1.Clinical value of the interleukin-19 level in peripheral blood of patients with bronchial asthma

Objective To study the level of interleukin(IL)-19 in peripheral blood of patients with asthma and investigate its importance.Methods Serum samples were obtained from 45 asthmatic patients (acute attack group and catabasis group) and 45 subjects (control group).The level of IL-19 and IgE in peripheral blood was respectively determined with ELJSA.The accurate number of eosinophile granulocyte (EOS) in peripheral blood was counted with cytoanalyze.Results The levels of IL-19,IgE and the quantity of EOS were (327.92 ± 245.79) ng/L,(260.48 ± 108.77) kU/L,(498.29 ± 96.46) x 107L in acute attack group,(165.54 ± 114.76) ng/L,(164.33 ± 59.08) kU/L,(224.67 ± 86.28) x 106/L in catabasis group,and (49.67 ±40.48) ng/L,(68.12 ± 25.70) kU/L,(31.84 ± 28.27) x 106/L in control group.Compared with control group,the levels of IL-19,IgE and the quantity of EOS in acute attack group and catabasis group were obvious higher than those in control group (P < 0.05).The level of IL-19 and the quantity of EOS in acute attack group were higher than those in catabasis group (P <0.05),but there was no significant difference in the level of IgE between acute attack group and catabasis group (P > 0.05).Furthermore,the results of correlation analysis showed that the level of IL-19 positively correlated with the level of IgE and the quantity of EOS respectively (r = 0.826 >0.738,P< 0.05).Conclusions The ascending degree of the IL-19 level is different during various periods of asthma.It may be inferred that IL-19 can play an important role in the pathogenesis of asthma,and observing the change of the IL-19 level dynamically is useful in diagnosing asthma and monitoring patient's condition.
2.Combination of transcatheter arterial chemoembiolization and antitumor drugs in the treatment of Hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is a common world wide disease.Transcatheter arterial chemoembolization(TACE)has been the first choice for the unresectable midadvanced ones.However, TACE has its own limitation such as high recurrence rate and many complications, what's more, recurrence induced by neovascularization which is stimulated by activation of vascular endothelial growth factors promoted by ischemia and hypoxia in local tissues after the embolization always reduce the effect of treatment.And combination of TACE and anti-angiogenesis therapy is capable to make up the shortage of each other and reduce the recurrence rate, which has been a hot issue for non-surgical treatment of HCC.
3.Detection and significance of IL-25 ,eosinophil levels in induced sputum of patients with bronchial asthma

Objective To investigate the roles of interleukin(IL)-25,eosinophils in the occurrence of bronchial asthma.Methods Selected 30 cases of acute episode of asthma(asthma group),and ease a group of 23 cases of asthma treatment(remission group),20 cases of the normal control group.Using the method of gradient ultrasonic atomizing inhalation of Hyperosmotic saline collection of induced sputum specimens,by ELISA method for the determination IL-25 level in induced sputum,count of eosinophils,analysis of correlation between them.Results Group IL-25 in induced sputum of asthma and eosinophil levels[(313.12±75.64)ng/L vs(236.43±57.90)ng/L]were sig-nificandy higher than the control group[(0.386±0.267) × 109/L vs(0.005±0.002) × 109/L],and significantly higher than acute stage in remission stage[(268.63 + 40.19) ng/L,(0.120 + 0.016) × 109/L](P < 0.05).Conclusion IL-25 has an important role in asthma development.It provides a new way of thinking for monitoring and treatment.
4.A biomechanical study on torque resistance of distal radioulnar joint by extra-and intra-articular reconstruction

Objective To compare the torque resistance effect with different types of reconstruction of the distal radioulnar joint (DRUJ) dislocation,in order to explore biomechanical basis for clinical practice.Methods Six adult cadaver forearm were used,including 4 males and 2 females.The average death age was 64.9 years (range,36-78 years).Three were right side,3 were left side.The DRUJ dislocation model was made after the standardized resection of the palmar and dorsal DRUJ ligament.Clinic reconstruction methods of the DRUJ include trans-bone tunnel renconstruction (intra-articular reconstruction) and Looptie up reconstruction (extra-articular reconstruction).The mechanical properties as follows:torque velocity:5°/min,maximum torque angulation:90°,maximum torsion force:1 N· mm.Each limb was detected for the torque mechanic under four conditions:normal,DRUJ dislocation,intra-articular reconstruction and extra-articular reconstruction.Results Each limb was inspected for intact mechanic baseline and without the visual damage to the bone.The average pronation torque resistance on normal condition was (0.44±0.10) N·mm,(0.37±0.09) N·mm on DRUJ dislocation group,(0.45±0.09) N-mm on intra-articular reconstruction group,and (0.42±0.09) N·nun on extra-articular reconstruction group.Pronation torque resistance of DRUJ dislocation group was apparently lower than the normal group (t=2.043,P=-0.047).The pronation torque resistance reconstruction group had no significant difference with normal group.The average supination torque resistance on normal condition was (0.56±0.16) N·mm,while on DRUJ dislocation group was (0.42±0.11) N-mm,on intra-articular reconstruction group was (0.54±0.16) N·mm,on extra-articular reconstruction group was (0.51±0.13) N·mm.There were no apparent differences among four groups.Conclusion Both internal and external reconstruction could recover the rotation stability and normalize the torque resistance,without statistical difference.
5.Role of XBP1 signal transduction pathway in the ischemia/reperfusion injury of graft liver in rats

