2.DEEPLY SITUATED MENINGIOMAS
Medical Journal of Chinese People's Liberation Army 1981;0(04):-
The experience in the management of 108 cases of deeply situated meningiomas is presented. Complete removal of the tumor has been possible in 83 cases (76.9%), partial resection in 22 cases (20.3%). The surgical results were excellent in 72.2% and fair in 16.7%. Early diagnosis and an attempt to remove completely of such tumors should always be considered as the treatment of choice. In order to reach this goal, we propose:(a)wide exposure, induced hypotension, hypothermia, interruption of the blood supply at the base of the tumor, and careful hemos-tasis are essential; (b) in addition, microsurgical dissection to avoid injury of the neighbouring important structures as well as vital areas of the brain are mandatory. The removal of an unusual giant mening-ioma arising in the medial aspect of the sphenoid ridge illustrated these points well. The tumor was13?10?8cm in size and 864g in weight. The patient recovered uneventfully and his general condition has remained good for years. Recent CT scan showed no recurrence.
3.Correlation between heart rate recovery at 1 minute after exercise and long-term prognosis in patients with acute myocardial infarction treated by percutaneous coronary intervention
Chinese Journal of Postgraduates of Medicine 2021;44(1):29-34
Objective:To investigate the correlation between heart rate recovery at 1 min after exercise (HRR 1) and long-term prognosis in patients with acute myocardial infarction treated by percutaneous coronary intervention (PCI). Methods:The clinical data of 117 acute myocardial infarction patients who successfully underwent PCI in Qingdao Starr Heart Hospital from May 2014 to May 2017 were retrospectively analyzed. The patients were performed cardiopulmonary exercising test within 30 d after PCI, the peak heart rate (HR peak), resting heart rate, peak oxygen uptake (VO 2peak) were detected, and the HRR 1, heart rate reserve and oxygen pulse were calculated. The correlation between HRR 1 and cardiopulmonary exercise related indexes was analyzed. The incidence of major adverse cardiovascular events (MACE) was recorded 3 years after PCI. The risk factors affecting the prognosis in patients with acute myocardial infarction were analyzed by Cox regression. Results:Among 117 patients, HRR 1 didn′t decrease in 74 cases (non-reduced group), and HRR 1 decreased in 43 cases (reduced group). The proportion of Killip heart function grade Ⅱ to Ⅲ, proportion of triple coronary artery lesions, Gensini score, resting heart rate and oxygen pulse in reduced group were significantly higher than those in non-reduced group: 90.7% (39/43) vs. 70.3% (52/74), 53.5% (23/43) vs. 17.6% (13/74), (81.46 ± 21.55) scores vs. (58.06 ± 17.52) scores, (81.48 ± 11.17) times/min vs. (69.06 ± 10.10) times/min and (13.13 ± 2.93) times/min vs. (9.19 ± 3.06) times/min, the VO 2peak, HR peak, HRR 1 and heart rate reserve were significantly lower than those in non-reduced group: (20.19 ± 9.39) ml/(kg·min) vs. (26.38 ± 9.40) ml/(kg·min), (124.98 ± 22.79) times/min vs. (149.47 ± 22.88) times/min, (16.63 ± 5.92) times/min vs. (27.24 ± 10.21) times/min and (55.51 ± 10.29) times/min vs. (69.07 ± 12.27) times/min, and there were statistical differences ( P<0.01). Correlation analysis results showed that HRR 1 was positively correlated with heart rate reserve, HR peak, VO 2peak ( r = 0.793, 0.835 and 0.800; P<0.01), and negatively correlated with resting heart rate ( r = -0.748, P<0.01); but there was no correlation between HRR 1 and oxygen pulse ( P>0.05). Twenty-one patients did not complete 3 years′ follow-up, with 8 cases in reduced group and 13 cases in non-reduced group. The incidence of MACE in reduced group was significantly higher than that in non-reduced group: 62.9% (22/35) vs. 13.1% (8/61), and there was statistical difference ( χ2 = 12.546, P<0.01). Multivariate Cox regression analysis result showed that the number of coronary artery lesions and HRR 1 were independent risk factors affecting the prognosis in patients with acute myocardial infarction ( HR = 3.124 and 0.248, 95% CI 1.044 to 9.342 and 0.080 to 0.771, P<0.05). Conclusions:The reduction of HRR 1 is related to the exercise cardiopulmonary index in patients with acute myocardial infarction, which can be used as one of the important predictors of long-term prognosis of patients.

