1.Teaching of chronic renal failure in a reasonable application of proprietary Chinese medicines

Chronic kidney disease has become a global public health problem, and a new pandemic, which seriously endanger people's health and life. With the gradual increase in kidney disease treatment of traditional Chinese medicine, western medicine doctors to rational use of Chinese patent medicine is also worthy of attention and discussion. In this research, we discussed the reliability of traditional Chinese medicine in chronic renal failure with practicability, to the analysis of the current status of Chinese patent medicine in General hospital, and summarizes the effect of traditional Chinese medicine in the treatment of renal diseases and analyzed the disadvantages. Finally, from the clinical research, we concluded that the reasonable application of traditional Chinese medicine in chronic renal failure has a long way to go and we need to make unremitting efforts.
2.A study on cryopreservation of human fetal islet

A study on the ideal method for cryopreserving human fetal islet tissues was carried out. The viability and the secretory function of fetal islets cryopreserved with 1.0,1.5,and 2.0 mol/L DMSO and with balancing for 30 and 60 minutes under room temperature and 4℃ as well as the influence of different cooling rates were compared. The results of the study showed that the viability of islets cryopreserved with slow cooling rate at 0.3℃/min, was the best and the insulin content of the media and the amount of insulin secretion of the insulin releasing test were significantly higher than those of the control group.In addition,morphologic examination with HE and immunocytochemical staining showed that the morphology of the islets cryopreserved with 1.5 mol/L DMSO,balancing under 4℃ for 60 minutes and slow cooling rate was much better,i.e.the acinar cells appeared to be degenerated and necrosed,clusters of islets with insulin-positive cells and the ductule-like cells were clearly visible.
3.Analysis of the characteristics of dyslipidaemia in women with polycystic ovary syndrome

A cross-sectional study of 92 women with polycystic ovary syndrome(PCOS) was performed.The patients were composed of 4 groups according to the Rotterdam criteria and were also divided into 2 groups according to body mass index and divided into 2 groups according to insulin-releasing test.Anthropometric measurements were recorded,and hormonal profile was assessed by measuring FSH,LH,prolactin,and testosterone.The metabolic profile was investigated by measuring glucose and insulin level during oral glucose tolerance test (OGTT),then calculating the homeostasis model assessment (HOMA).Serum lipid levels,including total cholesterol,triglycerides,high-density lipoprotein-cholesterol,and low-density lipoprotein-cholesterol were determirned.The result suggests that the classification according to the revised 2003 Rotterdam consensus on diagnosis does not reflect dyslipidemia in PCOS.Dyslipidemia in hyperinsulinemic group and obesity group were more severe than that in non-hyperinsulinemic group and non-obese group ; this finding needs our attention in early treatment and prevention of late complications of PCOS.
4.Nosocomial lower respiratory tract infection caused by extended-spectrum β-lactamases-producing bacteria: drug resistance and risk factors

Objective To investigate the antibiotic resistance of extended-spectrum β-lactamases (ESBLs)-producing bacteria,and the risk factors for patients with hospital-acquired lower respiratory tract infections caused by these bacteria.Methods The clinical data of 80 cases of hospital-acquired lower respiratory tract infections caused by ESBLs + bacteria in the People's Hospital of Qingtian in Zhejiang Province from July 2008 to December 2011 were collected; and 160 cases of nosocomial infection caused by ESBLs- bacteria were randomly selected as controls.Univariate analysis and multivariate logistic regression were used to analyze the risk factors for ESBLs + bacteria infection.The resistance of sixteen antibiotics was detected by K-B method.Results Univariate analysis revealed that the risk factors for nosocomial lower respirator tract infection caused by ESBLs+ bacteria were hospitalization ≥ 10 d,readmission to hospital,oxygen inhalation,invasive operations,using antacid > 7 d,using glucocorticoids > 7 d,using immunosuppressant > 7 d,using third-generation cephalosporins,using fluoroquinolone antibacterials,and the combined use of antibiotics≥3 d.Multivariate logistic regression analysis identified five independent risk faetors:oxygen inhalation (OR =8.613,95% CI:1.870-39.659),invasive operations (OR =5.900,95%CI:1.885-18.467),using third-generation cephalosporins (OR =7.465,95% CI:2.364-23.578),using fluoroquinolone antibacterials (OR =4.481,95% CI:1.197-16.781) and combined use of antibiotics≥3 d (OR =5.346,95% CI:1.717-16.643).ESBLs + Escherichia coli and Klebsiella pneumoniae strains were sensitive to imipenem,meropenem,cefoperazone /sulbactam and piperacillin/tazobactam (with the resistance rates lower than 30.00%),but were highly resistant to most β-lactam,aminoglycoside,fluoroquinolone and sulfonamide antibacterials.Conclusion ESBLs + isolates were highly resistant to most antibacterials,and hospital-acquired lower respiratory tract infections with ESBLs+ bacteria are mainly related with invasive operations and the use of antibiotics.
5.Studies on a kind of new biodegradable material--polycaprolactone and developments in medical area.

Nowadays, there are more and more researches on characters and applications of polycaprolactone (PCL). This paper reviews the researches of polycaprolactone, including its synthesis, physical and chemical properties, biodegradation, absorption properties, and its applications in clinical use and drug delivery systems. PCL can be from open ring polymerizaion. It is a kind of semi-crystalline polymer, and has good flexibility. The degradation reaction of PCL is hydrolysis. PCL is not cumulated in the body, and can be totally excreted. It can be used as suture, material for fixation of bone fracture, vector of drug, etc. PCL is widely used in medical area because of its safety, low toxicity, biodegradability.

