1.The progression of laboratory diagnosis for urinary tract infection

Urinary tract infection (UTI)is a commonly encountered disease during routine clinical care. In the mean time, it is also a commonly encountered hospital acquired infection. UTI possesses various clinical symptoms, and usually the symptoms are atypical. The detection of leukocyturia and bacteriuria is an essential index for screening and confirmatory diagnosis of UTI. This article summarized the pathogenesis of UTI, diagnostic criteria, clinical significance and application evaluation of detection of the WBC and bacteria in urine.
2.The signification and method of HBeAg gray zone setting and its cut-off verification in chemiluminescent assay

Objective To explore the signification and method of Cut-off verification and gray zone setting in chemiluminescent assay.Methods NCCLS EP-12 A2 document defines that C50 is the analyte concentration of cut off value for immunology qualitative test and C5-C95 interval is the range of analyte concentration that yields 5% positive results to 95% positive results for immunology qualitativc test.The C50 and C5-C95 interval of HBeAg in ARCHITECT i2000 were worked out according to the cut off value provided by HBeAg reagent calibrated in ARCHITECT i2000,which were verified to approve the character declaimed by manufactory or not.Gray zone was set and the procedure of cut off verification and gray zone set in chemiluminescent were built; A set of quality control was detected 20 times with two different lot HBeAg reagent kits,S/CO was caculated and compared with t test.Results C50 and C5-C95 interval of reagent (lot 06087L100,96378HN00) were 0.171 PEI U/ml,0.125 PEI U/ml; >0.154 PEI U/ml to 0.188 PEI U/ml,0.119 PEI U/ml to <0.150 PEI U/ml,respectively.S/CO of negative quality control and positive quality control were (0.550 ±0.038),(2.422 ±0.084) and(0.334 ±0.063),(3.587 ±0.321),respectively.They all approved the character (the sensitivity at cut off was less than 0.5 PEI U/ml)declaimed by manufactory,and the results of S/CO between two lot kits were obvious difference (t =9.944,15.499,P <0.01).Conclusion C50 and C5-95 interval can be used to verify cut off value and set gray zone in chemiluminescent assay;They may vary in different lot reagents and they must be verified to approve the character declaimed by manufactory.
3.Evaluation of triphasic contrast enhanced spiral CT for diagnosing hepatocellular adenoma (report of 5 cases)

Objective To evaluate the diagnostic and differential value of triphasic, especially the arterial phase, contrast enhanced CT scans for the hepatocellular adenoma, and to improve CT diagnostic accuracy. Methods Spiral CT scanning of pre and post contrast arterial phase, portal venous phase and delayed phase was performed in 5 patients with hepatocellular adenomas proved surgically and pathologically. The enhancement was administrated with venous injection of 3 ml/sec, totally 100 ml angiografin. Arterial phase of contrast enhanced scanning started 20~30 sec, portal venous phase 60~70 sec and delay scanning 3 min after the injection of contrast medium. The degrees of the enhancement of adnoma and normal tissues of liver in the three contrast enhanced phases were calculated and compared statistically. Results In plain CT, the lesions were iso density in 4 cases and slight hypodensity in 1 case, which was hardly distinguished from the normal tissue. In all 5 cases, the lesions showed marked enhancement in the arterial phase compared with normal tissue of liver (38 HU high), and there were markedly statistical difference ( t =18 94, P 0.05). The mean CT value of hepatocellular adenoma in the arterial phase was much higher than that in portal venous and delay phase, there were statistical differences between them ( F =18.39, P
4.Effect of donor hepatocyte preinfusion on the survival of islet xenografts

The rat islets were isolated by using collagenase digestive method, purified by Fi-coll density gradient centrifugation, and rat hepatocytes isolated simultaneously.Twenty-four cas-es of diabetic models, developed in mice with streptozotocin, were divided into 3 groups randomly.In experimental group, donor rat hepatocytes were infused first, followed by transplantation of cultured islets from same donor via portal vein on the 6th day.In Control group 1 no intervention was performed, and in control group 2, simple infusion of islets was done.The results showed group.Significant difference was noted between the two groups(P<0.05).The decreased blood glucose level were maintained about a week in experimental group.This result reveals that hepa-tocytes can induce immunotolerance and prolong the survival of xenografts to some extent.
5.Experimental study on vascular allografts' cold storage/sequential cryopreservation