BACKGROUND:BioFlex system as a new pedicle screw fixation of dynamic stabilization device has less been reported concerning its biomechanics.
OBJECTIVE:To study the effect of BioFlex system fixation at different decompression ranges on disc stress at adjacent segments.
METHODS:Eight samples of fresh calf spines were used. Under physiologic axial loads (500, 900, 2 300 N), electronic universal testing machine was used to simulate the lumbar spine at three physiological states (standing, sitting and bending, standing on a portable 20 kg weight and bending). Progressive decompression modeling for each specimen and dividing into five groups:(1) complete status group;(2) complete status+BioFlex group;(3) partial laminectomy+BioFlex group;(4) 1/2 medial facetectomy+BioFlex group;(5) total facetectomy+BioFlex group. Strain gauges were used to record the stress of disc annulus. Electronic universal testing machine was used to record load-displacement curve and calculate stiffness.
RESULTS AND CONCLUSION:(1) The stress of the adjacent segment of the intervertebral disc increased with the expansion of the range of decompression. Compared with the complete status, stress obviously increased after BioFlex fixation, showing significant differences (P<0.05). The stress was significantly increased in the 1/2 medial facetectomy+BioFlex group compared with the partial laminectomy+BioFlex group (P<0.05). However, no significant difference was detected between the partial laminectomy+BioFlex group and complete status+BioFlex group, and between total facetectomy+BioFlex group and 1/2 medial facetectomy+BioFlex group (P>0.05). (2) Axial stiffness reduced with the expansion of the range of decompression. Compared with the complete status, axial stiffness noticeably increased after BioFlex fixation. The difference was not significant among four kinds of reconstruction structures. (3) These findings confirmed that after BioFlex fixation, with the expansion of the range of decompression, the stress of adjacent segments of intervertebral disc gradual y increased, but different ranges of decompression cannot affect the stiffness of reconstruction structure.

Chitosan(CS)surface was modified with hydrophobic octyl groups to prepare N-octyl chitosan(nitro-gen-octyl chitosan;OC).Then hydrophilic group carboxyl-polyethylene glycol-amino (PEG);tumor-targeting lig-and D-glucosamine(DG);and membrane-penetrating peptide 9-D-arginine(9R)were linked to OC successively. Then the DG and 9R modified chitosan micelle (DG/9R-PEG-OC)with tumor-targeting and transmembrane effect was prepared.By hydrogen nuclear magnetic resonance spectrometer (1 H NMR)and sodium dodecyl sul-fate polyacryl amide gel electrophoresis(SDS-PAGE);the successful formation of DG/9R-PEG-OC was certified;with particle size of 151.8 nm and Zeta potential of 16.5 mV.The morphology of chitosan micelle observed by transmission electron microscope was homogeneous spherical structure. The drug loading content (DLC)(using fluorescein as a model drug)and encapsulation efficiency (EE)were about 28.2% and 75.0% measured by UV-visible spectrophotometer.Meanwhile;the drug showed a controlled releasing profile out of the micelle.Cellu-lar uptake experiments indicated DG/9 R-PEG-OC micelle had a significant tumor-tageting and transmembrane effects;especially on HepG2 cells;which exbihited high expression of the glucose transporter. Thus DG/9R-PEG-OC micelle could be a promising drug targeted delivery system of hydrophobic antitumor drugs.

A methodology for preparing and certifying the reference material of 3-amino-2-oxazolidinone(AOZ) in eel muscle lyophilisates was presented. Furazolidone was accessed to eel by dipping fish in pond with furazolidone solution at a dosage of ca 0.16 mg/L. With the metabolism of furazolidone in eel, the muscles contain a certain concentration of AOZ as furazolidone metabolite was obtained. Lyophilization of the muscles was performed in one batch and 400 bags of samples were obtained by the procedure of homogenation, cryodesiccation and irradiation. The homogeneity and stability of the sample was examined. The value of the chemical constituent of the sample was certified through the collaborative analysis program participated by 11 laboratories using isotope dilution liquid chromatography-tandem mass spectrometry, and the uncertainty assessment was performed. The reference materials have been approved as certified reference materials by AQSIQ, China (State General Administration of the People′s Republic of China for Quality Supervision and Inspection and Quarantine) in 2009 after one year of trial period. The serial numbers is GBW(E)100180.
