1.A new understanding of the pathogenesis of nonalcoholic fatty liver disease

The incidence rate of nonalcoholic fatty liver disease (NAFLD) has been increasing in China in recent years and has reached 11%-15%.At present,the multiple-hit hypothesis is considered the main pathogenesis of NAFLD,yet the specific pathogenesis remains unknown.This article elaborates on the roles of carcinoembryonic antigen-related cell adhesion molecule 1 highly expressed in the liver,vitamin D/vitamin D receptor axis,and psychological factors in the development and progression of NAFLD,in order to provide new directions for the research on the pathogenesis of NAFLD in future.
2.Therapeutic effect of hyperbaric oxygen therapy on patients with severe craniocerebral injury and its influence ;on serum interleukin -1 beta and interleukin -6 levels

Objective To explore the clinical curative effect of hyperbaric oxygen therapy for severe cranio-cerebral injury,and its influence on the level of serum interleukin -1 beta(IL -1 )and interleukin -6(IL -6). Methods 150 patients with severe craniocerebral injury were selected and divided into two groups by random and single blind method.75 patients of the control group were treated with conventional therapy,75 patients of the observation group were given hyperbaric oxygen on the basis of the conventional treatment,and the clinical efficacy was compared between the two groups.Results The total effective rate of the observation group was 94.67%,which was significantly higher than 77.33% of the control group,the difference was statistically significant(χ2 =7.83,P <0.05).There were no significant differences in GCS and ADL scores between the two groups(t =1.22,2.39,all P >0.05).After treat-ment,the GCS and ADL scores of the observation group were (12.18 ±2.13)points,(67.46 ±12.55)points,which were significantly higher than those before treatment(t =8.01,7.76,all P <0.05),and there were significant differ-ences between the two groups(t =7.23,6.82,all P <0.05).Before treatment,there were no significant differences in the serum levels of IL -6 and IL -1 between the two groups(t =2.10,1.01,all P >0.05).After treatment,the serum levels of IL -1 and IL -6 in the observation group were (127.48 ±16.35)pg/mL,(89.66 ±13.47)pg/mL, which were significantly lower than before treatment(t =8.34,7.89,all P <0.05),there were significant differences between the two groups(t =7.65,7.22,all P <0.05).Conclusion Implementation of hyperbaric oxygen in patients with severe traumatic brain injury on the basis of conventional symptomatic treatment can effectively improve the clinical symptoms and signs,reduce the serum IL -1 and IL -6 levels,promote the recovery of brain function and ability of daily life,and which is worth to be promoted in clinical.
3.The effect of early mechanical ventilation combined with mild hypothermia in the treatment of severe brain injury and its effect on serum C-reactive protein and hemorheology

Objective To explore the clinical effect of early mechanical ventilation coupled with mild hypothermia in the treatment of severe traumatic brain injury(STBI)and its effect on serum C -reactive protein (CRP)and hemorheology.Methods According to the order of admission,110 patients with STBI were divided into two groups.55 cases in the control group were given conventional symptomatic treatment and early mechanical ventilation, while 55 patients in the observation group were given early mechanical ventilation coupled with mild hypothermia.The clinical efficacy was compared between the two groups.Results The good prognosis rate was 74.55% in the observation group,which was significantly higher than 56.36% of the control group (χ2 =7.65,P <0.05).The serum CRP in the observation group admitted to hospital 2,3,5,7d were significantly decreased compared with the control group(t =7.48,8.04,7.34,7.82,all P <0.05).After treatment,the hear whole blood viscosity was (6.03 ±0.89)mPa·s, low shear whole blood viscosity was (3.18 ±0.19)mPa·s,plasma viscosity was (1.21 ±0.18)mPa·s,hematocrit was (37.56 ±3.23)%,and those were decreased compared with before treatment(t =8.45,7.54,7.04,8.02,all P <0.05).And there were significant differences compared with the control group(t =7.77,6.69,6.44,7.49,all P <0.05).Conclusion The effect of early mechanical ventilation coupled with mild hypothermia in the treatment of STBI has good effect,and it can reduce serum CRP levels,improve blood rheology,which is conducive to patients'prognosis.
4.Comparison on the effect of two kinds of mucosal protective agents combined with smoking cessation on the treatment of peptic ulcer

Objective To investigate the effect of two kinds of mucosal protective agents combined with smoking cessation on peptic ulcer. Methods In our hospital from January 2014 to December 2016, 244 cases with peptic ulcer with smoking as the research object, randomly divided into the control groupand the observation group, 122cases in each group. The control group were given omeprazole and smoking cessation and bismuth potassium citrate, the observation group were given omeprazole and smoking cessation and misoprostol. The therapeutic effect and adverse reaction were compared between the two groups. Results The total efficiency in the observation group 97.54% was significantly higher than the control group 87.70% (P<0.05). The incidence of adverse reactions in the observation group 2.46% was significantly lower than in the control group 7.38% (P<0.05). Conclusion For the treatment of refractory peptic ulcer with the habit of smoking, smoking cessation combined with Losec and misoprostol can effectively improve the effect, and reduce the occurrence of adverse reaction, which is worthy of promotion.
5.Cytokine-induced killer cell for interventional chemotherapy of hepatocellular carcinoma