6.Clinical study of clearing liver fire, removing dampness, strengthening slpeen and tonifying kidney methods in treating middle-aged and old woman with chronic urinary tract infection

ObjectiveTo observe the clinical effect of the treatment of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney in treating middle-aged and old woman with chronic urinary tract infection.Methods82 cases were randomly divided into two groups.42 cases in treatment group were treated with herbal prescription of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney,and 40 cases in control group were treated with sensitive antibiotics.The clinical effect,safety and changes of immune index were compared between two groups.ResultsIn the treatment group,the recent cure rate was 76.8%,the total effective rate was 90.5% and the complete cure rate was 61.9%; while in the control group,the recent cute rate was 50%,the total effective rate 65% and the complete cure rate 27.5%.It showed significant difference between two groups (Z=2.740,P<0.05).There was no significant difference between two groups in hepatic and renal function,serum and urine β2-MG level before and after treatment and it showed a better clinical safety in treatment group.The level of IgA,IgG and IgM increased after the treatment in treatment group.Comparing with the control group,the results showed significant differences(t=9.1,10.5 and 11.2 respectively,P<0.01).In treatment group the level of CD4+was increased,CD8+was decreased,and the ratio of CD4+/CD8+ was increased obviously after the treatment.The differences before and after the treatment and between groups were significant(t=13.3,8.6,P<0.01).In control group the level of CD4+、CD8+、CD4+/CD8 +did not change significantly.The differences before and after the treatment had no statistical significance (P>0.05).ConclusionThe treatment with herbs of clearing liver fire,removing dampness,strengthening slpeen and tonifying kidney had better effect,less side effect and good research prospect.
7.The establishment and application of five-heart nursing service model

Objective To establish and apply the systemic caring and nursing model,the five-heart nursing service model. Methods The five-heart nursing service content was designed,the five-heart nursing service organization was perfected,the five-heart nursing service manual books were compiled and the satisfaction questionnaires for five-heart nursing service were designed.Three tryout departments were selected and the nursing staff was trained by human theory and five-heart nursing service amenity and flow according to the five-heart nursing service requirement. Results The patients satisfaction degree, medical entanglemant complaint and praise after the application of five-heart nursing service were greatly improved compared with those of before its application (P < 0.05). Conclusion The application of five-heart nursing service supplied systemic caring and nursing for patients and it proved effective to improve patients satisfaction degree.
9.Application of ultrasound-guided fine needle aspiration and core needle biopsy in diagnosis of thyroid nodules

Objective To compare the clinical evaluation in diagnosis of thyroid nodules between fine needle aspiration(FNA) and core needle biopsy(CNB).Methods 82 cases with thyroid nodules undergoing FNA and 33 cases with thyroid nodules undergoing CNB were selected.Cytology,histopathology,and postoperative results were compared with postoperative pathology as the gold standard.Results The accuracy,sensitivity,specificity,positive predictive value,negative predictive value,false positive rate,false negative rate of FNA and CNB were 91.5% vs 87.9%,93.2% vs 87.5%,87.0% vs 88.9%,94.8% vs 95.5%,83.3% vs 72.7%,13.0% vs 11.1%,6.8% vs 12.5%,respectively.There was no significant difference between FNA and CNB (P>0.05).Conclusion Both of FNA and CNB examination show high accuracy,sensitivity and specificity in diagnosis of benign and malignant thyroid nodules,but FNA is more economical,safe,convenient and thus has more extensive clinical application.For lymphoma or borderline tumor,CNB examination combined with immunohistochemistry is of more advantages.
10.Effects of Tangbikang on the Expressions of p38 MAPK of Sciatic Nerve and Plasma TNF-αin Diabetic Rats

Objective To explore the effects of Chinese herbal compound Tangbikang on the expressions of p38 MAPK of sciatic nerve and plasma TNF-α in diabetic rats. Methods Ten of the sixty male SD rats were selected randomly as normal group, and the rest were fed with high-fat diet and low-dosage STZ was used to induce type Ⅱdiabetic rat models. Model rats were randomly divided into model group, mecobalamine group and Tangbikang low-, medium-, and high-dosage groups, 10 rats in each group. Each medication group was intervened with relevant medicine. Rat unilaterals sciatic nerves were taken after 16 weeks. The content of TNF-α in plasma was determined by radioimmunoassay. Western blot method was used to detect the expressions of p38 and p-p38 MAPK protein of sciatic nerve. Results Compared with normal group, the expressions of p38 and p-p38 protein and content of TNF-αin model group significantly increase (P<0.05, P<0.01). Compared with the model group, the expressions of p-p38 protein and the content of TNF-α significantly decreased after medicine intervention in different doses Tangbikang groups and mecobalamin group (P<0.05, P<0.01). The expression of p38 protein in Tangbikang high-dose group significantly decreased (P<0.05), with statistical significance (P<0.05). Conclusion Tangbikang can reduce the expression of p38 and p-p38 MAPK protein of the rat sciatic nerve, and reduce the content of TNF-α protein in rat plasma, which may be one of the effective targets of neuroprotection and abirritation of diabetic peripheral neuropathy.