Objective To explore the feasibility and secure cold storage time of human arteries during sequentially cold-cryopreservation by observing the cellular metabolic activity and structure after cold storage and cryopreservation. Methods Human iliac and splenic arteries were stored for 72 h, 1 week, 2 weeks, 3 weeks and 4 weeks in UW solution at 4 ℃. After the cold storage procedure, half of the vascular allografts were examined by NBT dye method, electron and light microscope. The other vascular allografts continued to be stored by - 80 ℃ cryopreservation procedure for 4 weeks, and then the vascular allografts were examined by NBT dye method, electron and light microscope. Results There was no statistically significant difference in NBT dyeing time between the groups stored in UW solution within 2 weeks and fresh group at 4 ℃ (P > 0. 05). After - 80 ℃ cryopreservation, there was also no statistically significant difference in NBT dyeing time between the groups stored by UW solution within 1 week and fresh group at 4 ℃ (P>0. 05). Along with the extension of cold storage time, the destruction of ultrastructure was aggravated. When vascular allograft was stored over 2 weeks at 4 ℃, the destruction was more obvious. As the cold storage time prolonged, the ultrastructural destruction of vascular allografts was aggravated, especially those stored over 1 week. Conclusion The optimal time limit for arteries stored at 4 ℃ in UW solution was 2 weeks. Cryopreservation at - 80 ℃ kept the arteries satisfactory metabolic activity and organizational structure. The arteries stored within 1 week at 4 ℃ in UW solution, which restored at - 80 ℃ , could maintain satisfactory metabolic activity and organizational structure.
6.Application on the thickness of nerve fiber layer of the optic disc in pathologic myopia eyes by OCT

AIM: To analyze the application on frequency domain optical coherence tomography ( OCT ) technology of pathologic myopia optic disc neurosensory retinal thickness changes and its relationship with axis oculi, sex and age, and help for the early diagnosis of pathological myopia and primary open angle glaucoma.
METHODS:Collected 96 eyes of normal eyes ( axis oculi 23-24mm) and 153 eyes of pathologic myopia eyes ( axis oculi 25-27mm 80 eyes, >27mm 73 eyes). We measured the thickness of nerve fiber layer of the optic disc by OCT and analyzed their relationship with axis oculi, sex and age with multiple linear regression analysis.
RESULTS: The observation group showed significant smaller average thickness of peripapillary, superior, inferior, nasal than the control group ( P<0. 05 ); the difference in the temporal quadrant between the groups were no statistically significant differences (P>0. 05); The partial correlation coefficient of peripapillary average thickness of nerve fiber layer and axis oculi was -1. 31, gender was 5. 21, age was -0. 12.
CONCLUSION:The thickness of nerve fiber layer of the optic disc in the pathologic myopia eyes are decreased than normal eyes, axis oculi, sex and age are influenced factors. The pathologic myopia patients should use different index combined with optic nerve fiber layer thickness decreased to help for the diagnosis of early primary open angle glaucoma.
7.Investigation of Human Papillomavirus-16 Infections and Its Mixed Infection in Yunnan Region

Objective To investigate the distribution of Human Papillomavirus 16 (HPV16) infection and its mixed infection with other HPV subtypes in the Yunnan region. Methods 16 166 cases of women were tested using flow fluorescence Luminex technology. Results (1) HPV16 infection rate and mixed infection rate was 2.2%and 28.0%, respectively; (2) The most common type of HPV16 mixed infection was HPV52, followed by HPV33. The two kinds of mixed infection accounted for 39.8% of the total infection rate; (3) There was a significant difference between each age group of HPV16 mixed infection (Chi=26.39, <0.01) . Conclusion The HPV16 infection was mainly HPV infection in Yunnan region. HPV16 mixed infection merged mainly with HPV52 and HPV33. HPV16 mixed infection was associated with age.
8.Inhibitory effects of argatroban on instant blood-mediated inflammatory reaction after islet transplantation in vitro

