1.Therapeutic Effect of Loosening Manipulation on Neck Tenderness Points for Cervicogenic Tinnitus

Objective To investigate the therapeutic effect of loosening manipulation on neck tenderness points for cervicogenic tinnitus . Methods Forty cervicogenic tinnitus patients were treated with loosening manipulation on neck tenderness points. Tinnitus grading was carried out before and after treatment by following Tinnitus Severity Grading Criteria for the evaluation of therapeutic effect. Results ( 1) Tinnitus was relieved after treatment for one week and 2 weeks, and at the end of treatment course (P<0.05 compared with that before treatment). (2) After treatment for 3 continuous weeks, the cure rate was 25.0% ( 10/40) , the markedly effective rate was 37.5% ( 15/40) , the effective rate was 27.5% ( 11/40) , the ineffective rate was 10.0% ( 4/40) , and the total effective rate was 90.0%. Conclusion Loosening manipulation on neck tenderness points is effective on relieving cervicogenic tinnitus.
2.Radiofrequency catheter ablation of atrial tachycardias related to myocardial scar or incision

Intra-atrial re-entrant tachycardias (IARTs) are common late after heart surgery. Conventional mapping and ablation is relatively difficult because of the complicated anatomy and multiple potential re-entry loops. In this study we aimed to evaluate the electrophysiological characteristics and radiofrequency catheter ablation of atrial tachycardia (AT) induced by myocardial scar or incision. Methods In 6 patients (three male and three female, aged 33.3+ 11.8 years) who had AT related to myocardial scar or incision,electrophysiological study and radiofrequency catheter ablation (RFCA) were performed. Earliest activation combined with entrainment mapping was adopted to determine a critical isthmus. Results Re-entry related to the lateral atriotomy scar was inducible in 5 of6 patients. With entrainment mapping, the PPI (post-pacing interval)-TCL (tachycardia cycle length) difference was <30 ms when pacing at the inferior margins of the right lateral atriotomy scar. Among them, 3 patients had successful linear ablation between scar area to inferior vena cava, and 2 patients between scar area to tricuspid annulus. Re-entry involving an ASD patch was demonstrated in 1 of 6 patients. PPI-TCL differences <30 ms were observed when entraining tachycardia at sites near the septal patch. But linear ablation failed in terminating AT. There was no complication during procedure. No recurrence of AT related to incision was observed during follow-up except for the failed patient. Conclusion Under conventional electrophysiological mapping, adopting linear ablation from scar area to anatomic barrier, successful ablation can be obtained in patients with IRATs related to myocardial scar or incision.
3.Genotype identification and breeding method of caveolin-1 gene knockout mice

Objective To investigate the identification and optimal breeding method of caveolin-1 knockout mice, and provide an ideal animal model for further study of the role of caveolin-1 in cerebral ischemic injury and repair. Meth?ods The introduced caveolin-1 gene knockout mice were reared in the SPF laboratory and genomic DNA was extracted from mouse tail tissue by the method of boiling lysis. According to the primer sequences provided by the Jackson Laboratory of America for polymerase chain reaction ( PCR) to detect the genotypes, with the four different ways of mating:caveolin-1 +/ -heterozygote intercrossing, heterozygous and homozygous caveolin-1 -/ -hybrid ( orthogonal and pay) as well as homo-zygous intercrossing. The pregnancy rate, shape characteristics of the filial generation mice and homozygous rate of the pa-rental mice were observed. Results Agarose gel electrophoresis results indicated that the size of molecular weight of the PCR products was about 200 bp and 661 bp, which were consistent with the expected target gene fragment, and identified caveolin-1 gene knockout mice of different genotypes successfully. The results of different mating patterns are basically in a-greement with Mendel rule, and the female and male aveolin-1 -/ -homozygous mice had a certain ability to reproduce, three different genotypes of mice had no significant differences between the shape features. Conclusions PCR can fast and reliably identify the genotypes of caveolin-1 knockout mice using genomic DNA through the method of boiling lysis. Combi- ning the breeding methods of intercrossing of caveolin-1 heterozygous mice and intercrossing of caveolin-1 homozygous mice may be a good way to obtain enough homozygous mice and homologous wild type mice in a short period.
4.Effects of caveolin-1 on expressions of interleukin-1βand interleukin-6 in the ischemic cortex after permanent middle cerebral artery occlusion in mice

