1.Improved implantion of urethral stents in patients with infra-sacral neurogenic voiding dysfunction

ObjectiveTo introduce an improved implantation for urethral stents in patients with infra-sacral neurogenic voiding dysfunction and to evaluate its outcome.MethodsTwelve patients with infrasacral neurogenic voiding dysfunction were treated by implantation of urethral stent according to an improved method.The following objective parameters were used to evaluate the outcome:voiding diary,urine residue volume(URV),renal hydronephrosis and urodynamic tests.ResultsThese parameters were improved significantly after implantation:detrusor leak point pressure,voiding pressure and URV decreased,voiding volume and urine flow increased.The main complications were incontinence,difficult voiding and hematuria,but these were transient and disappeared during a week.ConclusionsThis procedure is effective with fewer complications,protects the function of upper urinary tract and improves the life quality of the patients with infra-sacral neurogenic voiding dysfunction.
3.Utilization Analysis of Antidiabetic Essential Medicines in the Outpatient of Our Hospital from 2012 to 2014

OBJECTIVE:To provide reference for clinical rational use of antidiabetic essential medicines. METHODS:Retro-spective analysis was used to sort and analyze the consumption sum,DDDs and DDC of antidiabetic drugs in the outpatient of our hospital from May 1st,2012 to Apr. 30th,2014. RESULTS:The consumption sum and DDDs of insulin and oral antidiabetic drugs had upward trend. The top 3 consumption sum and DDDs of oral antidiabetic drugs were acarbose,metformin and glimepiri-de,and the highest DDDs of insulin was short-acting and premixed recombinant human insulin,all of them were in the National Essential Medicine List (2012 edition);DDDs of Glibenclamide tablets,Glipizide tablets and Chinese patent medicine were rela-tively small;DDC in long-acting insulin analogues was obviously higher than other kinds of insulin. The sales ratio of essential medicines was increased from 10.48% to 67.78% after National Essential Medicine List (2012 edition)carried out. CONCLU-SIONS:The use of antidiabetic drugs in the outpatient of our hospital is basically rational. Acarbose and metformin have the high-est DDDs,and DDDs of insulin analogues grow rapidly.
4.Chimeric antigen receptor-engineered T cells and its clinical application

Chimeric antigen receptor T-cell (CAR-T) technology is based on genetic modification technology to express T-cell expression tumor specific chimeric antigen receptor bind tumor antigen in an antigen-dependent anti-MCH way. Single chain antibody fragment (scFv) of tumor-associated antigen (TAA) combines with up-stream activating sequence of T-cell in vitro. The forming recombinant plasmid transfects the purified and large scale proliferating T-cell in vitro by transfection technique. This process starts and activates specific killing reaction of tumor. The clinical application of cell therapy shows high efficiency and good anti-tumor effect in treatment of malignant neoplasm, such as leukemia, lymphoma, melanoma, which made CAR-T become the mainstream method of cell therapy.
5.Effect of astragalus polysaccharides and soy isoflavones on glucose metabolism in diabetic rats

Objective To explore the effect of astragalus polysaccharides (APS) and soy isoflavones (SI) on glucose metabolism in diabetic rats. Methods Healthy 60 rats were equally and randomly divided into 5 groups, normal forage group (NF), high blood sugar model group(HBS), APS high dose group intervention group (APS-H), APS low dose group (APS-L), and SI intervention group (SI). Diabetic model was inflicted by intraperitoneal injection of streptozotocin (STZ). Ordinary feed and APS high dose,low dose APS and SI interfere were respectively given for 8 weeks. Blood glucose,glycated hemoglobin,glucose and lipid levels were determined in every group. Malignant mesothelioma is an aggressive malig na ncy that may be caused by environmental carcinogens(asbestos),viruses(SV40),and genetic predisposition.Diagnosis relies on radiographic studies as well as patho logy and mdecular bidogy tests.Most such patients are not suitable for surgical or radiotherapy treatment,and cytotoxic agents are the only options.Historically ,no single or combinations of agents consistently yielded response rates over 20 %.Recently,pemetrexed,a folate-based inhibitor of thymidylate synthase,has been evaluated in phase Ⅰ,ⅡandⅢ clinical trials with promising results.Moreover, low-dose folic acid and vitamin B 12 supplementation significantly reduced the toxicity observed with the use of pemetrexed.In light of these data,it is l ikely that pemetrexed/cisplatin will soon be recommended for malignant pleural m esothelioma as first-line standard chemotherapy.
7.Effect of 17α-hydroxyprogesterone caproate and medroxyprogesterone acetate on inflammation-induced preterm birth of mouse model