Objective To explore the regulation mechanism of X box binding protein 1 (XBP1)signal transduction pathway for TNF-α and effective approach in ischemia/reperfusion (I/R) injury of liver transplantation for short hairpin RNA (shRNA) interference used to gene therapy in liver graft.Methods Male Sprague-Dawley rats were divided into three groups: the cold ischemia transfection group (CIT), the in vivo transfection group (IVT) and the control group. Experiments of orthotopic liver transplantation were performed by two cuff method. The rats in CIT were perfused with XBP1-shRNA plasmid (pSIXBP1) during cold ischemia phase, those in IVT received the equivalent volume (2 ml) of pSIIRAK 4 after portal vein inoculation, and those in the control group were not subjected to any treatment. Rats were killed at 60 or 180 min after restoring reperfusion of hepatic portal vein.Histopathological damage degree of graft liver was observed by light microscope. The expression levels of XBP1 gene and protein were detected by RT-PCR and Western blotting. The activities of NF-κB and the serum TNF-α level were detected by ELISA. Results All the indexes including the degree of histopathological damage, the expression levels of XBP1 mRNA and protein and the TNF-α level were significantly decreased in CIT as compared with IVT and control group (P<0. 05). However,there was no significant difference in NF-κB activity among the three groups (P>0. 05). Conclusion The role of XBP1 pathway in TNF-α gene regulation and that of NF-κB pathway in rat liver I/R injury are two relatively independent aspects, and the depression of XBP1 expression with XBP1 shRNA through portal vein perfusion during cold ischemia phase could effectively alleviate graft hepatic I/R
6.Effect of lipoxin A4 on the Na+-K+-ATPase in alveolar type Ⅱ epithelial cells of rats treated with endotoxin

Objective To study the protective role of pre-resolving mediator lipoxin A4(LXA4) in the NA+ -K+-ATPase in alveolar type Ⅱ (AT Ⅱ ) epithelial cells of rats exposed to lipopolysaccharide (LPS). Method The AT Ⅱ cells were isolated and purified, and divided randomly into control group (PBS), vehiculum (alcohol 0.7 μL/mL) group, LPS (1 μg/mL) group, LXA4(1/10 mol/mL) group and LPS (1 μg/mL LPS) + LXA4(1/10 mol/mL) group. After exposure to LPS and/or LXA4 for4 hours, NA+-K+ -ATPase and β1-subunits mRNA in AT Ⅱ epithelial cells were detected by using RT-PCR, and ATP, ADP, AMP, total adenine nucleotides (TAN) and energy charge (EC) were measured by using high performance liquid chromatography (HPLC), and then the activities of Na+-K+-ATPase were calculated accordingly. Results The NA+-K+-ATPase α-subunit and β-subunit mRNA were significantly decreased in LPS group ( P < 0.05 vs. control group). However, the expressions of NA+ -K+-ATPase mRNA were significantly enhanced by application of LXA4 to AT Ⅱ epithelial cells exposed to LPS (P <0.05 vs. LPS group). The activities of NA+ -K+ -ATPase were enhanced in LPS group (P <0.05 vs. control group). Compared with control group and LPS group, the activities of NA+-K+-ATpase in LPS + LXA4 group were significantly increased (P <0.01 vs. control group; P <0.05 vs. LPS group). The EC of AT Ⅱ epithelial cells were higher in LPS group ( P < 0.01 vs. control group). There were no significant differences in EC between control group and LPS + LXA4group(P >0.05). Conclusions The pro-resolving mediator LXA4 can enhance the expressions of NA + -K + -ATPase α-subunit and β-subunit mRNA, and the activities of NA + -K + -ATPase in AT Ⅱ epithelial cells or rats exposed to LPS, and ca also balance the metabolism of AT Ⅱ epithelial cells. These findings suggest that LXA4 plays an important role in lung edema clearance in lung injury induced by endotoxin, and the role is likely associated with the enhancement of the expressions of Na+ -K+ -AT-Pase α-subunit and β-subunit, and the activities of Na+ -K* -ATPase, maintaining the balance of metabolism of AT Ⅱ epithelial cells.
7.Behavior of bone formation around the porous-hollow cylindrical titanium implant composed of bone morphogenetic protein.

