1.Application of hypofractionated conformal radiotherapy in non-small cell lung cancer

Hypofractionated conformal radiotherapy is capable to deliver much higher doses to the cancer than is possible with standard techniques. Recently there is data suggesting that the early stage nonsmall cell lung cancer ( NSCLC) which is not suitable to surgery is likely to benefit from this regimen, with low lung toxicity. Manyphase Ⅰ-Ⅱ studies showed that the patients with locally advanced NSCLC are well-tolerated to hypofractionated conformal radiotherapy. The model of radio-physic and relative clinical studies suggest that hy-pofractionation would not increase the risk of radiation pneumonitis compared to standard therapy.
2.Radiotherapy of stage I and II non-small cell lung cancer in 168 patients

Objective To evaluate the efficacy and prognostic factors of radiotherapy in 162 patients with stage I and II non-small cell lung cancer(NSCLC).Methods Between December 1982 and August 1992, 168 patients with stage I and II NSCLC were treated, with 116 confirmed by histopathology and 52 by cytopathology. All patients received radiotherapy to a dose of 40-76 ?Gy and were followed up for more than five years. Results The overall 1-, 3- and 5-year survival rates were 67.6%, 28.0% and 15.7%. Absence of concurrent disease and Karnofsky performance status ≥80 were favorable prognostic factors. Conclusions Radiotherapy is effective for stage I and II non-small cell lung cancer patients who are contraindicated for surgery. Higher dose of radiotherapy is recommended to improve the local control and survival rate.
3.Proliferation and Immunophenotypic Analysis of CD3AK Cells Derived from Lymph Node

Objective: To study the propagation and phenotypes changes of killer cell (CD3AK cell activated by CD3 mAb in vitro. Methods: Lymph nodes taken from lung cancer patient is dissociated into single cell suspension by mechanical method and cultured in culture medium added CD3 mAb and a little dose IL-2. We analyze cell immunophenotype by flow cytometry and proliferation by trypan blue exclusion test per 2 days. Results: Immunophenotypic analysis showed that CD3AK expressing CD3, CD8, CD56, CD25 increased, and reached a peak value which is 2.33 times than before culturing in the 8 th day. Conclusion: CD3 mAb added to the culture medium can obviously activate CD3AK cell and stimulate proliferation and keep its killer activity.
4.Study on the Expression of Secretory Phospholipase A2-Ⅱ A in Lesions of Psoriasis Vulgaris

Objective To discuss the relationship between the expression of secretory Phospholipase A2-Ⅱ A(sPLA2-ⅡA) and the pathogenesis psoriasis vulgaris patients lesions.Methods Using Psoriasis Area Severity Index(PASI),50 psoriasis vulgaris were divided into three groups:mild (n =16),moderate (n =18) and severe group (n =16),and compared to the non-psoriasis control group (n=52).The serum level of sPLA2-ⅡA in psoriasis vulgaris patients and the normal control group were detected using ELISA methods.Using RT-PCR,mRNA expression levels of sPLA2-ⅡA and subtypes were detected.Using western blot,the expression levels of Akt and p-Akt were detected.Results The serum level of sPLA2-ⅡA in the psoriasis lesion's group was significantly higher than that in the healthy control (t=13.62,P<0.01).The mRNA expression of sPLA2-ⅡA in the lesion of psoriasis was significantly higher than that in health control (t=113.41,P<0.01).In the other subtypes,the mRNA expression of sPLA2 had no distinctive.Among the three groups (mild group,moderate group and severe group),the difference in serum level and mRNA expression of sPLA2-ⅡA were statistically significant (F =28.12,69.62,P<0.01).There was statistical significance among three groups (t=3.14,5.14,6.38,all P<0.01;t=4.77,10.42,10.58,all P<0.01).The expression of Akt/p-Akt in the psoriasis lesion's group were higher than that in health control (t=17.79,19.04,P<0.01).Conclusion The expression of sPLA2-ⅡA would be associated with order of severity in lesions of psoriasis vulgaris.The pathway was relate to expression of Akt and p-Akt.
5.Effect of Psychological Support Therapy on Anxiety Resulted from Pain after Stroke

Objective To observe the effect of psychological support therapy on anxiety resulted from pain after stroke. Methods 40 patients with anxiety after stroke were divided equally into psychological support treatment group and control group. They were assessed with Hamilton Anxiety Scale (HAMA) and Visual Analogue Scale (VAS) of pain before and after treatment. Results The scores of HAMA and VAS improved more in treatment group than in control group (P<.01). Conclusion The psychological support therapy can reduce the anxiety and pain after stroke.
6.The analysis of effect about different therapies on brain metastases from non-small cell lung cancer

Objective To evaluate the prognostic factors of brain metastasis from non-small cell lung cancer and suggest a individualized treatment method proposal with prognostic estimation. Methods From Dec. 2003 to Jan.2007, 183 patients received whole brain radiation therapy (WBRT) were retrospectively analyzed. Kaplan-Meier method was used to analyze the survival. Logrank test was used to evaluate the difference between the groups. Multivariate survival analysis was conducted using a Cox proportional hazard regression model with a backward stepwise procedure. Results The overall l-, 2- and 3-year survival rate was 40.6%, 16.6% and 11.3%, respectively, but with a median survival time of 10.0 months (95% CI 8.6-11.4 months). In multivariate analysis, RAP grouping, weight loss, LDH in blood serum and treatment method were independent prognostic factors. The median survival time of WBRT alone, WBRT with chemotherapy, surgery with chemoradiotherapy and WBRT with Gefitinib was 9.0, 9.0, 22.0 and 13.0 months, respectively, but their difference were statistical significant (X2 = 10.37, P = 0.016). Conclusions The main prognostic factors of brain metastasis from non-small cell lung cancer are RAP grouping, weight loss, LDH in blood serum and treatment method. The survival time is prolonged by proper multidiseiplinary management than WBRT alone. The effect of combined treatment of surgery with chemoradiotherapy is favorable for the patients operated with single region of metastasis.
7.Early life stress and schizophrenia:a retrospective case-control study

