in the tracheal aspirate of this case by means of detection with real time RT-PCR and the specific sequence of the non-structural protein (NS) gene of H5N1 virus was obtained through the detection with re-sequencing clip. Through the comparative study with the sequence in Genbank, it was proved to be the H5N1 nucleic acid of avian influenza viruses and excluded the possibility of infections with 30 subtypes of influenza viruses and 33 other respiratory tract pathogens. It is apparent that the pathogen detection with re-sequencing clip shows the high sensitivity and specificity and it plays an important role in the pathogen screening and identification for the firstly confirmed human case of avian influenza A in Beijing.
7.Effects of rosuvastatin on matrix metalloproteinase 2 expression and cell migration of rat vascular smooth muscle cells

Objective To observe the effects of rosuvastatin on the homocysteine (Hcy)-induced expression of matrix metalloproteinase 2 (MMP 2) and cell migration in rat vascular smooth muscle cells (VSMCs), and to explore the possible mechanism of Hcy-induced atherosclerosis and the role of statins in reversing atherosclerosis. Methods In one cell culture plate, the cultured rat VSMCs were incubated with different concentrations of Hcy (0, 50, 100, 500, 1000 μmol/L and 5000 μmol/L) in vitro for 24 h, 48 h and 72 h. And in another cell culture plate, the different concentrations of rosuvastatin (10-9, 10-8, 10-7, 10-6, 10-5 mol/L and 0 mol/L) were added to the cultured rat VSMCs (while the concentration of Hcy was 1000 μmol/L). The MMP 2 expression and enzyme activity were determined by gelatin zymography and Western blotting. The effects of Hcy and rosuvastatin on cell migration and invasiveness of VSMCs were observed. Results Hcy (50-5000 μmol/L) increased the protein expression, and Hcy (50-1000 μmol/L) increased enzyme activity of MMP 2 significantly. But Hcy (5000 μmol/L) inhibited activity of MMP 2 (F=9.31, 6.44 and 5.97, all P<0.05). Rosuvastatin (10-9-10-5 mol/L) inhibited Hcy-induced expression and enzyme activity increasing of MMP 2. The counts of cell migration of VSMCs were 18.32±2.17, 32.68±4.34, 44.75±4.08, 61.39±5.21, 79.74±5.54 and 90.78±5.83, while the concentration of Hcy was 0, 50, 100, 500, 1000 μmol/L and 5000 μmol/L respectively (F=5.31, P<0.05). The counts of cell migration of VSMCs were 79.74±5.54, 62.53±6.41, 48.37±5.66, 31.41±4.79, 19.27±3.62 and 11.17±2.33, while the concentration of rosuvastatin was 10-9, 10-8, 10-7, 10-6 and 10-5 mol/L respectively (F=4.99, P<0.05). Rosuvastatin could decrease the stimulation of Hcy-induced migration of VSMCs. Conclusions Hcy can influence the MMP 2 protein expression/activity in VSMCs, and rosuvastatin can inhibit augmentation of Hcy-induced MMP 2 expression/activity and migration of VSMCs. It may be one of the multiple-effects of rosuvastatin reducing atherosclerosis.
8.Cloning , Expression of Human sTNFR1 Gene and the Biological Activity of Its Recombinant Protein

Human sTNFR1 (soluble tumor necrosis factor receptor 1) gene was amplified by RT-PCR from Hela cells. A recombinant expression vector of sTNFR1-MBP was constructed in pMAL-c2x, and transformed into E. Coli JM109.It was sequenced and confirmed to be identifical to the sTNFR1 gene in data bank. Recombinant protein sTNFR1-MBP was induced by IPTG and purified by Amylose resin Affinity Chromatography. sTNFR1-MBP was binded to sTNFR1's antibody in Western-blotting. From MTT assays, the results showed that sTNFR1-MBP could effectively block the cytotoxicity mediated by TNF?on QSG7701 cells. Annexin V-FITC staining and flowcytometry were used to observe the recombinant protein's anti-apoptosis capacity and the recombinant protein has marked anti-apoptosis effect in vitro.sTNFR1-MBP had good biological activity and it will be employed in further study.
9.Study on the mechanism of amniotic extraction inhibiting haze formation after Epi-LASIK in rabbit cornea

AIM: To investigate the probably mechanism of amniotic extraction inhibiting haze formation after epipolis laser in situ keratomileusis (Epi-LASIK) in rabbit cornea.METHODS: Thirty rabbit corneas were performed with Epi-LASIK.All eyes were randomly divided into three groups: eyes treated with amniotic extraction (AE group),eyes treated with 1g/L dexamethasone (hormone group) and eyes treated with solvent (solvent control group).Haze grade evaluation was performed under the slit lamp after Epi-LASIK for 1,4 and 8wk.The repair of corneal epithelium was observed by using HE staining,and the expression of NF-kB protein P65 was detected by immunohistochemistry.The expression levels of inflammatory cytokines (TNF-α,TGF-β1 and IL-1) and anti-inflammatory cytokines (IL-4,IL-10 and IL-13) were determined by ELISA.RESULTS: HE staining showed that the basal cells of corneal epithelium were more uniform and arranged regularly in AE groups after Epi-LASIK for 1wk as compared with the hormone group and the solvent control group.After 4wk,there were a few of new collagen fibers in the superficial stroma of AE group,forming a small amount of scar.After 8wk,the corneal stroma of AE group showed a small amount of new collagen fibers,arranged regularly,and rarely formed scar.At the early stage (1 and 4wk),AE treatment has an obviously effect on inhibiting the secretion of inflammatory factors (TNF-α,TGF-β1 and IL-1) and anti-inflammatory factors (IL-4,IL-10 and IL-13),and the difference was statistically significant (P<0.01).Moreover,the activation of the NF-kB signaling pathway was significantly inhibited by treatment with AE in the early postoperative period (1 and 4wk).CONCLUSION: Amniotic extraction may reduce the inflammatory response in corneal epithelial cells by inhibiting the NF-kB signaling pathway,thereby inhibiting the formation of collagen and scar and the occurrence of haze.
10.Early postoperative enteral nutrition compared with parenteral nutrition after hepatectomy:A prospective randomized study

Objective To evaluate the impact of early enteral nutrition (EEN) compared with parenteral nutrition (PN) on patients after hepatectomy. Methods Seventy-eight patients undergoing liver resection were randomized prospectively into two groups: EEN group receiving early enteral nutrition (n=35) and PN group receiving parenteral nutrition (n=43). The patients in both groups received isocaloric and isonitrogenous nutritional formulas 24 h after operation and the formulas were stopped on postoperative day 7. The general conditions, liver function tests, clinical complications,and clinical nutritional variables at three time points that included preoperative phase, postoperative day 1 (POD 1) and postoperative day 8 (POD 8) were observed. Results No significant differences were found in length of hospital stay, liver function and clinical nutritional variables between the 2 groups. In the EEN group, the serum prealbumin level almost returned to the preoperative level on POD 8. The nutritional complication rate of the EEN group was increased significantly but it was milder than that of the PN group. The time of gut function recovery in the EEN group was shorter than that of the PN group. The costs of nutritional drugs showed a significant decrease in the EEN group.Conclusion Early enteral nutrition is safe, rational and effective in patients who have undergone hepatectomy. Early enteral nutrition is better than parenteral nutrition in promoting liver function recovery, liver protein synthesis, postoperative recovery of gut function and decreasing costs of nutritional drugs.