OBJECTIVEThe purpose of this study was to observe the behavior of bone formation around the titanium-hollow porous cylinder implant composed of bovine bone morphogentic protein(bBMP).
METHODSPorous-hollow cylinder titanium implant composed of bBMP was implanted into mandibule of dogs. Multiple fluorescent was labeled at different times and then LSCM was used to observe the newly formed bone around the complex implant.
RESULTSThe newly formed bone around the complex implant in experimental group was more obvious than that in other groups.
CONCLUSIONEarlier, longer and more new-bone formation can be induced by porous-hollow cylinder titanium implant composed of bBMP, and LSCM is an effective method to observe new bone formation around implant.

8.Computed tomography images with different resolutions in Sentinel system:a setup error analysis

Objective To study the impact of setup error caused by computed tomography ( CT) images with different resolutions in the Sentinel system on clinical treatment. Methods A phantom was scanned by large?aperture positioning CT with two different resolutions ( CT1:0. 5 mm × 0. 5 mm × 1. 0 mm, FOV 256 mm, Matrix 512, thickness 1 mm;CT3:1. 0 mm×1. 0 mm×3. 0 mm, FOV 500 mm, Matrix 512, thickness 3 mm) . The CT images were transferred to the planning system. The radiation fields were designed and transferred to MOSAIQ and Sentinel systems. Ten fixed setup errors were applied to a six degree of freedom couch. The Sentinel system was used to position the two groups of CT images and generate the setup errors. The comparison of two datasets was made by paired t?test. Cone?beam CT was used for independent verification. Results The setup errors in x?, y?, and z?directions were significantly smaller on CT1 than on CT3(0.19±0. 11 vs. 0.33±0. 16 mm, P=0. 061;0.59±0. 79 vs. 1.07±1. 09 mm, P=0. 008;0.67±0. 75 vs. 1.16±1. 30 mm, P=0. 043). There were no significant differences in rotational errors in x?, y?, or z?directions between the two datasets ( P=0. 494;P=0. 182;P=0. 298) . Conclusions The Sentinel system has a higher setup accuracy in the 0. 5 mm×0. 5 mm×1. 0 mm mode than in the 1. 0 mm×1. 0 mm×3. 0 mm mode. However, the later mode is still an acceptable choice in clinical treatment.
9.Reconstruction of the skin and soft defects of hand and foot by free medial sural artery perforator flap

ObjectiveTo evaluate the curative effect of reconstruction of the hand and foot defects with bones and tendons exposure using free medial sural artery perforator flap(MSAP). MethodsRadiographs of 2 cadavers injected with a modified lead oxide-gelatin mixture were digitally analyzed. Between April 2007 and December 2010, thirty-four patients with soft tissue defects in the distal limb were treated with the free MSAP flap transplantation. The sizes of the defect ranged 6 cm × 4 cm-13 cm × 8 cm, and the flaps ranged 7 cm× 5 cm-14 cm × 9 cm. These clinical cases included 25 hands and 9 feet, all of them with bones and tendons exposure.In these defects,twenty-two were clean,twelve got infections.In our cases, twenty-three flaps were nourished with single perforator vessel and else 11 with two;perforator vessel fifteen flaps were dissected one superficial vein to anastomose with that of the recipient sites in addition to accompanying vein anastomosis;The sensation of 9 flaps recovered the hands were reconstructed with cutaneous nerve anastomosis. ResultsA partition of the calf skin blood vessels,and three-dimensional reconstruction image of the sural artery were obtained.All flaps survived,five of them appeared partially violet and bubbles. Followed up 6-21 months, the cosmetic results were satisfactory and without apparent bulkiness.The flap colors were similar to recipient sites. The flap senses reconstructed with neural anastomosis recover to S2-S3. ConclusionThe new flap is very suitable to repair the soft tissue defect in the distal limbs,because the fairly constant perforator vessel,the reliable blood supply and the cosmetic shape of the MSAP flap are all advantages of it in addition to no damage to low leg chief artery and gastrocnemius.
10.Findings of MRI in Cervical Spinal Cord Injury without Fracture and Dislocation and Correlation between Apparent Diffusion Coefficient of Foci and Severity of Injuries

Objective To study the MRI of cervical spinal cord injury without fracture and dislocation and explore of the apparent diffusion coefficient (ADC) of injured site related to the severity of injury. Methods 46 patients with cervical spinal cord injury without fracture and dislocation and 20 healthy controls were scaned with routine MRI and diffusion weighted imaging. The ADC value of site of injury and grades of Frankel's classification were analyze with the correlation. Results There were 22 cases with spinal cord edema, 8 cases with intramedullary hemorrhage, 14 cases with edema and hemorrhage, 2 cases without abnormal finding. The ADC of controls and patients were (1.05±0.12)×10-3 mm2/s, (1.21±0.23)×10-3 mm2/s (t=0.704, P<0.05). The ADC values positively correlated with the grades of Frankel's classification (r=0.407, P<0.05). Conclusion MRI may help to find the cervical cord injury in those without fracture and dislocation, and the ADC may be resposible to the severity of injury.