ObjectiveToinvestigatetheeffectsofcaveolin1(Cav1)onexpressionsofproinflammatory cytokine interleukin (IL)-1βand IL-6 in the ischemic cortex after permanent middle cerebral artery occlusion in mice. Methods The Cav-1 knockout mice (n=40) and wild-type mice (n=40) were randomly divided into ischemia groups and sham operation groups (n=20 in each group). They w ere also redivided into ischemia or sham operation at 3, 7, 10 and 14 d time points ( n=5 in each time point). A permanent middle cerebral artery occlusion model w as induced by the suture method. Immunohistochemical method w as used to detect the expressions of IL-1βand IL-6 in the ischemic cortex. Results The expression levels of IL-1βand IL-6 in the ischemic cortex at each time point in the ischemia group in Cav1 knockout mice w ere significantly higher than those in the ischemia group in the w ild-type mice ( al P< 0.05 ). Conclusions The upregulations of proinflammatory cytokines IL-1βand IL-6 in the ischemic cortex in Cav1 knockout mice suggests that Cav1 plays an important role in aleviating inflammation after cerebral ischemia.

6.Analysis of basic research in preventive medicine supported by National Natural Science Foundation in Nangjing Medical University during 2009-2019

Objective:Analyzed the funding data of the National Natural Science Foundation of China in the field of preventive medicine from 2009 to 2019 in Nanjing Medical University, to provide scientific basis for scientific research management and decision-making.Methods:Descriptive statistics was used to reveal the status of application and funding in the field of preventive medicine, the distribution and development trends of each secondary code, the research hotspots and key themes in this field.Results:From 2009 to 2019, Nanjing Medical University submitted 566 applications in total and 230 projects were funded with a total funding of 144.03 million in the field of preventive medicine. The top five codes in the funded projects were H2610, H2607, H2603, H2611 and H2601.The funding of H2610 was mainly concentrated in four major areas including cancer, cardiovascular and cerebrovascular diseases, diabetes and hematological system diseases.H2607 funded a large amount of projects for Non-coding RNA and related pathways in recent years. Funding in the H2603 were mainly focused on tumors, glucose metabolism disorders and vitamin A requirements. The funding rate in the H2605 was 9.26%, which was significantly lower than other secondary disciplines in preventive medicine.Conclusions:The National Natural Science Foundation of Preventive Medicine in the Nanjing Medical University has a good development trend, but it also faced challenges. In the future, evidence-based scientific management will be adopted to promote the development of preventive medicine according to the characteristics of different secondary disciplines.
7.Clinical analysis of 43 episodes of cyst infection in autosomal dominant polycystic kidney disease

Objective To summarize the clinical characteristics and outcome of renal cyst infection in patients with autosomal dominant polycystic kidney disease (ADPKD). Methods Clinical data of 40 ADPKD patients with 43 episodes of renal cyst infection admitted in Shanghai Changzheng Hospital from 1st January 1991 to 31st December 2010 were retrospectively analyzed.Differences of microbiological data and treatments between 1st January 1991 to 31st December 2000 and 1st January 2001 to 31st December 2010 were compared. Results Among 473 identified patients with ADPKD and 662 episodes of hospitalization,40 patients had 43 episodes of renal cyst infection,including 8 definite and 35 likely cases.Microbiological documentation was available for 34 episodes (79.0%),Escherichia coli accounting for 82.4% of all retrieved bacterial strains.Resistant Escherichia coli to quinolone and certain β-lactamine increased in recent decade.Clinical efficacy of initial antibiotic treatment was noted in 69.8% of episodes. Antibiotic treatment modification was more frequently required for patients receiving initial monotherapy compared with those receiving combination therapy.In the first ten-year group,initial combination therapy and clinical efficacy were noted in 30.0% and 60.0% of episodes respectively,and hospital stay was (20.2±6.7) d.In the second ten-year group,initial combination therapy and clinical efficacy were noted in 61.9% and 78.2% of episodes respectively,and hospital stay was (16.3±3.2) d.Large infected cysts (diameter >5 cm) frequently required drainage. Conclusions In renal cyst infection,the source of the organisms is often a gram negative enteric organism.Empiric therapy is often initiated with two antibiotics.The drainage of large infected cysts remains the main treatment for cyst infection.
8.Evaluation of problem-based learning combined with scene simulation in cardiopulmonary resuscitation teaching of standardized training of general practitioner