A total of 105 patients with first-episode schizophrenia (male =51,female =54) and 99 normal controls (male =51,female =48) were included into this retrospective case-control study.Childhood trauma questionnaire-28 item short form (CTQ-SF) was used to assess the experience of childhood abuse.The result of binary logistic regression showed that emotional abuse (β =0.630,P < 0.05) and emotional neglect(β =0.270,P < 0.05) were included into the final model of predicting schizophrenia.It indicates that patients with first-episode schizophrenia experienced more early life stress than controls.Particularly emotional abuse and emotional neglect may play important roles in the onset of schizophrenia.
8.Dosimetric evaluation of different IMRT treatment plans for non-small cell lung cancer

Objective To investigate the optimal treatment planning of intensity modulated radiotherapy (IMRT) for non-small cell lung cancer (NSCLC) .Methods Two types of treatment plans were designed for 11 patients with inoperable NSCLC disease.In the first plan(PTV60 plan) ,60 Gy was prescribed to the planning target volume(PTV) which was created using CTV(GTV +6-8 mm) plus the margin for organ motion and setup uncertainties.In the second plan(PTV70 plan) ,70 Gy was prescribed to the PTV which was created using GTV plus the margin for organ motion and setup uncertainties.The dose-volume histogram,the planning target volume coverage,and other dosimetric parameters of normal structures were compared between the two plans.Results These two plans were not significantly different in the dose heterogeneity,but commpared with the PTV60 plan,the PTV70 plan significantly improved 60 Gy volume with regard to PTV coverage.The values of V_(20) and V_5 of lung were reduced in average by (1.69 ±0.42) % ,and (1.29 ±1.09) % (t = 0.047,0.264,P = 0.002) ,respectively.Conclusions The planning of 70 Gy to the PTV using GTV plus the margin for organ motion and setup uncertainties might be better than that of 60 Gy to the PTV using CTV(GTV +6-8 mm) plus the margin for organ motion and setup uncertainties.
9.Prognosis analysis of esophageal carcinoma patients with tumor loco-regional recurrence after curative surgery

Objective To evaluate the outcome of radiotherapy for recurrent esophageal cancer after surgery and to determine the prognostic factors. Methods From Jan 2004 to Dec 2009, 93 patients of esophageal carcinoma with loco-regional recurrence as the first site of failure after surgery were retrospectively reviewed. Kaplan-Meier method was used to analyze the survival. Logrank test was used to evaluate the difference between the groups. Multivariate survival analysis was conducted using a Cox proportional hazard regression model with a backward stepwise procedure. Results The overall survival rates at 1, 2 and 3 years were 40. 9% , 10. 1% and 6. 7% ,respectively,but with a median survival time of 11.0 months(95% CI 9. 4-12. 6) . In univariate analysis, age,PS, radiation dose and retreatment methods were independent prognostic factors. In multivariate analysis, only radiation dose and retreatment methods were independent prognostic factors for overall survival. Conclusions The prognosis of patients with postoperative loco-regional recurrence of esophageal cancer is poor. However, a long-term survival maybe expected by definitive chemoradiotherapy.
10.Protetive effect of substance P on hyperoxia lung injury and its regulation

Objective To investigate the expression of JNK2 in hyperoxic lung injury ,and explore the protective effect of sub-stance P (SP) on hyperoxic lung injury and its mechanism .Methods Sixteen SD rats were divided into four groups with 4 rats in each group :room-air and f 9 g/L saline group (group A) ,room-air and SP group (group B) ,hyperoxia injury group and f 9 g/L sa-line group (group C) ,hyperoxia injury group and SP group (group D) .Rats ingroup B and D were injected with SP 1 × 10-6 mol · L -1 · kg -1 · d-1 intraperitoneally ,group A and group C were injected with an equal volume of 9 g/L saline .The animals were sac-rificed after 14 days of experiment .Lung pathology was examined with light microscopy ,lung wet/dry (W/D) ratio and the level of SP and PCNA and TUNEL in lung were evaluated .The Superoxide dismutase (SOD) ,malondialdehyde (MDA) and glutathione (GSH) level were assayed respectively in lung tissue .The quanlity of JNK2 protein was detected by Western blot analysis .Results Compared with group A ,the high oxygen groups all had different degrees of lung injury ,,while the lung pathological pictures in group D was improved significantly compared with group C .Western blot showed that level of JNK2 in group C was obviously higher than that of group A ;After the intervention ,level of JNK2 in group D was lower than that of group C .The lung W/D retio , TUNEL and PCNA expression and distribution SOD ,MDA and GSH was consistent with the trends of JNK2 protein expression . Conclusion High oxygen stress can activate damage lung tissue JNK 2 activity ;SP protection mechanism of high oxygen lung injury may be induced by cutting high oxygen activation of JNK 2 to inhibit oxidative damage .