Objective To evaluate the clinical effect of hepatocellular carcinoma treatment with a combination therapy of transcather arterial super liquefied lipiodol embolization and cytokine-induced killer cell(CIK) infusion.Methods There were 3 groups in this study,group 1:38 cases of HCC patients treated with a combination therapy of transcather arterial super liquified lipiodol embolization and CIK infusion;group 2:80 cases of HCC patients treated with a combination therapy of transcather arterial super liquefied lipiodol embolization and percutaneous intratumoral ethanol injection;group 3:134 cases of HCC patient treated with transcather arterial super liquefied embolization.Finally,the outcomes of the 3 groups were compared.Results The short term effective rates of group 1,2 and 3 were 76.1%,41.3% and 14.9% respectively,simultaneously with significant difference of changes concerning AFP value among the three groups especially in group 1 the AFP decrease to normal level while those of the other two groups still remain in higher levels.Conclusions The living quality and survival rate of HCC patients could be improved by a combination therapy of transcather arterial super liquefied lipiodal embolization and CIK infusion.(J Intervent Radiol,2007,16:235-239)
6.Microsurgical removal of suprasellar craniopharyngioma via pterional approach

Objective To explore the microsurgical techniques for total removal of the suprasellar cranio pharyngiomas via the pterional approach.Methods The clinical data of 28 patients with suprasellar craniopharyngio mas resected by the microsurgery was analyzed retrospectively.Results The tumors were total removed in 24 cases (86%)and subtotal resected in 4 cases(14%).The frequent postoperative complications are diabetes insipidus,hy perthermy,imbalance of water and electrolyte and lower level of hormone.Conclusion It is the key to get fine operation effect that removal sclerotin of base of skull as far as possible and releasing cerebrospinal fluid sufficiently,paying attention to protect the perforating branch and nourishing artery in each operative spaces.
7.Short pedicle screw segment with bone graft through unilateral pedicle channel and pedicle screw fixation for thoracolumbar burst fractures by posterior operation

BACKGROUND:Short-segment pedicle screw technology has been extensively used in the treatment and repair of thoracolumbar burst fractures in the clinic. However, it requires further investigations whether bone graft through unilateral pedicle channel has superiority. <br> OBJECTIVE:To observe the perioperative effects on thoracolumbar burst fractures posterior by bone graft through unilateral pedicle channel with short pedicle screw segment fixation. <br> METHODS:Clinical data of 57 cases of thoracolumbar burst fractures without neurological symptoms treated by short segment pedicle screw internal fixation therapeutic method at the Department of Orthopedics, The First People’s Hospital of Yinchuan City from January 2009 to December 2013 were retrospectively analyzed. According to therapeutic methods, they were divided into three groups:pedicle screw fixation through injured vertebrae (n=21), unilateral pedicle screw fixation with vertebrae pedicle screw (n=19), and the short segment pedicle screw fixation with unilateral pedicle screw fixation and bone graft through pedicle channel (n=17). We observed the changes in operation time, the blood loss volume in operation, postoperative drainage blood (at the period of removing the drainage tube), injured vertebrae height’s ratio, the Cobb’s angel of injured vertebrae sagittal plane, vertebral canal encroachment rate and visual analog scale. <br> RESULTS AND CONCLUSION:(1) The unilateral pedicle screw fixation and bone graft by pedicle channel could prolong operation time and increase postoperative drainage blood volume (P<0.05). (2) Three operation methods improved vertebral body height. (3)There were no significant differences in Cobb’s angle postoperation among the three groups (P>0.05). These data suggested that the bone graft through pedicle channel was not the factors to improve the spine kyphosis. (4) Bone graft by pedical channel improved vertebrae channel value. (5) Bone graft through pedicle channel combined with screw implantation could improve pain after treatment. These findings indicated that the therapeutic technology of short pedicle screw fixation on thoracolumbar vertebral burst fracture with unilateral pedicle fixation and bone graft through pedicle channel is a safe reliable operation method.
8.CNTNAP3 Copy Number Variation and its Significance in Crohn's Disease