Objective:To evaluate the effect of problem-based learning (PBL) combined with scene simulation in the cardiopulmonary resuscitation (CPR) teaching of standardized training of general practitioner, and explore a high-quality teaching method that will help students develop their clinical thinking and practical skills.Methods:Forty students of the Grade 2018 general practitioner training program in Yixing Hospital Affiliated to Jiangsu University were randomly divided into experimental group ( n=20) given PBL combined with scene simulation teaching method and control group ( n=20) given traditional teaching method. The pre-class preparation, classroom discussions, actual combat simulations, and analysis and summary were conducted in the experimental group (4-hour CPR training), while classroom teaching, teaching rounds, analysis and discussion, skills training were conducted in the control group (4-hour CPR training). Furthermore, both groups received the same theoretical test, skill assessment, and questionnaires. t-test or chis-quare test was performed for comparison between the two groups using SPSS 18.0. Results:The average score of theoretical test in the experimental group was significantly higher than that in the control group [(87.5±4.3) vs. (81.2±4.7), P<0.05]; the average score of the skill assessment in the experimental group was significantly higher than that in the control group [(91.0±1.5) vs. (83.1±1.8), P<0.05]. The questionnaire survey showed more students of the experimental group who reported greater improvement in their abilities than those of the control group ( P<0.05). Conclusion:PBL combined with scene simulation teaching method can effectively improve the quality of CPR teaching for standardized training of general practitioners, help students improve clinical thinking and comprehensive processing capabilities.
9.Causes analysis of 652 hospital stays in patients with autosomal dominant polycystic kidney disease

Objective To analyze the causes of 652 hospitalizations in the patients with autosomal dominant polycystic kidney disease (ADPKD).Methods The medical records of all ADPKD inpatients in our hospital from January 1,1990 to December 31,2010 were collected.The differences of hospitalization causes in different age,gender and period were analyzed.Results (1)In 652 hospitalizations,the most common cause was lumbar pain (15.2%),followed by cystic bleeding (14.6%),aggravating renal failure (10.1%),dialysis-related problems (9.4%),renal transplant related issues (8.3%),renal replacement therapy for ESRD (8.0%),urinary tract infection (6.4%),end stage renal failure (5.8%),hypertension (4.1%),renal cyst volume enlargement (3.7%),finding polycystic kidney disease (2.1%),urinary lithiasis (1.8%) and others (10.4%).(2)Younger patients were admitted into hospital because of polycystic kidney bleeding and finding PKD.With the increase of patients age,hospitalization due to dialysis-related problems increased,while many middle-aged patients were hospitalized because of back pain.(3)Male patients were admitted into hospital for aggravating renal failure,ESRD,kidney transplantation-related problems and urinary lithiasis,while female patients mainly for lumbar pain,dialysis-related problems and urinary tract infection.(4)The proportion was significantly reduced with time of finding PKD,renal failure and polycystic kidney bleeding,the proportion of renal cysts increasing and aggravating renal failure increased,there was a significant increase in the proportion of patients with hypertension,while a significant decrease in the proportion of patients with uncontrolled hypertension,and the average SBP was also significantly reduced.Conclusions The highest rate of hospitalization of ADPKD patients is in 40 to 60 age group.Cause of admission varies with age and gender,and changes with the change of time.Over the past decade,the proportion of hospitalization due to renal cysts enlargement and renal failure aggravation increased significantly.The incidence of hypertension is higher than that in the first 10 years,but hypertension control rate increases compared with the previous.Prevention should focus on finding the suppression measures of renal cysts enlargement.

Compared with developed countries, China Disaster Nursing Research started relatively late and is still in its infancy, there are many projects to be studied and developed in disaster nursing.
