1.Current clinical research of intraperitoneal perfusion chemotherapy for gastric cancer

Gastric cancer is the first in morbidity and mortality among cancers in China. The presence of peritoneal dissemination is the most common cause of recurrence after radical surgery, and has become a pivotal to the curative resection. Whether intraperitoneal chemotherapy can prevent peritoneal dissemination,lymph node metastasis, eradicate invisible seeding tumor cells, reduce peritoneal metastasis, improve survival and quality of life is always under investigation. This review focus on the theory, clinical applications and problems of intraperitoneal chemotherapy for gastric cancer.
2.Reversal of multidrug resistance in human liver cancer cells BEL-7402 by erythromycin

AIM To study the reversal effect of erythromycin on multidrug resistance of human liver cancer cells BEL 7402. METHODS Cultivated the parental cells BEL 7402 in the presence of doxorubicin to obtain a multidrug resistant subline designated BEL 7402/ADM. The expression of P glycopro tein in BEL 7402 and BEL 7402/ADM cells was determined by using cell ELASA method. The cytotoxicity of drugs was determined by using MTT assay, and the intracellular concentration of doxorubicin in BEL 7402/ADM was determined using a spectrofluorometer. RESULTS After growing BEL 7402 cells in the presence of ADM for some time, the resulting cells (BEL 7402/ADM) became not only resistant to ADM but cross resistant to VCR and MMC, the P gp on the subline cells' surface was highly expresssed. Erythromycin, which is a lipophilic antibiotic, enhanced the anti tumor functions of ADM, VCR and MMC to BEL 7402/ADM cells and increased the intracellular concentration of ADM with no effect on the expression of the P gp in BEL 7402/ADM cells. CONCLUSUON Erythromycin can significantly reverse the multidrug resistance in BEL 7402/ADM cells, it may do so by saturating the P gp pathway to reduce the efflux of intracellular concentration of the drugs.
3.Utility of full-field digital mammography and localization in breast biopsy in diagnosis of nonpalpable breast cancer

Objective To investigate the utility of full-field digital mammography (FFDM) and localization and biopsy in diagnosis of nonpalpable breast cancer. Methods 42 cases with suspect breast cancer by routine film reading but negative clinical palpation were performed localization and biopsy under X-ray orientation and diagnosed with breast cancer by pathology. The characters of those cases were studied.Results Among 42 cases, 5 cases showed micronodules, 8 cases demonstrated local dense infiltration, 11cases had architecture distortion, 18 cases demonstrated micro-calcifications. The type of pathology comprised of intraductal carcinoma (23 cases), tabular carcinoma in situ (2 cases), early infiltration of ductal carcinoma (12 cases), infiltrating ductal carcinoma (3 cases), papilloma carcinoma (2 cases). Conclusion FFDM can clearly show direct and indirect X-ray signs of breast cancer, combined with localization and biopsy the nonpalpable breast cancer can be easily diagnosed.
4.Progress of clinical research of neoadjuvant chemotherapy for breast cancer

Breast cancer is a systemic disease, only through multidisciplinary treatment including surgery, radiotherapy, chemotherapy, endocrine, can we enhance its effectiveness. In recent years, neoadjuvant chemotherapy for breast cancer has aroused great interest in tumor academia. We hope that this therapy can not only improve breast conservation rate, but also extend the overall survival through a series of clinical studies. This review presents the research progress of neoadjuvant chemotherapy for breast cancer.
5.Comparison of narcotrend index and bispectral index during propofol-remifentanil anesthesia administered by TCI

Objective To compare bispectral index (BIS) with narcotrend index (NI) during propofol-remifentanil anesthesia administered by target-controlled infusion (TCI).Methods Ten ASA Ⅰ or Ⅱ pafients aged 18-56 yr weighing 52-67kg undergoing abdominal surgery lasting>1h were included in this study.BIS and NI were monitored simultaneously.Anesthesia was induced with TCI of propofol with target plasma concentration (Cp) of 3~4μg/ml and remifentanil (Cp 3-4ng/ml).Tracheal intubation was facilitated with cis-atracurium 0.3 mg/kg.The patients were mechanically ventilated.PETCO2 was maintained between 30-35 mm Hg.Anesthesia was maintained with TCI of propofol and remifentanil by the anesthesiologist bhnded to BIS and NI values.according to hemedynamic parameters.BIS and narcotrend values were recorded every minute and compared by another anesthesiologist.All data were compared by Bland-Altman analysis and with Kappa coefficient for agreement.The correlation between BIS and NI was tested by Spearman correlation analysis.The number of error ofjudgement (Type Ⅰ was defined as BIS<40 and NI>62;Type Ⅱ was defined as BIS>60 and NI<20)Was counted.Results The correlation and agreement between BIS and NI during maintenance of propofol-remifentanil anesthesia administered by TCI showed good consistency.Conclusion Both NI and BIS Can help anesthesiologist control the depth of anesthesia during TCI of propofol-remifentanil.
6.Correlation between Cystatin C and target organ damage in elderly hypertension