Objective To investigate the inhibitory effects of Argatroban on the instant bloodmediated inflammatory reaction(IBMIR)after islet transplantation.Methods Rat islets were isolated and purified rat islets,and were divided into blank control group,control group and experimental group.In the control group,the blood and the islets were directly mixed,and in the experimental group the Argatroban was added to the mixture based on the control group.while the blank control group was added with blood alone without the islets.Each group was reacted at 37℃for 60min,and then the content was filtered through trap valve of 70 μm.The residual thrombus and tissues were filtered by the trap valve in both the experimental and control groups,detected by the thinprep cytologic test(TCT),and the filtrate received blood routine test,and the function of islet was determined using insulin releasing test.Results The number of blood platelets,white blood cells,mononuclear cells,and lymphocytes percentage in the experimental group were significantly higher than in the control group after 60 min(P<0.05).Under the environment of the high and low concentrations of glucose,the insulin release in experimental group was significantly increased as compared with the control group,and the insulin release index of former was 2.25±0.18,significantly higher than that of the latter 3.36±0.18(P<0.05).The residual thrombus and tissues had few islet cells in the control group,the structure was damaged seriously,the capsule was not intact,and there were a large mumber of micro-thrombosis around the islets formed by red blood cells.But there were a large number of islet cells in the experimental group.the structure was intact in a mass,and no obvious micro-thrombosis around the islets was found.Conclusion Argatroban can effectively inhibit IBMIR in vitro,and alleviate the damage to the islet cells.
9.Abnormal Functional Connectivity of Insular in Primary Insomnia Based on Resting-state fMRI Study

[Objective] To explore the activity aberration of primary insomnia (PI) patients with resting-state fMRI.[Methods]Resting-state fMRI datasets of 60 PI and 60 healthy controls were acquired.We investigated the cortical connectivity patterns of the insula in PI and independent-sample t-test were used to compare the brain activity abnormalities between two groups.[Results] In PI,we found enhanced connectivity between left insular with the left middle cingulate cortex,the Frontal_Sup_Media and right Parietal_Inf,as well as decreased connectivity with the left precentral gyrusand the right fusiformgyrus (P < 0.05).The right insular show increased FC with the right middle cingulate cortex,the right fusiform gyrus and the right middle frontal gyrus,as well as decreased FC with the right precentral gyrus and the right middle temporal gyrus (P < 0.05).[Conclusion] This study provides additional evidence of brain functional integration alterations in PI.Those may help us understand the possible neural mechanisms of PI.
10.The role of tumor suppressor factor CYLD in lung injury associated with acute necrotizing pancreatitis

Objective Acute necrotizing pancreatitis (ANP) may cause lung injury.This study explores two factors that are associated with lung damage from ANP,the expression of tumor suppressor factor CYLD and nuclear factor-kappa B (NF-κB).Methods 72 adult Sprague-Dawley rats were randomly divided into 3 groups:sham operation,ANP,and GdCl3 treatment groups (n=24 for each group).A retrograde injection of 5% sodium taurocholate into the biliopancreeatic duct of rats induced ANP,and the animals were killed 1,3,6,and 12 hours after the ANP induction.AMs were harvested by bronchoalveolar lavage technique,and TNF-a and IL-1β levels in bronchoalveolar lavage fluid (BALF) were evaluated.Lung tissue was checked with histological examinations,and the activity of NF-κB and CYLD in AM were measured by western blot.Results TNF-α and IL-1β secreted by AM were gradually elevated,peaked on the sixth hour,had maximums of (491.3 ±20.3)ng/L and (178.83±11.32)ng/L respectively,and decreased on the twelfth hour.The levels of TNF-α and IL-1β in the ANP group were significantly higher than the sham operation group (P<0.05),and the GdC13 group levels were obviously lower than ANP group.In the sham operation group,the expression of NF-κB was low and CYLD was high.In the ANP group,when compared to the sham operation group,the expression of NF-κB rose after 3 hours and continued to rise with time progression (P<0.05).In contrast,CYLD protein expression in the ANP group dropped after 3 hours and continued to gradually decrease (P<0.05).The CYLD and NF-κB protein expression in GdCl3 groups had similar trends as the ANP group.GdCl3 group CYLD levels began to rise at 6 hours (P<0.05),and NF κB levels began to fall at 1 hour (P<0.05).The expression of NF-κB and CYLD possessed a negative correlation in both the ANP and GdCl3 groups (r =-0.918,r=-0.723,P< 0.01).Conclusions Therefore,in acute lung injury associated with acute pancreatitis,CYLD expression decreased with evident phases,such as a decrease in levels after 3 hours,and NF κB expression increased.Also,GdCl3 may be responsible for upregulation of CYLD expression and downregulation of NF-κB expression,and confirmed that CYLD had a negative effect on NF-κB.Perhaps GdCl3 could be used in the future to ameliorate the lung injury associated with ANP.