Objective To study the effect of 17α-hydroxyprogesterone caproate (17P) and medroxyprogesterone acetate (MPA) on expression of tumor necrosis factor-α (TNF-α)and cyclooxygenase-2 (COX-2) in placenta and uterine myometrium of inflammation-induced preterm birth mouse model to investigate the mechanism of preventing inflammation-induced preterm birth by progestogen.Methods Thirty clean CD-1 mice were divided into 6 groups (5 mice in each group) at 15th day of gestation:control group,lipopolysaccharides (LPS) group,17P 1 mg+LPS group,17P 2 mg+ LPS group,MPA 1 mg+LPS group and dimethyl sulfoxide (DMSO) + LPS group.Progestogens at different dosage were administered 1 h before LPS and 6 h after LPS administration.After these mice were sacrificed,TNF-α and COX-2 levels in the myometrium and placenta were detected by real-time PCR and immunohistochemistry.Data were analyzed by ANOVA,and comparisons between groups were adopted LSD method.Results 1.The comparison of relative expression of COX-2 mRNA and TNF-α mRNA in myometrium and placenta among groups:(1) Expressions of COX-2 mRNA and TNF-α mRNA in myometrium and placenta in the study groups were obviously higher than those of control group (P<0.05).(2) COX-2 mRNA expression in myometrium of 17P 1 mg+LPS group (11.410±3.931),17P 2 mg+LPS group (8.352±3.209) and MPA 1 mg+LPS group (11.920± 2.905) were obviously lower than that of LPS group (20.540± 4.147) and DMSO+ LPS group (18.620 ± 4.156) (P<0.05,respectively) ; although TNF-α mRNAexpression had similar trends among these groups,there were no statistical significance (P>0.05,respectively).(3) COX-2 mRNA expression in placenta of 17P 1 mg+LPS group (10.864±3.777),17P 2 mg+LPS group (7.084±1.667) and MPA 1 mg+LPS group (11.830±3.652) were obviously lower than that of LPS group (18.920±4.106) and DMSO+LPS group (23.820±7.554)(P<0.05,respectively).(4) TNF-α mRNA expression in placenta in 17P 1 mg+LPS group (14.340±1.618),17P 2 mg+ LPS group (11.488 ± 2.910) and MPA 1 mg+ LPS group (13.040 ± 2.982) were obviously lower than that of LPS group (24.240±7.059) and DMSO+LPS group (23.040±5.896) (P<0.05,respectively).2.The comparison of protein expression of COX-2 and TNF-α in placenta among groups:(1) The expression of COX-2 protein in placenta of the study groups was significantly higher than that of control group (P<0.05).(2) There were no differences among the COX-2 protein expression of 17P 1 mg+ LPS group (14 360.92± 1766.01),17P 2 mg+ LPS group (13 340.18±965.35) and MPA 1 mg+LPS group (12 870.81±1521.97)(P>0.05),while the COX-2 protein expressions of them were significantly lower than that of LPS group (16 426.64 ± 1823.87) and DMSO+LPS group (16 761.23±2388.17)(P<0.05,respectively).(3) There were no differences among the TNF-α protein expression of 17P 1 mg+LPS group (22 750.96±4656.68),17P 2 mg+LPS group (22766.24± 3500.34) and MPA 1 mg+LPS group (20770.01±3318.48)(P>0.05),while the TNF-α protein expressions of them were significantly lower than that of LPS group (26204.49±5090.34) and DMSO+LPS group (27346.18±3269.30)(P<0.05,respectively).Conclusions 17P and MPA might prevent the preterm parturition of inflammation-induced mouse model by inhibiting inflammation cytokines and prostaglandins.
8.Laparoscopic peritoneal vaginoplasty versus sigmoid loop vaginoplasty: A comparison study

Objective To compare the therapeutic efficacy between laparoscopic peritoneal vaginoplasty and sigmoid loop vaginoplasty. Methods A total of 29 cases of congenital absence of vagina received either laparoscopic peritoneal vaginoplasty (Peritoneal Group, 14 cases) or sigmoid loop vaginoplasty (Sigmoid Group, 15 cases) from March 1998 to April 2003. Results The operation failed in 1 case in the Peritoneal Group, requiring a conversion to open sigmoid loop vaginoplasty. Both groups had no surgical complications or postoperative pyrexia. The operating time of the Peritoneal Group (118 9?19 0 min) was significantly shorter than that of the Sigmoid Group (202 0?18 6 min) ( t =-11 674, P =0 000). And the intraoperative blood loss of the former (36 2?12 6 ml) was significantly less than that of the latter (105 3?46 3 ml) ( t =-5 205, P =0 000). The conditions of reconstructed vagina of both groups were basically the same, but the vagina length of the Sigmoid Group (10 8?1 1 cm) was significantly greater than that of the Peritoneal Group (9 0?0 8 cm) ( t =-4 882, P =0 000). Among the Sigmoid Group, 3 cases had somewhat more amount of vaginal secretion and 4 cases had abnormal-smelling secretion. Patients in both groups reported the same satisfactory degree of sexual life. Conclusions Laparoscopic peritoneal vaginoplasty and sigmoid loop vaginoplasty present a similar therapeutic efficacy, but the former gives less surgical invasion than the latter.

10.Biological properties of biphasic tricalcium phosphate bioceramics/calcium sulfate bone cement porous three-dimensional scaffolds

BACKGROUND:With the development of tissue engineering, porous bioceramics are more and more used to repair bone defects. Current research focuses on the biological synthesis of this bioceramics and its performance evaluation.
OBJECTIVE:To discuss the preparation of a new kind of bone cement and to determine its physicochemical properties and biocompatibility with osteoblasts.
METHODS:Biphasic tricalcium phosphate powders were prepared using co-precipitation method. The powder was turned into granular stuff by arabic gum. After sintering, porous hydroxyapatite/tricalcium phosphate bioceramics were harvested, and then mixed with alpha-hemihydrate to prepare the bone cement.
RESULTS AND CONCLUSION:X-ray diffraction confirmed that the synthetic substance was a kind of biphasic calcium phosphate ceramic having a porous structure. The bone cement could be in the plastic state within 3 minutes. The curing time was 15 minutes, and the curing temperature was 36.5℃. The maximum compression strength was 5.82 MPa, and the MTT toxicity was level 0. Osteoblasts could grew wel on the material surface.