DNA copy number variation is an important pathogenic factor of human diseases and might be involved in the pathogenesis and pathological process of inflammatory bowel disease (IBD).Aims: To investigate the copy number variation of CNTNAP3 gene and its significance in Crohn''s disease (CD).Methods: A total of 101 active CD patients admitted from Jul.2009 to Dec.2010 at Renji Hospital, School of Medicine, Shanghai Jiao Tong University were enrolled.Eighty healthy subjects were served as controls.Peripheral blood or intestinal mucosa samples of CD patients were collected, and the copy number variation of CNTNAP3 gene was screened and validated by array-based comparative genomic hybridization (aCGH, n=8) and real-time PCR (n=93);expression of CNTNAP3 encoding protein was determined by ELISA (n=55).Results: A large fragment copy number amplification was revealed by aCGH at chromosome 9p13 region (including CNTNAP3 gene) in untreated CD patients.Real-time PCR confirmed that the copy number of CNTNAP3 gene was amplified in peripheral blood of CD patients, especially steroid-naive patients as compared with the normal controls (208 616.4±126 984.7 and 233 453.3±113 520.8 vs.161 750.2 ±53 940.3, P<0.05 and P<0.01).In the clinical parameters analyzed in this study, only smoking was significantly correlated with CNTNAP3 copy number amplification (P<0.05).However, there was no significant difference in plasma CNTNAP3 level between CD patients with amplified copy number and normal controls (P>0.05).Furthermore, the plasma CNTNAP3 level in CD patients with amplified copy number was not correlated with the simplified endoscopic score for CD (P>0.05).Conclusions: Copy number amplification of CNTNAP3 gene might be involved in the pathogenesis of CD in Chinese population.Glucocorticoid treatment and smoking might affect the copy number variation of CNTNAP3 gene.Plasma CNTNAP3 level cannot discriminate CD patients from healthy subjects.Conclusions of this study needs to be further demonstrated and discussed.
9.Investigation on rural environmental sanitation and summer diarrhea among rural residents in Zhejiang Province

Objective :
To understand the status of rural environmental sanitation and summer diarrhea among rural residents in Zhejiang Province,and to provide evidence for improvement of rural environmental sanitation and prevention and control of summer diarrhea.
Methods :
The stratified random sampling method was used. Five or six counties each were selected from the east,middle and west of Zhejiang Province;five townships(excluded Chengguan Town)were selected from each selected county;four villages were selected as field survey points from each township;five households were selected from each survey point;one family member(householder or housewife)was selected from each selected household. The information about the environmental sanitation status and prevalence of summer diarrhea were obtained by literature review,interviews,questionnaire suvey,field investigation and laboratory tests. Logistic regression model was used to analyze the influencing factors for occurrence of summer diarrhea among rural residents.
Results :
There were 2 000 households surveyed. In the past two weeks,108 households had families with diarrhea and the prevalence of diarrhea was 5.40%. A total of 1 828 households disposed the household waste in the garbage bins or pools,accounting for 91.40%. There were 1 926 households with organized sewage discharge,accounting for 96.30%. There were 768 households with nymphs found in the kitchen,accounting for 38.40%;404 households with live egg sheaths found,accounting for 20.20%;592 households with cockroach traces found,accounting for 29.60%;564 households with rat traces found,accounting for 28.20%;and 1 206 households with flies found,accounted for 60.30%. There were 1 314 households with disease vectors around the house,accounting for 65.70%. There were 1 944 household with sanitary toilets,accounting for 97.20%. A total of 1 915 households had habits of drinking boiled water,accounting for 96.57%. The results of multivariate logistic regression analysis showed that rat traces found in the kitchen(OR=2.026,95%CI: 1.362-3.013)were a risk factor for summer diarrhea in rural residents,while drinking boiled water(OR=0.272,95%CI: 0.091-0.811)was a protective factor.
Conclusion
The sanitary environment and family health habits of rural residents were related to the prevalence of diarrhea in summer. Paying attention to kitchen hygiene and drinking boiled water could prevent summer diarrhea.
10.Curative effect of somatostatin on acute adhesive intestinal obstruction and its protection effect on intestinal mucous membrane barrier function

Objective To discuss curative effect of somatostatin on acute adhesive intestinal obstruction and its protec-tion effect on intestinal mucous membrane barrier function. Methods All 90 cases of patients with acute adhesive in-testinal obstruction were divided into two groups, with 45 cases of patients in research group and control group respec-tively. The patients in control group were given routine medical treatment, such like fasting, water-fast, continuous gas-trointestinal decompression, antibiotics to prevent infection, relieving spasm and pain, adjustment of water-electrolyte imbalance, total parenteral alimentation(TPA)and etc. The patients in research group were additionally given 3 mg so-matostatin injection, 50 mL normal saline(NS) with minipump by 4 to 6 mL per hour, until cacation or exhaust from anus, given transit operation treatment or termination of treatment, while the patients in control group were given the same medical treatment as that in research group except for somatostatin. The changes of serum endotoxin, diamineoxi-dase (DAO)and procalcitonin(PCT)levels of patients in two groups before and 5 days after medical treatment were ob-served and compared, and the clinical curative effect and untoward effect were evaluated as well. Results After 5 days'medical treatment, the serum endotoxin, DAO and PCT levels of patients in two groups were obviously declined than before (P<0.05 or P<0.01), and the declining rate of patients in research group was much higher than that in control group(P<0.05), and meanwhile, the total clinical efficiency of patients in research group(95.56%)was much higher than that in control group(80.00%)(χ2=5.07, P<0.05). 3 cases(6.67%)and 5 cases(11.11%)of untoward effect were appeared on patients in control group and research group during treatment, and no statistical differences were appeared after comparison(χ2=0.14, P>0.05). Conclusion The somatostatin has reliable curative effect on adhesive intestinal ob-struction after abdominal operation, and can obviously improve clinical symptom with high security, whose mechanism of action may have close effect on reducing serum endotoxin, DAO and PCT levels and protecting and repairing in-testinal mucous membrane barrier function.