Objective To investigate correlation between cystatin C (Cys C) and target organ damage (TOD) in elderly hypertension and its clinical significance.Methods One hundred patients with TOD in elderly hypertension were selected as an observation group.One hundred patients without TOD in elderly hypertension were recruited as the controll group.And sixty healthy aged persons were recruited as healthy group.Serum concentration of Cys C in all cases was measured by the particle enhanced nephelometric immunoassay.At the same time,serum creatinine (Scr),urine albumin (mALB) and left ventricle mass index (LVMI) via ultrasonography were also detected.Correlation of Cys C with the above measurements were analyzed.Results The Cys C concentration was significantly higher in observation group [(1.84±0.32) mg/L] than in contrast [(0.92±0.36) mg/L] and healtbygroups [(0.85±0.34) mg/L],F=88.43,P=0.000.However,the difference in level of Cys C was not statistically significantly different between controll group and healthy group (P >0.05).In observation group,serum concentration of Cys C was positively correlated with Scr,mALBand LVMI (r=0.420,0.526,0.470,P=0.021,0.019,0.034) after adjusting age.Conclusions Serum level of Cys C is markedly elevated and is positively correlated with Scr,mALB and LVMI in patients with TOD in elderly hypertension.It can be considered as one of the useful factors for evaluating TOD in elderly hypertension.
7.Prevalence of children's eating problems among 1 to 7 years old and its correlation with their physical development

Objective To investigate the eating problems of outpatient infants,preschool age children(1 to 7 years old) enrolled in the Department of Child Health Care,Nanjing Maternal and Child Health Hospital Affiliated to Nanjing Medical University,and to analyze its correlation with children's physical development,so as to establish strategies for preventing abnormal eating habit in children.Methods A toll of 2458 children met the criteria,and caregivers (mothers) completed the Children's Eating Behavior Questionnaires (CEBQ) in Department of Child Health Care,Nanjing Maternal and Child Health Hospital Affiliated to Nanjing Medical University were selected and the children's sociodemographic data and the morbidity of children eating problems were investigated.The correlation between children's body mass index(BMI) with children's eating problems was determined by using Chi-square test and multiple regression analysis.Results About 66.2% (1627/2458 cases) had normal weight,and 10.8% (257/2458 cases) and 8.5 % (210/2458 cases) were overweight (BMI > P85-P95) or obese (BMI ≥ P95),respectively.The prevalence of eating behavior problems was detected during 25-36 months.For 1-to-7-year-old children,the highest detection rate of eating problems was inattention and eating at non-permanent locations,occupying 64.7% (1590/2458 cases)and 50.5% (1241/2458 cases),respectively;the prevalence rate of preferring to junk food was the lowest,accounting for 19.3% (474/2458 cases).The children's eating problems had a high association with the children's BMI.Among them,children with eating problems,such as difficultly in accepting the varying food stuff[at the age of 12 month,odds ratio(OR)=11.50,95% confidence interval(CI):1.84-72.16] and eating at non-permanent locations(at 25-36month,OR=1.77,95 % CI:1.11-2.83),were prone to be wasting away;children with eating problems,such as preferring to junk food (at 12 month,OR=5.08,95 % CI:1.43-18.00;13-18 month,OR=2.17,95 % CI:1.06-4.44),rarely eating vegetables or fruit (19-24 month,OR=4.06,95%CI:1.46-11.31) and inattention (12 month,OR=3.85,95 % CI:1.52-9.79),were associated with overweight or obesity (all P<0.01).Conclusions There was a high prevalence of eating problems in children between 12-84 month(1-7 years old) in Nanjing.Improper children's eating behaviors can increase the risks of wasting away or children's overweight/obesity.
8.Supercritical extraction technique for essential oils in Keganliyan Oral Liquor

Objective To optimize supercritical extraction technique from four medicinal materials, which are the part components in the recipe of Keganliyan Oral Liquor and are extracted traditionally for essential oils. Methods Extraction ratio and menthol extraction quantity were taken as evaluated indexes. Supercritical extraction technique was researched with orthogonal tests, gas chromatography, and SAS statistic. Results Within the test levels, temperature and time showed evident effect on extraction ratio and menthol extraction quantity, while pressure did not show any evident effect on them、 The preferable technique to extraction ratio is temperature at 55 ℃, time for 120 min, and extracted pressure at 27 MPa; and the preferable technique to menthol extraction quantity is temperature at 45 ℃, time for 120 min, and extracted pressure at 22 MPa. Conclusion The optimized supercritical extraction technique for Keganliyan Oral Liquor is feasible.
9.Effect of transcription factor SOX18 on proliferation, migration and invasion of cervical cancer cell line HeLa

Objective To explore the effect of transcription factor SOX18 on proliferation, migration and invasion of HeLa cells and its related mechanism. Methods HeLa cells were divided into blank control group, negative control group, pCI-SOX18 group. The expression of SOX18 and matrix metalloproteinase 7 (MMP-7) was detected by Western blot. The cell proliferation of different group was measured by MTT analysis. The effect of transcription factor SOX18 on migration and invasion of HeLa cells were measured by Transwell assay. Results HeLa cells were successfully transfected with pCI-SOX18. According to the results of MTT analysis, the cell number of pCI-SOX18 group showed no significant difference compared with blank control group and negative control group (P> 0.05). Compared to blank control group (0.23 ±0.05) and negative control group (0.28±0.07), SOX18 expression in pCI-SOX18 group (1.16±0.13) was significantly higher (F=218.14, P<0.05). Meanwhile, MMP-7 expression in pCI-SOX18 group (0.95±0.09) was increased significantly compared with blank control group (0.71±0.07) and negative control group (0.68±0.06) (F=116.84, P < 0.05). The number of cell migration in pCI-SOX18 group (175.71 ±17.17) was increased significantly compared with that in negative control group (95.19 ±8.18) and that in blank control group (93.34±9.72) (t=75.47, t=77.35, P<0.05). Compared with that in negative control group (98.63±14.62) and that in blank control group (96.57 ±13.95), the number of cell invasion in pCI-SOX18 group (247.12 ±36.82) was significantly higher (t=98.02, t=99.34, P<0.05). Conclusion Overexpression of SOX18 has no effect on the proliferation of HeLa cells, but has a significant role in promoting cell migration and invasion probably by regulating MMP-7 expression positively.
